"ABSTRACT
Purpose To evaluate the status of surgical training in Japan through a national level needs assessment. Methods A survey was sent to all 909 graduating residents (GRs) and their 611 program directors (PDs) for the year 2016. A working group of surgical educators from around the country was formed under the education committee of the Japan Surgical Society. The survey items were developed by consensus of this working group. The survey investigated the knowledge and problems of the current curriculum, and the status of the current residency training.
Results
The response rates were 56,3% of the GRs and 76,8% of the PDs. Among the participants, 47,6% of the GRs and 29,4% of the PDs believed that the residency curriculum did not match the clinical experience. Over 80% of the GRs and PDs agreed on the importance of training outside of the OR, whereas only 13% of the GRs had received such training regularly. Trainees also reported a lower satisfaction rate about the opportunity to train outside of the OR.
Conclusion
This national level needs assessment of surgical training in Japan identified several gaps in the curriculum. These results provide valuable data to assist the ongoing efforts for surgical residency curriculum improvement."

"Perawat sebagai pemberi asuhan mempunyai peran penting untuk dapat meningkatkan derajat
kesehatan perempuan. Perawat menjalankan peran dan fungsi sebagai pemberi pelayanan
keperawatan, konselor, advokat, educator, koordinator, pengelola, kolaborator, dan peneliti
dalam upaya meningkatkan kesehatan reproduksi perempuan, selama melaksanakan praktek baik
di rumah sakit maupun di komunitas. Salah satu masalah reproduksi pada perempuan adalah
molahidatidosa, teori yang diaplikasikan adalah fokus penerapan adaptasi Roy dan Caring
Swanson pada ibu dengan post kuretase mola hidatidosa. Penerapan teori ini terbukti dapat

"Latar Belakang: Pergeseran dominasi antara laki-laki dan perempuan pada dunia kedokteran terjadi dari waktu ke waktu di berbagai belahan dunia, termasuk di bidang bedah plastik di Indonesia. Profesi yang semula didominasi oleh laki-laki, saat ini didominasi perempuan. Pergeseran dominasi perempuan ini memungkinkan terjadinya masalah-masalah yang berpengaruh pada pendidikan dan pelayanan bedah plastik. Penelitian ini bertujuan untuk mengeksplorasi fenomena dominasi perempuan pada pendidikan spesialis di Indonesia. Metode: Penelitian ini bersifat kualitatif berupa studi fenomenologi. Penelitian dilakukan pada 3 program pendidikan dokter spesialis bedah plastik rekonstruksi dan estetik di Indonesia. Penelitian dimulai pada bulan Januari 2020. Populasi penelitian terdiri dari 4 kelompok, yaitu peserta didik, dosen, pengelola program studi, dan pengguna lulusan. Responden penelitian dipilih menggunakan metode maximum variation sampling. Setiap responden mendapatkan informed consent, seluruh informasi yang diberikan sifatnya rahasia dan tidak memengaruhi proses pendidikan responden. Metode pengumpulan data berupa studi dokumen, Focus Group Discussion (FGD), dan In-Depth Interview. Data penelitian yang diperoleh dari berbagai metode diatas, kemudian dianalisis dan diolah lebih dalam secara tematik. Hasil: Peneliti membagi tema berdasarkan garis waktu proses pendidikan, yaitu: prapendidikan, intra-pendidikan, dan pascapendidikan. Masing-masing proses memiliki tema yang saling memengaruhi proses pendidikan. Pada masa prapendidikan terdapat karakter personal yang dipengaruhi oleh persepsi positif maupun negatif dari masyarakat. Sedangkan iklim lingkungan kerja, dampak dominasi perempuan, dan dimensi budaya memengaruhi kelancaran intra-pendidikan. Pasca pendidikan dan memasuki dunia kerja, peserta didik menginginkan suatu kondisi lingkungan kerja yang ideal dan terdapat preferensi tempat bekerja tertentu untuk mencapai kondisi well-being. Kesimpulan: Dampak dominasi perempuan selama pendidikan hanya akan berpengaruh pada dinamika kehidupan antar peserta didik dan antara peserta didik dengan dosen sebagai mentor. Namun dominasi ini tidak akan memengaruhi kualitas pendidikan dan beban kerja yang diberikan. Pada penelitian ini juga didapatkan fenomena kesenjangan kepemimpinan tidak terjadi selama pendidikan tetapi terjadi pada pascapendidikan. Namun kesenjangan kepemimpinan bukanlah akibat tekanan dalam komunitas, melainkan kecenderungan dari pribadi perempuan pada umumnya di kelompok masyarakat feminim.

