"Praktik kerja profesi apoteker di Apotek Kimia Farma 202 Kejayaan bertujuan mengkaji resep obat dari salah satu resep pasien diabetes melitus di Apotek Kimia Farma Kejayaan bulan Mei 2023 dari segi administrasi, farmasetik dan klinis. Serta memahami permasalahan yang ditemukan dari hasil pengkajian resep di Apotek Kimia Farma Kejayaan bulan Mei 2023. Metode pelaksanaan yang dilakukan dalam laporan ini adalah studi literatur. Resep yang diperoleh adalah dari resep bulan Mei tahun 2023 di Apotek Kimia Farma 202 Kejayaan. Resep dipilih karena kompleksitas yang ada di dalam resep dan dapat dijadikan bahan ajar tentang pengkajian resep. Pengkajian salah satu resep di Apotek Kimia Farma 202 Kejayaan dilihat dari aspek administratif, yaitu melihat kelengkapan data pasien, dokter, serta obat yang diberikan pada resep. Aspek farmasetik dilihat dengan menyesuaikan nama obat serta kekuatan obat dengan literature. Aspek klinis dilakukan untuk melihat indikasi, dosis, efek samping, kontra indikasi, dan potensi interaksi obat yang ada pada resep. Permasalahan yang sering ditemukan pada penulisan resep yaitu terkait aspek administrasi masih ditemukan beberapa poin yang jarang dicantumkan dalam penulisan resep. Ada pula permasalahan terkait potensi interaksi obat yang harus diwaspadai yang dapat terjadi kepada pasien.

The professional pharmacist work practice at Apotek Kimia Farma 202 Kejayaan aims to review a prescription from a diabetes mellitus patient at Apotek Kimia Farma Kejayaan in May 2023 in terms of administrative, pharmaceutical, and clinical aspects. Additionally, it seeks to understand the issues found from the prescription review at Apotek Kimia Farma Kejayaan in May 2023. The implementation method used in this report is literature study. The prescription reviewed is from May 2023 at Apotek Kimia Farma 202 Kejayaan. This prescription was chosen due to its complexity and can be used as a teaching material for prescription review. The review of the prescription at Apotek Kimia Farma Kejayaan is conducted from three aspects: Administrative aspect involves checking the completeness of the patient data, doctor information, and the medications prescribed. Pharmaceutical aspect involves verifying the drug names and their strengths against the literature. Clinical aspect involves assessing the indications, dosage, side effects, contraindications, and potential drug interactions in the prescription. Common issues found in prescription writing include some administrative details that are often omitted. Additionally, there are potential drug interaction problems that need to be closely monitored to prevent adverse effects on the patient."

"Salah satu standar pelayanan kefarmasian yang dilakukan di apotek adalah pengkajian resep. Pengkajian resep bertujuan untuk menganalisis adanya masalah yang terkait dengan peresepan obat. Masalah terkait obat yang mungkin terjadi akibat kesalahan penulisan resep diantaranya obat yang tidak tepat, dosis kurang atau berlebih, alergi obat, inkompatibilitas obat, interaksi obat, duplikasi pengobatan, serta penggunaan obat yang tidak tepat. Prevalensi diabetes mellitus di Indonesia mengalami peningkatan dari 1,5% pada tahun 2013 menjadi 2,0% pada tahun 2018 berdasarkan diagnosis dokter pada penduduk umur ≥ 15 tahun. Penanganan penyakit diabetes dengan cara pendekatan pasien memerlukan kolaborasi tenaga kesehatan, dalam hal ini dokter sebagai penentu diagnosa, apoteker mendampingi khususnya dalam terapi obat merupakan salah satu tugas profesi kefarmasian. Proses pengkajian dilakukan dengan observasi dan studi literatur. Pengkajian resep secara administratif yang dilakukan terhadap resep terdapat beberapa aspek administratif yang tidak lengkap, seperti jenis kelamin, berat badan pasien, dan nomor telepon dokter tidak tercantum pada resep. Hal ini dapat diatasi dengan konfirmasi oleh petugas farmasi atau apoteker. Berdasarkan pengkajian aspek klinis terdapat adanya polifarmasi dan interaksi obat. Hal tersebut dapat diatasi dengan memberikan edukasi atau pemberian informasi obat kepada pasien.

