"Gangguan pada muskuloskeletal menimbulkan dampak fisiologis, psikologi, sosial dan ekonomi bagi individu yang mengalaminya. Peran dan kontribusi tenaga kesehatan khususnya perawat spesialis medikal bedah peminatan muskuloskeletal sangat diperlukan dalam menangani permasalahan kesehatan ini. Pemberian asuhan keperawatan berbasis bukti penelitian mendukung layanan kesehatan yang berkualitas. Hal ini menjadi landasan dalam melaksanakan peran perawat spesialis sebagai pemberi asuhan, pendidik, peneliti dan inovator. Residensi perawat spesialis yang dilaksanakan selama dua semester memberikan pengalaman dan kesempatan untuk menerapkan model teori keperawatan Self Care Orem pada 30 kasus resume dan satu kasus utama yaitu klien dengan multipel trauma fraktur tertutup femur, fraktur terbuka olecranon dan distal ulna. Peran perawat peneliti dilakukan pada penerapan evidence base nursing berupa pengaturan posisi lateral pasca pembedahan tulang belakang untuk menurunkan intensitas nyeri dan meningkatkan mobilitas. Inovasi keperawatan yang dilakukan oleh residen berupa penerapan pengkajian risiko deep vein thrombosis pada klien dengan masalah muskuloskeletal ekstremitas bawah. Pengkajian ini bertujuan sebagai skrining pada klien yang berisiko terjadi deep vein thrombosis sehingga dapat diberikan tindakan pencegahan sebelumnya.
Musculoskeletal disorders have physiological, psychological, social, and economic effects on the individuals who experience them. The role and contribution of health workers, especially medical surgical specialist nurses specializing in musculoskeletal care, is needed to deal with this health problem. The provision of evidence-based nursing care supports quality health care. This became the basis for the role of specialist nurses as caregivers, educators, researchers, and innovators. The specialist nurse residency, which was held for two semesters, provided experience and opportunity to apply Orem's self-care nursing theory model to 30 resume cases and one main case, namely a client with multiple traumatic closed fractures of the femur, open fractures of the olecranon, and distal ulna. The role of the research nurse was to apply evidence-based nursing in the form of setting the lateral position after spinal surgery to reduce pain intensity and increase mobility. Residents of the nursing facility implemented an innovative approach of assessing the risk of deep vein thrombosis in clients with lower extremity musculoskeletal issues. This study aimed to screen clients who are at risk for deep vein thrombosis so that preventive measures can be provided beforehand."

"Professionalime ilmu keperawatan adalah bagian disiplin ilmu keperawatan sampai dengan keperawatan sub spesialisasi dimana perawat spesialis sebagai seorang perawat yang dipersiapkan melampaui level seorang perawat generalis yang berwenang untuk praktek sebagai spesialis dengan keahlian dalam cabang bidang keperawatan. Ners spesialis keperawatan medikal bedah dengan peminatan muskuloskletal berperan dalam perawatan kekhususan masalah dan kasus kelolaan yang berkaitan dengan orthopedi atau muskuloskletal. Tujuan utama memberikan asuhan keperawatan secara komprehensif pada pasien kasus Spinal Cord Injury dengan trauma melalui pendekatan keperawatan model teori self-care Orem. Asuhan berupa pengelolaan 1 kasus kelolaan uatam dan 30 resume yang terdiri 3 bagian kasus besar yaitu kasus Spine, ekstremitas atas, dan ekstremitas atas dengan menggunakan teori model keperawatan Self-Care Orem yang menekankan tentang kebutuhan perawatan diri. Pencegahan terjadinya luka tekan dengan pemberian Aloe vera gel sebagai bagian dari tindakan mandiri keperawatan berdasarkan bukti penelitian ilmiah. Proyek inovasi berupa pengkajian resiko DVT dengan menggunakan Autar Scale adalah bentuk pengembangan dari intervensi keperawatan untuk pencegahan resiko terjadinya DVT pada pasien dengan kasus muskuloskletal. Tujuan seluruh kegiatan yang dilakukan selama kegiatan residensi adalah mewujudkan asuhan keperawatan yang komprehensif berdasarkan bukti ilmiah terkini.
Professional expertise is part of the discipline of expertise up to sub-specialization where specialist nurses as nurses who are prepared go beyond the level of a generalist nurse who is classified to practice as a specialist with expertise in their field of expertise. Medical surgeon specialist nurses with specialization in musculoskeletal play a role in treating specific problems and managing cases related to orthopedics or musculoskeletal issues. The main goal is to provide comprehensive care assistance to patients with spinal cord injury cases with trauma through the Orem self-care model of therapy. The care is in the form of managing 1 main case and 30 resumes consisting of 3 major case sections namely the Spine, upper extremity and upper extremity cases using a theoretical model including Orem's Self Care who is depressed about the need for self-care. Prevention of pressure sores by administering Aloe Vera Gel as part of an independent act of kindness based on scientific research evidence. The innovation project in the form of DVT risk assessment using the Autar Scale is a form of developing interventions to prevent the risk of DVT in patients with musculoskeletal cases. The aim of all activities carried out during the residency activities is to realize comprehensive group care based on the latest scientific evidence."

