"Aspek kenyamanan merupakan fokus keperawatan namun aspek ini cenderung diabaikan terutama didalam melakukan pengkajian. Luka kronik yang dialami pasien menimbulkan ketidaknyamananyang dapat mempengaruhi kondisi psikologis, spiritual, sosial, dan kultural pasien. Penelitian ini bertujuan untuk menguji pemodelan teoritis kenyamanan pada pasien ulkus diabetikum. Penelitian ini dilakukan melalui 2 fase, fase I menghasilkan instrumen ketidaknyamanan luka kronik, yaitu Discomfort Evaluation of Wound Instrument (DEWI). Fase kedua dilakukan dengan menggunakan desain potong lintang pada 140 pasien ulkus diabetikum. Hasil pemodelan menunjukkan bahwa luka dapat menimbulkan ketidaknyamanan, luka juga dapat mempengaruhi status emosional psikologis. Ketidaknyamanan yang ditimbulkan dari adanya luka dapat mempengaruhi terjadinya gangguan status emosional psikologis. Dukungan keluarga dapat menurunkan gangguan status emosional psikologis. Sifat hubungan ketidaknyamanan dan status emosional psikologis bersifat timbal balik dimana karakteristik individu tidak mempengaruhi hubungan ketidaknyamanan dan status emosional psikologis. Penelitian ini merekomendasikan agar perawatan yang bersifat holistik diterapkan dalam merawat luka terutama luka kronik. Manajemen pengelolaan stress perlu diajarkan dan diterapkan pada pasien ulkus diabetikum dengan meningkatkan peran keluarga.

Comfort is focus of nursing that tend to have less attention when conducting assessment. Wound chronic occurs discomfort on patient that influences psychologicl, spiritual, social, and cultural aspect. This study was to examine theoretical model of comfort on diabeticum ulcer. This study consisted of two phases. Phase 1 developed an instrument of wound chronic discomfort, namely Discomfort Evaluation of Wound Instrument (DEWI). Phase 2 was conducted with cross sectional involving 140 diabetic patient with ulcer. The result of modelling shows that chronic wound can occur discomfort. Chronic wound also affects emotional psychological status. This discomfortable contributes to emotional psychological disturbance. The relationship discomfort and emotinal psychological status is recursive which individual characteristics does not this fundings recommend to provide holistic care in wound care specifically chronic wound. Stress management needs to teach and apply on diabeticum ulcer patients with improving of family role.

"ABSTRAK
Delegasi keperawatan merupakan salah satu aktifitas kepala ruangan melaksanakan fungsi manajemen dalam pemberian pelayanan keperawatan. Mekanisme delegasi keperawatan yang efektif menjamin kualitas pendelegasian keperawatan. Penelitian ini bertujuan memperoleh model delegasi keperawatan yang efektif dan meningkatkan kualitas pendelegasian keperawatan kepala ruangan. Penelitian ini dilakukan tiga tahap yaitu tahap 1 identifikasi masalah melalui penelitian deskriptif kualitatif fenomenologi, tahap 2 pengembangan model, dan tahap 3 penelitian kuasi eksperimen pre-posttest control group design. Metode sampling tahap 1 dengan purposive sampling dengan 4 partisipan. Tahap 3 sampel dipilih secara probability sampling yaitu simple random sampling sejumlah 70 responden 35 responden kelompok intervensi dan 35 kelompok kontrol . Hasil penelitian tahap 1 didapatkan 5 tema yaitu proses kegiatan delegasi keperawatan belum sesuai prinsip, prosedur delegasi keperawatan tidak standar, kegiatan delegasi tidak terdokumentasi, perbedaan pemahaman antar partisipan tentang prosedur delegasi, dan belum ada penerapan penghargaan atau sanksi untuk proses delegasi yang telah dilaksanakan. Tema tersebut dikembangkan pada tahap ke 2 menjadi model delegasi keperawatan yang diberi nama lsquo;Relactor rsquo;. ABSTRACT
Nursing delegation becomes one of management function of head nurse 39 s activity in providing nursing care. The mechanism of effective nursing delegation assures quality of nursing delegation. This research aimed to discover effective nursing delegation model and improve quality of nursing delegation of head nurse. This study undergone 3 steps include step 1 problem identification through descriptive qualitative phenomenology research, step 2 model development, and step 3 quasi experimental research pre post test control group design. Sampling method of step 1 with purposive sampling involved 4 participants. Seventy respondents 35 respondents at each group had been collected by using simple random sampling probability sampling during step 3. Research result of step 1 obtained 5 themes, those were inappropriate nursing delegation process, unstandardized nursing delegation procedure, undocumented delegation activity, different understanding among participants about delegation procedure, and the absence of reward and punishment for implemented delegation. Those themes were developed at step 2 that hereinafter called nursing delegation model lsquo Relactor rsquo . Step 3 was successfully proved that there was significant influence of nursing delegation model lsquo Relactor rsquo toward quality of delegation. The study recommended that lsquo Relactor rsquo model can be applied in nursing service and be executed continuous monitoring to measure success."

