"Praktek ners spesialis keperawatan medikal bedah peminatan muskuloskeletal bertujuan untuk mengaplikasikan peran perawat sebagai pemberi asuhan, pendidik, pengelola, peneliti dan agen pembaharu. Perawat spesialis sebagai pemberi asuhan dengan melakukan pengeloaan pasien yang menjadi kasus utama dan resume. Kasus utama diberikan pada pasien ankylosing hip post operasi THR (Total Hip Replacement) dengan penerapan teori model Virginia Henderson. Perawat spesialis sebagai peneliti dengan menerapkan Evidence-Based Nursing Practice dalam melakukan tindakan keperawatan. The practice of medical-surgical nursing specialists with musculoskeletal specialization aims to apply the role of nurses as caregivers, educators, managers, researchers, and agents of change. Specialist nurse as a caregiver by managing patients as the main case and resume. The main case was the application of Virginia Henderson's model theory to post THR (Total Hip Replacement) - ankylosing hip surgery patients. As a researcher, the nurse applied Evidence- Based Nursing Practice in implementing nursing interventions. The application of strength and mobilization training was safe for the patient and improved ROM (Range of Motion) and gait. In addition, as an agent of change, the nurse implemented audio-visual (web)-based education (EduHart), and the readiness to go back home increased significantly. This education method was not limited by time and place to be accessed as needed and help improve the personal status, knowledge, coping abilities, and support for hip arthroplasty patients. A series of activities carried out in residency practice aims to improve the quality of holistic nursing care services."

Peran sebagai pemberi asuhan keperawatan dilakukan dengan mengelola 30 pasien dengan gangguan pada sistem endokrin dan 1 pasien kelolaan utama dengan diabetes melitus Tipe 2 yang memiliki komplikasi selulitis, sepsis, AKI, hipoalbumin, hiperkoagulitas, anemia, hipertensi dan dislipidemia menggunakan pendekatan teori model adaptasi Calista Roy. Diabetes melitus (DM) merupakan penyakit kronik yang membutuhkan perawatan seumur hidup, sehingga keterlibatan pasien secara aktif dalam manajemen DM sangat diperlukan. Keyakinan (self efikasi) pasien terhadap manajemen DM mempengaruhi outcome (HbA1c) pasien. Peran sebagai peneliti dan pendidik dilakukan dengan menerapkan asuhan keperawatan berbasis bukti (evidence based practice) screening self efikasi dan edukasi pasien DM dengan Thai Diabetes Management Self Efficacy Scale (TDMSES). The practice of medical surgical nurses specializing in the endocrine system was carried out for approximately one year with the aim of implementing the role of nurses as care givers, educators, and innovators. The role as a care giver is given by managing 30

patients with disorders in endocrine system and 1 main patient with type 2 diabetes mellitus with complications of cellulitis, sepsis, AKI, hypoalbumin, hypercoagulability, anemia, hypertension and dyslipidemia using the Calista Roy adaptation theory approach. Diabetes mellitus (DM) is a chronic disease that requires a lifetime management, so active involvement of patient in management of DM is very important. Confidence (self- efficacy) of patients on DM management would affects patient outcomes (HbA1c). Roles as researchers and educators are carried out by implementing evidence-based practice in nursing by screening self-efficacy and educating patients with the Thai Diabetes Management Self Efficacy Scale (TDMSES). The role as innovator is carried out by developing the Hypoglycemia Nursing Early Warning Score System (Hy-news) as innovation project to screen the risk of severe hypoglycemia in type 2 DM patients."

