"ABSTRAK
Keperawatan adalah inti dalam organisasi rumah sakit karena perawat menduduki posisi terbesar dalam pola ketenagaan di rumah sakit. Hal ini menyebabkan perlu dibentuk wadah untuk perawat yang berfungsi menjembatani antara staf perawat fungsional dengan jalur struktural. Dimana Komisi Keperawatan ini dibentuk untuk membahas pengembangan mutu sumber daya keperawatan, pembinaan etik profesi, penyusunan standar pelayanan dan penelitian keperawatan. Komisi Keperawatan ini dalam struktur organisasi rumah sakit merupakan wadah non struktural, dimana pola kegiatan dari Komisi Keperawatan ini sangat dinamis sehingga dapat menciptakan stabilitas dan nilai-nilai sosial.
Penelitian ini bertujuan untuk mendapatkan informasi tentang pengorganisasian Komisi Keperawatn di Pelayanan Kesehatan Sint Carolus dengan pendekatan kualitatif. Pengumpulan data kualitatif dilakukan dengan metode wawancara mendalam (indepth interview). Variabel yang diukur adalah struktur organisasi, peran dan fungsi, tugas dan tanggung jawab, serta kegiatan yang dilakukan.
Dari hasil wawancara mendalam dan pengamatan dokumentasi sebagai triangulasi data teridentifikasi bahwa Komisi keperawatan telah dapat menjalankan fungsinya dengan baik, terbukti dengan adanya kebijakan tertulis dari Direktur Keperawatan meskipun Departemen Kesehatan Republik Indonesia sendiri belum ada kerangka acuannya serta dokumen-dokumen lain yang mendukung bahwa kegiatan dilaksanakan sesuai dengan jadwal yang sudah ditentukan. Pada akhirnya dengan adanya Komisi Keperawatan di PKSC telah membantu perawat dalam mengembangkan diri baik secara individu maupun secara profesional.
Perbedaan dengan konsep terutama terjadi pada struktur organisasi dimana struktur organisasi Komisi Keperawatan PKSC masih berbentuk struktural sehingga pengorganisasian Komisi Keperawatan masih sangat tergantung kepada manajemen puncak, tidak ada hubungan langsung dengan pelaksana asuhan. Sedangkan pelayanan keperawatan sudah diakui sebagai pelayanan profesional. Sekiranya dapat disarankan untuk pengorganisasian Komisi Keperawatan agar dirubah dengan menganut poly pengasuhan bersama "Shared Governance", suatu organisasi yang memajukan profesional keperawatan, seharusnya organisasi Kornite Keperawatan di Rumah Sakit dilindungi oleh undang-undang tidak hanya dilindungi oleh pimpinan rumah sakit.

ABSTRACT
Nursing of the core of hospital organization because nurse has the biggest position in hospital labour. Because of this, we need organization for nurse to be a mediator between fungsional and structural nurse. Where nursing committee is made for talking about human resources quality developrtient, building profesion, ethic, arranging serve sign and nursing research. Nursing committee at hospital is non structural institution, where activity model for system nurse is very dynamic until it can stability creative and social value.
This research purpose is to get information about nursing organization committee in Sint Carolus Health Service with qualitative research approach. Collecting data is done with in depth interview. Variable measured are organization structure, characteristic function, duty and responsibility including acting who was done.
From in depth interview and documentation observation as a date triangulation, were identified that nursing committee has carried out its function well. It is proved with approach approval from nursing director although Indonesia Health Ministry is not yet give framework that support activity appropriate schedule. Finally with nursing committee in structure health service can help in developing self individually and professionally. Differences with concept especially occurred on organization structure where organizational structure of Sint Carolus Health Services nursing committee still structured model with the result that nursing committee organization still depend on top management. There is no relationship with direct care giver, while nursing services has recognized as a professional.
This result suggested that Sint Carolus Health Services to change nursing committee organization to "Shared Governance" an organization to advancing professional nursing. Nursing Committee should method the hospital should be protected by law of Indonesian government not only be protected by the board of trustees of the hospital."