One of the standard pharmaceutical services carried out in pharmacies is prescription review. The prescription review aims to analyze the existence of problems related to drug prescribing. Drug-related problems that may occur due to prescription errors include incorrect medication, insufficient or excessive dosage, drug allergies, drug incompatibility, drug interactions, duplication of medication, and inappropriate drug use. The prevalence of diabetes mellitus in Indonesia has increased from 1.5% in 2013 to 2.0% in 2018 based on doctor's diagnosis in residents aged ≥ 15 years. Handling diabetes using a patient approach requires collaboration between health workers, in this case the doctor as the determiner of the diagnosis, the pharmacist assisting, especially in drug therapy, is one of the duties of the pharmaceutical profession. The administrative review of prescriptions carried out on prescriptions contained several administrative aspects that were incomplete, such as the patient's gender, weight, and the doctor's telephone number not being listed on the prescription. This can be overcome by confirmation by a pharmacy officer or pharmacist. Based on the clinical aspect assessment, there is polypharmacy and drug interactions. This can be overcome by providing education or providing drug information to patient."

"Diabetes melitus (DM) adalah penyakit metabolik kronis yang ditandai dengan peningkatan kadar glukosa darah. Pengelolaan penyakit diabetes ini dilaksanakan di setiap fasilitas pelayanan kesehatan salah satunya adalah apotek. Kegiatan pelayanan farmasi klinik di apotek salah satunya adalah pengkajian dan pelayanan resep. Tujuan dari tugas khusus ini adalah untuk menganalisis kajian resep pengobatan diabetes melitus di Apotek Kimia Farma No. 147 Duren Sawit periode Februari 2023. Analisis resep dilakukan dengan cara memilih dua resep terkait diabetes yang kemudian dilakukan pengkajian berdasarkan aspek administratif, farmasetika, dan klinis. Hasil menunjukkan pada kedua resep terdapat beberapa informasi administratif yang tidak tercantum pada resep. Kajian kesesuaian farmasetik pada kedua resep menunjukkan tidak terdapat masalah pada stabilitas dan kompatibilitas. Kajian pertimbangan klinis pada kedua menunjukkan ketepatan indikasi, dosis, cara dan lama penggunaan obat yang sudah sesuai. Kesimpulannya adalah secara umum kedua resep tersebut sudah memenuhi aspek kesesuaian farmasetik dan pertimbangan klinis, tetapi belum memenuhi aspek administratif.

Diabetes mellitus (DM) is a chronic metabolic disease characterized by increased blood glucose levels. Management of diabetes is carried out in every health care facility, one of which is a pharmacy. One of the activities of clinical pharmacy services in pharmacies is the assessment and service of prescriptions. The purpose of this special assignment was to analyze a review of diabetes mellitus medical prescriptions at Apotek Kimia Farma No. 147 Duren Sawit for the period of February 2023. Prescription analysis was done by selecting two diabetes-related prescriptions, which were then assessed based on administrative, pharmaceutical, and clinical aspects. The results showed that both prescriptions contained some administrative information that was not included in the prescriptions. Pharmaceutical suitability studies on both prescriptions showed no problems with stability and compatibility. The study of clinical considerations on both showed the appropriate indications, dosage, method, and duration of drug use. The conclusion is that, in general, the two recipes have fulfilled the aspects of pharmaceutical suitability and clinical considerations but not the administrative aspects."

