Abstrak :
Industri farmasi dituntut untuk dapat memproduksi obat yang sesuai dengan tujuan penggunaannya, memenuhi persyaratan registrasi, dan aman bagi pengguna. Apoteker memegang peran penting dalam mengawasi pengkajian mutu produk (PMP) atau product quality review (PQR) serta studi stabilita. Penelitian ini menggunakan metode studi literatur untuk memahami prinsip CPOB, quality assurance, PQR, dan studi stabilita. Penyusunan laporan PQR menggunakan Ms. Excel dan aplikasi Minitab untuk analisis deskriptif statistik dan pembuatan peta kendali (control chart) yang bertujuan untuk memonitor tren produk. Studi stabilita dilakukan menggunakan Ms. Word dan Ms. Excel untuk membuat dan menganalisa tren produk. Pada tugas khusus ini, dilakukan PQR terhadap produk sirup H 100 mL dengan 64 batch. Berdasarkan analisis data trending, produk ini memenuhi semua spesifikasi dalam PQR. Pengujian stabilita dilakukan pada 3 batch produk sirup H 100 mL, yang menunjukkan produk mempertahankan spesifikasi terkait penampilan, pH, identifikasi bahan aktif, dan uji mikrobiologi. Hasil dari studi ini menegaskan peran penting apoteker dalam memastikan kualitas produk farmasi sesuai dengan standar yang ditetapkan, serta memonitor stabilitas produk untuk memastikan keamanan dan efektivitas selama masa simpan. The pharmaceutical industry faces demands to produce drugs that meet their intended use, registration requirements, and ensure safety for users. Pharmacists play a crucial role in overseeing Product Quality Reviews (PQR) and stability studies. This research utilized literature review methods to understand Good Manufacturing Practice (GMP) principles, quality assurance, PQR, and stability studies. The PQR report was compiled using Ms. Excel and Minitab for descriptive statistical analysis and control chart generation to monitor product trends. Stability studies were conducted using Ms. Word and Ms. Excel for product trend analysis. In this specific task, PQR was performed on the H 100 mL syrup product across 64 batches. Based on trending data analysis, the product met all specifications outlined in the PQR. Stability testing was conducted on 3 batches of H 100 mL syrup, demonstrating that the product maintained specifications related to appearance, pH, active ingredient identification, and microbiological tests. The findings underscore the critical role of pharmacists in ensuring pharmaceutical product quality meets established standards and monitoring product stability to ensure safety and efficacy throughout its shelf life. This study highlights the importance of implementing Product Quality Review practices and stability studies in the pharmaceutical industry as part of efforts to ensure regulatory compliance and provide assurance of product safety to consumers.

Abstrak :
Apoteker dituntut harus memiliki kompetensi yang memadai dalam menjalankan tugas dan tanggung jawab kefarmasian di apotek. Oleh karena itu, calon apoteker harus dipersiapkan dengan baik sebelum memegang tugas dan tanggung jawab tersebut. Salah satu program untuk mempersiapkan calon apoteker yaitu melalui Praktik Kerja Profesi Apoteker (PKPA). Pelaksanaan PKPA dilakukan mulai tanggal 3-31 Juli 2023 di Apotek Roxy Depok. Metode yang digunakan dalam penyusunan laporan tugas khusus ini menggunakan metode berupa studi literatur dan penelusuran terkait interaksi dan efek samping obat yang dapat terjadi pada resep polifarmasi pada periode bulan Juli 2023. Berdasarkan 5 resep yakng dikaji, hasil penelitian menunjukkan adanya interaksi yang bersifat moderat pada resep 2,4 dan 5. Oleh karena itu interaksi tersebut perlu dimonitor agak tidak membahayakan keselamatan pasien. Apoteker memiliki peran penting untuk memberikan konseling bagi pasien yang mendapatkan resep polifarmasi serta berusahan untuk melakukan kerja sama yang baik dengan dokter pembuat resep dalam rangka memberikan pengobatan yang efektif dan aman bagi pasien.Peran apoteker dalam meminimalkan risiko obat yang membahayakan bagi pasien sangat diperlukan. Apoteker dapat memaksimalkan konseling terhadap pasien untuk menyampaikan efek samping yang dapat terjadi terkait dengan obat serta penanganannya. Selain itu apoteker memiliki peran penting untuk mencegah adanya interaksi yang membahayakan pasien terkait polifarmasi yakni dengan memaksimalkan kolaborasi dengan dokter pembuat resep terkait pengobatan yang efektif bagi pasien. ...... Pharmacists are required to have adequate competence in carrying out pharmaceutical duties and responsibilities in pharmacies. Therefore, prospective pharmacists must be well-prepared before assuming these tasks and responsibilities. One program aimed at preparing prospective pharmacists is through the Pharmacist Professional Internship Program (PKPA). The PKPA was conducted from July 3-31, 2023, at Roxy Pharmacy Depok. The method used in preparing this specific task report employed literature review and investigation methods related to drug interactions and side effects that may occur in polypharmacy prescriptions during the July 2023 period. Based on the analysis of five prescriptions, the study results showed moderate drug interactions in prescriptions 2, 4, and 5. Therefore, these interactions need to be monitored to avoid endangering patient safety. Pharmacists play a crucial role in counseling patients receiving polypharmacy prescriptions and strive to collaborate effectively with prescribing doctors to ensure effective and safe treatment for patients. The pharmacist's role in minimizing the risk of dangerous drug effects for patients is essential. Pharmacists can maximize counseling for patients to convey potential drug-related side effects and their management. Additionally, pharmacists play a critical role in preventing dangerous interactions related to polypharmacy by maximizing collaboration with prescribing doctors regarding effective patient treatment.