"Gangguan pada muskuloskeletal menimbulkan dampak fisiologis, psikologi, sosial dan ekonomi bagi individu yang mengalaminya. Peran dan kontribusi tenaga kesehatan khususnya perawat spesialis medikal bedah peminatan muskuloskeletal sangat diperlukan dalam menangani permasalahan kesehatan ini. Pemberian asuhan keperawatan berbasis bukti penelitian mendukung layanan kesehatan yang berkualitas. Hal ini menjadi landasan dalam melaksanakan peran perawat spesialis sebagai pemberi asuhan, pendidik, peneliti dan inovator. Residensi perawat spesialis yang dilaksanakan selama dua semester memberikan pengalaman dan kesempatan untuk menerapkan model teori keperawatan Self Care Orem pada 30 kasus resume dan satu kasus utama yaitu klien dengan multipel trauma fraktur tertutup femur, fraktur terbuka olecranon dan distal ulna. Peran perawat peneliti dilakukan pada penerapan evidence base nursing berupa pengaturan posisi lateral pasca pembedahan tulang belakang untuk menurunkan intensitas nyeri dan meningkatkan mobilitas. Inovasi keperawatan yang dilakukan oleh residen berupa penerapan pengkajian risiko deep vein thrombosis pada klien dengan masalah muskuloskeletal ekstremitas bawah. Pengkajian ini bertujuan sebagai skrining pada klien yang berisiko terjadi deep vein thrombosis sehingga dapat diberikan tindakan pencegahan sebelumnya.

The role and contribution of health workers, especially medical surgical specialist nurses specializing in musculoskeletal care, is needed to deal with this health problem. The provision of evidence-based nursing care supports quality health care. This became the basis for the role of specialist nurses as caregivers, educators, researchers, and innovators. The specialist nurse residency, which was held for two semesters, provided experience and opportunity to apply Orem's self-care nursing theory model to 30 resume cases and one main case, namely a client with multiple traumatic closed fractures of the femur, open fractures of the olecranon, and distal ulna. The role of the research nurse was to apply evidence-based nursing in the form of setting the lateral position after spinal surgery to reduce pain intensity and increase mobility. Residents of the nursing facility implemented an innovative approach of assessing the risk of deep vein thrombosis in clients with lower extremity musculoskeletal issues. This study aimed to screen clients who are at risk for deep vein thrombosis so that preventive measures can be provided beforehand."

The clinical practice of medical-surgical nursing residency in osteomyelitis patients aims to develop and implement the role of specialist nurses as professional nursing care providers, educators, researchers, and reformers innovators. The role of nursing care provider is carried out using the Orem Self Care Theory approach in patients with infected union of left tibia post debridement and bone graft application from iliac wing and 30 other patients with various musculoskeletal system disorders. The role of educator and researcher is carried out by specialist nurses to patients and also colleagues in hospital service settings. The implementation of EBN (Evidence Based Nursing) is by applying diaphragmatic breathing exercise interventions in postoperative Total Knee Replacement patients to reduce pain and anxiety. Innovation projects are also carried out as a manifestation of the role as a reformer or innovator, namely through the implementation of the Autar scale DVT Risk Assessment in order to improve the quality of professional nursing care in the hospital."

"Karya Ilmiah ini merupakan hasil analisis pengalaman praktik residensi spesialis keperawatan medikal bedah kekhususan peminatan neurologi. Laporan ini bertujuan untuk memberikan gambaran pengalaman praktik residensi spesialis keperawatan medikal bedah dalam mengolola berbagai kasus dengan gangguan neurologis. Karya ilmiah ini terdiri asuhan keperawatan pada kasus kelolaan utama pada klien dengan tuberculosis of spine dengan menggunakan pendekatan model adaptasi Roy, penerapan Evidence Based Nursing (EBN) dengan intervensi mengunyah permen karet untuk pemulihan gastrointestinal pasca operasi tulang belakang, proyek inovasi nurse-led early mobilization, dan analisis pada 30 kasus resume pada klien dengan gangguan neurologis. Hasil analisis diagnosa pada 30 kasus resume ditemukam tiga diagnose keperawatan yang paling sering pada klien dengan gangguan neurologis yaitu penurunan kapasitas adaptif intrakranial, hambatan mobilitas fisik dan nyeri. Intervensi keperawatan yang paling sering dilakukan dalam menjalankan asuhan keperawatan diantaranya promosi perfusi serebral, pemantauan tekanan intrakranial, dan terapi fisik: kontrol otot.