"ABSTRAK
Batik workers are a group at highest risk of acquiring occupational contact
dermatitis, especially hand dermatitis. Prevention of using personal protective
equipment (PPE) does not provide optimal protection therefore other interventions
are needed for secondary prevention. The development of topical agents that
improve skin barrier function is a promising approach for the management of
hand dermatitis. Topically applied lipids and humectants may interfere with skin
barrier function, and facilitate skin barrier repair. The aim of this study is to
evaluate the effectiveness of Cocos nucifera and glycerine for secondary
prevention of hand dermatitis among batik workers.
In a double-blind, cross-over randomized study, the effect of glycerine coconut
oil-containing cream vs. glycerine alone on skin barrier functions was determined
by assessment after repeated application of moisturizers over a 2-week period in
batik workers with occupational hand dermatitis. As much as 32 batik dyeing
and/or rinsing process workers were enrolled in this study for a minimum of 6
months with mild to moderate occupational hand dermatitis. Assessment of
transepidermal water loss (TEWL), skin capacitance and the Hand Eczema
Severity Index (HECSI) were made at baseline and day 14.
The research showed that the type of hand dermatitis among subjects that occurs
were 56 palmar hyperkeratosis, 15.6 interdigital eczema, 12.5 nummular
hand eczema, 9.4 pulpitis and 6.3 recurrent vesicular hand eczema. As much
as 96.97 of the subjcts have nail discoloration. 40.6 of the subjects always use
PPE were at risk of developing hand dermatitis.
After applying 14 days, both moisturizers showed clinical improvement by 20
decreased of HECSI and TEWL and 20 increased of skin capacitance. There
was no statistically significant treatment effect between both moisturizers. Skin
stripping result was analyzed using chromatography method showed penetration
at 3rd hour (uptake) and still detected at 12th hour (clearance).
The findings of the study support the skin health program for batik workers and
routine usage of moisturizers are recommended for a minimal of 12 hours per day
to prevent hand dermatitis."

"Ketepatan aktivasi kode trauma dan tindakan keperawatan emergensi yang terstruktur merupakan salah satu faktor dalam keberhasilan penanganan pasien politrauma. Penanganan pasien politrauma yang segera dan terstruktur dapat menurunkan risiko perburukan kondisi pasien, menurunkan angka kecacatan dan menyelamatkan nyawa pasien. Ketidaktepatan penapisan pasien politrauma di triage disebabkan karena belum adanya instrumen penapisan dengan indikator yang sensitif dan spesifik. Selain itu, kompleksitas kondisi pasien politrauma menuntut adanya penanganan yang cepat, tepat, komprehensif, dan terstruktur. Oleh karena itu, penelitian ini bertujuan untuk mengembangkan instrumen skrining politrauma di triage yang sensitif dan spesifik serta melakukan restrukturisasi intervensi keperawatan emergensi untuk pasien trauma yang disebut ELLASI. Penelitian ini menggunakan metode sequential exploratory mixed method yang meliputi 3 tahap. Tahap I: pengembangan instrumen Skrining Politrauma Universitas Indonesia – Cipto Mangunkusumo (SPIC) melalui literature review, studi kualitatif, diskusi pakar (pannel expert), dan studi kuantitatif (pembuatan model skoring). Tahap II: uji nilai diagnostik instrumen SPIC dalam menapis politrauma dengan uji formulasi model skoring. Tahap III: uji efektivitas kombinasi instrumen SPIC+ELLASI dibandingkan SPIC+non ELLASI menggunakan randomized control trial (RCT) single blind. Penelitian ini menghasilkan instrumen SPIC yang sensitif (91%) dan telah mendapatkan HKI. Kombinasi instrumen SPIC dan ELLASI terbukti lebih efektif dalam meningkatkan waktu respons (p = 0,000), mencegah kondisi perburukan pasien (skor EWS p = 0,000), dan menjaga status metabolik pasien (pH p = 0,04; HCO3 p = 0,03) dibandingkan dengan kombinasi instrumen SPIC dan non ELLASI. SPIC dapat digunakan sebagai instrumen penapisan pasien politrauma di triage. Kombinasi SPIC dan ELLASI dapat meningkatkan luaran pasien.

