Background: Periostin plays role as mediator of inflammatory processes including inflammation and kidney fibrosis. However, data on the significance of periostin in acute kidney injury are limited. We investigated the correlation of urine periostin levels with kidney function in malignant patients receiving high-dose cisplatin therapy.
Methods: This prospective cohort study was conducted in the 8th floor HOM care room in building A, in the RSCM from November 2019 until the minimum sample size was fulfilled by consecutive sampling. Data were analyzed using SPSS version 23.0 according to the purpose of study.
Results: Of the 37 respondents known to be 70.3% male, 29.7% aged 41-50 years, 59.5% suffer from NPC, and 64.9% have Karnofsky score of 80. Urea levels and blood creatinine of respondents increased from pre-chemotherapy to 1 week after chemotherapy I. Likewise, the eGFR value decreases. Changes in periostin levels decreased during chemotherapy I and II, rising again 1 week after chemotherapy III with a p value> 0.05. In the correlation test of urinary periostin levels with other kidney function variables, no significant domain was found (p> 0.05) with a weak correlation coefficient (r = 0.017-0.254) and some domains had a negative correlation direction.
Conclusion: No significant correlation was found in urine periostin levels with blood urea levels, blood creatinine and glomerular filtration rates of malignant patients with high-dose cisplatin therapy.

Kedua hal diatas menyebabkan terjadinya penurunan kadar Ca darah. Pada penderita GGT kerusakan masa gnjal menyebabkan penurunan kadar kalsitriol sehingga tetjadi gangguan homeostasis Ca darah terutama kadar ion Ca. Hal iru menyebabkan terjadinya peningkatan kadar hormon paratiroid.

Protein Pattern And Β-Nag Activity In The Urine From Patients With Several Different Kidney AbnormalitiesKidney disease and its manifestation require different kind of laboratory tests to complete the data of the patients. In general, these tests are used as a diagnostic aid, and also in monitoring the therapy and the progress of the disease. It is important to differentiate between glomerular and tubular disorders. Sodium dodecyl sulfate polyacrylanude gel electrophoresis (SDS-PAGE) is one of the techniques that can be used to differentiate these abnormalities by looking at the protein excretion pattern in the urine. For renal tubular disorder, the determination of β-N-acetylglucosaminidase (β-NAG) is a useful test to detect the alteration of the tubular apparatus. This test is more specific than the others, such as β2-microglobulin, because β-NAG is produced by the lysosomes of the cells of proximal tubules.
The aims of our study is to find a correlation between urine protein excretion pattern and β-NAG activity, and the abnormalities of the kidney. We found that β-NAG activity was increased in patients with tubular and mixed type pattern of proteinuria. The increased activity of this enzyme was also seen in 2 patients (4.7%) with glomerular pattern; one with nephrotic syndrome and the other with hypertension. In nephrotic syndrome with non-selective proteinuria, it is conceivable that some of the urine β-NAG was plasma origin. In hypertension, the mechanisme of the increasing of β-NAG activity in the urine is still unknown."

Metode Penelitian : Studi Pre experimental dengan before and after design.

ABSTRACT
The circadian pattern of blood pressure (BP) is a 24 hours blood
pressure (24hrs BP) curve which increases in the morning, decreases in the
afternoon/evening and the lowest state is at night/bedtime. 24 hrs Ambulatory
Blood Pressure Monitoring (ABPM) is a BP measuring device that is
accurate and can exhibit a circadian pattern of 24 hrs BP. The fall of BP 10-􀀃20%
at night is called as a dipper, while less than 10% is called as a nondipper. The
increasing of 24 hrs BP and nondipper are the risk factor for
cardiovascular morbidity and mortality. The prevalence of
hypertension and nondipper in Chronic Kidney Disease stage 5 on Dialysis
(CKD 5D) are still very high. The main factors causing hypertension in CKD 5D
are decreased Glomerular Filtration Rate (GFR) and increased extracellular
fluid. Kidney transplantation will improve BP and nondipper
by GFR improvement, increases urine production and decreases extracellular
fluid. However, one month after kidney transplantation, the dose
of immunosuppressant drugs is relatively high, which is an obstacle to decrease
BP.
Aim: To determine differences in the circadian pattern of BP, the data
of dipper and nondipper, and the mean of 24 hrs BP in CKD before, and one
month after kidney transplantation.
Methods: Design of the study is before and after design. Subjects of the study
were patients with CKD 5D before kidney transplantation, aged 18-60 years,
were conducted in Cipto Mangunkusumo Hospital during October to
December 2014. 15 subjects were included in the study. 24 hrs urine
collection, GFR, 24 hrs BP measurement with 24 hrs ABPM were recorded in all
subjects, before and one month after kidney transplantation. McNemar test and t
dependent test were used in statistical analysis.
Results: Before kidney transplantation, 12 of 15 subjects were nondippers while
the others 3 subjects were dippers. After kidney transplantation, all subjects (15
patients) were nondippers. McNemar test can not be used because all subjects one
month after kidney transplantation were nondippers (homogeneous). The
decreasing of the mean of 24 hrs systolic BP was found in all CKD one
month after kidney transplantation, but statistically not significant (p>0.05),
while decreasing of the mean of 24 hrs diastolic BP was statistically significant
(p<0.05).
Conclusion: There were still no improvement in nondipper patients
one month after kidney transplantation. There were a decrease in the mean of 24
hrs systolic BP but statistically not significant and a decrease in the mean of 24
hrs diastolic BP which is statistically significant in patients one month after

hrs systolic BP but statistically not significant and a decrease in the mean of 24
hrs diastolic BP which is statistically significant in patients one month after
kidney􀀃transplantation.;Background: The circadian pattern of blood pressure (BP) is a 24 hours blood
pressure (24hrs BP) curve which increases in the morning, decreases in the
afternoon/evening and the lowest state is at night/bedtime.
(p<0.05).