"Pelayanan kefarmasian di apotek merupakan pelayanan yang bersentuhan langsung dengan masyarakat, sehingga dituntut untuk mampu menjamin ketersediaan obat yang aman, bermutu, dan berkhasiat. Diabetes mellitus (DM) merupakan gangguan metabolisme kronis yang ditandai dengan terjadinya hiperglikemia persisten. Terapi pasien DM tipe dua bertujuan untuk mempertahankan tingkat konsentrasi glukosa dalam darah dalam batas normal, dan untuk mencegah berkembangnya komplikasi jangka panjang dari kondisi diabetes. Penulisan Tugas Khusus ini dilakukan agar penulis lebih memahami secara mendalam terkait analisis dan pengkajian resep di apotek, khususnya resep pasien diabetes. Metode pelaksanaan analisis resep dilakukan dengan pendekatan pengkajian, pelayanan, dan screening resep terhadap resep pasien diabetes di KFA Matraman sesuai dengan yang tertera pada buku Petunjuk Teknis Standar Pelayanan Kefarmasian di Apotek. Kesimpulan yang didapat dari analisis resep pasien diabetes kali ini yaitu secara umum seringkali terdapat kombinasi pengunaan obat pada pasien (polifarmasi), sehingga potensi terjadi interaksi obat pada pasien dapat meningkat. Menyikapi hal ini, apoteker perlu memastikan setiap kombinasi terapi yang diterima pasien merupakan kombinasi terapi yang rasional, sehingga efektif untuk menunjang kesembuhan pasien.

Community pharmaceutical services in pharmacies are services that are in direct contact with the community, so they must be able to guarantee the availability of safe, quality, and efficacious drugs. Diabetes mellitus (DM) is a chronic metabolic disorder characterized by persistent hyperglycemia. Therapy for type 2 DM patients aims to maintain blood glucose concentration levels within normal limits, and to prevent the development of long-term complications from diabetes. This Special Assignment was written so that the author could get better understanding in the analysis and assessment of prescriptions in pharmacies, especially prescriptions for diabetes patients. The analysis of diabetes prescription in this assignment was carried out using the approach of assessment, service, and screening of prescriptions for diabetes patients at KFA Matraman in accordance with that stated in the Technical Instructions for Standard Pharmaceutical Services in Pharmacies. The conclusion obtained from the analysis of diabetes patient prescriptions this time is that in general there is often a combination of drug use in patients (polypharmacy), so that the potential for drug interactions in patients can increase. In response to this, pharmacists need to ensure that every combination of therapy received by patients is a rational combination of therapy, so that it could be effective in supporting patient recovery."

"Pelayanan resep merupakan salah satu kegiatan pelayanan kefarmasian yang berupa kegiatan menyiapkan permintaan tertulis (resep) dari dokter, dokter gigi dan dokter hewan kepada apoteker untuk menyediakan dan menyerahkan obat yang tepat kepada pasien dan sesuai dengan peraturan yang berlaku. Akan tetapi, terkadang masih terdapat beberapa resep yang kurang tepat (resep irasional) yang disebabkan oleh pemilihan obat, dosis, dan cara penggunaan yang kurang tepat. Terdapat beberapa dampak yang mungkin diakibatkan karena kesalahan peresepan diatas, mulai dari biaya pengobatan yang lebih mahal hingga kematian. Salah satu pelayanan resep yang sering diberikan kepada pasien di apotek Kimia Farma cabang Pos Pengumben merupakan pelayanan resep PRB (pasien rujuk balik). Pasien rujuk balik (PRB) merupakan pemberian obat-obatan untuk pasien penyakit kronis dengan kondisi stabil untuk kebutuhan 1 bulan dan dapat diulang sebanyak 3 kali. Salah satu jenis penyakit kronik yang dilayani pada program pasien rujuk balik adalah penyakit diabetes. Diabetes melitus merupakan penyakit gangguan metabolisme yang menyumbangkan angka kematian tertinggi ketiga di Indonesia. Menurut analisis yang dilakukan terhadap beberapa resep pasien program rujuk balik penderita diabetes, dapat disimpulkan bahwa bentuk ketidakrasionalan resep PRB pada pasien diabetes melitus di Apotek Kimia Farma Pos Pengumben berupa duplikasi obat, terjadinya interaksi obat, dan kurang spesifiknya cara pemberian obat pada resep.

