"Pengkajian resep merupakan suatu rangkaian kegiatan yang terdiri dari penerimaan, pemeriksaan ketersediaan, pengkajian resep, penyiapan sediaan farmasi, termasuk peracikan obat dan penyerahan disertai pemberian informasi. Tujuan pengkajian resep adalah untuk menganalisa adanya masalah terkait obat dan dilakukan untuk semua resep yang masuk tanpa ada kriteria khusus pasien. Pengkajian resep dapat dilakukan di fasilitas kesehatan seperti rumah sakit, apotek, dan puskesmas. Pada tugas khusus ini, pengkajian resep dilakukan untuk pasien dengan ulkus kornea di Apotek Roxy Sawangan. Ulkus kornea merupakan defek epitel kornea sampai stroma yang disertai dengan inflamasi. Ulkus kornea secara garis besar dibagi berdasarkan penyebabnya yaitu infeksi dan non-infeksi. Beberapa gejala dari ulkus kornea bakteri yang dapat dialami pasien adalah mata nyeri, kemerahan, penurunan visus, fotofobia, lakrimasi dan sensasi benda asing. Terapi farmakologi untuk pasien ulkus kornea bakteri adalah antibiotik golongan fluorokuinolon atau kombinasi fortified antibiotic topical. Berdasarkan pengkajian resep Nyonya D di Apotek Roxy Sawangan, dapat disimpulkan bahwa tidak ada medication error pada resep Nyonya D. Akan tetapi, terdapat ketidaksesuaian aspek administratif pada resep Nyonya D. Kemudian, berdasarkan aspek farmaseutik dan klinis yang telah dikaji dapat disimpulkan bahwa Nyonya D mengalami ulkus kornea yang disebabkan infeksi bakteri.

The purpose of prescription review is to analyse medication error and to be conducted for all prescription without any special criteria. Prescription review can be done in health care facilities such as hospital, drugstore, and public health center. On this report, prescription review was done for patient with corneal ulcer in Apotek Roxy Sawangan. Corneal ulcer is a defect in cornea epithelium until stroma with inflammation. In general, corneal ulcer classified based on its cause, with infection and non-infection. Some of the symptoms of bacterial corneal ulcers are eye pain, redness. Pharmacological therapy for patient with bacterial cornea ulcer is fluoroquinolone antibiotic or combination fortified antibiotic topical. Based on prescription review of Miss D in Apotek Roxy Sawangan, it can be concluded that there is no medication error in Miss D’s prescripition. However, there was discrepancy of administrative aspects in Miss D’s prescripition. Furthermore, based on pharmaceutical and clinical aspects of Miss D’s prescription, it can be concluded that Miss D has corneal ulcer caused by bacterial infection."

"Kegiatan pengkajian resep meliputi kajian administrasi, kesesuaian farmasetik, dan pertimbangan klinis. Jika ditemukan adanya ketidaksesuaian dari hasil pengkajian apoteker harus menghubungi dokter penulis resep. Pada setiap tahap alur pelayanan Resep dilakukan upaya pencegahan terjadinya kesalahan pemberian obat (medication error). Penyusunan tugas khusus Praktik Kerja Profesi Apoteker di Apotek Roxy Sawangan bertujuan untuk mengidentifikasi kelengkapan resep obat terapi gangguan pencernaan berdasarkan aspek administratif, aspek farmasetik dan aspek klinis yang terdapat pada Apotek Roxy Sawangan periode April 2023.Tugas khusus ini disusun dengan metode deskriptif dan melakukan kegiatan pengkajian resep pada dua resep pasien di Apotek Roxy Sawangan. Obat yang terdapat pada kedua resep tersebut memiliki indikasi yang beririsan yaitu sebagai terapi gangguan pencernaan. Berdasarkan pengkajian resep yang dilakukan terhadap kedua resep dapat disimpulkan pada resep 1 aspek administratif dan farmasetik sudah lengkap tetapi pada aspek klinis menunjukkan ketidaktepatan dosis pemberian sirup sukralfat. Selanjutnya pada resep 2, aspek administratif kurang lengkap karena tidak ada alamat , paraf dokter dan berat badan pasien pada aspek administratif dan farmasetik sudah lengkap dan sesuai.

The preparation of a special assignment for Pharmacist Professional Work Practice at Roxy Sawangan Pharmacy aims to identify the completeness of the prescription of indigestion therapy drugs based on administrative aspects, pharmaceutical aspects and clinical aspects found at Roxy Sawangan Pharmacy for the period April 2023.This special assignment was prepared using a descriptive method and carried out prescription review activities on two patient prescriptions at Roxy Sawangan Pharmacy. The drugs contained in the two prescriptions have indications that overlap, namely as therapy for indigestion. Based on the prescription review carried out on the two prescriptions, it can be concluded that in recipe 1 the administrative and pharmaceutical aspects are complete but the clinical aspects show the inaccuracy of the dose of sucralfate syrup. Furthermore, in prescription 2, the administrative aspect is incomplete because there is no address, the doctor's initials and the patient's weight in the administrative and pharmaceutical aspects are complete and appropriate."