"Praktik Spesialis Keperawatan Medikal Bedah dengan kekhususan pada sistem muskuloskeletal ini bertujuan untuk mengaplikasikan peran perawat sebagai pemberi asuhan, pengelola, pendidik dan peneliti. Peran sebagai pemberi asuhan dilakukan dengan mengelola satu kasus utama; fraktur tertutup intertrokhanter femur dan 30 pasien yang mengalami masalah pada sistem muskuloskeletal dengan pendekatan teori adaptasi Roy. Peran perawat sebagai peneliti dilakukan dengan penerapan tindakan keperawatan yang berbasis bukti ilmiah (Evidence-Based Nursing Practice) yaitu dengan menerapakan foot massage untuk mengurangi nyeri dan kecemasan pada pasien paska pembedahan fraktur ekstremitas bawah. Sedangkan peran sebagai pengelola dilakukan dengan menyusun protokol pengembangan ERAS untuk pasien yang akan menjalani operasi hip. Seluruh rangkaian kegiatan tersebut bertujuan untuk mewujudkan asuhan keperawatan yang komprehensif dalam rangka meningkatkan kualitas pelayananan keperawatan.
Clinical Practice of Medical-Surgical Nursing Spesialialist in specialty of orthopaedic nursing aims to apply the role of nurse as a direct patient care provider, managers, educators and researchers in the clinical setting. Role as a care provider has done by one patient (major managed cases) with closed fracture intertrochanter femur and 30 patients with musculoskeletal problems using adaptation Roy Nursing theory approach. The role of nurses as a researchers has contructed by applying the nursing action based by evidence is foot massage to reduce pain and anxiety in patients after surgery for lower limb fracture. While the role as a nursing manager is done by comliling the ERAS development protocol for patients who undergo hip surgery. "ABSTRAK
Saat ini perkembangan ilmu dan pengetahuan keperawatan akan kesehatan beriringan dengan usaha untuk meningkatkan pelayanan bagi pasien. Tingginya prevalensi kasus kardiovaskuler menjadi perhatian sekaligus tantangan bagi tenaga kesehatan dalam memberikan pelayanan dan melakukan program pencegahan. Perawat diharapkan berkontribusi dalam upaya penanganan dengan menjalankan peran sebagai pemberi asuhan, pendidik, peneliti, dan inovator. Peran sebagai pemberi asuhan keperawatan dan pendidik diterapkan pada 31 pasien dengan berbagai kasus kardiovaskuler baik medikal maupun surgical. Pendekatan teori model yang digunakan adalah teori Self Care Dorothea Orem. Peran sebagai pendidik tidak hanya dijalankan untuk pasien dan keluarga, tetapi juga dijalankan dengan melakukan journal reading bagi rekan sejawat. Hasil analisis praktik menunjukkan bahwa teori Self Care dapat diterapkan pada asuhan keperawatan pada pasien dengan gangguan kardiovaskuler. Peran sebagai peneliti dilakukan pada saat penerapan tindakan keperawatan berbasis bukti ilmiah, dengan intervensi Progressive Muscle Relaxation PMR untuk mengatasi fatigue. Hasil penerapan didapatkan bahwa PMR dapat menurunkan level fatigue. Peran sebagai inovator dijalankan dengan menyusun Form Discharge Planning, dengan hasil evaluasi didapatkan bahwa form layak untuk digunakan. The high prevalence of patients with cardiovascular diseases makes a concern as well as a challenge for health provider to give treatments and prevention programs. Nurses are expected to contribute in health services by performing their roles as a care provider, educator, researcher, and innovator. The role as a care giver and educator was applied in 31 patients with various case in medical and surgical cases. The nursing theory model approach used is Dorothea Orem 39 s Self Care Theory. The role as a educator not only perfomed by educating patients and family, but also doing reading journal for other nurses. The analysis showed that Dorothea Orem 39 s Self Care Theory can be applied