Code trauma activation and emergency nursing intervention are factors affecting the success of polytrauma patient management. These factors can help prevent deterioration and death. There is no instrument to screen polytrauma patients in triage. The complexity of polytrauma patients’ condition requires fast, correct, comprehensive and structured intervention. This study aims to develop a polytrauma screening instrument and standardized emergency nursing intervention called ELLASI. This study used a sequential exploratory mixed method, which consisted of 3 phases. Phase 1: To develop an instrument of Skrining Politrauma Universitas Indonesia – Cipto Mangunkusumo (SPIC) by using literature review, qualitative study, pannel expert, and quantitative. Phase 2: To test the diagnostic value of SPIC to screen polytrauma patients. Phase 3: To examine the effectiveness of SPIC and ELLASI in preventing deterioration, increasing response time, and maintaining the metabolic status of polytrauma patients using randomized control trials (RCT) single blind. This study produced SPIC with high sensitivity (91%). SPIC and ELLASI are effective in increasing response time (p = 0,000), preventing deterioration (EWS score p = 0,000), and maintaining metabolic status (pH p = 0,04; HCO3 p = 0,03) of polytrauma patients compare to SPIC and non ELLASI. SPIC can be used as a screening tool for polytrauma patients in Triage. SPIC and ELLASI can increase patients outcome."

"Komunikasi yang dilakukan para perawat belum dilakukan secara optimal sesuai standar yang benar. Banyak keluhan pasien terhadap komunikasi yang belum sesuai dengan budaya masyarakat. Hasil study phenomenologi menunjukkan, bahwa selama perawat melakukan intervensi, 1) klien tidak pernah mengetahui nama perawat yang merawatnya, 2) klien tidak pernah mendapat penjelasan tentang prosedur intervensi, 3) klien tidak pernah ditanya kesediaan waktu untuk pengkajian dan intervensi. Kompetensi komunikasi perawat masih rendah. Prosedur komunikasi belum dilakukan sesuai prosedur standar. Klien dan keluarga merasa tidak puas dengan komunikasi perawat dalam pelayanan keperawatan. Tujuan penelitian untuk menghasilkan Pola komunikasi perawat pelaksana dalam pelayanan keperawatan yang mencirikan perawat profesional yang sesuai dengan sosial budaya Priangan-Jawa Barat – Indonesia. Desain penelitian yang digunakan adalah action research dengan pendekatan studi etnografi. Partisipan perawat pelaksana yang bekerja di ruang rawat inap di Rumah Sakit Immanuel Bandung berjumlah 31 orang Partisipan di ambil secara purposive sampling. Pengumpulan data dilakukan selama satu tahun, dengan wawancara mendalam, observasi partisipan, catatan lapangan dan wawancara mendalam kepada subyek yang terkait dengan partisipan. Temuan penelitian; Perawat harus mengetahui kondisi klien dan mempersiapkan penampilan, perawat peduli terhadap kliennya, komunikasi mulai dilakukan secara efektif, Perawat berperilaku ramah, menampilkan karakteristik budaya Priangan, pengkajian sosial budaya klien lebih lengkap. Rekomendasi yang disampaikan hendaknya memperbaiki rumusan kurikulum, dengan mengadaptasi dan mengadopsi karakteristik budaya Priangan atau budaya setempat sesuai tempat institusi itu berada. Melakukan penelitian studi etnografi dengan latar belakang budaya klien pada etnik yang lainnya yang ada di Indonesia. Pelatihan komunikasi yang berbasis budaya Priangan bagi seluruh perawat pelaksana, agar komunikasi dalam pelayanan keperawatan dapat berkualitas dan memuaskan klien, keluarga, rekan kerja dan tim kesehatan lain. Menegakan kembali kompetensi budaya dan kompetensi komunikasi dalam ranah kompetensi perawat.