"Obat merupakan suatu bahan atau paduan bahan yang dapat digunakan sebagai pencegahan, penyembuhan, pemulihan, serta peningkatan kesehatan dan kontrasepsi untuk manusia (Menteri Kesehatan RI, 2016). Obat dapat diperoleh secara bebas atau berdasarkan resep dari dokter. Pasien yang memiliki masalah kesehatan tertentu dan menjalani terapi obat, umum diberikan resep obat dari dokter (Megawati & Santoso, 2017). Resep merupakan permintaan tertulis dari dokter atau dokter gigi baik tertulis pada kertas maupun secara elektronik kepada apoteker, untuk menyediakan dan menyerahkan obat bagi pasien sesuai dengan peraturan yang berlaku (Menteri Kesehatan RI, 2016). Resep dari dokter harus memuat informasi yang jelas dan memenuhi aspek administratif, farmasetik, serta pertimbangan klinis agar apoteker serta petugas kefarmasian dapat memahami obat yang akan diberikan kepada pasien (Menteri Kesehatan RI, 2016). Tugas khusus ini dilakukan dengan mendokumentasikan serta secara metode deskriptif, dilakukan pengkajian resep terhadap aspek administratif, farmasetik, dan klinis. Pengkajian resep dilakukan terhadap dua resep yang berbeda tetapi memiliki indikasi beririsan, yaitu sebagai terapi obat gangguan kardiovaskular. Berdasarkan kedua resep tersebut, aspek yang dikaji secara administratif, kesesuaian farmasetik, dan pertimbangan klinis cukup lengkap dan memberikan informasi yang cukup untuk apoteker dan tenaga kefarmasian lainnya dalam menyediakan dan memberikan obat yang sesuai kepada pasien.

Drug is a material or combination of ingredients that can be used as prevention, cure, recovery, and health improvement and contraception for humans (Menteri Kesehatan RI, 2016). Drug can be obtained with or without prescription from a doctor. Patients who have certain health problems and undergo drug therapy, are generally given drug prescriptions from the doctors (Megawati & Santoso, 2017). Prescription is a written request from a doctor or dentist either written on paper or electronically to the pharmacist, to provide and deliver drugs for patients in accordance with applicable regulations (Menteri Kesehatan RI, 2016). Prescriptions from doctors must contain clear information and meet administrative, pharmaceutical, and clinical considerations aspects so that pharmacists and pharmaceutical personnel can understand the drugs to be given to patients (Menteri Kesehatan RI, 2016). This task is carried out by documenting and reviewing prescriptions on administrative, pharmacological, and clinical aspects with descriptive method. The review of prescriptions was carried out on two different prescriptions but had intersecting indications, specifically for cardiovascular disorders. Based on these two prescriptions, the aspects reviewed administratively, pharmaceutical suitability, and clinical considerations are quite complete and provide sufficient information for pharmacists and other pharmaceutical personnel in providing and delivering appropriate drugs to patients."

"Apoteker memiliki peran penting dalam pekerjaan kefarmasian. Berpartisipasi langsung dalam bidang praktik kefarmasian menjadi salah satu hal penting yang perlu dilakukan untuk menjadi seorang apoteker profesional. Praktik kerja profesi apoteker dilakukan sebagai bekal agar calon apoteker memahami peran apoteker, serta meningkatkan kemampuan pada lapangan yang sebenarnya sebelum memasuki dunia kerja. Praktik Kerja Profesi Apoteker (PKPA) dilaksanakan di PT Hexpharm Jaya Laboratories periode Januari - Februari 2022 dan Apotek Roxy Sawangan periode April 2022. The Professional Practice of Pharmacist (PKPA) is held at PT Hexpharm Jaya Laboratories for the January - February 2022 period and Apotek Roxy Sawangan for the April 2022 period. Prospective pharmacists are expected to be able to gain experience, knowledge, insight, and appropriate skills to carry out pharmaceutical work through the PKPA process in pharmaceutical idnsutry and pharmacy."

January 2022 - February 2022 and Roxy Sawangan Pharmacy for March 2022.

Through the implementation of this pharmacist professional work practice, the

future pharmacists are expected to be able to understand the competencies needed,

gain insight and be able to implement work practices in the field of pharmacy."

Tujuan penulisan ini adalah untuk memperoleh gambaran terkait kelengkapan resep di Apotek Roxy Depok berdasarkan pengkajian administratif, farmasetika, dan klinik periode Januari 2023. Metode pelaksanaan dilakukan secara retrospektif dengan melakukan pengkajian pada beberapa resep. Berdasarkan pengkajian, diperoleh bahwa resep telah memenuhi aspek administratif, farmasetik, dan klinik. Namun, masih ditemukan ketidaklengkapan terkait aspek administratif pada identitas dokter penulis resep.

