"Praktek residensi keperawatan medikal bedah pada sistem muskuloskeletal bertujuan untuk mengaplikasikan peran perawat sebagai pemberi asuhan, pendidik, pengelola, peneliti dan agen pembaharu. Perawat spesialis sebagai pemberi asuhan dengan melakukan pengeloaan pasien yang menjadi kasus utama dan resume. Kasus utama diberikan pada pasien dengan multiple fraktur dengan pendekatan teori model Virginia Henderson. Perawat spesialis sebagai peneliti dengan menerapkan Evidence-Based Nursing Practice dalam melakukan tindakan keperawatan. Penerapan terapi musik menjadi salah satu pilihan tindakan keperawatan yang berbasis bukti untuk mengurangi kecemasan pada pasien rawat inap ortopedi. Sedangkan perawat spesialis sebagai agen pembaharu dilakukan dengan menerapkan program Enhanced Recovery After Surgery pada pasien dengan Total Hip Replacement. Rangkaian kegiatan yang dilakukan dalam prakteik residensi bertujuan untuk meningkatkan kualitas pelayanan asuhan keperawatan yang holistik.

Clinical practice of Medical-Surgery Nursing residency in orthopaedic nursing aims to apply the role of nurses as patient care providers, educators, manager, researchers and reform agents. Nurse specialists as providers of care by conducting patient management which is the main case and resume. The main case was given to patients with multiple fractures with Virginia Hendersons model theory approach. Nursing specialists as researchers by applying Evidence Based Nursing Practice in caryying out nursing actions. The application of the music therapy is one of the choices of evidence based nursing actions to reduce anxiety in erthopaedic inpatients. Nurse specialists as reform agents are carried out by implementing enhanced recovery after surgery programs in patients with Total Hip Replacement. The series of activities carried our in the residency practice aims to improve the quality of holistic nursing care services."

"Asuhan keperawatan yang diberikan oleh perawat sangat mempengaruhi kualitas pelayanan kesehatan yang akan diterima oleh pasien. Upaya yang dilakukan untuk meningkatkan kualitas asuhan keperawatan tersebut salah satunya dengan menerapkan peran ners spesialis. Tujuan penulisan Karya Ilmiah Akhir (KIA) ini adalah mampu menampilkan peran ners spesialis sebagai pemberi asuhan keperawatan lanjut, pembaharu dan peneliti. Metodologi dan pendekatan yang digunakan adalah dengan menggunakan studi literature. Karya Ilmiah Akhir berisi tentang : penerapan teori Virginia Henderson pada pasien dengan Neglected Fracture Left Shaft Femur, penerapan Progressive Muscle Relaxation (PMR) sebagai Evidance Based Nursing dan proyek inovasi yaitu Aplikasi Peran Perawat dalam Program Enhanced Recovery Orthopedic Surgery (EROS) pada Pasien Operasi Total Hip Replacement (THR). Hasil : Penerapan teori Virginia Henderson efektif diterapkan pada kasus muskuloskeletal. Intervensi Progressive Muscle Relaxation (PMR) juga efektif dalam menurunkan Anxiety pasien. Kesimpulan : bahwa teori Virginia Henderson dapat digunakan pada pasien dengan Neglected Fracture Left Shaft Femur untuk meningkatkan kemandirian akibat perubahan fisik dan psikologis. Intervensi Progressive Muscle Relaxation (PMR) dapat digunakan sebagai salah satu alternatif intervensi keperawatan mandiri dalam membantu mengurangi Anxiety pasien fraktur ekstremitas yang akan di lakukan tindakan operasi elektif.

The purpose of this Final Scientific Writing is able to show the role of specialist ners as nurse care nurse, change agent and researcher Methodology and approach used is by using literature study. The final work contains: the application of Virginia Hendersons theory to patients with Neglected Fracture Left Shaft Femur, and the Application Progressive Muscle Relaxation as Evidance Based Nursing and innovation projects Application of the Role of Nurses in Enhanced Recovery Orthopedic Surgery (EROS) in Patient Surgery Total Hip Replacement (THR). Results: The application of Virginia Hendersons theory was effectively applied to musculoskeletal cases. The intervention of Progressive Muscle Relaxation (PMR) is also effective in reducing patient anxiety. Conclusion: that Virginia Hendersons theory is appropriately used in patients with Neglected Fracture Left Shaft Femur to improve independence due to physical and psychological changes. The intervention of Progressive Muscle Relaxation (PMR) can be used as one of the alternative independent nursing interventions to help reduce Anxiety patients in limb fracture elective surgery."