in nursing care to patients with cardiovascular diseases. The role as a researcher was excuted by applying evidence based nursing practice with the topic is Progressive Muscle Relaxation PMR to resolve fatigue. The analysis showed that PMR can decrease fatigue level. As a innovator, the practician trying to created Discharge Planning Form, with the evaluation showed that the form is acceptable. Peran sebagai edukator dan peneliti dilakukan oleh perawat spesialis terhadap pasien dan juga rekan sejawat di tatanan pelayanan rumah sakit. Penerapan EBN (Evidence Based Nursing) yaitu dengan mengaplikasikan intervensi diaphragmatic breathing exercise pada pasien pasca operasi Total Knee Replacement untuk mengurangi nyeri dan kecemasan. Proyek inovasi juga dilakukan sebagai perwujudan peran sebagai pembaharu atau inovator yaitu melalui pelaksanaan berupa Pengkajian Risiko DVT skala Autar dalam rangka meningkatkan kualitas asuhan keperawatan profesional di rumah sakit.
The clinical practice of medical-surgical nursing residency in osteomyelitis patients aims to develop and implement the role of specialist nurses as professional nursing care providers, educators, researchers, and reformers innovators. "Praktik residensi keperawatan medikal bedah sistem muskuloskeletal merupakan program lanjutan pendidikan magister keperawatan yang berfokus pada konsep dan prinsip dasar medikal bedah dengan menerapkan ilmu dan tehnologi keperawatan untuk memenuhi kebutuhan pasien dewasa yang mengalami perubahan fisik dengan atau tanpa adanya gangguan struktural pada sistem muskuloskeletal. Salah satu model keperawatan yang dapat diterapkan untuk memenuhi kebutuhan pasien gangguan muskuloskeletal adalah pendekatan Teori Keperawatan adaptasi Roy. Pendekatan ini merupakan model asuhan keperawatan yang dapat membantu pasien beradaptasi dengan berbagai permasalahan yang ditimbulkan akibat gangguan sistem muskuloskeletal. Kegiatan lain dari praktik residensi adalah melakukan penerapan evidence based nursing (EBN) melalui intervensi program ambulasi pada pasien post operasi fraktur ekstremitas bawah yang merupakan salah satu kegiatan intervensi untuk membantu pasien meningkatkan kemampuan gerak. Selain itu kegiatan melalui aplikasi peran perawat dalam penerapan program enhanced recovery after surgery (ERAS) pada pasien operasi panggul menjadi proyek inovasi yang dapat dilaksanakan di ruang perawatan untuk mempercepat proses pemulihan, mengurangi lama rawat (LOS), mencegah terjadinya komplikasi serta menekan morbiditas dan mortalitas paska operasi.
The practice of residency in medical surgical nursing of the musculosceletal system is an advanced program of nursing master education that focuses on the concepts and basic principles of medical surgery by applying nursing science and technology to meet the needs of adult patients who experience physical changes with or without structural disorders of the musculosceletal system. One of nursing model that can be applied to meet the need of patients with musculosceletal disorder is Roys Adaptation Theory approach. This approach is a model of nursing care that can help patient to adapt various problems caused by disorder of the musculosceletal system. Another activity of residency practice is implementing evidence based nursing (EBN) through ambulation program intervention in postoperative patients lower limb their mobility. In addition, activities through the application of the role of nurses in implementing of "enhanced recovery after surgery (ERAS)" program in hip surgery patients are innovative projects that can be implemented in the surgical ward to speed up the recovery process, reduce length of stay (LOS), prevent complication, also reduce post morbidity and mortality operation."