"This research study ilustrattes the basic ofthe turn over ofthe nurses . It also pertains to the pations opinion on the unsatisiied treatment received from the hospital nurses attending spesilically with regard to the unpolite, unfriendly manner, slow attention and the overall implication of not good or quality services. In general the research study aims to analyzed the job performance and the skill of nurses at Tebet hospital at the present year.
The research used the cross sectional method by getting the sample of the total population of the nurses at Tcbet hospital on their actual work during the month of May untill June 2001. The data were gathered from the given quetionaire at the respective respondents and was properly interpreted and explained on the basis of satistical reports with technical analysis distribution frequency, chi squere and logistic regretion. Based on the univariat analysis the research study implication is that; job satisfaction and job performsnce ofthe nurses at Tebet hospital were almost the same.
The research study further illustrates interestingly that comparison with oter hospital research study researches Tebet hospital got lowest grade as compared to them. The analysis also implied that the relation of bivariat between job satisfied and performance are not useful on thc contrary, the useful relation of the said factors were based on the condition of the environment.
The analysis also proved that job satisfaction of the nurses at their work condition are important to recognize by the administration / management of the hospital for continuous care and profesional growth of the nurses. The researchers recommendation is focused on the achivement of the job satisfaction of nurses and further development of their job -performance . This would simply be possible by providing them better working environment like good lighting as good ambience , and complete facilities and equipments at the hospital."

"Intensive Care Unit (ICU) adalah suatu bagian dari rumah sakit yang tezpisah, dengan staf yang khusus dan perlengkapan yang khusus yang ditujukan nntuk observasi, perawatan dan terapi pasien-pasien yang menderita penyakit, cedera atau penyulit-penyulit yang mengancam jiwa atau potensial mengancam jiwa dengan prognosis dubia. Pelayanan keperawatan adalah bagian integral dari pelayanan kesehatan di Rurnah Sakit sebingga sering dijadikan cerrnin keberhasilan pelayanan. Perawat merupakan komponen penting di Rumah Sakit, sehingga perlu diperhatikan ketidakpuasan kerjanya karena nantinya akan mempengaruhi produktivitas.
Penelitian ini bertujuan untuk mengetahui informasi mengenai faktor-faktor yang mempengaruhi ketidakpuasan kerja para perawat di ICU. Dalan1 penyusnnan kerangka konsep penelitian ini, sebagai variabel dependen adalah ketidakpuasan kerja perawat, sedangkan variabel independennya adalah Karakterisik individu (usia,jenis kelamin,status perkawinan,latar belakang pendidikan,lama kerja) dan Lingkungan kerja (Struktur tugas, pola kepemimpinan, penghargaan, ketersediaan sarana kelja, desain pekeljaan, pendidikan dan pelatihan,pola kerjasama).Penelitian ini menggunakan data knantitatif (kuisioner bagi perawat di ICU 18 RS) dan kualitatif (wawancara mendalam di 2 RSUD di Jakarta). Data knantitatif yang diperoleh, di analisa secara univariat, bivariat, multivariate.Selain itu juga dilakukan analisis berdasarakan diagram Kartesius dan gap score.
Dari hasil penelitian didapatkan bahwa 46,7 % dari responden merasa ketidakpuasan kerja. Dan yang paling dominan mempengaruhi ketidakpuasan terlihat pada dimensi penghargaan. Sehingga dari basil penelitian ini diharapkan dapat bermanfaat untuk perbaik:an-perbaikan pihak Depkes, Rumah Sakit,Perawat pelaksana, maupun Program Kajian Administrasi Rumah Sakit, Universitas Indonesia.

Intensive Care Unit (ICU) is a separate part of the hospital, with special staff and equipment that are specific fur observing, treating and the therapy of patients who are injured, or have life threatening or potentially life threatening conditions, with prognosis dubia. The nursing service is an integral part of the health services in a hospital, hence has frequently been a portrayal of the success of service. The nursing staff is an important component of a hospital, so it must be closely scrutinized, as it will affect productivity.
This research was carried out to gain information on the filctors affecting the work dissatisfaction of the nurses at the ICU. In making the concept of this research, the dependent variable was the work dissatisfaction of nurses, while the independent variable was individual characteristics (age, sex:, marital status, educational background, period of employment) and work environment (task structure, mode of leadership, appraisals, availability of equipment, task design, education and training, pattern of cooperation). This research used quantitative (questionnaires for nurses at 18 hospital ICU's) and qualitative (in depth interviews at two state hospitals in Jakarta) data. The obtained quantitative data was analyzed univariately, bivariately and multivariately. Furthermore, an analysis was performed based on the Kartesius diagram and the gap score.
From the research, 46.7 % of the respondents were dissatisfied with their work. In addition, what dominantly affected their dissatisfaction were their appraisals. Therefore, the result of this research is hoped to promote improvements at the Ministry of Health, the hospitals, the working nurses and the Hospital Administrative Research Program at the University of Indonesia.

