"Prakktik residensi merupakan bagiam dari pendidikan profesi yang bertujuan untuk memebentuk perawat spesialis. Beberapa kompensasi yang harus dicapai dalam praktik residensi keperawatan medikal ebdah adalah manajemen kasus   menggunakan teori model keperawatan, penerapan model keperawatan berbasis bukti, dan proyek inovasi. Model adaptasi Roy digunakan dalam asuhan keperawatan dengan pasien dengan gangguan system pernapasan. Pendekatan model adaptasi Roy bertujuan untuk mempertahankan integritas adaptasi manusia. Model adaptasi Roy berperan dalam membantu mengembangkan mekanisme koping yang adaptif terhadap stimulusdari lingkungan. Pada target kompetensi penerapan asuhan berbasis bukti dengan penerapan Aroma Hand Massage untuk mengurangi nyeri, tingkat kecemasan dan depresi pada pasien dengan kanker paru terminal.Hasil penerapan menunjukkan bahwa terapi aroma hand massage menurunkan skor nyeri dan depresi pada pasien dengan kanker paru terminal. Proyek inovasi adalah penerapan mobile phone application untuk pengkajian dan manajemen keperawatan mandiri pada pasien dengan kanker paru. Hasil penerapan aplikasi tersebut  menunjukkan bahwa kemanfaatan, mudah dalam penggunaan dan kepuasan responden dalam mengaplikasikannya.

The residency practice is a part of professional education to build specialist nurses. Some of the competencies must be achieved in residency practice of medical surgical are case management using nursing model theory, the application of  evidence-based nursing practice, and innovation projects. Roys adaptation mode is used in nursing care in patients with impaired respiratory function. Roys adaptation model aimed to develop adaptive coping mechanism from environment stimuli.

The implementation of evidence based nursing used Aroma Hand Massage was effective and useful significantly to reduce score pain and depression on patient with terminal lung cancer. Innovation project was the use of mobile phone application for assessment and management nursing intervention  on patient with lung cancer it was  useful, easy and fun to be applied."

"Pendekatan model adaptasi Roy menggunakan penanganan stimulus terhadap perubahan perilaku fisik, konsep diri, fungsi peran dan interdependensi membantu pasien berperilaku adaptif terhadap perubahan status kesehatan yang terjadi. Respon fisiologis yang khas dikaji pada pasien kelolaan yaitu berfokus pada fungsi respirasi. Pasien mengalami gangguan respirasi yang disebabkan oleh adanya infeksi kronik pada paru oleh Mycobacterium tuberculosis. Selain itu, pasien juga memiliki penyakit penyerta yaitu Diabetes Melitus tipe 2, dan komplikasi berupa piopneumotoraks. Berdasarkan hasil pengkajian menggunakan pendekatan model adaptasi Roy, didapatkan tiga masalah keperawatan diantaranya gangguan pertukaran gas, resiko ketidakseimbangan cairan dan elektrolit, dan resiko ketidakstabilan kadar glukosa darah. Selama 12 hari perawatan, pasien mengalami perkembangan perilaku inefektif menuju perilaku adaptif dari hari ke hari. Sehingga dapat disimpulkan bahwa model adaptasi Roy efektif diterapkan pada pasien dengan tuberkulosis karena dapat mengkaji pasien secara komprehensif hingga evaluasi perilaku adaptif dan inefektif selama perawatan di rumah sakit.

Roy's adaptation model approach uses stimulus handling to changes in physical behavior, self-concept, role function and interdependence to help patients behave adaptively to changes in health status. The typical physiological response was examined in managed patients, focusing on the function of respiration. Patients experienced respiratory problems caused by chronic infection of the lungs by Mycobacterium tuberculosis. In addition, patients also had comorbidities of type 2 diabetes mellitus, and the complications was pyopneumothorax. Based on the results of the study using Roy's adaptation model approach, there were four nursing problems including gas exchange disorder, risk of fluid and electrolyte imbalance, and risk of instability in blood glucose levels. During the 12 days of nursing care, patients showed improvement from ineffective behavior towards adaptive behavior day by day. It can be concluded that Roy's adaptation model is effectively applied to patients with tuberculosis because it can comprehensively assess patients and can evaluate adaptive and ineffective behavior during hospital care."