The above prescribing errors could have a wide range of effects, from increased medical costs to patient death. one of the prescription services that is often carried out at apotek Kimia Farma cabang Pos Pengumben is the Pasien rujuk balik (PRB) prescription service. Pasien rujuk balik (PRB) is the administration of medications for a month to those with stable chronic diseases. One type of disease covered in the PRB program is diabetes. Diabetes mellitus is a metabolic disorder that is responsible for Indonesia's third-highest mortality rate. According to the analysis conducted on several PRB program prescriptions for diabetics, it can be concluded that the irrational forms of prescribing for PRB patients with diabetes mellitus at Apotek Kimia Farma Pos Pengumben are in the form of drug duplication, drug interactions, and a lack of detail in the directions for administering medications."

"Penggunaan obat yang rasional adalah faktor penting dalam keberhasilan pelayanan kefarmasian. Pengkajian resep obat asma di Apotek Kimia Farma 202 Kejayaan Depok dilakukan untuk menilai kesesuaian resep dengan standar yang berlaku. Penelitian ini bertujuan untuk mengkaji resep obat asma dari segi administrasi, farmasetik, dan klinis serta memahami permasalahan yang muncul dari pengkajian resep tersebut. Pengkajian dilakukan terhadap resep asma yang diterima di Apotek Kimia Farma 202 selama bulan Juli 2023. Data dikumpulkan melalui observasi langsung dan wawancara dengan apoteker. Analisis dilakukan berdasarkan aspek administrasi, farmasetik, dan klinis sesuai Peraturan Menteri Kesehatan No. 73 Tahun 2016. Hasil analisis menunjukkan bahwa sebagian besar resep sudah memenuhi standar administrasi, namun terdapat beberapa kekurangan dalam aspek farmasetik dan klinis, seperti ketidaktepatan dosis dan bentuk sediaan obat yang tidak tercantum dengan lengkap. Peningkatan kualitas pelayanan resep di Apotek Kimia Farma 202 dapat dicapai melalui peningkatan pengawasan dan edukasi kepada tenaga kesehatan mengenai penulisan resep yang lengkap dan jelas.

Rational drug use is a critical factor in the success of pharmaceutical services. The evaluation of asthma medication prescriptions at Apotek Kimia Farma 202 Kejayaan Depok was conducted to assess the conformity of prescriptions with applicable standards. This study aims to review asthma drug prescriptions in terms of administration, pharmaceutics, and clinical aspects, as well as to understand the issues arising from this evaluation. The review was carried out on asthma prescriptions received at Apotek Kimia Farma 202 during July 2023. Data were collected through direct observation and interviews with pharmacists. Analysis was conducted based on administrative, pharmaceutical, and clinical aspects in accordance with Minister of Health Regulation No. 73 of 2016. The results of the analysis indicated that most prescriptions met administrative standards; however, there were some deficiencies in pharmaceutical and clinical aspects, such as inaccurate dosages and incomplete drug formulations. Improving the quality of prescription services at Apotek Kimia Farma 202 can be achieved through enhanced supervision and education for healthcare professionals regarding the writing of complete and clear prescriptions."

"Standar yang digunakan dalam penelitian ini berdasarkan Peraturan Menteri Kesehatan No. 73 tahun 2016, dimana kegiatan pengkajian resep dimulai dari persyaratan administrasi, persyaratan farmasetika dan persyaratan klinis. Apotek Kimia Farma 202 Kejayaan adalah salah satu apotek yang melayani resep BPJS dan non BPJS, dengan jumlah resep masuk rata-rata 100 resep perhari. Banyaknya resep yang diterima memerlukan proses penanganan yang tepat dan cepat, termasuk resep-resep untuk terapi diabetes. Resep tersebut kemudian dianalisa kelengkapannya dan diidentifikasi, sehingga diharapkan dapat membantu meningkatkan kualitas pelayanan kepada pasien untuk mendapatkan outcome terapi yang optimal serta mendukung pelaksanaan keamanan pada pasien di Apotek Kimia Farma 202 Kejayaan.