Introduction: Shifting in gender dominance between men and women in the medical field has occurred from time to time globally, including in Indonesia’s plastic surgery. The profession, which was initially dominated by men, is currently dominated by women. This shift in female dominance might allow problems that affect the education and clinical settings of plastic surgery. This study aims to explore the phenomenon of women's dominance in medical residency education in Indonesia. Method: This study is a qualitative study of phenomenology. It was conducted on 3 medical residency programs specializing in reconstructive and aesthetic plastic surgery in Indonesia. The study began in January 2020. The research population consisted of 4 groups, namely students, lecturers, study program managers, and graduate users. Research subjects were selected using the maximum variation sampling method. Each respondent was provided with informed consent, all information given was confidential and did not affect the educational process of the respondent. Data collection methods include document study, Focus Group Discussion (FGD), and In-Depth Interview. Research data obtained from various methods above was analyzed and processed thematically. Results: The themes were categorized based on the educational process timeline, namely: pre-education, intra-education, and post-education. Each timeline had several themes which mutually influenced the educational process. During pre-education there were personal characters which were affected by positive and negative perceptions from society. Whereas the work environment atmosphere, the impact of women's dominance, and the cultural dimension affected the intra-educational process. After graduating from residency program and entering the career life, students expected an ideal working environment and had certain workplace preferences to achieve their well-being. Conclusion: The impact of women's dominance during education affected the daily dynamics among students and their interaction with lecturers as mentors. However, this dominance did not affect the quality of education and workload. We also found that the phenomenon of leadership disparity did not occur during education but occurred in post-education setting. This leadership disparity was not resulted by pressure in the community, but due to the tendency of the women’s personality in general among the feminine community."

"Pelayanan kesehatan yang bermutu sudah merupakan kebutuhan bagi masyarakat pada saat ini, rumah sakit sebagai bagian dari mata rantai pelayanan kesehatan dituntut untuk terus mengembangkan diri. Salah satu cermin pelayanan rumah sakit adalah pelayanan keperawatan, karena berhubungan langsung dengan pasien. Untuk itulah sebagai tenaga pelaksana terbanyak di rumah sakit kiranya perawat perlu mendapat perhatian mengenai faktor-faktor apa yang dapat meningkatkan motivasi kerja mereka sehingga dapat melaksanakan tugasnya seperti yang diharapkan.
Saat ini hampir setengah jumlah perawat di Rumah Sakit Umum Daerah Kabupaten Cianjur adalah perawat dengan status honorer rumah sakit, khususnya di Instalasi Rawat inap ada sebanyak 53,7% perawat honorer yang penggajiannya dianggarkan dari dana operasional rumah sakit, sehingga jumlah gajinyapun belum dapat memadai.
Pihak manajemen berusaha untuk dapat menambah kesejahteraan pegawainya termasuk perawat, baik yang berstatus Pegawai Negeri Sipil maupun honorer rumah sakit, salah satunya dengan pemberian uang Insentif yang dimaksudkan untuk dapat meningkatkan motivasi kerja mereka, adapun sumber biayanya didapat dari penghasilan rumah sakit sendiri dengan pengaturan pembagiannya diatur oleh Surat Keputusan Direktur.
Dilakukan penelitian mengenai adakah hubungan pemberian insentif terhadap motivasi kerja perawat di Instalasi Rawat Inap Rumah Sakit Umum Daerah Kabupaten Cianjur, penelitian ini merupakan penelitian deskriptif yang dilanjutkan dengan analisis hubungan antar variabel, dilakukan secara cross sectional, dengan pengumpulan data melalui kuesioner terhadap 96 orang responden yang terdiri dari perawat di Instalasi Rawat Inap Rumah Sakit Umum Daerah Kabupaten Cianjur. Analisis data dilakukan dengan analisis univariat dan multivariat.
Kesimpulan dari hasil penelitian, ternyata terdapat hubungan yang bermakna antara pemberian insentif dengan motivasi kerja perawat di instalasi rawat inap Rumah Sakit Umum Daerah Kabupaten Cianjur pada tahun 2004, sementara itu pada beberapa variabel imbalan non finansial justru didapatkan hasil yang sangat bermakna hubungannya dengan motivasi kerja.