"Praktik residensi keperawatan medikal bedah adalah rangkaian dari program pendidikan magister keperawatan yang dilaksanakan dalam tatanan pelayanan keperawatan. Keperawatan medikal bedah berfokus pada konsep dan prinsip dasar medikal dan bedah dalam penerapan ilmu dan tehnologi keperawatan untuk memenuhi kebutuhan pasien dewasa yang mengalami perubahan fisik dengan atau tanpa gangguan struktural. Praktik residensi keperawatan dilaksanakan di RSPAD Gatot Soebroto dan RSUP Fatmawati. Kegiatan Residensi terdiri dari pemberian asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Virginia Henderson. Asuhan keperawatan telah diberikan pada pasien dengan fraktur, osteoarthritis, SCI, osteosarcoma, spondylitis TB dan beberapa kasus lainnya. Pendekatan teori Henderson membantu pasien untuk memenuhi kebutuhannya selama mengalami gangguan muskuloskeletal. Penerapan evidence based nursing (EBN) melalui intervensi edukasi pada pasien yang akan menjalani operasi tulang belakang membantu pasien dalam menurunkan kecemasan dan nyeri. Intervensi edukasi membantu pemahaman pasien dalam menghadapi operasi sehingga mengurangi kecemasan dan dapat mengelola nyeri dengan lebih baik. Proyek inovasi melalui aplikasi peran perawat dalam program Enhace Recovery for Orthopedic Surgery (EROS) pada pasien operasi Total Hip Replacement (THR). Aplikasi peran perawat membantu pasien dalam persiapan operasi sampai dengan rehablitasi paska operasi.

The residency program in medical medical nursing is a series of nursing master education programs carried out in the order of nursing services. The scope of the implementation of the residency program is the achievement of clinical competencies and the application of the role of specialist nurses in advanced nursing practice. This process was carried out with the application of Virginia Henderson's Nursing theory approach to the oncology system, the application of Evidence Based Nursing music therapy to reduce cancer pain, and the application of innovation to the ERAS protocol in mastectomy and thyroidectomy patients at Cipto Mangunkusumo General Hospital, Jakarta.

The Henderson virginian theory approach is very suitable to be applied in meeting the needs of patients with complex malignancies because it is comprehensive and holistic including physical, psychological, social, and spiritual needs. The application of evidence based nursing music therapy shows that there is a decrease in pain after being given music therapy. The ERAS innovation protocol shows a decrease in the number of care days and a reduction in postoperative complications.

It is suggested to nurses to further develop the use of Virginia Henderson's theoretical approach in meeting patients' holistic needs and making music therapy an alternative choice in reducing cancer pain and developing ERAS protocols for mastectomy and thyroidectomy surgery."

Sebagai peneliti, mahasiswa residensi telah mengaplikasi penerapan skrining Mual dan Muntah pasien HIV/AIDS menggunakan Index of Nausea, Vomiting and Retcing dan hasil dari evidence based nursing ini menunjukkan bahwa kuisioner INVR akurat dan reliable untuk menilai status mual dan muntah pada pasien HIV. Sedangkan sebagai innovator, telah dilakukan inovasi optimalisasi teknologi digital dalam meningkatkan kepatuhan minum obat ARV pada pasien dengan HIV / AIDS yang mana media teknologi digital berupa laman web yang dengan berbagai topik edukasi dengan web yang dapat di akses menggunakan barcode. Hasil penerapan proyek inovasi ini menunjukkan manfaat yang sangat signifikan bagi peningkatan pengetahuan dan kemudahan pasien dalam mengakases informasi penting tentang penyakit HIV sehingga dapat meningkatkan kepatuhan dalam minum obat ARV.

Nursing Specialist Practice is an important process of professional education in order to apply the role of specialist nurses that have an impact on efforts to improve the quality of nursing services. This practice is carried out and developed based on science and technology through various researches. Virginia Henderson's Theory and Bandura's Social Cognitive Theory at RSUP Fatmawati Jakarta. The three competencies that must be achieved in this residency practice are providing nursing care using a nursing theory approach, implementing evidence-based nursing practice, and carrying out nursing innovation projects that are useful for the land where the residency practice. As a nursing care provider, residency students have provided nursing care using Roy's adaptation theory approach in 1 major management case and 30 resume cases of HIV/AIDS patients with various opportunitistic infections. As researchers, residency students have applied the application of Nausea and Vomiting screening for HIV/AIDS patients using the Index of Nausea, Vomiting and Retcing and the results of this evidence-based nursing show that the INVR questionnaire is accurate and reliable to assess the status of nausea and vomiting in HIV patients. Meanwhile, as an innovator, digital technology optimization innovations have been carried out in improving adherence to taking ARV drugs in patients with HIV / AIDS where digital technology media is in the form of web pages with various educational topics with the web that can be accessed using barcodes. The results of the implementation of this innovation project show very significant benefits for increasing knowledge and ease of patients in accessing important information about HIV disease so as to improve adherence in taking ARV drugs."