The standards used in this study are based on the Regulation of the Minister of Health no. 73 of 2016, where prescription review activities start from administrative requirements, pharmaceutical requirements and clinical requirements. Kimia Farma 202 Kejayaan Pharmacy is a pharmacy that serves BPJS and non-BPJS prescriptions, with an average number of incoming prescriptions of 100 prescriptions per day. The large number of prescriptions received requires a precise and fast handling process, including prescriptions for diabetes therapy. The prescription is then analyzed for completeness and identified, so that it is hoped that it can help improve the quality of service to patients to obtain optimal therapeutic outcomes and support the implementation of patient safety at the Kimia Farma 202 Kejayaan Pharmacy."

"Diabetes Melitus (DM) merupakan penyakit tidak menular yang memerlukan pengobatan yang kompleks. Banyaknya risiko komplikasi pada pasien DM menyebabkan peningkatan jumlah obat-obat yang harus dikonsumsi oleh pasien yang mengarah pada polifarmasi. Pemberian obat polifarmasi memiliki risiko yang tinggi terhadap terjadinya Drug Related Problems (DRPs) yang dapat membahayakan pasien. Laporan ini bertujuan untuk mengkaji resep obat pada pasien DM di Apotek Kimia Farma Kejayaan untuk mengetahui ada tidaknya DRPs. Pengumpulan informasi dilakukan dengan studi literatur terhadap setiap obat yang diresepkan. Pada hasil pengkajian resep, diperoleh masih banyaknya aspek yang belum terpenuhi, yaitu aspek administratif, berat badan, usia, nomor telepon dan SIP dokter perlu dicantukan. Pada aspek farmasetik bentuk sediaan dan data stabilitas perlu ditanyakan kembali kepada dokter penulis resep. Sementara pada aspek klinis, perlu dicek kembali dosis obat (resep 1), penambahan aturan pakai dan risiko interaksi obat yang dapat mengarah pada efek samping yang serius terhadap pasien.

Diabetes Mellitus (DM) is a non-communicable disease that requires complex treatment. The many risks of complications in DM patients cause an increase in the number of drugs that must be consumed by patients which leads to polypharmacy. Administering polypharmacy drugs has a high risk of developing Drug Related Problems (DRPs) which can harm the patient. This report aimed to assest drug prescriptions in DM patients at Kimia Farma Kejayaan Pharmacy to determine whether DRPs were present. Information collection was carried out by studying the literature on each drug prescribed. Based on the results of the review of prescriptions, it was found that there were still many aspects that had not been fulfilled, namely administrative aspects, weight, age, telephone number and doctor's SIP that needed to be included. Regarding the pharmaceutical aspects of dosage forms and stability data, it is necessary to ask the prescribing doctor again. While on the clinical aspect, it is necessary to re-check the dosage of the drug (recipe 1), add the rules for use and the risk of drug interactions which can lead to serious side effects in patients."

"Penyakit Paru Obstruksi Kronik (PPOK) merupakan penyakit yang ditandai dengan kelainan pada alveolar yang disebabkan oleh paparan signifikan terhadap partikel atau gas berbahaya sehingga mengakibatkan penyumbatan pada paru-paru dan keterbatasan aliran udara. Dalam upaya pencegahan dan pengobatan PPOK, apoteker berperan peting dalam pelayanan farmasi klinik yang berkaitan dengan sediaan farmasi untuk meningkatkan kualitas hidup pasien. Pengkajian resep merupakan salah satu pelayanan farmasi klinik yang dapat dilakukan oleh apoteker untuk menganalisa masalah terkait obat serta mencegah terjadinya kesalahan pemberian obat. Penelitian ini bertujuan untuk mengkaji aspek administrasi, kesesuaian farmasetik, dan pertimbangan klinis, serta kerasionalan 5 resep pasien paru di Apotek Kimia Farma Raya Bogor periode April 2023. Penelitian ini dilakukan dengan pengumpulan dan pengkajian resep pasien paru pada bulan april 2023. Hasil penelitian menunjukkan kelima resep pasien paru di Apotek Kimia Farma Raya Bogor telah memenuhi aspek administratif, aspek kesesuaian farmasetik, aspek pertimbangan klinis, dan resep rasional.