Abstrak :
Pemantauan Terapi Obat (PTO) merupakan aspek krusial dalam memastikan terapi obat yang aman, efektif, dan rasional bagi pasien. Penelitian ini bertujuan untuk memahami implementasi PTO di Rumah Sakit Pusat Angkatan Darat (RSPAD) dalam konteks pendidikan praktik kerja profesi apoteker (PKPA). Fokus utama adalah pada analisis PTO pada pasien dengan CVD berulang, hipertensi, diabetes, dan dislipidemia. Selama lima hari PKPA di Paviliun Soehardo Kertohusodo (PSK), data pasien dikumpulkan melalui SIM RS, rekam medis, dan wawancara dengan pasien, keluarga, serta tenaga kesehatan lainnya. Hasil kajian menunjukkan beberapa temuan penting, termasuk identifikasi masalah terkait pengobatan seperti kebutuhan akan terapi gout dengan allopurinol pada pasien dengan nilai asam urat dan ureum tinggi yang belum diobati. Selain itu, ditemukan kebutuhan akan terapi anemia pada pasien dengan nilai hemoglobin, hematokrit, eritrosit, trombosit, dan MCV rendah namun belum mendapatkan perlakuan yang sesuai. Rekomendasi pengobatan hasil PTO diberikan setelah diskusi dengan apoteker penanggung jawab kepada Dokter Penanggung Jawab Pasien (DPJP), dengan rencana pemantauan yang ditetapkan untuk memastikan keberhasilan terapi. Studi ini menyoroti pentingnya kolaborasi yang baik antara dokter dan apoteker dalam pelayanan kesehatan untuk meningkatkan hasil terapi pasien dan mencegah komplikasi yang tidak diinginkan selama perawatan. Dengan demikian, upaya ini diharapkan dapat memberikan kontribusi positif dalam pengembangan praktik PTO yang lebih efektif dan terfokus di lingkungan rumah sakit. ...... Therapy Drug Monitoring (TDM) is a crucial aspect in ensuring safe, effective, and rational drug therapy for patients. This research aims to understand the implementation of TDM at the Indonesian Army Central Hospital within the context of the Pharmacist Professional Internship. The main focus is on analyzing TDM in patients with recurrent CVD, hypertension, diabetes, and dyslipidemia. Over five days of PKPA at Pavilion Soehardo Kertohusodo (PSK), patient data was collected through hospital information system, medical records, and interviews with patients, families, and other healthcare professionals. The study findings revealed several important observations, including the identification of treatment-related issues such as the need for gout therapy with allopurinol in patients with high uric acid and urea levels that have not been treated. Additionally, there was a need for anemia therapy in patients with low hemoglobin, hematocrit, red blood cells, platelets, and MCV values but have not received appropriate treatment. Treatment recommendations from TDM were provided after discussions between the responsible pharmacist and the Attending Physician, with a monitoring plan established to ensure therapy success. This study highlights the importance of effective collaboration between doctors and pharmacists in healthcare delivery to improve patient therapy outcomes and prevent unwanted complications during treatment. Thus, these efforts are expected to positively contribute to the development of more effective and focused TDM practices within hospital settings.

