"Diabetes Melitus (DM) merupakan penyakit tidak menular yang memerlukan pengobatan yang kompleks. Banyaknya risiko komplikasi pada pasien DM menyebabkan peningkatan jumlah obat-obat yang harus dikonsumsi oleh pasien yang mengarah pada polifarmasi. Pemberian obat polifarmasi memiliki risiko yang tinggi terhadap terjadinya Drug Related Problems (DRPs) yang dapat membahayakan pasien. Laporan ini bertujuan untuk mengkaji resep obat pada pasien DM di Apotek Kimia Farma Kejayaan untuk mengetahui ada tidaknya DRPs. Pengumpulan informasi dilakukan dengan studi literatur terhadap setiap obat yang diresepkan. Pada hasil pengkajian resep, diperoleh masih banyaknya aspek yang belum terpenuhi, yaitu aspek administratif, berat badan, usia, nomor telepon dan SIP dokter perlu dicantukan. Pada aspek farmasetik bentuk sediaan dan data stabilitas perlu ditanyakan kembali kepada dokter penulis resep. Sementara pada aspek klinis, perlu dicek kembali dosis obat (resep 1), penambahan aturan pakai dan risiko interaksi obat yang dapat mengarah pada efek samping yang serius terhadap pasien.

Diabetes Mellitus (DM) is a non-communicable disease that requires complex treatment. The many risks of complications in DM patients cause an increase in the number of drugs that must be consumed by patients which leads to polypharmacy. Administering polypharmacy drugs has a high risk of developing Drug Related Problems (DRPs) which can harm the patient. This report aimed to assest drug prescriptions in DM patients at Kimia Farma Kejayaan Pharmacy to determine whether DRPs were present. Information collection was carried out by studying the literature on each drug prescribed. Based on the results of the review of prescriptions, it was found that there were still many aspects that had not been fulfilled, namely administrative aspects, weight, age, telephone number and doctor's SIP that needed to be included. Regarding the pharmaceutical aspects of dosage forms and stability data, it is necessary to ask the prescribing doctor again. While on the clinical aspect, it is necessary to re-check the dosage of the drug (recipe 1), add the rules for use and the risk of drug interactions which can lead to serious side effects in patients."

"Praktik kerja profesi apoteker di Apotek Kimia Farma 202 Kejayaan bertujuan mengkaji resep obat dari salah satu resep pasien diabetes melitus di Apotek Kimia Farma Kejayaan bulan Mei 2023 dari segi administrasi, farmasetik dan klinis. Serta memahami permasalahan yang ditemukan dari hasil pengkajian resep di Apotek Kimia Farma Kejayaan bulan Mei 2023. Metode pelaksanaan yang dilakukan dalam laporan ini adalah studi literatur. Resep yang diperoleh adalah dari resep bulan Mei tahun 2023 di Apotek Kimia Farma 202 Kejayaan. Resep dipilih karena kompleksitas yang ada di dalam resep dan dapat dijadikan bahan ajar tentang pengkajian resep. Pengkajian salah satu resep di Apotek Kimia Farma 202 Kejayaan dilihat dari aspek administratif, yaitu melihat kelengkapan data pasien, dokter, serta obat yang diberikan pada resep. Aspek farmasetik dilihat dengan menyesuaikan nama obat serta kekuatan obat dengan literature. Aspek klinis dilakukan untuk melihat indikasi, dosis, efek samping, kontra indikasi, dan potensi interaksi obat yang ada pada resep. Permasalahan yang sering ditemukan pada penulisan resep yaitu terkait aspek administrasi masih ditemukan beberapa poin yang jarang dicantumkan dalam penulisan resep. Ada pula permasalahan terkait potensi interaksi obat yang harus diwaspadai yang dapat terjadi kepada pasien.

