Abstrak :
Salah satu reaksi inflamasi yang ditimbulkan Mycobacterium tuberculosis yang masuk ke dalam alveoli adalah sekresi lendir yang berlebihan pada saluran napas. Penanganan dalam menurunkan produksi sputum pada pasien TBC dapat diberikan secara non farmakologi sebagai terapi pelengkap atau disebut juga terapi komplementer yaitu penggunaan minyak essensial untuk aromaterapi adalah Eucalyptus (minyak kayu putih). Tujuan penelitian ini untuk mengetahui efek aromaterapi Eucalyptus dalam menurunkan produksi sputum pada pasien paru. Penelitian ini menggunakan pendekatan quasi eksperimen dengan desain one group pretest-posttest design dengan jumlah 20 sampel dengan teknik non probability sampling dengan metode consecutive sampling. Analisis statistik menggunakan uji t-dependen yang bertujuan mengetahui adanya pengaruh intervensi aromaterapi Eucalyptus sebelum dan sesudah dilakukan intervensi untuk menurunkan produksi sputum pada pasien tuberkulosis. Hasil penelitian menunjukkan bahwa perbedaan signifikan jumlah produksi sputum antara sebelum intervensi, sesudah intervensi hari pertama, kedua dan ketiga dengan nilai p value 0,000. Praktik keperawatan residensi yang dilakukan di Rumah Sakit Umum Pusat Persahabatan dengan kekhususan respirasi diharapkan dapat mengatasi masalah pernapasan. Selama proses residensi menerapkan intervensi berdasarkan evidence based nursing, mampu menjadi seorang Clinical Care Manager (CCM) sebagai konsultan staf keperawatan, dan memberikan terapi keperawatan yang profesional. Dalam memberikan asuhan keperawatan pada kasus kelolaan pasien dengan Bronkiektasis dan 30 kasus resume menggunakan teori Henderson 14 kebutuhan dasar manusia yang menunjukkan peran perawat sebagai pemberi asuhan keperawatan dalam membantu individu baik dalam keadaan sehat maupun sakit, melalui usahanya melakukan berbagai aktivitas guna mendukung kesehatan dan penyembuhan individu atau proses meninggal dengan damai. Masalah keperawatan yang banyak muncul yaitu tentang pemenuhan kebutuhan bernapas normal. Penerapan EBN cold application dan latihan napas dan batuk untuk menurunkan tingkat nyeri pada pasien terpasang chest tube. Pelaksanaan proyek inovasi pengembangkan video edukasi monitoring dan perawatan di rumah pasien terpasang WSD sebagai media self-management bagi perawat ruang rawat dan pasien yang terpasang WSD. Hasil analisis praktik residensi keperawatan didapatkan bahwa asuhan keperawatan menggunakan pendekatan teori Henderson bertujuan untuk membantu sesegera mungkin kemandirian pasien. Kanker merupakan salah satu penyakit utama penyebab kematian di dunia. Salah satu jenis kanker yang sering dijumpai dari seluruh jenis tumor tulang adalah soteosarkoma. Tujuan penulisan Karya Ilmiah Akhir KIA ini adalah mampu menampilkan peran ners spesialis sebagai pemberi asuhan keperawatan lanjut, pembaharu dan peneliti Metodologi dan pendekatan yang digunakan adalah dengan menggunakan studi literature. Karya Ilmiah Akhir berisi tentang : penerapan model teori Virginia Henderson pada pasien dengan osteosarkoma, dan penerapan Evidance Based Nursing yang merupakan bagian dari proyek inovasi yaitu pemberian intervensi cryoterapi oral pada pasien kanker yang menjalani kemoterapi. Hasil : Penerapan teori model keperawatan Virginia Henderson efektif diterapkan pada pasien dengan kasus onkologi dan intervensi cryoterapi oral pada pasien kanker yang menjalani kemoterapi efektif dalam mengurangi angka kejadian mukositis pada pasien yang menjalani kemoterapi Kesimpulan : bahwa teori Virginia Henderson tepat digunakan pada pasien dengan osteosarkoma untuk meningkatkan kemandirian akibat perubahan fisik dan psikologis serta efek samping pengobatan kanker yang dijalaninya. Intervensi cryoterapi oral dapat digunakan sebagai salah satu alternatif intervensi keperawatan mandiri dalam mengurangi kejadian mukositis pada pasien kanker yang menjalani kemoterapi. The purpose of this Final Scientific Writing is able to show the role of specialist ners as nurse care nurse, change agentr and researcher Methodology and approach used is by using literature study. The final work contains: the application of Virginia Henderson 39;s theory model to patients with osteosarcoma, and the application of Evidance Based Nursing that is part of an innovation project that provides oral cryotherapy intervention in cancer patients undergoing chemotherapy. Results: The application of the Virginia Henderson nursing model theory was effectively applied to patients with oncology cases and oral cryotherapy interventions in cancer patients undergoing effective chemotherapy in reducing the incidence of mucositis in patients undergoing chemotherapy Conclusion: that Virginia Henderson 39;s theory is appropriately used in patients with osteosarcoma to improve independence due to physical and psychological changes and side effects of cancer treatment that lived. Oral cryotherapy interventions can be used as an alternative to self-nursing intervention in reducing the incidence of mucositis in cancer patients undergoing chemotherapy. Praktik residensi keperawatan medikal bedah merupakan rangkaian pendidikan magister keperawatan yang dilaksanakan dalam tatanan pelayanan kesehatan. Keperawatan medikal bedah berfokus pada penerapan ilmu dan tehnologi keperawatan untuk memenuhi kebutuhan pasien dewasa yang mengalami perubahan fisik dengan atau tanpa gangguan struktural. Praktik residensi keperawatan dilaksanakan di RSPJN Harapan Kita, kegiatan residensi terdiri dari pemberian asuhan keperawatan pada pasien dengan gangguan kardiovaskuler menggunakan pendekatan teori Virginia Henderson. Penerapan evidence based nursing (EBN) menggunakan terapi Spiritual Emotional Freedom Technique (SEFT) untuk menurunkan kecemasan pasien pre operasi bedah jantung, dan proyek inovasi rehabilitasi jantung fase 1 pada pasien SKA. Hasil akhir dari proses residensi ini menunjukkan bahwa pendekatan teori Virginia Henderson relevan dan dapat diterapkan pada pasien dengan gangguan sistem kardiovaskuler. EBN terapi Spiritual Emotional Freedom Technique (SEFT) dapat diterapkan diruang pre operasi bedah jantung karena secara signifikan dapat menurunkan kecemasan. Hasil proyek inovasi rehabilitasi jantung fase 1 dapat diterapkan pada pasien SKA karena SPO mudah dipahami dan mudah untuk dilakukan. Rehabilitasi jantung fase 1 pada pasien SKA dapat mempertahankan fungsi jantung secara optimal.