This scientific work is the result of an analysis of the practical experience of a medical surgical nursing specialist residency with a specialization in neurology. This report aims to provide an overview of medical surgical nursing specialist residency experience in managing various cases with neurological disorders. This scientific work consists of nursing care in primary managed cases for clients with tuberculosis of the spine using the Roy Adaptation Model approach, application of Evidence Based Nursing (EBN) with chewing gum interventions for gastrointestinal recovery post operative spine surgery, nurse-led early mobilization innovation projects, and analysis on 30 case resumes on clients with neurological disorders. The results of the analysis of diagnoses in 30 resume cases found the three most common nursing diagnoses in clients with neurological disorders, namely decreased intracranial adaptive capacity, impaired physical mobility, and pain. The most frequently performed nursing interventions in carrying out nursing care include promoting cerebral perfusion, monitoring intracranial pressure, and physical therapy: muscle control.

Pencegahan terjadinya luka tekan dengan pemberian Aloe vera gel sebagai bagian dari tindakan mandiri keperawatan berdasarkan bukti penelitian ilmiah. Proyek inovasi berupa pengkajian resiko DVT dengan menggunakan Autar Scale adalah bentuk pengembangan dari intervensi keperawatan untuk pencegahan resiko terjadinya DVT pada pasien dengan kasus muskuloskletal. Tujuan seluruh kegiatan yang dilakukan selama kegiatan residensi adalah mewujudkan asuhan keperawatan yang komprehensif berdasarkan bukti ilmiah terkini.

"Ners Spesialis peminatan kardiovaskular didapatkan setelah melalui program praktik residensi keperawatan. Praktek residensi keperawatan terdiri dari 2 program yaitu praktek residen umum dan praktek residensi peminatan yaitu kardiovaskular. Kegiatan praktek residensi peminatan termasuk peran perawat sebagai pemberi asuhan keperawatan, peneliti, dan inovator. Peran pemberi asuhan keperawatan yaitu melakukan asuhan keperawatan pada 1 kasus utama dan 30 kasus resume pasien gangguan sistem kardiovaskular menggunakan model konservasi Levine. Peran peneliti adalah melakukan penerapan EBN yaitu akupresur terhadap kecemasan pasien preoperasi jantung. Hasil penerapan EBN; akupresur sebagai intervensi nonfarmakologi, nonivasif, mudah, sederhana dan aman dapat menurunkan kecemasan preoperasi jantung. Terakhir, peran inovator yaitu penggunaan skala Munro untuk mencegah kejadian perioperative related pressure injury pada pasien bedah jantung. Instrumen skala Munro sebagai pengkajian risiko pressure injury spesifik pada perioperatif, dilakukan 3 tahapan (preoperatif, intraoperatif, dan postoperatif), valid dan reliabel. Instrumen dapat digunakan sebagai bagian dari satuan intervensi pencegahan perioperative related pressure injury bersama dengan pengkajian kulit.

Nursing residensy practice consists of 2 programs, namely general residency practice and specialization residency practice, namely cardiovascular. Specialization residency practice activities include the roel of nurses as nursing care providers, researchers and innovators. The role of nursing care provder is to manage nursing care in 1 main case and 30 resume cases of pastients with cardiovascular system disorders using Levine conservation model. The role of researcher is to implement EBN, namely acupressure on the anxiety of preoperative cardiac patients.

Results of the EBN application; acupressure as a non-pharmacological intervention, non-invasive, simple, easy and safe can reduce anxiety of preoperative cardiac. Finally, the rola of an innovator is the use of Munro scale to prevent perioperative related pressure injury in cardiac surgery patients. Instrument of Munro scale as an assessment of the risk of specific in perioperative pressure injury, carried out 3 stages (preoperative, intraoperative, dan postoperative), valid and reliable. Instrument can be used as bundle of an intervention for the prevention of perioperative related pressure injury along with skin assessment."