The professional pharmacist work practice at Apotek Kimia Farma 202 Kejayaan aims to review a prescription from a diabetes mellitus patient at Apotek Kimia Farma Kejayaan in May 2023 in terms of administrative, pharmaceutical, and clinical aspects. Additionally, it seeks to understand the issues found from the prescription review at Apotek Kimia Farma Kejayaan in May 2023. The implementation method used in this report is literature study. The prescription reviewed is from May 2023 at Apotek Kimia Farma 202 Kejayaan. This prescription was chosen due to its complexity and can be used as a teaching material for prescription review. The review of the prescription at Apotek Kimia Farma Kejayaan is conducted from three aspects: Administrative aspect involves checking the completeness of the patient data, doctor information, and the medications prescribed. Pharmaceutical aspect involves verifying the drug names and their strengths against the literature. Clinical aspect involves assessing the indications, dosage, side effects, contraindications, and potential drug interactions in the prescription. Common issues found in prescription writing include some administrative details that are often omitted. Additionally, there are potential drug interaction problems that need to be closely monitored to prevent adverse effects on the patient."

"Hipertensi atau tekanan darah tinggi merupakan salah satu penyakit sistem kardiovaskular yang memiliki tingkat prevalensi tinggi di Indonesia. Untuk itu dibutuhkan terapi yang efektif dalam pengobatan penyakit tersebut. Pada kebanyakan kasus, dokter banyak memberikan terapi polifarmasi terutama pada pasien yang lebih tua dengan komorbiditas. Hal ini menyebabkan kemungkinan dapat terjadinya permasalahan terkait pengobatan diantaranya: obat yang tidak tepat, dosis obat yang berlebih atau kurang, cara pakai yang tidak tepat, inkompatibilitas obat, alergi obat, duplikasi obat, interaksi obat, dan sebagainya. Oleh karena itu, untuk mencegah terjadinya permasalahan pengobatan tersebut, penting bagi seorang apoteker untuk melakukan pengkajian resep. Penelitian ini bertujuan untuk memberikan gambaran pengkajian resep pada pasien dengan hipertensi yang menebus obat di Apotek Kimia Farma 202 Kejayaan. Penyusunan laporan ini dilakukan melalui hasil pencarian studi literatur dan juga analisis pengkajian resep yang dilakukan pada resep dari pasien yang menerima resep obat hipertensi di Apotek Kimia Farma 202 Kejayaan. Hasil pengkajian resep pada pasien dengan hipertensi menunjukkan terdapatnya informasi yang belum dicantumkan pada salinan resep. Selain itu, terdapat interaksi terhadap obat-obatan yang diresepkan oleh dokter tersebut, sehingga untuk mencegah terjadinya interaksi obat dapat dilakukan dengan memisahkan waktu konsumsi antara obat yang berinteraksi tersebut. Hasil laporan pengkajian resep ini dapat dijadikan sebagai sumber informasi untuk apoteker dalam melakukan proses pengkajian resep.

Hypertension or high blood pressure is a disease of the cardiovascular system that has a high prevalence rate in Indonesia. For this reason, effective therapy is needed in the treatment of this disease. In most cases, doctors provide polypharmacy therapy, especially in older patients with comorbidities. This causes the possibility of problems related to treatment, including: inappropriate medication, excessive or insufficient dosage of medication, incorrect method of use, drug incompatibility, drug allergies, drug duplication, drug interactions, and so on. Therefore, to prevent these medication problems from occurring, it is important for a pharmacist to review the prescription. This study aims to provide an overview of prescription assessments in patients with hypertension who redeem medication at Kimia Farma 202 Kejayaan Pharmacy. The preparation of this report was carried out through the results of a literature study search and also analysis of prescription studies carried out on prescriptions from patients who received prescriptions for hypertension medication at Kimia Farma 202 Kejayaan Pharmacy. The results of reviewing prescriptions for patients with hypertension showed that there was information that had not been included in the prescription copy. Apart from that, there are interactions with the drugs prescribed by the doctor, so to prevent drug interactions you can do this by separating the consumption time between the interacting drugs. The results of this prescription review report can be used as a source of information for pharmacists in carrying out the prescription review process."

