Farmasia Inovasi Megatrading bertujuan untuk mengidentifikasi alur proses pengadaan dan menganalisis dokumentasi pengadaan impor yang ada di PT. Farmasia Inovasi Megatrading. Penulisan laporan ini menggunakan alur proses pengadaan yang digunakan oleh PT. Farmasia Inovasi Megatrading kemudian akan dikembangkan sesuai dengan hasil analisis alur proses tersebut secara efektif dan efisien serta diimplementasikan di kemudian hari dengan mempertimbangkan berbagai aspek sesuai dengan PT. Farmasia Inovasi Megatrading. Hasil yang didapatkan adalah alur proses yang ada di perusahaan dimulai dari customer akan menghubungi sales perusahaan (untuk customer lama) hingga pengiriman barang sampai ke tangan customer. Dari alur proses tersebut, diambil contoh bahan baku yang bersifat fast moving di Indonesia, yaitu amlodipin besilat. Dari 10 industri farmasi yang masuk ke dalam industri farmasi terbesar di Indonesia, 9 (sembilan) diantaranya menggunakan zat aktif amlodipin besilat. Oleh karena itu, disarankan untuk pengadaan bahan baku yang bersifat fast moving sebaiknya dilakukan secara stok buffer dengan mempertimbangkan riwayat pembelian pelanggan dan menggunakan metode penerapan fuzy tsukamoto. Namun dikarenakan perusahaan masih terbilang perusahaan baru, maka untuk pengadaan bahan baku masih tidak bersifat ready stock karena pasar penjualan bahan baku tidak stabil.

Farmasia Inovasi Megatrading with the purpose to identified the procurement process flow and to analyzed the existing import procurement documentation at PT. Farmasia Inovasi Megatrading. The writing of this report was used the procurement process flow of PT. Farmasia Inovasi Megatrading and will be developed according to the results of the analysis of the process flow effectively and efficiently that will be implemented in the future by considering various aspects according to PT. Farmasia Inovasi Megatrading. The results was obtained that the process flow of the company started from the customer will contact the company’s sales (for old customers) until the delivery of goods to the customer’s hand. From the process flow, an example of fast moving raw materials is Indonesia is amlodipine besylate. From the 10 pharmaceuticals company industries that make up the largest pharmaceutical industry in Indonesia, 9 of them is used the active substance amlodipine besylate. Therefore, it’s recommended that the procurement of fast moving raw materials should be done in b buffer stock by considering customer purchasing history and using the fuzy tsukamoto application method. However, because the company is still a new company the procurement of raw materials is still to doing a buffer stock applicable because the raw materials sales market is not stable."

"Penulisan laporan tugas khusus praktik kerja profesi Apoteker (PKPA) di Apotek Kimia Farma 048 Matraman bertujuan untuk mengidentifikasi penyakit kronis dan menganilisis resep untuk pasien rujuk balik (PRB) berdasarkan kajian yang berlaku di Apotek. Penulisan laporan ini menggunakan data resep pasien rujuk balik sebanyak 20 resep yang memiliki riwayat penyakit kronis kemudian rujuk balik ke Apotek Kimia Farma 048 Matraman. Hasil yang didapatkan adalah rata – rata pasien yang dirujuk balik ke Apotek memiliki obat untuk penyakit kronis seperti Hipertensi yaitu Amlodipin Besilat, Bisoprolol. Selain itu, penyakit kronis lainnya terdapat Diabetes Melitus dengan contoh obat yang didapatkan seperti Metformin, Glimepirid, dan Insulin baik insulin kerja panjang maupun kerja pendek. Dari obat – obat tersebut dianalisis bagaimana mekanisme kerjanya dan indikasi serta dosis yang direkomendasikan di resep telah sesuai atau belum sesuai. Tidak hanya analisis resep, namun pengkajian kesesuaian resep juga dilakukan berdasarkan persyaratan administratif, farmasetik, dan klinis. Dari analisis 20 resep tersebut, diketahui bahwa resep telah sesuai dengan persyaratan administratif, farmasetik, dan klinis sehingga disimpulkan bahwa telah memenuhi aturan yang berlaku.