Pemberian asuhan keperawatan diterapkan kepada satu pasien kelolaan utama dengan kasus PPOK dan 30 kasus resume menggunakan pendekatan model adaptasi Roy. Penerapan tindakan keperawatan berbasis bukti ilmiah yang dilakukan yaitu pemberian posisi condong kedepan dan Latihan nafas pursed lips breathing pada pasien dyspnea dengan penyakit paru. Proyek inovasi yang diterapkan yaitu Pengembangan Self Manajemen Dengan Video Latihan Breathing Exercise : ACBT Dalam Meningkatkan Airway Clearance Pada Pasien Dengan Penyakit Paru Obstruksi Kronis (PPOK). Hasil analisis praktik menunjukkan bahwa model adaptasi Roy dapat digunakan sebagai pendekatan dalam asuhan keperawatan pasien dengan gangguan sistem pernapasan untuk mengembangkan mekanisme adaptif pasien. Pemberian posisi condong kedepan dan Latihan nafas pursed lips breathing dapat menurunkan sesak napas pada pasien dengan penyakit paru. Selain itu, Latihan Breathing Exercise : ACBT Dalam Meningkatkan Airway Clearance Pada Pasien mendapat evaluasi yang positif dan dapat diterapkan di tatanan klinis sebagai inovasi pengkajian dan manajemen keperawatan mandiri pada pasien PPOK selama di rumah.

Medical surgical nursing residency practice is part of professional education that aims to produce specialist nurses. Specialist nurses are expected to carry out their role in providing nursing services in cases with respiratory system disorders. The role is in line with the competencies to be achieved during residency practice in the form of providing nursing care, implementing evidence-based nursing practice and innovation projects. Nursing care was applied to one primarily managed patient with a case of COPD and 30 resume cases using Roy's adaptation model approach. The application of scientific evidence-based nursing actions carried out is the provision of a forward leaning position and pursed lips breathing exercises in dyspnea patients with lung disease. The innovation project applied is the Development of Self Management with Video Breathing Exercise: ACBT in Increasing Airway Clearance in Patients with Chronic Obstructive Pulmonary Disease (COPD). The results of the practice analysis show that Roy's adaptation model can be used as an approach in nursing care of patients with respiratory system disorders to develop patient adaptive mechanisms. Forward leaning and pursed lips breathing exercises can reduce shortness of breath in patients with lung disease. In addition, Breathing Exercise: ACBT in Increasing Airway Clearance in Patients received a positive evaluation and can be applied in clinical settings as an innovation in self-assessment and nursing management in COPD patients while at home."

"Penyakit Paru Obstruksi Kronis (PPOK) merupakan penyakit paru yang menetap yang dapat dicegah dan diobati, ditandai dengan adanya keterbatasan aliran udara yang menetap dan bersifat progresif dikarenakan abnormalitas saluran napas dan/atau alveolus yang biasanya disebabkan oleh pajanan gas atau partikel berbahaya. Laporan ini dimaksudkan untuk menganalisis pemberian Asuhan Keperawatan dengan penerapan Model Adaptasi Roy pada pasien kelolaan utama dan 30 pasien resume dengan menggunakan metode studi kasus, laporan tentang penerapan latihan pernapasan untuk mengurangi sesak dan meningkatkan toleransi aktivitas pada pasien PPOK dan penerapan RDOS untuk menilai tanda-tanda distres pernapasan. Hasil menunjukkan masalah keperawatan yang muncul adalah hambatan pertukaran gas, nutrisi kurang dari kebutuhan tubuh, ketiddakberdayaan dan ketidakefektifan manajemen kesehatan. Model Adaptasi Roy dapat diterapkan dalam asuhan keperawatan pada pasien PPOK untuk mampu beradaptasi terhadap penyakitnya.

Chronic Obstructive Pulmonary Disease (COPD) is a persistent lung disease that can be prevented and treated, characterized by limited persistent and progressive air flow due to airway abnormalities and / or alveoli which is usually caused by exposure to harmful gases or particles. This report aims to analyze the apllication of Roy Adaptation Model to primary care nursing and the 30 patient resumes using case study methods, the application of respiratory training to reduce dispnea and increase activity tolerance in COPD patients and the application of RDOS to assess the signs of respiratory distress. The results showed nursing problems arise were impaired gas exchange, imbalanced nutrition less than body requirements, powerlessness and ineffective health management. Roy  Adaptation Model can be applied in nursing care for patients with COPD  to adapted the ability of their the disease."

Peran selanjutnya sebagai innovator, proyek inovasi yang dilakukan berupa penggunaan mobile phone application untuk pengkajian dan manajemen keperawatan mandiri pada pasien kanker paru. Hasil penerapan menunjukkan bahwa mobile phone application bermanfaat bagi pasien dalam manajemen gejala secara mandiri.