Chronic Obstructive Pulmonary Disease (COPD) is a disease characterized by alveolar abnormalities caused by significant exposure to harmful particles or gases, resulting in blockage of the lungs and limited air flow. In efforts to prevent and treat COPD, pharmacists play an important role in clinical pharmacy services related to pharmaceutical preparations to improve the quality of life of patients. Prescription review is one of the clinical pharmacy services that can be carried out by pharmacists to analyze drug-related problems and prevent medication administration errors. This research aims to examine administrative aspects, pharmaceutical suitability and clinical considerations, as well as the rationale for 5 prescriptions for pulmonary patients at Kimia Farma Raya Bogor for the period April 2023. This research was conducted by collecting and reviewing pulmonary patient prescriptions in April 2023. The research results showed that the five prescriptions for pulmonary patients at Kimia Farma Raya Bogor Pharmacy had fulfilled administrative aspects, pharmaceutical suitability aspects, clinical consideration aspects, and rational prescriptions."

"Sebagai seorang apoteker, salah satu cara untuk memastikan pasien telah mendapatkan pengobatan yang rasional adalah melalui pengkajian resep, yang meliputi ketepatan penilaian kondisi pasien, tepat diagnosis, tepat indikasi, tepat jenis obat, tepat dosis, tepat cara dan lama pemberian, tepat informasi, dengan memperhatikan keterjangkauan harga, kepatuhan pasien, dan waspada efek samping. Pengkajian resep dilakukan berdasarkan aspek administratif, farmasetik dan klinis. Pengkajian resep dilakukan pada pasien yang menebus obat di Apotek Kimia Farma Dramaga 348 yang memiliki diagnosis diabetes tipe 2, hipertensi, dan diabetes tipe 2 yang disertai dengan hipertensi. Resep-resep yang akan dikaji umumnya berisi obat-obatan rutin pasien yang diberikan selama 1 bulan. Dari hasil kaji resep, Aspek administratif belum memenuhi persyaratan karena kurangnya informasi mengenai berat badan dan alamat pasien. Namun, kekurangan tersebut dapat diatasi dengan mengkonfirmasi kembali ke pasien dan dokter yang bersangkutan. Aspek farmasetik sudah memenuhi persyaratan, sementara aspek klinis belum memenuhi persyaratan karena ada beberapa obat yang saling berinteraksi dan merupakan polifarmasi. Namun, hal tersebut dapat diintervensi dengan mengedukasi pasien untuk menjeda konsumsi pemberian obat dan memotivasi pasien pentingnya untuk meminum obat secara rutin untuk mendapatkan hasil terapi yang optimal.

Prescription reviews are carried out based on administrative, pharmaceutical and clinical aspects. Prescription reviews were carried out on patients who redeemed medicines at Kimia Farma Dramaga 348 Pharmacy who had a diagnosis of type 2 diabetes, hypertension, and type 2 diabetes along with hypertension. The prescriptions that will be studied generally contain the patient's routine medicines that are given for 1 month. From the results of the prescription review, the administrative aspects did not meet the requirements due to lack of information regarding the patient's weight and address. However, this deficiency can be overcome by confirming it again with the patient and the doctor concerned. The pharmaceutical aspect meets the requirements, while the clinical aspect does not meet the requirements because there are several drugs that interact with each other and polypharmacy. However, this can be intervened by educating patients to separate the consume between interacted medications and motivating patients to the importance of taking medication regularly to get optimal therapeutic results."