A special assignment report pharmacist professional internship is writing at Apotek Kimia Farma with the purpose to identified chronic disease and to analyzed prescriptions for patients with referral back (PRB) based on applicable studies at pharmacy. The writing of this report used prescriptions data for 20 referral patients who have a history of chronic disease and then referral back to Apotek Kimia Farma 048 Matraman. The results was obtained that the average of the patients who has referral back to drugstore have drugs for chronic disease such as hypertension, like Amlodipine Besylate and Bisoprolol. In addition to, another chronic disease there’s Diabetic with the example of drugs are Metformin, Glimepiride and Insulin both long acting and short-acting insulin. From these drug, it was analyzed how the mechanism of action and the indications and the dosage was recommended in the prescriptions was appropiate or not. Not only prescriptions was analyzed, but also the assessment of the prescriptions suitability was carried out based on administrative, pharmaceutical, and clinical requirements. From the analyzed of the 20 prescriptions, it was found that the prescriptions were in accordance with administrative, pharmaceutical, and clinical requirements so that it was concluded that they had met the applicable regulations."

"Penulisan laporan tugas khusus praktik kerja profesi Apoteker (PKPA) di PT Kalventis Sinergi Farma bertujuan untuk menganalisis persyaratan pengguna mengenai personal training dan menghasilkan penilaian risiko berdasarkan kontrol dan tingkat prioritas PT. Kalventis Sinergi Farma. Penulisan laporan ini menggunakan suatu sistem yang digunakan oleh perusahaan yaitu Sistem X yang berbasis  COTS (commercial off-the shelf) dan perangkat lunak berbasis SaaS (software as a service). Proses dokumentasi yang diajukan kepada sistem x memiliki beberapa persiapan seperti menggunakan diagram proses yang terdiri dari rencana pelatihan, manajemen pelatihan, analisis pelatihan, laporan pelatihan, dan pencarian pelatihan. Spesifikasi fungsional dianalisis sesuai tingkat risiko yaitu tinggi, sedang, rendah. Dari analisis tingkat risiko, dapat disimpulkan untuk menetapkan strategi tes dari masing-masing rencana pelatihan yaitu challenge testing, nominal testing, dan exploratory testing. Penilaian risko dari persyaratan pengguna spesifikasi fungsional dapat mengurangi kegagalan pada GxP dan risiko kegagalan bisnis.

Kalventis Sinergi Farma with the purpose to analyze user requirements of personal training and to make a risk assesment based on the controls and the priority levels of PT. Kalventis Sinergi Farma. This report is using a system used by the company, namely system X which is based on COTS (Commercial off-the shel) and SaaS (Software as a Service). The documentation of the validation is processing by submitted to system has several preparations like a diagram process consisting of training plans, training management, traing analysis, training reports, and training search. Specification of functional was analyzed that accorded by the risk level (high, medium, low). From analysis of risk level, it can be concluded to determine the test strategy of each training plan (challenge testing, nominal testing, and exploratory testing). Risk assessment of user requirements of functional specifications can reduce failures in GxP and business failure.

"Penulisan laporan tugas khusus praktik kerja profesi Apoteker (PKPA) di RSUP Persahabatan bertujuan untuk mengetahui dan melakukan pemantauan terapi obat pada salah satu pasien autoimun di RSUP Persahabatan periode Juni 2023. Penulisan laporan ini menggunakan data terapi obat salah satu pasien yang terdiagnosa Evans Syndrome ditandai dengan gejala yang muncul seperti penurunan trombosit dibawah batas normal dan hasil tes coombs (antiglobulin test). Hasil yang didapatkan yaitu selama perawatan, jumlah trombosit pasien menurun drastis pada hari ke-1 sebesar 2000. Oleh karena itu, pasien mendapatkan transfusi darah sehingga pada hari ke-3 hasil trombosit pasien meningkat menjadi 13.000 dari batas normal yaitu 150.000 – 400.000. Terapi obat pasien terdapat DRP (drug related problem) yaitu ada indikasi tanpa obat. Dari terapi pasien, diketahui belum ada penambahan terapi menggunakan kortikosteroid. Oleh karena itu, diberikan saran kepada DPJP untuk penambahan terapi kortikosteroid untuk terapi evans syndrome.

