"Diabetes melitus (DM) merupakan gangguan metabolisme kronis yang ditandai yang dengan tingginya kadar glukosa darah (hiperglikemia) akibat defisiensi sekresi insulin atau resistensi insulin. Penelitian epidemiologi menunjukkan adanya kecenderungan peningkatan angka insidensi dan prevalensi DM di berbagai penjuru dunia. Peningkatan pesien DM mempengaruhi peningkatkan pola peresepan obat dan risiko terjadinya medication error (ME). ME merupakan setiap kejadian yang dapat merugikan keselamatan pasien akibat pemakaian obat selama dalam pengawasan tenaga kesehatan atau pasien yang sebetulnya dapat dicegah. Salah satu upaya yang dapat dilakukan untuk pencegahan ME adalah pengkajian resep yang meliputi aspek administrasi, farmasetik, dan klinis. Penelitian ini bertujuan untuk melakukan pengkajian 5 resep pada pasien DM di Apotek Kimia Farma Raya Bogor pada periode April 2023. Pelaksanaan pengakajian resep dilakukan berdasarkan studi literatur dari resep pasien DM pada bulan April 2023. Hasil kajian 5 resep penyakit DM di Apotek Kimia Farma Raya Bogor menunjukkan bahwa pada aspek farmasetik dan klinik telah terpenuhi dan sesuai dengan literatur. Sedangkan pada aspek administrasi belum sesuai persyaratan yaitu tidak ada informasi alamat dan berat badan pasien pada seluruh resep.

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by high blood glucose levels (hyperglycemia) due to insulin secretion deficiency or insulin resistance. Epidemiological research shows a trend of increasing incidence and prevalence of DM in various parts of the world. The increase in DM patients influences the increase in drug prescribing patterns and the risk of medication errors (ME). ME is any event that can harm patient safety due to the use of drugs while under the supervision of health workers or patients that can actually be prevented. One effort that can be made to prevent ME is reviewing prescriptions which includes administrative, pharmaceutical and clinical aspects. This study aims to conduct a review of 5 prescriptions for DM patients at Kimia Farma Raya Bogor Pharmacy in the period April 2023. The prescription assessment was carried out based on a literature study of DM patient prescriptions in April 2023. The results of the study of 5 DM prescriptions at Kimia Farma Raya Bogor Pharmacy shows that the pharmaceutical and clinical aspects have been fulfilled and are in accordance with the literature. Meanwhile, the administration aspect does not meet the requirements there is no information on the patient's address and weight on all prescriptions."

"Pengkajian resep dilakukan untuk menganalisa adanya masalah terkait obat, bila ditemukan masalah terkait obat harus dikonsultasikan kepada dokter penulis resep. Apoteker harus melakukan pengkajian resep sesuai persyaratan administrasi, persyaratan farmasetik, dan persyaratan klinis. Praktik Kerja Profesi Apoteker (PKPA) di Apotek dilaksanakan selama 1 bulan, dimulai dari 1 April – 29 April 2022 di Apotek Kimia Farma 382 K2. Metode Pelaksanaan dilakukan secara deskriptif, kemudian dipilih sebanyak 10 (satu) resep untuk dianalisa. Berdasarkan dari resep tersebut terdapat duplikasi atau polifarmasi, hal ini dapat memicu terjadi medication error, mengingat informasi yang terdapat dalam aspek klinis berperan penting dalam rasionalitas terapi pengobatan pasien, seperti indikasi, aturan pakai, dosis, efek samping, kontra indikasi dan mekanisme kerja yang dapat mempengaruhi adanya interaksi obat pada resep pasien. Hal ini dapat terjadi karena dokter kurang berhati-hati dalam menulis resep yang diberikan, sehingga terjadi kesalahan pemberian (medication error). Untuk dapat mengatasi hal tersebut dapat diganti dengan obat alternatif lain yang tidak menimbulkan efek medication error sehingga aman diberikan kepada pasien agar mendapatkan hasil yang maksimal.

This can occur because doctors are less careful in writing the prescriptions given, resulting in medication error. To be able to overcome this, it can be replaced with other alternative drugs that do not cause the effect of medication errors so that it is safe to give to patients in order to get maximum results."

