"Lansia di perkotaan rentan mengalami berbagai masalah kesehatan, salah satunya yaitu masalah hambatan mobilitas fisik. Masalah hambatan mobilitas fisik dapat diatasi dengan salah satu intervensi keperawatan yaitu penggunaan alat bantu jalan. Penulisan karya ilmiah ini menganalisis praktik klinik keperawatan kesehatan masyarakat perkotaan yang dilakukan penulis di PSTW Budi Mulia 1 Cipayung. Kasus yang diangkat yaitu klien Ibu S (73 tahun) dengan diagnosa keperawatan hambatan mobilitas fisik. Hasil analisis intervensi menunjukkan bahwa penggunaan alat bantu jalan walker mampu meningkatkan mobilitas fisik, kemandirian, dan penurunan risiko jatuh. Perawat dan care giver di panti perlu meningkatkan pengetahuan dan keterampilan terkait penggunaan alat bantu jalan untuk mengoptimalkan mobilitas fisik lansia.

Elderly in urban community are susceptible to a variety of health problems. One of them is impaired physical mobility. One of nursing intervention, training the use of walker, can resolve this problem. This scientific paper analyzes clinical nursing practice in urban community at PSTW Budi Mulia 1 Cipayung as a writing method. The case in this paper is Mrs. S (73 years old) with impaired physical mobility.

The result showed that walker can increase physical mobility, independency, and decrease the risk of falls. Nurses and care giver need to improve the knowledge and skills of the use of walker to optimize the physical mobility in elderly."

"Konstipasi merupakan salah satu masalah kesehatan yang sering dikeluhkan lansia. Konstipasi pada lansia terjadi akibat beberapa faktor yang meliputi perubahan fungsi pencernaan, kurangnya asupan serat, intake cairan, dan kurangnya aktivitas fisik. Penelitian ini bertujuan untuk menggambarkan asuhan keperawatan lansia dengan masalah konstipasi. Metodologi penelitian yang digunakan adalah analisis kasus berdasarkan pengkajian melalui wawancara, observasi, dan tinjauan literatur pada klien di wisma Cattleya Panti Sosial Tresna Werdha Budi Mulia 1 Cipayung. Hasil analisis intervensi terapi latihan kayuh sepeda secara teratur yang dilakukan dua kali sehari dalam tiga minggu intervensi pada Kakek SH dengan konstipasi menunjukkan bahwa ada pengaruh latihan kayuh sepeda terhadap pola defekasi klien. Peneliti merekomendasikan untuk melakukan program latihan fisik secara teratur untuk meningkatkan pola normal defekasi klien.

Constipation is one of problem that commonly occurs in elderly. Constipation in elderly caused by several factors such as gastrointestine changes, low fiber intake, low fluid intake, and physical activity . This research aimed to describe nursing interventions in elderly with constipation.

This research used case analysis method based on the data obtained by interview, observation, and literature review of client at Cattleya housing, Panti Sosial Tresna Werdha Budi Mulia 1 Cipayung.

Analysis results of bicycle movement therapy given 2 times a day in three weeks on Mr. SH with constipation showed that there's significant relation between physical activity with defecation pattern fom before.

This research suggested to have a regular physical activity to maintain normal defecation pattern in elderly."

"Latar Belakang : Salah satu dampak sistemik dari penyakit paru obstruktif kronik (PPOK) yaitu modifikasi tipe otot skeletal. Tujuan penelitian ini adalah untuk menilai tingkat kelelahan kaki dengan menilai skala Borg lelah dan asam laktat perifer sebagai penanda kelelahan kakiMetode : Desain penelitian ini adalah studi potong lintang, terdiri dari 34 subjek PPOK dan 25 subjek kontrol sehat yang seusia dengan usia subjek PPOK. Subjek dinilai skala Borg kelelahan kaki lelah dan asam laktat perifer sebelum dan sesudah uji jalan 6 menit (UJ6M).Hasil : Terdapat peningkatan lebih tinggi median asam laktat yang tidak bermakna (p > 0,05) secara statistik antara subjek PPOK (0,5) dibandingkan kontrol (0,45). Terdapat peningkatan median skala Borg lelah yang bermakna (p < 0,001) antara subjek PPOK (5,0) dibandingkan subjek kontrol (1,0). Terdapat jarak tempuh yang lebih besar secara bermakna pada subjek kontrol dibandingkan subjek PPOK (p < 0,05).Kesimpulan : Kelompok PPOK memiliki peningkatan asam laktat yang tidak bermakna dibandingkan kelompok kontrol. Kelompok PPOK memiliki peningkatan skala Borg kaki lelah yang lebih besar dan berbeda bermakna dibandingkan dengan kontrol.

Background : One of systemic effects of COPD is a modification of skeletal muscle fiber types. The objective of this study is to determine the increase of leg fatigue by using Borg scale leg fatigue and lactic acid level.

Methods : This is a cross-sectional study design. The samples were 34 COPD patients and 25 healthy adults with the same age as COPD patients as the control. The lactic acid level and Borg leg fatigue scale were measured before and after six minute walking test (6MWT).

Results : There was an unsignificantly difference change of median of lactic acid level (p > 0,05) between COPD (0,5 mMol) compared to control (0,45 mMol). There was a statistically significant difference (p < 0,001) change of leg fatigue Borg scale between COPD (5,0) compared to control (1,0). There was a significantly (p < 0,05) higher mean of distance of 6MWT in control subjects (411,62 meters) compared to COPD (364 meters).

Conclusion : COPD patients had an unsignificantly increase of lactic acid level after the 6MWT compared to control subjects. COPD patients had a significantly higher leg fatigue Borg scale compared to control after the 6MWT."