ABSTRACT
The residency program in medical nursing is a series of nursing master education programs carried out in the order of nursing services. Medical surgical nursing focuses on concepts and basic medical and surgical principles in the application of nursing science and technology to meet the needs of adult patients who experience physical changes with or without structural disorders. The practice of nursing residence was carried out at Harapan kita Heart and Vascular Hospital Jakarta, residency activities consist of giving nursing care to patients with cardiovascular disorders using the theoretical approach of Virginia Henderson. An evidence based nursing (EBN) application uses the therapy of Spiritual Emotional Freedom Technique (SEFT) to reduce the anxiety of patients preoperative cardiac surgery, and an innovative phase 1 cardiac rehabilitation project in SKA patients. The final results of this residency process show that Virginia Henderson's theoretical approach is relevant and can be applied to patients with cardiovascular system disorders. EBN Spiritual Emotional Freedom Technique (SEFT) therapy can be applied in the preoperative room because it can significantly reduce anxiety. The results of phase 1 cardiac rehabilitation innovation projects can be applied to SKA patients because SPO is easy to understand and easy to do. Phase 1 cardiac rehabilitation in SKA patients can maintain heart function optimally.

Praktek Spesialis Keperawatan merupakan sebuah proses penting dari pendidikan profesi dalam rangka mengaplikasikan peran seorang perawat spesialis yang memberikan dampak dalam upaya meningkatkan kualitas layanan keperawatan. Praktek ini dilakukan dan dikembangkan berdasarkan ilmu pengetahuan dan teknologi melalui penelitian yang berbasis fakta dan dengan pendekatan teori Henderson dan teori sosial kognitif Bandura di RS Persahabatan dan RS Fatmawati Jakarta. Tiga kompetensi yang harus dicapai dalam praktek residensi ini adalah memberikan asuhan keperawatan menggunakan pendekatan teori keperawatan, menerapkan implementasi keperawatan yang berbasis bukti ilmiah (evidence based nursing practice), dan melakukan proyek inovasi keperawatan yang berguna bagi lahan yang menjadi tempat praktek residensi. Mahasiswa residensi sebagai pemberi asuhan keperawatan telah memberikan asuhan keperawatan menggunakan pendekatan teori Henderson pada satu kasus kelolaan utama dan 30 kasus resume pasien HIV/AIDS dengan berbagai infeksi oportunitis. Tugas mahasiswa residensi sebagai peneliti telah mengaplikasi penerapan efektifitas terapi musik untuk menurunkan tingkat stes pada pasien dengan penyakit kronis dan hasil dari evidence based nursing ini menunjukkan bahwa terapi musik memberikan hasil yang signifikan untuk menurunkan tingkat stres pada pasien dengan penyakit kronis. Fungsi sebagai innovator, mahasiswa residensi telah melakukan inovasi yaitu pembuatan modul peer group dalam meningkatkan dukungan emosional dan kepatuhan minum obat pada ODHIV dan hasil penerapan proyek inovasi ini menunjukkan manfaat yang sangat signifikan bagi ODHIV dalam meningkatkan kepatuhan minum obat ART. ......Nursing Specialist Practice is an important process of professional education in order to apply the role of a specialist nurse which has an impact in efforts to improve the quality of nursing services. This practice is carried out and developed based on science and technology through fact-based research and using Henderson's theoretical approach and Bandura's social cognitive theory at Persahabatan Hospital and Fatmawati Hospital, Jakarta. The three competencies that must be achieved in this residency practice are providing nursing care using a nursing theory approach, implementing nursing implementation based on scientific evidence (evidence based nursing practice), and carrying out nursing innovation projects that are useful for the land where the residency practice takes place. Residency students as providers of nursing care have provided nursing care using Henderson's theoretical approach to one primary management case and 30 resume cases of HIV/AIDS patients with various opportunistic infections. The job of residency students as researchers has been to apply the effectiveness of music therapy to reduce stress levels in patients with chronic illnesses and the results of this evidence-based nursing show that music therapy provides significant results in reducing stress levels in patients with chronic illnesses. Functioning as innovators, residency students have innovated, namely creating a peer group module to increase emotional support and medication adherence for PLWHA and the results of implementing this innovation project show very significant benefits for PLWHA in increasing adherence to taking ART medication.