Nursing Specialist Practice is an important part of professional education in order to apply the role of specialist nurses to improve service quality. Nursing specialist practice is carried out and developed based on science and technology. Competency that must be achieved during the residency practice of surgical medical nursing is to provide nursing care using the theory of nursing models, application of scientific evidence-based nursing practice, and do innovation projects. "Praktik Spesialis Keperawatan Medikal Bedah Kekhususan Respirasi di RSUP Perasahabatan Jakarta merupakan aplikasi peran dan fungsi sebagai ners spesialis dengan menerapkan Model Adaptasi Roy dalam memberikan asuhan keperawatan pada pasien dengan gangguan sistem respirasi baik pada pasien kelolaan utama maupun 30 pasien resume. Selain sebagai pemberi asuhan keperawatan peran perawat spesialis respirasi adalah menerapkan tidakan berbasis bukti ilmiah pada area respirasi yaitu dengan memberikan terapi suara berbasis alam dalam mengurangi kecemasan, agitasi, dan stress pada pasien terpasang ventilator mekanik. "Praktik spesialis keperawatan medikal bedah merupakan rangkaian dari pendidikan ilmukeperawatan yang dilaksanakan dalam tatanan pelayanan kesehatan. Selama menjalani proses residensi, residen berperan menjadi Clinical Care Manager yang bertugas sebagai pemberi asuhan keperawatan, sebagai peneliti, konsultan keperawatan bagi staf keperawatan, serta pendidik. Guna mencapai peran tersebut, residen menerapkan teori keperawatan model adaptasi Roy sebagai alat untuk mempermudah pemberian asuhan keperawatan dalam membantu individu, baik dalam kondisi sakit maupun pada proses penyembuhan dan peningkatan pemeliharaan kesehatan di setiap mode fisiologis, konsep diri, fungsi peran dan interdependen secara sistematis. Pada kasus kelolaan dengan tuberkuloma paru diperoleh hasil bahwasanya ada perbaikan kondisi setelah pemberian asuhan keperawatan selama 15 hari dan hasil pada 30 kasus resume pasien dengan penyakit sistem pernapasan terbanyak adalah tuberkulosis paru. Penerapan Evidence Based Nursing menggunakan terapi Eye Movement Training (EMT) didapatkan hasil bahwa kelompok intervensi EMT memiliki nilai perubahan kualitas tidur rerata lebih tinggi sebelum dan sesudah diberikan terapi dibandingkan kelompok kontrol. Pada proyek inovasi, dilakukan program peningkatan kemampuan perawat dalam melakukan perawatan trakeostomi pada pasien dewasa dengan 6 elemen bundle trakeostomi diperoleh hasil bahwa perawat ruangan merasa materi yang diberikan sangat bermanfaat dan mampu laksana sehingga tindak lanjutnya adalah terbentuknya booklet perawatan trakeostomi dan teraplikasi dalam Electronic Medical Record (EMR).

Medical-surgical nursing specialist practice is a series of nursing science education carried out in health care settings. During the residency process, the resident acts as a Clinical Care Manager in charge of providing nursing care, as a researcher, nursing consultant for nursing staff, and educator. In order to achieve this role, residents apply the Roy adaptation model nursing theory as a tool to facilitate the provision of nursing care in helping individuals, both in sickness and in the process of healing and improving health maintenance in each physiological mode, self-concept, role function and interdependence systematically. In the case of management with pulmonary tuberculosis, the results showed that there was an improvement in condition after providing nursing care for 15 days and the results in 30 cases of patient resumes with the most respiratory system diseases were pulmonary tuberculosis. The application of Evidence Based Nursing using Eye Movement Training (EMT) therapy found that the EMT intervention group had a higher mean sleep quality change value before and after therapy than the control group. In the innovation project, a programme to improve the ability of nurses to perform tracheostomy care in adult patients with 6 elements of the tracheostomy bundle was carried out. The results showed that the room nurses felt the material provided was very useful and applicable so that the follow-up was the formation of a tracheostomy care booklet and applied in the Electronic Medical Record (EMR)."

"Praktik spesialis Keperawatan Medikal Bedah peminatan sistem perkemihan ini bertujuan untuk melakukan asuhan keperawatan medikal bedah dengan pendekatan Model Adaptasi Roy pada pasien gangguan sistem perkemihan dan menerapkan peran sebagai care provider. Sebagai penyedia asuhan keperawatan dilakukan kepada 30 orang pasien dengan gangguan sistem perkemihan. Surgical-medical nursing residency practice on the specificity of urinary system is a process which must be taken to become a nursing specialist. During the residency practice, the student played several roles. As a care provider, the student delivered professional nursing actions with Roy adaptation model theory on the main case of chronic renal failure stage V on haemodialysis with subarachnoid haemorrhagic complication and thirty urinary cases. As a researcher, the student applied scientific evidence-based nursing actions with holy Qur'an to decrease depression score. Depression screening was conducted as the initial stage. And as an innovator, the student applied self-instructional module SIM to improve haemodialysis patients knowledge and obedience. The results of the residency practice were Roy Adaption model was effective to deliver nursing care on urinary system, holy Qur rsquo;an was successful to decrease depression score with Beck Depression inventory II used in the initial screening , and SIM was also effective to improve haemodialysis patients rsquo; knowledge and obedience."