The Relationships between the Incentive Program and the Work Motivations of Nurses in the Inpatient Facility of Cianjur Residency General Hospital Year 2004A good healthcare system undoubtedly has become a significant factor to create better societies. Consequently, a hospital as a part of the system is demanded to improve its quality of services to the community surroundings. One of the services that has direct contact to patients is nursing facility. Since nurses play a key role in every hospital operations, it is extremely important for the management to comprehend factors that are able to boost nurses morale and motivations that in turn will create a high performance organization in a hospital.
Nowadays, it is known that almost a half of the total numbers of nurse in Cianjur Residency General Hospital is in the status of honorary employees. Even in its Inpatient Facilities, 53.7% of the honorary employees are paid from the hospital's operational expenses. As a result, their wages are far below the standard.
Considering there is a direct relationship between reward system and employees' motivations, the management is committed to raise the employees' welfares (including both -the permanent and honorary nurses) by implementing an incentive program. Such a program allows the organization to give the eligible employees a certain amount of money as an incentive to increase the employees' performance in conducting their duties. The source of money is taken from the hospital's earnings which is based on and arranged in a decree released by the director.
In connection with the aforementioned matter, this paper analyzed the relationships between the ongoing incentive program and the nurses' motivations and work performances in the inpatient facilities of Cianjur Residency General Hospital. This paper includes descriptive research and followed by inter-variables analysis. The inter-variables analysis was conducted in cross sectional method; undertaking questioners and collecting data from 96 respondents. The respondents are nurses who work at the inpatient facility in Cianjur Residency General Hospital. Data analysis was calculated by using univariate and multivariate analyses.
Interestingly, based on the study of this paper, it is concluded that there is significant relationship between incentives and nurses' work motivations in the Inpatient Facility of Cianjur Residency General Hospital throughout year 2004. Meanwhile, it is known that several variables of non-financial rewards have made significant relationships to the motivation of the workers.

"Prevalensi pasien dengan gangguan sistem kardiovaskuler semakin meningkat. Perawat diharapkan memiliki kontribusi dalam penanganan pasien dengan gangguan sistem kardiovaskuler dengan menjalankan peran sebagai pemberi asuhan, pendidik, peneliti dan inovator. Praktik residensi spesialis keperawatan medikal bedah yang telah dilaksanakan selama 1 tahun (2 semester) bertujuan untuk melakukan penerapan dan pendalaman pada peran-peran tersebut dengan pendekatan Model Adaptasi Roy (MAR).
Peran sebagai pemberi asuhan diterapkan pada 30 pasien dengan berbagai gangguan kardiovaskuler dan satu pasien kasus kelolaan utama dengan STEMI. Peran sebagai pendidik dijalankan dengan pembimbingan perawat sejawat dan mahasiswa keperawatan.
Peran sebagai peneliti dijalankan dengan menerapkan tindakan keperawatan berbasis pembuktian ilmiah (evidence-based nursing) yaitu tindakan relaxation response untuk menurunkan tingkat stres pada pasien CAD. Peran sebagai inovator dijalankan dengan menerapkan format pemantauan komplikasi dan algoritma pada pasien post percutaneus coronary intervention (PCI).
Hasil analisis praktik menunjukkan bahwa MAR efektif digunakan sebagai pendekatan dalam asuhan keperawatan pada pasien dengan gangguan kardiovaskular untuk meningkatkan tingkat adaptasi dan tindakan relaxation response efektif diterapkan untuk menurunkan skor stres dan marker fisiologis stres pada pasien CAD. Selain itu, format pemantauan dan algoritma dapat diterapkan untuk antisipasi dan penanganan komplikasi setelah menjalani PCI.

The prevalence of patients with cardiovascular disorders are increasing. Nurses are expected to contribute on managament of patients with cardiovascular disorders by running their roles as a care giver, educator, researcher and innovator. Residency clinical practice of medical-surgical nursing specialists had been carried out for 1 year (2 semesters) and aimed to implement and explore these roles with Roy?s Adaptation Model (RAM) approach
The role as a care giver was applied to 30 patients with various cardiovascular disorders and a patient with STEMI as the primary case. The role as an educator was performed by educating patients, nurses and nursing students.
The role as a researcher was carried out by applying the evidence-based nursing, relaxation response intervention to reduce stress levels in patients with CAD. The role as an innovator was executed by applying the complications monitoring format and algorithms for patients with post percutaneous coronary intervention (PCI).
The analysis showed that the MAR can be apllied effectively in nursing care for patients with cardiovascular disorders to improve adaptation level and the relaxation response intervention can be applied effectively to reduce stress scores and physiological markers of stress in patients with CAD. In addition, the post PCI monitoring format and algorithm can be applied effectively to anticipate and manage the complications after PCI.

"ABSTRAK
Praktik residensi keperawatan medikal bedah dengan bidang peminatan sistem
kardiovaskuler telah memfasilitasi mahasiswa residensi untuk menjalankan
perannya sebagai seorang perawat spesialis. Peran yang dijalankan pada pasien
dengan gangguan sistem kardiovaskuler adalah peran sebagai advokat, inovator,
pendidik, peneliti dan pemberi layanan asuhan keperawatan. Gangguan
kardiovaskuler merupakan salah satu penyakit kronis yang tidak menular yang
memerlukan kemampuan pasien dalam merawat dirinya. Untuk itu pilihan
penggunaan Teori self care Orem pada asuhan keperawatan kasus utama pasien
CAD 3 VD yang dilakukan Coronary Artery ByPass Grafting (CABG) dan 30
kasus kardiovaskuler lainnya merupakan pilihan yang tepat dengan tujuan pasien
mampu merawat dirinya untuk meningkatkan kesehatannya. Asuhan keperawatan
yang diberikan mempertimbangkan tingkat ketergantungan pasien dan berbasis
bukti (evidence-based nursing practice). Untuk mengatasi depresi pada pasien,
residen menggunakan intervensi spiritual dengan mendengarkan Al Quran surat ar
rohman, dan terbukti menurunkan skala depresi walaupun secara klinis tidak terlihat
jelas. Untuk peran sebagai inovator, residen melakukan penapisan pasien ends of
life dengan instrumen cristal dan hasil evaluasi menunjukkan instrumen ini layak
digunakan khususnya pada pasien di atas 65 tahun.