Abstrak :
Laporan ini membahas peran apoteker sebagai penanggung jawab dalam pengelolaan sediaan farmasi di Pusat Distribusi Farmasi (PBF), terutama terkait produk narkotika. PKPA dilaksanakan di KFTD Cabang Jakarta 2 untuk memahami praktik pengadaan, penyimpanan, dan distribusi produk farmasi sesuai dengan peraturan perundang-undangan dan standar CDOB (Cara Distribusi Obat yang Baik) dan SOAP (Standar Operasional Prosedur). Metode studi literatur dan pengamatan langsung digunakan untuk mengevaluasi implementasi pengelolaan produk narkotika di KFTD 2. Hasilnya menunjukkan bahwa pengelolaan produk narkotika di KFTD 2 telah mematuhi ketentuan yang berlaku, dengan apoteker memainkan peran sentral dalam memastikan kepatuhan terhadap proses distribusi. Meskipun demikian, PKPA berdurasi singkat mengakibatkan keterbatasan dalam pemahaman mahasiswa PKPA terhadap proses seperti stock opname dan pelaporan narkotika bulanan. Oleh karena itu, diperlukan upaya untuk mengoptimalkan waktu yang tersedia agar mahasiswa PKPA dapat memperoleh pengetahuan yang komprehensif dalam praktik distribusi farmasi, khususnya produk narkotika. ...... This report discusses the pharmacist's role as the responsible party in managing pharmaceutical preparations at the Pharmaceutical Distribution Center (PBF), particularly concerning narcotic products. The Pharmacist Professional Internship (was conducted at KFTD Branch Jakarta 2 to understand the practices of procurement, storage, and distribution of pharmaceutical products in accordance with legal regulations and Good Distribution Practices (CDOB) and Standard Operating Procedures (SOP). Literature review and direct observation methods were employed to evaluate the implementation of narcotic product management at KFTD 2. The results indicated that the management of narcotic products at KFTD 2 complied with applicable regulations, with pharmacists playing a central role in ensuring compliance with distribution processes. However, the short duration of practiced resulted in limitations in students' understanding of processes such as stock-taking and monthly narcotic reporting. Therefore, efforts are needed to optimize the available time so that PKPA students can acquire comprehensive knowledge in pharmaceutical distribution practices, especially regarding narcotic products.

Abstrak :
HIV/AIDS tetap menjadi perhatian utama dalam kesehatan global, dengan tantangan utama dalam diagnosis, pencegahan, dan perawatan yang efektif menggunakan antiretroviral (ARV). Praktik Kerja Profesi Apoteker dilakukan pada periode 1 April hingga 17 April 2023 di Puskesmas Kecamatan Cengkareng, yang bertujuan untuk meningkatkan edukasi masyarakat tentang HIV/AIDS melalui pengembangan leaflet dan brosur. Metode studi literatur digunakan untuk menyusun laporan ini, dengan fokus pada analisis data pasien HIV/AIDS di Puskesmas. Penggunaan aplikasi grafis Canva mempermudah pembuatan materi edukatif seperti leaflet dan brosur. Hasil penelitian menunjukkan bahwa dari 506 pasien terdaftar pada periode 25 Desember 2021 hingga 31 Desember 2022, masih ada yang tidak aktif dalam mengambil terapi ARV di Puskesmas. Kesimpulan dari laporan ini menyoroti pentingnya edukasi melalui leaflet dan brosur sebagai metode yang efektif untuk menjangkau masyarakat lebih luas dalam memahami HIV/AIDS, termasuk informasi tentang pengertian HIV/AIDS, tanda dan gejala, penularan, upaya pencegahan, serta pengobatan yang tersedia di Puskesmas. Peran apoteker dalam memberikan informasi, konseling, dan monitoring penggunaan ARV juga disoroti sebagai bagian integral dari manajemen HIV/AIDS. Saran untuk penelitian mendatang mencakup perlunya investigasi lebih lanjut terkait alasan tidaknya pasien mengambil terapi ARV di Puskesmas, serta perluasan studi literatur dan analisis data demografi untuk mengevaluasi efektivitas leaflet dan brosur dalam mendukung edukasi masyarakat tentang HIV/AIDS di wilayah tersebut. ...... HIV/AIDS remains a major concern in global health, with primary challenges in diagnosis, prevention, and effective treatment using antiretroviral therapy (ARV). The Pharmacist Professional Internship was conducted from April 1 to April 17, 2023, at Cengkareng Sub-District Health Center, aimed at enhancing public education about HIV/AIDS through the development of leaflets and brochures. Literature review methods were used to compile this report, focusing on the analysis of HIV/AIDS patient data at the health center. The use of graphic design application Canva facilitated the creation of educational materials such as leaflets and brochures. The research findings showed that out of 506 registered patients from December 25, 2021, to December 31, 2022, some were inactive in picking up their ARV therapy at the health center. The conclusion of this report highlights the importance of education through leaflets and brochures as effective methods to reach a broader audience in understanding HIV/AIDS. This includes information on the definition of HIV/AIDS, signs and symptoms, transmission, prevention efforts, and available treatments at the health center. The role of pharmacists in providing information, counseling, and monitoring the use of ARV therapy is also emphasized as an integral part of HIV/AIDS management. Recommendations for future research include the need for further investigation into the reasons why some patients do not pick up their ARV therapy at the health center, as well as expanding literature studies and demographic data analysis to evaluate the effectiveness of leaflets and brochures in supporting public education on HIV/AIDS in the region.