"Penerapan asuhan keperawatan pada pasien gangguan sistem perkemihan dengan menggunakan teori adaptasi Roy memiliki tujuan untuk meningkatkan kemampuan pasien dalam beradaptasi terhadap perubahan gangguan tubuh yang disebabkan oleh stimulus fokal, stimulus residual, dan stimulus konstektual. Diagnosa keperawatan yang sering muncul pada pasien dengan gangguan sistem perkemihan yaitu kelebihan volume cairan berhubungan dengan disfungsi ginjal. Implementasi keperawatan yang dilakukan untuk mengatasi diagnosa keperawatan tersebut adalah manajemen cairan, monitoring cairan, dan terapi pengganti ginjal berdasarkan intervensi keperawatan yang meliputi aktivitas regulator dan aktivitas kognator. The application of nursing care to patients with urinary system disorders using Roy`s Adaptation theory has the aim of increasing the patient's ability to adapt to changes in bodily disorders caused by focal stimuli, residual stimuli, and contextual stimuli. Nursing diagnoses that often appear in patients with urinary system disorders are excess fluid volume associated with kidney dysfunction. Nursing implementation carried out to overcome nursing diagnoses is fluid management, fluid monitoring, and renal replacement therapy based on nursing interventions which include regulator activities and cognator activities. Evidence-based nursing practice with music therapy in 11 hemodialysis patients proved to be unable to reduce intradialysis hypertension. Meanwhile, the implementation of innovation projects with the provision of early ambulation education after kidney transplantation through media back sheets can improve patient knowledge."

In addition, activities through the application of the role of nurses in implementing of "enhanced recovery after surgery (ERAS)" program in hip surgery patients are innovative projects that can be implemented in the surgical ward to speed up the recovery process, reduce length of stay (LOS), prevent complication, also reduce post morbidity and mortality operation."

Hasil dari evidence based nursing menunjukan bahwa intervensi mindfulness dapat meningkatkan kualitas hidup khusunya pada wanita yang terdiagnosa HIV. Sedangkan proyek inovasi yang telah di lakukan oleh residensi keperawatan tentang edukasi dalam meningkatkan kedisiplinan minum AR dimana media edukasi yang akan di berikan kepada psien HIV ber bentuk wab page sehingg dapat di akses melalui barcode dan hasil dari penerapan proyek inovasi dapat menunjukan manfaat yang terlihat secara signifikan dalam meningkatkan pengetahuan dan dalam pencegahan infeksi oportunitik pada ODHA.

Results from evidence-based nursing show that mindfulness interventions can improve the quality of life, especially for women diagnosed with HIV.Meanwhile, the innovation project that has been carried out by the nursing residency is about education in improving AR drinking discipline where the educational media that will be given to HIV patients is in the form of a web page so that it can be accessed via barcode and the results of implementing the innovation project can show significant visible benefits. in increasing knowledge and in preventing opportunistic infections in PLWHA."

Sedangkan sebagai innovator, telah dilakukan inovasi edukasi dan informasi melalui optimalisasi teknologi digital dalam meningkatkan kepatuhan minum obat ARV pada pasien HIV/AIDS melalui web page, flayer yang dapat diberikan melalui daring dan luring serta banner dan kode kris. Dan hasil penerapan proyek inovasi menunjukkan hasil yang signifikan terhadap peningkatan pengetahuan dan sikap terhadap kepatuhan dalam minum obat pada ODHA.

Nursing specialist programs must be able to provide holistic bio-psycho-social-spiritual needs, provide a sense of security and comfort, prioritize patient safety, manage nursing care skillfully and innovatively and make promotive, preventive, curative and rehabilitative efforts, as well as conducting clinical evidence-based research as well as answering science and technology questions according to their specialist field. The competencies that must be met include three things, namely providing nursing care to primary care patients and resumes, then implementing Evidence Based Nursing (EBN) and creating innovation projects related to patient needs and practice areas. The application of Virginia Henderson's nursing theory was chosen as a theory in providing nursing care to primary management cases in the ward to be able to help patients achieve independence regarding their health and recovery and 30 resume cases of HIV/AIDS patients with various opportunistic infections. As researchers, residency students apply the Jacobson's Progressive Muscle Relaxation (JPMR) technique and the results show that JPMR can reduce levels of anxiety and depression in HIV patients at the Wijaya Kusuma polyclinic. Meanwhile, as an innovator, he has carried out educational and information innovations through optimizing digital technology to increase adherence to ARV medication in HIV/AIDS patients through web pages, flayers that can be provided online and offline as well as banners and crisis codes. And the results of implementing the innovation project show significant results in increasing knowledge and attitudes towards medication adherence among PLWHA."