Karya Ilmiah Akhir ini merupakan laporan dari rangkaian kegiatan praktik residensi Ners Spesialis Keperawatan Medikal Bedah, dengan penerapan Teori Virginia Henderson pada proses keperawatan pasien gangguan sistem kardiovaskular, penerapan praktik keperawatan berdasarkan pembuktian, dan melakukan inovasi keperawatan. Pengkajian pasien dengan teori Virginia Henderson berfokus pada 14 kebutuhan dasar pasien. Dimana kebutuhan pasien sama dengan kebutuhan pada manusia sehat umumnya. Asumsi utama dalam teori Orlando sesuai dengan konsep utama yaitu person (manusia), kesehatan dan keperawatan dan lingkungan. Kebutuhan untuk dibantu (diagnosis keperawatan) yang umum terjadi pada pasien antara lain : penurunan curah jantung, gangguan pertukaran gas, inefektif bersihan jalan napas, risiko perdarahan, nyeri akut dan risiko infeksi. Pelaksanaan praktik keperawatan berdasarkan pembuktian, dilakukan untuk pengelolaan kecemasan pada pasien ACS. Intervensi dilakukan dengan memberikan stimulasi murottal Al-Qur`an dengan durasi 30 menit. Pelaksanaan inovasi keperawatan berupa optimalisasi pemberian asuhan keperawatan dimensi spiritual pasien dengan gangguan sistem kardiovaskular di ICVCU RSJPDHK. ......This scientific work is report from series of practice residency Nurse Medical Surgical Nursing Specialist, by implementing The Virginia Henderson`s Theory at nursing process with cardiovascular disorder, implementation of nursing practical based on evidence and making innovation in nursing. Assessment to patient with Virgini Henderson`s Theory was focusing fourteen  basic patient  needs. Where are same basic need of patients with healthy human needs. The assumption in Virginia Henderson`s theory according with the main concept, i.e person (human), environment, health and nurse. The need to get help (diagnostic of nursing) generally happened to patient, i.e.: decreased of cardiac output, impaired gas exchange, ineffective airway clearance, risk of hemorrhage, acute pain, and risk for infection. Tiga kompetensi yang harus dicapai dalam praktek residensi ini adalah memberikan asuhan keperawatan menggunakan pendekatan teori keperawatan, menerapkan implementasi keperawatan yang berbasis bukti ilmiah (evidence based nursing practice), dan melakukan proyek inovasi keperawatan yang berguna bagi lahanyang menjadi tempat praktek residensi. Sebagai pemberi asuhan keperawatan, mahasiswa residensi telah memberikan asuhan keperawatan menggunakan pendekatan teori Virginia Henderson pada 1 kasus kelolaan utama dan 30 kasus resume pasien HIV/AIDS dengan berbagai infeksi oportunitis. Sebagai peneliti, mahasiswa residensi telah mengaplikasi penerapan skrining Mual dan Muntah pasien HIV/AIDS menggunakan Index of Nausea, Vomiting and Retcing dan hasil dari evidence based nursing ini menunjukkan bahwa kuisioner INVR akurat dan reliable untuk menilai status mual dan muntah pada pasien HIV. Sedangkan sebagai innovator, telah dilakukan inovasi optimalisasi teknologi digital dalam meningkatkan kepatuhan minum obat ARV pada pasien dengan HIV / AIDS yang mana media teknologi digital berupa laman web yang dengan berbagai topik edukasi dengan web yang dapat di akses menggunakan barcode. Meanwhile, as an innovator, digital technology optimization innovations have been carried out in improving adherence to taking ARV drugs in patients with HIV / AIDS where digital technology media is in the form of web pages with various educational topics with the web that can be accessed using barcodes. The results of the implementation of this innovation project show very significant benefits for increasing knowledge and ease of patients in accessing important information about HIV disease so as to improve adherence in taking ARV drugs.