Phenomenology study result showed that nurses during intervention: 1) patients never know the name of the nurse who take care for them; 2) patients has never received an explanation about the procedure; 4) patient never asked for time consent for assessment and intervention. Nurse communication competency still considered low. Communication procedures have not been carried out according to standard procedures. Client and family are not satisfied with the communication of nurses during nursing care. The purpose of research to produce nurse practitioner communication patterns in nursing care that characterize professional nursing in accordance with the sociocultural Priangan West Java - Indonesia. The design research is action research with ethnographic study approach. Using purposive sampling, the participants were chosen for approximately 31 nurse practitioner who are working in the inpatient at Immanuel Hospital Bandung Participants. Data collection was carried out for one year, with in-depth interviews, participant observation, field notes and interview subjects related to the participants. The findings of the study: Nurses should know the condition of the client and prepare appearances; Nurses care for clients; Communication should be done effectively; Nurses behave friendly, show Priangan cultural characteristics: Comprehensive sociocultural assessment. The recommendations should improve formulation of the curriculum by adapting and adopting cultural characteristics Priangan or culturally appropriate where the institution is located. Conducting research using ethnographic study with others cultural background or ethnicity in Indonesia. Communication training based on Priangan culture for all nurse practitioner; thus, communication in nursing care will be qualified and satisfied client, family, co-workers, and other health professionals. Re-establish cultural competence and communication competence in the realm of competence of nurses."

"Sumber daya manusia keperawatan yang berkualitas merupakan kepribadian yang tidak cacat emosionalnya seperti kesalahan/kekurang telitian dalam pekerjaan, keterlambatan dalam menyelesaikan pekerjaan, semangat kerja yang buruk, bekerja secara asal-asalan dan kesal hati. Sebaliknya sumber daya manusia keperawatan yang berkualitas adalah perawat yang memiliki kegairahan dalam bekerja, kreatif, proaktif, mempunyai kehangatan dan mudah tersenyum, terkait dengan kompetensi perawat pelaksana didapatkan bahwa kompetensi inti yang dimiliki oleh perawat pelaksana adalah kepemimpinan, cara kerja, interpersonal/softskill, dan pengusaan lingkungan. Softskill sendiri dapat dikembangkan dan ditumbuhkan melalui berbagai cara, dan faktor yang dapat membentuk softskill diantaranya pelatihan, tantangan yang didapat, lingkungan, dan pendidikan.
Penelitian ini bertujuan mengetahui efektifitas model Kepemimpinan Transformasional Kepala Ruang berbasis softskill, mendapatkan modul dan panduan panduan model yang terdiri dari panduan, modul dan portopolio model Kepemimpinan Transformasional Kepala Ruang berbasis softskill Metode yang digunakan dalam penelitian ini adalah action research dengan desain riset pengembangan dengan studi eksperimen kuasi, dengan menggunakan penilaian times series. Penelitian terdiri dari 3 tahap yaitu tahap studi pendahuluan, tahap pengembangan model, dan tahap implementasi model, alat ukur yang digunakan yaitu Kuesioner Transformational Leadership Questionnaire (TLQ) dan Nurse Soft skill Questionnaire (NSQ), panduan model yang terdiri dari panduan, modul dan portopolio model Kepemimpinan Transformasional Kepala Ruang berbasis softskill, pada tahap implementasi dilakukan sosialisasi, pelatihan, pendampingan penerapan panduan model yang terdiri dari panduan, modul dan portopolio model Kepemimpinan Transformasional Kepala Ruang berbasis softskill. Populasi dalam penelitian ini terdiri dari 18 kepala ruang yang ada dimasingmasing Rumah Sakit sebagai kelompok kontrol dan kelompok perlakuan (total sampel), dan 47 perawat pelaksana yang diambil secara random sampling.
Hasil penelitian didapatkan panduan model yang telah divalidasi dan diuji keterterapannya oleh 2 orang pakar dan 2 orang praktisi, hasil implementasi model diperoleh adanya efektifitas model dalam meningkatkan softskill perawat pelaksana dalam hal kemampuan beradaptasi, berkomunikasi, bekerjasama tim, memecahkan masalah, percaya diri, disiplin dan teliti, didapatkan perbedaan kemampuan softskill pada bulan ke 1 dengan ke II, dan ke I dengan ke III setelah implementasi model. Model kepemimpinan transformasional kepala ruang berbasis soft skill sangat mungkin diterapkan ditatanan Unit perawatan di seluruh Rumah Sakit yang ada, untuk dapat menjawab tantangan dunia terhadap perbaikan sumber daya manusia melalui perbaikan kinerja dalam hal kemampuan interpersonal dan intrapersonal, kegiatan harian perawat kegiatan ini yang sering terbengkalai, portopolio lembar kerja yang sangat efektif dalam penerapan model.