ABSTRACT
The practice of residency in medical surgical nursing with the field of specialization
in the cardiovascular system has facilitated residency students to carry out their
role as a specialist nurse. The role played in patients with cardiovascular system
disorders is the role of advocates, innovators, educators, researchers and nursing
care providers. Cardiovascular disorders are one of the non-communicable chronic
diseases that require the patient's ability to care for him. For this reason, the choice
of use for Orem's self-care theory in the main case nursing care of CAD 3 VD
patients performed by Coronary Artery ByPass Grafting (CABG) and 30 other
cardiovascular cases is the right choice with the aim of patients being able to treat
themselves to improve their health. Nursing care provided considers the level of
patient dependence and evidence-based nursing (evidence-based nursing practice).
To overcome with depression in patients, residents use spiritual intervention by
listening to the Al-Quran Surat Ar Rohman, and it has been shown to reduce the
scale of depression even though it is not clinically obvious. For the role of an
innovator, residents did screening end of life patients with cristal tools and,
evaluation results indicate the form is feasible to use especially in patients over 65
years."

Latihan napas yang diberikan kepada pasien salah satunya dengan mindful breating, Latihan napas ini dilakukan pasien dengan bimbingan perawat dan focus terhadap pernapasannya sehingga menimbulkan perasaan tenang dan dampak dapat mengriangi sesak napas yang dirasakan. Inovasi keperawatan dengan mengembangkan video edukasi monitoring dan perawatan di rumah pasien terpasang WSD sebagai media self-management bagi perawat ruang rawat dan pasien yang terpasang WSD.

One of the breathing exercises given to patients is mindful breathing. This breathing exercise is carried out by the patient under the guidance of a nurse and focuses on his breathing so that it creates a feeling of calm and the impact can relieve the shortness of breath that is felt. Nursing innovation by developing educational videos for monitoring and caring for patients with WSD installed as a self-management medium for ward nurses and patients with WSD installed."

Residen menemukan berbagai macam penyakit kardiovaskuler selama praktik residensi dalam waktu satu tahun di Rumah Sakit Jantung dan Pembuluh Darah Harapan Kita antara lain Acute Coronary Syndrome (ACS) dengan STEMI, NSTEMI dan UAP, aritmia, kelainan katup, kelainan septal jantung, myxoma, aneurisma aorta dan diseksi aorta. Selama praktik residensi, Residen sebagai perawat spesialis telah menjalankan peran sebagai care provider yang diterapkan pada satu pasien kasus kelolaan utama yakni NSTEMI post operasi Coronary Artery Bypass Grafting dan 30 pasien kasus resume dengan pendekatan model adaptasi Roy dalam memberikan asuhan keperawatannya. Peran sebagai researcher dengan menerapkan EBNP menggunakan soft icepack gel dalam menurunkan nyeri dan cemas pada 12 pasien paska pembedahan jantung di ruang Intermediate Ward Bedah yang akan dilakukan pelepasan selang drain. Peran sebagai innovator telah dilakukan di ruangan ICVCU selama 5 hari dimana residen telah membuat format dokumentasi handover perawat antarshift yang dinilai efektif dan efisien dalam penerapannya.

The Cardiovascular disease is a health problems faced in various countries. Resident has found a wide range of cardiovascular disease during residency practice within a year at the Harapan Kita hospital cardiovascular including Acute Coronary Syndrome (ACS) patients with STEMI, NSTEMI and UAP, arrhythmias, valve abnormalities, abnormal septal heart, myxoma, aortic anerysme and aortic dissection. During residency practice, resident as a care provider has been implemented to one primary case patients with NSTEMI post operative coronary artery bypass grafting and 30 patients cases of resume by appliying the Roy’adaptation model in providing nursing care. Role as a researcher by applying EBNP use a soft icepack gel to reducing pain and anxiety in 12 patients after cardiac surgery at intermediate ward surgery. The role as an innovator was completed at Intermediate Cardiovascular Care Unit for 5 days, resident made a documentation format nurse handover among shift are considered effective and efficient in its application"