A pharmacy is a pharmaceutical service facility where pharmaceutical practice is carried out by pharmacists. Reviewing and providing prescription services is the first activity in providing clinical pharmacy services. The purpose of this writing is to obtain an overview regarding the completeness of prescriptions at Apotek Roxy Depok based on administrative, pharmaceutical and clinical studies for the period January 2023. The implementation method is carried out retrospectively by reviewing several prescriptions. Based on the assessment, it was found that the prescription met administrative, pharmaceutical and clinical aspects. However,incompleteness was still found regarding the administrative aspects of the identity of the prescribing doctor."

"Migrain merupakan gangguan nyeri kepala primer dengan prevalensi tinggi dan cukup berdampak pada masalah sosial ekonomi. Studi yang dilakukan yaitu pengkajian kelengkapan resep serta analisis terhadap resep obat migrain yang diterima oleh Apotek Roxy Galaxy selama periode Januari 2023.Pengkajian dilakukan dengan mengumpulkan resep yang diterima selama bulan Januari tahun 2023 kemudian memilih dua resep resep yang memuat obat migran untuk dilakukan pengkajian resep.Pengkajian resep obat migrain yang diterima di Apotek Roxy selama periode bulan Januari 2023 ditemukan bahwa resep yang diterima sudah rasional namun kelengkapan administratif dan farmasetik tidak tercantum dengan lengkap. Resep yang diterima sudah rasional namun perlu dilakukan pengkajian resep dengan data yang lebih banyak dan periode pengkajian lebih panjang sehingga data yang diperoleh lebih banyak dan beragam.

Migraine is a primary headache disorder with a high prevalence and quite an impact on socio-economic problems. The study carried out was a review of the completeness of prescriptions and an analysis of migraine drug prescriptions received by Apotek Roxy Galaxy during the January 2023 period. A review of migraine drug prescriptions received at the Apotek Roxy during the January 2023 period found that the prescriptions received were rational but the administrative and pharmaceutical aspects were not listed completely. The prescriptions received are rational, but it is necessary to review the prescriptions with more data and a longer review period so that the data obtained is more numerous and varied."

Dari aspek klinis pada resep 1 perlu dilakukan penambahan aturan pakai medixon dan alopurinol sementara resep 2 terdapat potensi interaksi obat.

The use of drugs in large quantities can lead to an increased risk of medication errors (drug interactions and duplication of therapy), non-compliance and side effects of drugs, so it is necessary to study prescriptions, both administrative, pharmaceutical, and clinical to minimize the risk of medication errors due to the administration of polypharmacy drugs. This report aims to conduct a prescription review which includes administrative review, pharmaceutical review, and clinical review of polypharmacy prescriptions that enter the Roxy Depok Pharmacy. The method used is literature study. The results obtained show that the administrative aspects in the form of weight data and the doctor's telephone number need to be completed on both prescriptions and from the pharmaceutical aspect it is necessary to ask the prescribing doctor regarding the dosage form (prescription 1) and drug stability data (recipes 1 and 2). From a clinical aspect, prescription 1 needs to be added to the rules for using medixon and allopurinol, while prescription 2 has the potential for drug interactions."

"Pengkajian dan pelayanan resep serta dispensing merupakan bagian dari standar pelayanan farmasi klinik di apotek. Pelayanan resep yang teliti dengan waktu tunggu yang singkat menjadi salah satu upaya dalam meningkatkan kepuasan serta kenyamanan pasien. Evaluasi mengenai waktu tunggu pelayanan penting dilakukan sebagai salah satu indikator evaluasi mutu pelayanan kefarmasian di apotek untuk mengetahui kecepatan pelayanan farmasi dalam meningkatkan kepuasan juga kenyamanan pasien. Evaluasi dilakukan melalui observasi langsung dan pencatatan waktu tunggu pelayanan tiap resep obat jadi dan obat racikan di Apotek Roxy Poltangan. Hasil evaluasi menunjukkan rata-rata waktu pelayanan baik obat jadi maupun racikan sudah sesuai dan dapat dikatakan baik karena masih berada dalam rentang 15-30 menit. Faktor-faktor yang memengaruhi waktu pelayanan resep di Apotek Roxy Poltangan yaitu jenis resep, jumlah staf yang bertugas, jumlah obat yang diambil, dan sistem komputer yang digunakan.

Evaluation of waiting time for important services is carried out as an indicator for evaluating the quality of pharmaceutical services in pharmacies to determine the speed of pharmaceutical services in increasing patient satisfaction and comfort. Evaluation was carried out through direct observation and recording of waiting times for each finished drug prescription and concoction drug at the Roxy Poltangan Pharmacy. The evaluation results show that the average service time for both finished and concoction drugs is appropriate and can be said to be good because it is still in the range of 15-30 minutes. Factors that affect prescription service time at the Roxy Poltangan Pharmacy are the type of prescription, the number of staff on duty, the number of drugs taken, and the computer system used."