"Pelayanan dan pengkajian resep merupakan salah satu pelayanan farmasi klinik di apotek. Resep harus selalu ditulis dengan benar karena resep memiliki peran yang besar dalam terapi pengobatan dan kesehatan pasien. Sebagai penyakit yang termasuk ke dalam lima besar penyebab kematian di dunia dengan prevalensi 17,4%, obat asma banyak diresepkan kepada pasien sehingga dalam penulisan resepnya perlu diperhatikan untuk menghindari pemberian obat yang tidak rasional akibat dari kesalahan penulisan resep. Oleh karena itu, tugas khusus ini bertujuan untuk melakukan pengkajian resep obat pada pasien asma agar dapat mengetahui kesesuaian dan kelengkapan resep serta mendeteksi adanya kesalahan pada peresepan seperti adanya interaksi obat. Tugas khusus ini disusun melalui pengkajian resep untuk pasien asma yang menggunakan layanan BPJS. Setiap resep yang diterima akan dilakukan skrining resep terkait dengan kelengkapan administrasinya. Setelahnya resep diserahkan kepada tim penyiapan resep untuk pengambilan obatnya dari rak, kemudian penulisan etiket, dan terakhir penulisan kelengkapan lainnya seperti copy resep (jika ada) dan Kartu Kendali untuk pasien BPJS. Setelahnya akan dilakukan pengkajian bagi resep yang mengandung obat-obatan asma sesuai persyaratan farmasetik dan klinisnya. Dua buah resep yang dikaji pada tugas khusus ini belum memenuhi seluruh aspek administrasi, farmasetik, dan klinis.

Therefore, this special task aims to assess drug prescriptions for asthma patients to determine the suitability and completeness of the prescription and detect errors in prescribing such as drug interactions. This special task is prepared by reviewing prescriptions for asthma patients who use BPJS services. Every prescription received will be screened regarding the completeness of its administration. After that, the prescription is handed over to the prescription preparation team to take the medicine from the shelf, then write the label, and finally write other equipment such as a copy of the prescription (if any) and a Control Card for BPJS patients. After that, a review will be carried out for prescriptions containing asthma medicines according to pharmaceutical and clinical requirements. The two prescriptions studied in this special assignment did not meet all administrative, pharmaceutical, and clinical aspects."

"Diabetes melitus (DM) telah menjadi ancaman bagi kesehatan global dengan prevalensi penderita DM baru di Indonesia mencapai 25% tahun 2018. Salah satu penanganan penyakit DM ini dapat dilakukan dengan pelayanan farmasi klinik langsung oleh apoteker di apotek untuk meningkatkan outcome terapi dan meminimalkan risiko terjadinya efek samping obat. Apoteker harus membangun hubungan dengan apotek dan fasilitas kesehatan lain untuk memudahkan komunikasi, kerjasama dan konfirmasi terkait pelayanan resep pasien. Pelayanan obat dari pasien Program Rujuk Balik dengan jaminan BPJS Kesehatan, terbatas pada jumlah dan lama pemakaian obat mengikuti pedoman Formularium Nasional sehingga peresepan obat rasional. Tujuan penyusunan tugas khusus ini adalah untuk mengkaji resep pasien rujuk balik BPJS penyakit diabetes melitus tipe 2 di apotek Kimia Farma 48 Matraman periode Oktober 2023 berdasarkan kajian administratif, kesesuaian farmasetik, dan pertimbangan klinis. Metode pelaksanaan tugas khusus ini menggunakan sampel berupa resep pasien rujuk balik BPJS dengan penyakit diabetes melitus tipe 2 di Apotek Kimia Farma 48 Matraman. Berdasarkan hasil pengkajian resep pasien rujuk balik BPJS penyakit diabetes melitus tipe 2 di Apotek Kimia Farma 48 Matraman, diperoleh bahwa sebagian besar resep pasien telah memenuhi kajian administratif, kesesuaian farmasetik, dan pertimbangan klinis namun masih terdapat beberapa data yang kurang lengkap khususnya dari aspek administratif seperti data jenis kelamin pasien, berat badan pasien, nomor telepon dan paraf dokter.