"Penyelenggaraan pelayanan kefarmasian oleh Apoteker di Apotek wajib mengikuti standar pelayanan kefarmasian yang tercantum dalam Peraturan Menteri Kesehatan Republik Indonesia No 73 tahun 2016. Implementasi penyelenggaraan pelayanan kefarmasian yang dilakukan oleh Apoteker di Apotek berawal dari kegiatan manajerial (menjamin ketersediaan dan pengelolaan sediaan farmasi, alat kesehatan yang aman, bermutu serta terjangkau) hingga pelayanan farmasi klinis yang berorientasi pada pasien untuk meningkatkan kualitas hidup pasien. Pelayanan swamedikasi terkait dengan keluhan batuk pilek merupakan salah satu pelayanan kefarmasian klinis yang umum dilakukan oleh Apoteker di Apotek. Dokumentasi pelayanan kefarmasian di Apotek Kimia Farma Jalan Raya Lenteng Agung No 39 dilakukan dengan melakukan observasi langsung dan membandingkan kesesuaiannya dengan peraturan yang berlaku. Berdasarkan hasil observasi, pelaksanaan kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan BMHP serta pelayanan swamedikasi di Apotek Kimia Farma Jl Raya Lenteng Agung No 39 sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016.

The implementation of pharmaceutical services by pharmacists at pharmacies must follow the pharmaceutical service standards listed in the Regulation of the Minister of Health of the Republic of Indonesia No. 73 of 2016. Implementation of pharmaceutical services by pharmacists at pharmacies starts with managerial activities (ensuring the availability and management of pharmaceutical preparations, medical devices safe, quality and affordable) to patient-oriented clinical pharmacy services to improve the quality of life of patients. Self-medication services related to complaints of cough and cold are one of the clinical pharmacy services that are commonly performed by pharmacists at pharmacies. Documentation of pharmaceutical services at Kimia Farma Pharmacy Jalan Raya Lenteng Agung No. 39 is carried out by direct observation and comparing compliance with applicable regulations. Based on the results of observations, the implementation of management activities for pharmaceutical preparations, medical devices and BMHP as well as self-medication services at Kimia Farma Pharmacy Jl Raya Lenteng Agung No 39 is quite good and in accordance with PMK No. 73 of 2016."

"Apoteker memiliki peran penting dalam pekerjaan kefarmasian. Salah satu hal penting yang perlu dilakukan untuk menjadi seorang apoteker profesional adalah berpartisipasi langsung dalam bidang praktik kefarmasian. The Professional Practice of Pharmacist is held at Apotek Kimia Farma period February 2022 and PT Mahakam Beta Farma period March – April 2022. Through the activities in the pharmacy and pharmaceutical industry, prospective pharmacists are expected to be able to get appropriate experience, knowledge, insight, and skills to perform pharmaceutical practice."

"Pelayanan dan pengkajian resep merupakan salah satu pelayanan farmasi klinik di apotek. Resep harus selalu ditulis dengan benar karena resep memiliki peran yang besar dalam terapi pengobatan dan kesehatan pasien. Sebagai penyakit yang termasuk ke dalam lima besar penyebab kematian di dunia dengan prevalensi 17,4%, obat asma banyak diresepkan kepada pasien sehingga dalam penulisan resepnya perlu diperhatikan untuk menghindari pemberian obat yang tidak rasional akibat dari kesalahan penulisan resep. Oleh karena itu, tugas khusus ini bertujuan untuk melakukan pengkajian resep obat pada pasien asma agar dapat mengetahui kesesuaian dan kelengkapan resep serta mendeteksi adanya kesalahan pada peresepan seperti adanya interaksi obat. Tugas khusus ini disusun melalui pengkajian resep untuk pasien asma yang menggunakan layanan BPJS. Setiap resep yang diterima akan dilakukan skrining resep terkait dengan kelengkapan administrasinya. Setelahnya resep diserahkan kepada tim penyiapan resep untuk pengambilan obatnya dari rak, kemudian penulisan etiket, dan terakhir penulisan kelengkapan lainnya seperti copy resep (jika ada) dan Kartu Kendali untuk pasien BPJS. Setelahnya akan dilakukan pengkajian bagi resep yang mengandung obat-obatan asma sesuai persyaratan farmasetik dan klinisnya. Dua buah resep yang dikaji pada tugas khusus ini belum memenuhi seluruh aspek administrasi, farmasetik, dan klinis.