This study aims to determine the effectiveness of transformational leadership model of Head Room-based soft skills, gain module guides and guide model consisting of guides, modules and portfolio models Transformational Leadership Head Room-based soft skills. The method used in this study is an action research with research design the development of quasi-experimental studies, using the assessment times series. The study consisted of three phases: a preliminary study, the model development stage, and the implementation phase models, The instruments used Transformational Leadership Questionnaire (TLQ) and Nurse Soft skills Questionnaire (NSQ), guide model consisted of guides, modules and portfolio Transformational Leadership Model Head Room-based soft skills, at the implementation stage by socialization, training, mentoring guide application model consisted of guides, modules and portfolio models Transformational Leadership Head Room-based soft skills. The population in this study consisted of 18 head rooms in each of the existing hospital as a control group and the experimental group (total sampling), and 47 nurses were taken by random sampling.
The results was obtained the model guide has been validated and tested by 2 experts and 2 practitioners, the results obtained by the model of implementation effectiveness of the model in improving the soft skills of nurses in terms of adaptability, communication, team work, problem solving, self-confidence, discipline and meticulous, There were significant differences at 1st months and 2nd months, and 1st months and 3rd months after the implementation of the model. Transformational leadership model of head room-based soft skills are very likely applicable in care units of hospital, to be able to meet the challenges of the world to the improvement of human resources through improved performance in terms of interpersonal and intrapersonal skills, daily activities of the nurse's activities are often neglected, portfolio worksheet very effective in the application of the model.

"Perilaku remaja yang berisiko terhadap IMS, HIV/AIDS merupakan salah satu masalah kesehatan yang membutuhkan layanan kesehatan yang tepat bagi remaja. Penelitian ini bertujuan untuk mengidentifikasi pengetahuan remaja tentang kesehatan reproduksi dan seksual, sikap, self-efficacy, life skills pencegahan perilaku berisiko dan menguji efektivitas model layanan kesehatan reproduksi (life skills) berbasis sekolah terhadap perilaku berisiko IMS, HIV/AIDS, berdasarkan kebutuhan dan permasalahan remaja melalui pelatihan keterampilan hidup (life skills) di sekolah untuk meningkatkan pengetahuan, sikap, self-efficacy, life skills dan perilaku pencegahan perilaku berisiko. Sejumlah 340 responden remaja ikut berpartisipasi pada penelitian tahap survey ini, kemudian diambil 40 responden terpilih untuk diberikan pelatihan layanan kesehatan reproduksi (life skills) berbasis sekolah terhadap perilaku berisiko IMS, HIV/AIDS selama 4 bulan. Pendidikan kesehatan reproduksi dasar diberikan pada 40 responden sebagai kontrol. Model layanan kesehatan reproduksi (life skills) berbasis sekolah terhadap perilaku berisiko IMS, HIV/AIDS ini merupakan bagian dari layanan keperawatan yang dapat diberikan melalui usaha kesehatan sekolah (UKS) dan merupakan aspek penting dari peran perawat dalam mendukung tujuan dari millennium development goals (MDGs) yaitu penanganan berbagai penyakit menular berbahaya diantaranya adalah HIV/AIDS.

Behaviors of adolescents which are risky for STIs and HIV/AIDS are one of health problems that require appropriate health services. This study aims to identify the knowledge of adolescents about reproductive and sexual health, attitudes, self- efficacy, and life skills to prevent risky behaviors and to test the effectiveness of school-based reproductive health services (life skills) for risky behaviors of STI and HIV/AIDS based on the needs and problems of the youth through life skill training. This training is aimed to improve the knowledge, attitudes, self-efficacy, life skills and behavioral prevention of risk behaviors. A number of 340 adolescent respondents participated in the research survey. As many as 40 respondents were selected to be trained in school-based reproductive health services (life skills) for risky behaviors of STI and HIV/AIDS for 4 months. Education about basic reproductive health was given to 40 respondents as the control group. The model of school-based reproductive health services (life skills) for risky behaviors of STI and HIV/AIDS is a part of nursing services that can be provided through the school health unit (UKS). It is also an important aspect of the nurse's role in supporting the objectives of the millennium development goals (MDGs) which is the handling of dangerous infectious diseases such as HIV/AIDS."