"Penelitian ini bertujuan untuk mengetahui kepuasan keluarga pelanggan terhadap keperawatan unit perawatan intensi pada 18 Rumah Sakit di Indonesia. Dalam penyusunan kerangka konsep penelitian ini sebagai variabel dependen adalah variabel kepuasan total yang meliputi harapan dan persepsi responden dimana tingkat kepuasan dihitung berdasarkan kesenjangan nilai antara keduanya sedangkan sebagai variabel independen adalah mencari diantara lima dimensi servual yaitu tangible, reiiability, responsiveness, assurance dan emphary yang memiliki pengaruh paling dominan dalam kepuasan keluarga peianggan serta penilaian dalam pelaksanaan asuhan keperawatan yang tercakup dalam lima prosw, yaitu pengkajian, diagnosa, rencana, tindakan dan evaluasi keperawatan.
Metode penelitian yang dilaksanakan adalah gabungan penelitian kuantitatif dan kualitatih dimana menggunakan suwey kepada 246 responden perawat dan |22 reponden keluarga pelanggan dalam upaya mengumpulkan data kuantitatifi Sedangkan untuk mengumpulkan data kualitatif digunakan metode cross-sectional terhadap rosponden infoman Kemudian data yang diperoleh dari hasil penelitian ini kemudian dilakukan analisis dengan menggunakan metode reliabilitas, korelasi, dan regresi yang kesemuanya menggunakan program perangkat lunak komputer.
Dari hasil peneiitian terlihat bahwa 62,3 % dari responden merasa puas dengan keperawatan unit perawatan intensif pada I8 rumah sakit di Indonesia Penggunaan metode servqual sebagai alat ukur kepuasan keluarga. pelanggan, memperlihatkan adanya hubungan bemakna antam kepuasan total responden dengan lima aspek dimensi metode servqual tersebut. Juga metode servqual memperlihatkan adanya kesenjangan antara jasa yang dipersepsikan dengan jasa yang diharapkan keluarga pelanggan.
Juga di dalam penelitian ini terlihat adanya hubungan tidak langsung antara pelaksanaan asuhan keperawatan dan kepuasan keluarga pelanggan, dikatakan telah terlaksana dengan baik oleh 93,9% responden dan hal ini memiliki keterkaitan tidak langsung dengan kepuasan total keluarga pelanggan unit perawatan intensif. Dalam hal ini, proses rencana keperawatan merupakan proses yang paling baik diiaksanakan dan proses tindakan keperawatan merupakan proses yang dinyatakan paling tidak baik dilaksanakan oleh para perawat pelaksana di unit perawatan intensif.
Sebagai kesimpulan dari penelitian ini bahwa penilaian asuhan keperawatan dan alat ukur metode servqual dapat mengidentifikasikan unsur-unsur pelayanan yang menyebabkan ketidak-puasan keluarga pelanggan terhadap mutu pelayanan serta unsur-unsur pelayanan mana yang memberikan pengaruh besar secara signifikan terhadap kepuasan total keluarga pelanggan. Oleh karena itu disarankan kepada pihak manajemen mmah sakit untuk menggunakan metode servqual dalam melakukan survei pengukuran kepuasan keluarga pelanggan. Karena penelitian ini membuktikan bahwa metode servqual dapat digunakan dan efektif untuk diterapkan di rumah sakit dalam mengukur kepuasan keluarga pelanggan di unit perawatan intensiti Dari hasil penelitian, saran untuk semua pihak yang terkait untuk memperhazikan keluarga pelanggan yang mempunyai harapan tinggi namun berpersepsi rendah, dimana hal tersebut menggambarkan tingginya tingkat ketidakpuasan keluarga pelanggan.