Therefore, this special task aims to assess drug prescriptions for asthma patients to determine the suitability and completeness of the prescription and detect errors in prescribing such as drug interactions. This special task is prepared by reviewing prescriptions for asthma patients who use BPJS services. Every prescription received will be screened regarding the completeness of its administration. After that, the prescription is handed over to the prescription preparation team to take the medicine from the shelf, then write the label, and finally write other equipment such as a copy of the prescription (if any) and a Control Card for BPJS patients. After that, a review will be carried out for prescriptions containing asthma medicines according to pharmaceutical and clinical requirements. The two prescriptions studied in this special assignment did not meet all administrative, pharmaceutical, and clinical aspects."

"Pelayanan kefarmasian di apotek merupakan pelayanan yang bersentuhan langsung dengan masyarakat, sehingga dituntut untuk mampu menjamin ketersediaan obat yang aman, bermutu, dan berkhasiat. Diabetes mellitus (DM) merupakan gangguan metabolisme kronis yang ditandai dengan terjadinya hiperglikemia persisten. Terapi pasien DM tipe dua bertujuan untuk mempertahankan tingkat konsentrasi glukosa dalam darah dalam batas normal, dan untuk mencegah berkembangnya komplikasi jangka panjang dari kondisi diabetes. Penulisan Tugas Khusus ini dilakukan agar penulis lebih memahami secara mendalam terkait analisis dan pengkajian resep di apotek, khususnya resep pasien diabetes. Metode pelaksanaan analisis resep dilakukan dengan pendekatan pengkajian, pelayanan, dan screening resep terhadap resep pasien diabetes di KFA Matraman sesuai dengan yang tertera pada buku Petunjuk Teknis Standar Pelayanan Kefarmasian di Apotek. Kesimpulan yang didapat dari analisis resep pasien diabetes kali ini yaitu secara umum seringkali terdapat kombinasi pengunaan obat pada pasien (polifarmasi), sehingga potensi terjadi interaksi obat pada pasien dapat meningkat. Menyikapi hal ini, apoteker perlu memastikan setiap kombinasi terapi yang diterima pasien merupakan kombinasi terapi yang rasional, sehingga efektif untuk menunjang kesembuhan pasien.

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by persistent hyperglycemia. Therapy for type 2 DM patients aims to maintain blood glucose concentration levels within normal limits, and to prevent the development of long-term complications from diabetes. This Special Assignment was written so that the author could get better understanding in the analysis and assessment of prescriptions in pharmacies, especially prescriptions for diabetes patients. The analysis of diabetes prescription in this assignment was carried out using the approach of assessment, service, and screening of prescriptions for diabetes patients at KFA Matraman in accordance with that stated in the Technical Instructions for Standard Pharmaceutical Services in Pharmacies. The conclusion obtained from the analysis of diabetes patient prescriptions this time is that in general there is often a combination of drug use in patients (polypharmacy), so that the potential for drug interactions in patients can increase. In response to this, pharmacists need to ensure that every combination of therapy received by patients is a rational combination of therapy, so that it could be effective in supporting patient recovery."

"Seorang apoteker memegang peranan penting di industri farmasi, apotek dan pemerintahan. Apoteker harus memenuhi standar kompetensi sebagai persyaratan untuk memasuki dunia kerja dan menjalani praktek profesi. standar kompetensi apoteker Indonesia terdiri dari sepuluh (10) standar kompetensi sebagai kemampuan yang diharapkan oleh apoteker saat lulus dan masuk ke tempat praktik kerja profesi. Sebagai bekal dan pengalaman calon apoteker untuk dapat memahami peran apoteker dan meningkatkan kompetensi, maka dilaksanakan Praktik Kerja Profesi Apoteker di PT Aventis Pharma, Badan POM, Apotek Kimia Farma selama periode bulan Januari - April 2019. Selama PKPA, diharapkan calon apoteker dapat memperluas wawasan, pemahaman, dan pengalaman untuk melakukan pekerjaan kefarmasian di tempat praktek kerja profesi.

Indonesian pharmacist competency standards consist of ten (10) competency standards as abilities expected by pharmacists when they graduate and enter professional work practices. As a preparation and experience of prospective pharmacists to be able to understand the role of pharmacists and improve competence, the Pharmacist Professional Work Practices were implemented at PT Aventis Pharma, BPOM, Kimia Farma Pharmacy during the period January - April 2019. During PKPA, it is expected that prospective pharmacists can broaden their horizons, understanding, and experience to do pharmaceutical work in professional workplaces."