Pharmacists must build relationships with pharmacies and other health facilities to communicate, collaborate, and confirm patient prescription services. Drug services for Refer-Back Program patients with BPJS Health Insurance are limited to the quantity and duration of drug use following the National Formulary Guidelines, for which drug prescribing is rational. The purpose of this special assignment is to review prescriptions for BPJS referral patients for type 2 diabetes mellitus at Kimia Farma 48 Matraman pharmacy for the period October 2023 based on administrative studies, pharmaceutical suitability, and clinical considerations. The method for carrying out this special assignment uses samples in the form of prescriptions from BPJS referral patients with type 2 diabetes mellitus at Kimia Farma 48 Matraman Pharmacy. Based on the results of the review of BPJS referral patient prescriptions for type 2 diabetes mellitus at Kimia Farma 48 Matraman Pharmacy, it was found that the majority of patient prescriptions had met administrative review, pharmaceutical suitability, and clinical considerations, but there were still some incomplete data, especially from administrative aspects such as data on patient gender, patient weight, contact information, and doctor's initials."

Tujuan dari observasi ini adalah untuk mengetahui kesesuaian pelayanan kefarmasian di Apotek Kimia Farma 007 Djuanda Bogor dengan standar pelayanan kefarmasian di Apotek, serta melakukan pengkajian resep pasien diabetes melitus Apotek Kimia Farma 007 Djuanda. Pengkajian resep dilakukan berdasarkan kelengkapan pada aspek administratif, farmasetik dan klinis, kemudian ditentukan apakah pemberian resep tersebut telah memenuhi persyaratan aspek atau belum. Berdasarkan hasil observasi dapat disimpulkan bahwa pelayanan kefarmasian yang diterapkan di Apotek Kimia Farma 007 Djuanda meliputi pengkajian resep, dispensing obat, PIO, konseling, pelayanan apotek digital sebagai pelayanan home pharmacy care, PTO, dan MESO serta hasil pengkajian resep terdapat belum memenuhi kelengkapan pada aspek administrasi dan aspek klinis berupa data diri pasien, alergi pasien, cara pemberian obat, duplikasi obat, dan interaksi obat.

The purpose of this observation is to determine the suitability of pharmaceutical services at the Kimia Farma 007 Djuanda Pharmacy in Bogor with the pharmaceutical service standards at the Pharmacy, and to assess the prescriptions of patients with diabetes mellitus at the Kimia Farma 007 Djuanda Pharmacy. Prescription review is carried out based on completeness in administrative, pharmaceutical and clinical aspects, then it is determined whether the prescription has fulfilled the required aspects or not. Based on the results of observations, it can be concluded that the pharmaceutical services applied at the Kimia Farma 007 Djuanda Pharmacy include prescription review, drug dispensing, PIO, counseling, digital pharmacy services as home pharmacy care services, PTO, and MESO and the results of the prescription review do not yet complete of the administration and clinical aspects in the form of patient personal data, patient allergies, how to administer drugs, drug duplication, and drug interactions."

"Salah satu standar pelayanan kefarmasian yang dilakukan di apotek adalah pengkajian resep. Pengkajian resep bertujuan untuk menganalisis adanya masalah yang terkait dengan peresepan obat. Hal ini dapat diatasi dengan konfirmasi oleh petugas farmasi atau apoteker. Berdasarkan pengkajian aspek klinis terdapat adanya polifarmasi dan interaksi obat. Hal tersebut dapat diatasi dengan memberikan edukasi atau pemberian informasi obat kepada pasien.

One of the standard pharmaceutical services carried out in pharmacies is prescription review. The prescription review aims to analyze the existence of problems related to drug prescribing. Drug-related problems that may occur due to prescription errors include incorrect medication, insufficient or excessive dosage, drug allergies, drug incompatibility, drug interactions, duplication of medication, and inappropriate drug use. The prevalence of diabetes mellitus in Indonesia has increased from 1.5% in 2013 to 2.0% in 2018 based on doctor's diagnosis in residents aged ≥ 15 years. Handling diabetes using a patient approach requires collaboration between health workers, in this case the doctor as the determiner of the diagnosis, the pharmacist assisting, especially in drug therapy, is one of the duties of the pharmaceutical profession. The administrative review of prescriptions carried out on prescriptions contained several administrative aspects that were incomplete, such as the patient's gender, weight, and the doctor's telephone number not being listed on the prescription. This can be overcome by confirmation by a pharmacy officer or pharmacist. Based on the clinical aspect assessment, there is polypharmacy and drug interactions. This can be overcome by providing education or providing drug information to patient."