This research's goal is to assess the level of the customer's family satisfaction on intensive Nursing unit at I8 Hospitals all over indonesia in compiling the frame of this research concept, the dependent variable is: total satisfaction variable that encompassed hope and perception of the respondents, where the level of satisfaction is calculated based on the different value between the two, while the independent variable is finding between consists of 5 dimensions: service quality, tangible, reliability, responsiveness, assistance and empathy that has most dominant effect in family's customer satisfaction and in executing nursing guidelines that consists of 5 process: review, diagnosis, planning, action and nursing evaluation.
Research method that is implemented is a combination of quantitative and qualitative research, where it surveyed 246 nurse respondents and 122 family customer respondents, in an effort to collect quantitative data. While in the process of gathering qualitative data, cross-sectional method was used against informant respondents. Then data obtained ii-om this research will be analyzed using reliability, correlation and regression method that is all available in the computer software.
From the results of the research, we can see that 62,3 % of thc respondents felt satisfied with the intensive nursing unit at I8 Hospitals all over Indonesia. The use of servqual method as a gauge for the level of customer's family satisfaction shows a meaningful correlation between total respondent satisfactions with the 5 aspect dimension of the servqual method. Also the sen/qual method shows there was a gap between the service that is perceived and the service that is hoped for by the customer's family.
The research also shows an indirect correlation between the executions of the nursing guidelines and customer's family satisfaction, it is stated that it was well executed by 93,9% respondents and this also have indirect correlation with the total satisfaction of the customer's family with regards to the intensive care unit ln this point, planning process of nursing guidelines is the best process they've been doing and the action process of nursing guidelines is the worst process in their statement.
In conclusion from this research, executing nursing guidelines and the servqual method could identify the service aspects that caused dissatisfaction for the customer's family against the quality of service and all aspect of service where it significantly influenced the customer's family total satisfaction. lt is recommended to the hospital management to use the servqual method in surveys, to gauge the level of the customer's family satisfaction. Due to the proof this research has shown, the servqual method could be used effectively in hospitals in order to gauge the level of the customer's family satisfaction in the intensive care unit.

"Saat ini rumah sakit dituntut untuk memberikan pelayanan kesehatan yang berkualitas. Tenaga keperawatan sebagai bagian integral dari sistem pelayanan kesehatan di rumah sakit, merupakan kelompok sumber daya manusia profesional yang sangat penting dalam menghasilkan pelayanan kesehatan yang berkualitas.
Rumah Sakit Azra memiliki visi mengunggulkan pelayanan keperawatan yang profesional. Untuk mewujudkan visi ini, pihak manajemen RS Azra perlu memberikan perhatian khusus kepada tenaga keperawatannya, salah satu yang terpenting ialah dalam pengembangan karirnya.
Penelitian ini bertujuan untuk menyusun perencanaan pengembangan karir tenaga keperawatan yang sesuai untuk RS Azra yang memiliki visi mengunggulkan pelayanan keperawatan yang profesional. Penelitian ini merupakan penelitian studi kasus dengan menggunakan analisis kualitatif berdasarkan tinjauan pustaka dan studi komparatif atas perencanaan pengembangan karir rumah sakit pembanding yaitu Pelayanan Kesehatan Saint Carolus Jakarta dan RS Awal Bros Pekanbaru. Selain itu peneliti melakukan wawancara mendalam dengan para informan dan mengumpulkan data sekunder di RS Azra. Dari semua yang dijelaskan diatas, peneliti kemudian melakukan sintesis sehingga didapatkan pola perencanaan pengembangan karir tenaga keperawatan RS Azra.
Hasil penelitian ini menunjukkan bahwa hingga saat ini RS Azra belum memiliki pola perencanaan pengembangan karir tenaga keperawatan yang baku dan terstruktur. Pengembangan karir yang ada baru terbatas pada kegiatan pelatihan yang dilaksanakan secara tidak terstruktur dan bersifat reaktif. Selain itu hasil penelitian ini menunjukkan bahwa pola perencanaan pengembangan karir tenaga keperawatan RS Azra dapat dikembangkan sesuai dengan sintesis dan pola perencanaan pengembangan karir tenaga keperawatan Pelayanan Kesehatan Saint Carolus dan RS Awal Bros.

Today, hospitals are requested to provide excellent health care services. Nurse as a part of the health care system in hospital, is a group of professional that is very important in providing excellent health care services.
The vision of Azra Hospital is to excel professional nursing care. To achieve this vision, the management of Azra Hospital should give special attention to their nursing personnel. One of the most important things is the nursing personnels career development.
The purpose of this research is to make a career planning and career development for nursing personnel, which is appropriate with Azra Hospital. The data was collected from in-depth interview and documentation in Azra Hospital. Further, the data were analyzed by qualitative method based on literature and using two hospitals as comparative models.
The important result of this research is that the nursing personnel career planning and development is limited in training activities, which unstructured. It is necessary to develop the nursing personnel career planning and development by synthesizes the pattern in comparative hospitals, Sint Carolus Health Services and Awal Bros Hospital.