"Standar pelayanan kefarmasian yang diatur dalam PMK No. 73 Tahun 2016 merupakan pedoman yang harus diimplementasikan di apotek untuk mengoptimalkan kesehatan di komunitas. Laporan ini mendeskripsikan implementasi pelayanan kefarmasian dan pengkajian resep pasien diabetes melitus tipe 2 dan hipertensi di apotek kimia farma 002 periode november 2022. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan BMHP di Apotek Kimia Farma 002 sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016. Pengkajian yang dilakukan pada satu resep belum memenuhi aspek kelengkapan resep, yaitu aspek administratif dan pertimbangan klinis. Petugas apotek perlu menghubungi pihak dokter atau rumah sakit untuk mengkonfirmasi resep. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan pelayanan swamedikasi di Apotek Kimia Farma 002 sudah cukup baik. Penggalian informasi pasien yang dilakukan sudah sesuai dengan literatur. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan PIO sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016, namun tidak didokumentasikan dengan baik.

The standard of pharmaceutical services regulated in Ministerial Decree No. 73 of 2016 is a guideline that must be implemented in pharmacies to optimize community health. The assessment conducted on a prescription did not meet the completeness aspects of a prescription, namely administrative aspects and clinical considerations. Pharmacy staff need to contact doctors or hospitals to confirm the prescription. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of self-medication services at Chemist Pharmacy 002 is already quite good. Patient information gathering is in accordance with the literature. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of Pharmaceutical Care Program activities is already quite good and in accordance with Ministerial Decree No. 73 of 2016, but it is not well documented."

"Apotek merupakan salah satu sarana kegiatan pelayanan kefarmasian yang bertujuan untuk meningkatkan kualitas hidup pasien. Pelayanan farmasi klinik di apotek mencakup pengkajian resep, dispensing, Pelayanan Informasi Obat (PIO), konseling, pelayanan kefarmasian di rumah (home pharmacy care), Pemantauan Terapi Obat (PTO), dan Monitoring Efek Samping Obat (MESO). Program Rujuk Balik (PRB) merupakan pelayanan kefarmasian yang diberikan oleh apotek Kimia Farma 007 Djuanda, dimana pemberian obat-obatan kepada pasien penyakit kronis dengan kondisi stabil. Diabetes Melitus merupakan gangguan metabolik yang ditandai dengan peningkatan kadar glukosa darah melebihi normal, menjadi salah satu jenis penyakit kronik yang dilayani pada PRB. Tujuan dari observasi ini adalah untuk mengetahui kesesuaian pelayanan kefarmasian di Apotek Kimia Farma 007 Djuanda Bogor dengan standar pelayanan kefarmasian di Apotek, serta melakukan pengkajian resep pasien diabetes melitus Apotek Kimia Farma 007 Djuanda. Pengkajian resep dilakukan berdasarkan kelengkapan pada aspek administratif, farmasetik dan klinis, kemudian ditentukan apakah pemberian resep tersebut telah memenuhi persyaratan aspek atau belum. Berdasarkan hasil observasi dapat disimpulkan bahwa pelayanan kefarmasian yang diterapkan di Apotek Kimia Farma 007 Djuanda meliputi pengkajian resep, dispensing obat, PIO, konseling, pelayanan apotek digital sebagai pelayanan home pharmacy care, PTO, dan MESO serta hasil pengkajian resep terdapat belum memenuhi kelengkapan pada aspek administrasi dan aspek klinis berupa data diri pasien, alergi pasien, cara pemberian obat, duplikasi obat, dan interaksi obat.

The Reverse Referral Program (PRB) is a pharmaceutical service provided by Kimia Farma 007 Djuanda pharmacy, which provides medicines to patients with chronic diseases with stable conditions. Diabetes Mellitus is a metabolic disorder characterized by an increase in blood glucose levels beyond normal, being one type of chronic disease served in PRB. The purpose of this observation is to determine the suitability of pharmaceutical services at the Kimia Farma 007 Djuanda Pharmacy in Bogor with the pharmaceutical service standards at the Pharmacy, and to assess the prescriptions of patients with diabetes mellitus at the Kimia Farma 007 Djuanda Pharmacy. Prescription review is carried out based on completeness in administrative, pharmaceutical and clinical aspects, then it is determined whether the prescription has fulfilled the required aspects or not. Based on the results of observations, it can be concluded that the pharmaceutical services applied at the Kimia Farma 007 Djuanda Pharmacy include prescription review, drug dispensing, PIO, counseling, digital pharmacy services as home pharmacy care services, PTO, and MESO and the results of the prescription review do not yet complete of the administration and clinical aspects in the form of patient personal data, patient allergies, how to administer drugs, drug duplication, and drug interactions."