"Penggunaan obat yang rasional adalah faktor penting dalam keberhasilan pelayanan kefarmasian. Pengkajian resep obat asma di Apotek Kimia Farma 202 Kejayaan Depok dilakukan untuk menilai kesesuaian resep dengan standar yang berlaku. Penelitian ini bertujuan untuk mengkaji resep obat asma dari segi administrasi, farmasetik, dan klinis serta memahami permasalahan yang muncul dari pengkajian resep tersebut. Pengkajian dilakukan terhadap resep asma yang diterima di Apotek Kimia Farma 202 selama bulan Juli 2023. Data dikumpulkan melalui observasi langsung dan wawancara dengan apoteker. Analisis dilakukan berdasarkan aspek administrasi, farmasetik, dan klinis sesuai Peraturan Menteri Kesehatan No. 73 Tahun 2016. Hasil analisis menunjukkan bahwa sebagian besar resep sudah memenuhi standar administrasi, namun terdapat beberapa kekurangan dalam aspek farmasetik dan klinis, seperti ketidaktepatan dosis dan bentuk sediaan obat yang tidak tercantum dengan lengkap. Peningkatan kualitas pelayanan resep di Apotek Kimia Farma 202 dapat dicapai melalui peningkatan pengawasan dan edukasi kepada tenaga kesehatan mengenai penulisan resep yang lengkap dan jelas.

Rational drug use is a critical factor in the success of pharmaceutical services. The evaluation of asthma medication prescriptions at Apotek Kimia Farma 202 Kejayaan Depok was conducted to assess the conformity of prescriptions with applicable standards. This study aims to review asthma drug prescriptions in terms of administration, pharmaceutics, and clinical aspects, as well as to understand the issues arising from this evaluation. The review was carried out on asthma prescriptions received at Apotek Kimia Farma 202 during July 2023. Data were collected through direct observation and interviews with pharmacists. Analysis was conducted based on administrative, pharmaceutical, and clinical aspects in accordance with Minister of Health Regulation No. 73 of 2016. The results of the analysis indicated that most prescriptions met administrative standards; however, there were some deficiencies in pharmaceutical and clinical aspects, such as inaccurate dosages and incomplete drug formulations. Improving the quality of prescription services at Apotek Kimia Farma 202 can be achieved through enhanced supervision and education for healthcare professionals regarding the writing of complete and clear prescriptions."

"Pemantauan terapi obat adalah salah satu metode untuk memastikan ketepatan penggunaan obat. Tujuan dari kegiatan ini adalah untuk memastikan terapi obat yang aman, efektif, dan rasional bagi pasien. Kimia Farma apotek merupakan perusahaan yang bergerak dalam bisnis penjualan obat dan alat kesehatan yang memiliki 1300 cabang di seluruh Indonesia. Dalam pembangunannya, terdapat beberapa outlet yang disertai dengan klinik dan dokter umum serta dokter spesialis. Oleh karena itu, diperlukan pemantauan terapi obat terhadap pasien yang mendapatkan terapi yang dilakukan di klinik dan penyiapan obat di kimia farma apotek. Pengambilan data diambil secara retrospektif terhadap data sekunder, yaitu resep pasien yang dilakukan pelayanan resep Ny. K. di apotek kimia farma. Berdasarkan hasil pemantauan terapi obat yang telah dilakukan, dapat ditarik kesimpulan bahwa pengobatan pada pasien Ny. K sudah sesuai indikasi, akan tetapi terdapat permasalahan terkait obat seperti potensi interaksi, ketidak sesuaian dosis dan interval penggunaan.

Kimia Farma Pharmacy is a company engaged in the business of selling drugs and medical devices which has 1300 branches throughout Indonesia. During its development, there are several outlets accompanied by clinics and general practitioners and specialist doctors. Therefore, it is necessary to monitor drug therapy for patients who receive therapy carried out in clinics and preparation of drugs in pharmacy pharmacies. Data collection was taken retrospectively on secondary data, namely patient prescriptions for Ny. K. in a chemical pharmacy pharmacy. Based on the results of monitoring drug therapy that has been carried out, it can be concluded that the treatment of Mrs. K is indicated according to indications, but there are problems related to drugs such as potential interactions, mismatched doses and intervals of use."