The practice of medical surgical nursing residency focuses on the application of science, research and nursing technology in meeting the basic needs of adult patients with cardiovascular disorders in a comprehensive manner. Cardiovascular medical surgical nursing clinical practice is carried out at Heart and Vascular Hospital Harapan Kita (RSJPHK) Jakarta, by providing nursing care using the Virginia Henderson theoretical approach in one managed case with a medical diagnosis of UAP TIMI 2/7 GRACE 139 CRUSADE 57, AV Block degree 1, LBBB ect NSTEMI With History of CABG x2 (SVG-OM, SVG-LAD) and mechanical AVRAV block grade I and 30 cases resumed. Implementation of Evidence Based Nursing (EBN) using a single session 20-minute mindful breathing intervention to reduce complaints of dyspnea in cute Decompensation Heart Failure (ADHF) patients. The residency also made innovations, namely the application of pressure injury assessment applications in perioperative patients using the Munro scale as an instrument for preventing pressure injury in patients planning cardiac surgery."

Karya Ilmiah Akhir ini adalah menganalisis kegiatan praktik Ners Spesialis Keperawatan Medikal Bedah berdasarkan standar kompetensi spesialis keperawatan medikal bedah kekhususan sistem muskuloskeletal. Ners Spesialis dalam melakukan asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy baik pada kasus kelolaan utama maupun resume. Penerapan Evidence Based Nursing (EBN) melalui intervensi latihan isometrik pada pasien pasca operasi fraktur ektremitas bawah, efektif menurunkan nyeri dan meningkatkan kekuatan otot. Proyek inovasi berupa pengembangan Enhanced Recovery after Surgery (ERAS) for orthopaedic untuk mempercepat proses pemulihan pasien operasi hip. Pemberian asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy dapat membantu pasien untuk beradaptasi terhadap penyakitnya. Intervensi latihan isometrik jika dikombinasikan dengan terapi non farmakologis yang lain seperti nafas dalam berpotensi bisa memberikan efek yang lebih baik terhadap penurunan nyeri.

The number of problems caused by musculoskeletal disorders requires the role of nurse
specialist musculoskeletal nursing to provide professional and advanced nursing care based on patient needs. The purpose of writing this final scientific paper is to analyze
the practical activities of nurse specialist medical surgical nursing based on the competency standards of medical surgical nursing specialists, specifically the musculoskeletal system. Nurse specialists in conducting nursing care in patients with
musculoskeletal disorders use the Roy Adaptation Model theory approach both in the
case of main and resume managed. The application of Evidence Based Nursing (EBN)
through isometric exercise intervention in patients with lower extremity fracture surgery, effectively reduces pain and increases muscle strength. The innovation project was in the form of developing Enhanced Recovery after Surgery (ERAS) for orthopedics to accelerate the recovery process for hip surgery patients. Giving nursing care to patients with musculoskeletal disorders using the Roy Adaptation Model theory approach can help patients adapt to the disease. An isometric exercise intervention if combined with other non-pharmacological therapies such as deep breathing has the potential to have a better effect on reducing pain."

Pengkajian kasus utama terhadap pasien karsinoma gaster dan 30 kasus resume pasien
bedah digestif, gastrointestinal serta hepatobilier berfokus pada respon adaptif dari
model adaptasi Roy. Pelaksanaan evidence based dilakukan untuk mengelola
kecemasan pasien sebelum menjalani operasi bedah digestif sebanyak 8 orang, dengan
intervensi mendengarkan Al-Qur`an Surah Al Waqi’ah dan terjemahannya selama 30
menit. Program inovasi keperawatan dilakukan, terhadap 7 orang pasien yang
direncanakan menjalani operasi kolostomi berupa optimalisasi pemberian edukasi
melalui media video. Kesimpulan: mendengarkan Al-Qur`an Surah Al Waqi’ah dan
terjemahannya dapat menurunkan tingkat kecemasan pasien sebelum operasi dan
edukasi melalui media video terhadap pasien yang akan menjalani operasi kolostomi
dapat meningkatkan pengetahuan pasien yang sangat memengaruhi terhadap keputusan
persetujuan pasien menjalani operasi.
The residency practice analysis is a series of reports on the activities of Surgical
Medical Nursing Specialists with the application of Roy's Adaptation Model in the
nursing care process of patients with digestive system disorders, the application of
evidence-based nursing practices and nursing innovations. Major case studies of gastric
carcinoma patients and 30 cases of digestive, gastrointestinal and hepatobiliary surgery
resumes focused on the adaptive response of Roy's adaptation model. Evidence based
implementation was carried out to manage the anxiety of patients before undergoing
digestive surgery as many as 8 people, with the intervention of listening to Al-Qur'an
Surah Al Waqi'ah and its translation for 30 minutes. Nursing innovation program
carried out, for 7 patients who were planned to undergo colostomy surgery in the form
of optimizing the provision of education through video media. Conclusion: listening to
Al-Qur'an Surah Al Waqi'ah and its translation can reduce the level of patient anxiety
before surgery and education through video media for patients who will undergo
colostomy surgery can increase patient knowledge which greatly influences the decision
of consent for patients undergoing surgery."

Activities carried out in undergoing residency practice Ners Medical Nursing Specialists Respiratory specialization surgery consists of managing lung cancer cases using Roy`s adaptation model, managing thirty case resumes, implementing evidence based nursing (EBN) Foot Reflexology effectiveness against anxiety levels and physiological changes in patients who Bronchoscopy will be carried out, as well as implementing nursing innovations using the Respiratory Distres Observation Scale (RDOS) to assess the mild severity of dyspnea in patients with respiratory disorders."