A special assignment report pharmacist professional work practice is writing at the RSUP Persahabatan with the purpose to find out and monitoring drug therapy for one of the autoimmune patients at RSUP Persahabatan on Juni 2023 period. This report used the drug therapy data of Evans Syndrome patients characterized by symptoms that appear with a decrease of platelets below the normal data and the results of the coombs test (antiglobulin test). The resutls was obtained that during the treatment, the patients’s platelet count decreased on the 1st day by 2000 from 150.000 – 400.000. Therefore, the patient was received a blood transfusion so on the 3rd the patient’s platelet was increased to 13.000 from 150.000 – 400.000. the patient’s drug therapy has a DRP which is a indications without drugs. From the patient’s therapy, it’s known that there hasn’t additional therapy using corticosteroids which the therapy of Evans Syndrome is using a corticosteroid. Therefore, the pharmacist advice to DPJP for the addition of corticosteroid therapy for Evans Syndrome."

"Penulisan laporan tugas khusus praktik kerja profesi Apoteker (PKPA) di Puskesmas Kecamatan Makasar bertujuan untuk mengetahui efek samping, alur pelaporan, dan monitoring efek samping dari obat TB dan HIV di Puskesmas Kecamatan Makasar. Penulisan laporan ini menggunakan sampel data 7 (tujuh) pasien yang mengkonsumsi obat TB dan HIV pada periode waktu Mei 2022 – Februari 2023 dengan metode observasional. Hasil yang didapatkan dari ketujuh pasien tersebut adalah sebanyak 3 (tiga) dari 7 (tujuh) pasien mengalami efek samping obat tuberkulosis yaitu pirazinamid ditandai dengan pasien mengalami mual dan muntah yang berlebih. Oleh karena itu, ketiga pasien dirujuk ke rumah sakit untuk ditangani lebih lanjut. Setelah itu, 2 (dua) dari 7 (tujuh) pasien mengalami efek samping levofloxacin dan 2 (dua) lainnya mengalami efek samping dari obat HIV yaitu efavirenz. Tindak lanjut dari pasien yang mengalami efek samping obat levofloxacin adalah obat belum bisa dihentikan dikarenakan masih tahap pengobatan TB-MDR sedangkan untuk tindak lanjut dari pasien yang mengalami efek samping efavirenz adalah penggantian terapi ARV menjadi KDT yang mengandung dolutegravir. Data monitoring efek samping obat yang didapatkan, kemudian dilakukan pelaporan ke e-MESO.

A special assignment report pharmacist professional work practice is writing at the Makasar District Health Center with the purpose to knowing about the side effects, monitoring the side effect and reporting flow of the side effects of tuberculosis and HIV drugs at the Makasar District Health Center. This report is using 7 total sample patients who consumes TB and HIV drugs with the period May 2022 – Februari 2023 with the observasional method. The results of this report from the seven of patients were as many as three of the seven patients was experienced the side effects of tuberculosis drug which is pyrazinamid by viewed nausea and vomiting. Therefore, that patients were referred to the hospital for the further treatment. The next is, two of seven patients experienced the side effects of levofloxacin and the others are efavirenz. The follow up of levofloxacin is the drug cant be discontinued because it’s still in the MDR-TB treatment. The follow up of efavirenz is the replacement of ARV therapy to KDT with dolutegravir. The data of monitoring side effects was obtained and then reported to e-MESO."