"Salah satu tolok ukur keberhasilan pelayanan kesehatan di rumah sakit adalah peningkatan mutu asuhan keperawatannya.
Sesuai visi dan misi Rumah Sakit Awal Bros untuk menjadi pusat pelayanan kesehatan yang didukung tenaga-tenaga profesional, merasa perlu meningkatkan pengetahuan dan ketrampilan Sumber Daya Manusianya terutama perawat yang dianggap merupakan tulang punggung suatu rumah sakit.
Belum adanya standar penilaian perawat yang mempunyai latar belakang dan pengalaman yang berbeda serta keseragaman pengetahuan dan ketrampilan yang tidak merata maka disusun Rancangan Standar Kompetensi Keperawatan oleh tim Inti Rumah Sakit Awal Bros.
Hasil penelitian dibuat suatu Rancangan Standar Kompetensi Keperawatan yang akan diuji cobakan di Rumah Sakit Awal Bros.
Penelitian dilaksanakan secara analisa kualitatif yang merupakan studi kasus berdasarkan kebutuhan Rumah Sakit Awal Bros dengan membuat perbandingan antara teori serta data dari beberapa rumah sakit yang telah mempunyai standar kompetensi.
Penerapan standar kompetensi terdiri dari 2 (dua) langkah yaitu pertama menetapkan tingkat golongan seorang perawat berdasarkan pendidikan dan pengalaman kerja, langkah selanjutnya menentukan kriteria penilaian dari masing-masing tingkat dengan menilai ketrampilan tehnikal, ketrampilan interpersonal dan ketrampilan intelektual.
Disarankan agar dalam uji coba standar kompetensi ini bekerjasama dengan konsultan keperawatan.

Program for Composition the Standards of Nursing Competence to Develop Nursing Process in Awal Bros Hospital, PekanbaruOne of the measuring standard for the success of health service in hospital is enhancement of nursing quality.
Based on its vision and mission to become a health service center which is supported by professional manpower, Awal Bros deems it necessary to enhance the knowledge and skill of its human resources, particularly of nurses who serve as the fundamental of a hospital.
As there have been no standard for evaluation of nurses who are of different background and experience, and as their knowledge and skills are not equally the uniform in Design of Nursing Competence Standards is prepared by the team of Awal Bros Hospital.
The Design of Nursing Competence Standards is prepared based on the results of survey and it will be tried-out in Awal Bros Hospital.
The survey is conducted by qualitative analysis which is a case study based on the literature and the hospitals who are applied competence standards.
The competence standards are applied in 2 (two) steps. First is determine the class level of a nurse based on education and work experience. The second step is to determine the criteria of evaluation of each level by evaluating technical skill, interpersonal skill and intellectual skill.
It is recommended to have these competence standards tried-out in cooperation with a nursing consultant.

"Dokumentasi keperawatan merupakan bukti dari pelaksanaan keperawatan yang menggunakan metode proses keperawatan, berisi tentang catatan respon pasien terhadap tindakan medis dan tindakan keperawatan serta merupakan indikator mutu asuhan keperawatan. Agar pelayanan keperawatan berkualitas maka perawat diharapkan dapat menerapkan asuhan keperawatan dengan pendokumentasian yang benar.
Penelitian ini bertujuan untuk menganalisis kelengkapan dokumen asuhan keperawatan terkait dengan faktor individu, faktor organisasi dan faktor psikologis, menggunakan metode kualitatif dengan teknik pengumpulan data observasi dan wawancara mendalam. Observasi memungkinkan peneliti mengamati langsung tantangan perawat dalam melengkapi dokumen pengkajian, diagnosa keperawatan, rencana tindakan, implementasi, evaluasi dan catatan keperawatan.
Hasil penelitian menunjukkan bahwa secara umum pengisisan kelengkapan dokumentasi asuhan keperawatan di rumah sakit Santo Antonio masih di bawah standar Depkes. Perawat sudah menyadari pentingnya pendokumentasian asuhan keperawatan. Kendala yang dihadapi antara lain kurangnya tenaga dan kurangnya fasilitas yang ada seperti petunjuk teknis pengisian dokumentasi asuhan keperawatan. Selain itu, belum pernah dilakukan pelatihan terkait dengan pendokumentasian asuhan keperawatan. Perawat memanfaatkan hasil dokumentasi sebagai materi komunikasi kemajuan kondisi pasien, namun dokter belum memanfaatkan secara maksimal hasil dokumentasi yang dibuat oleh perawat.
Saran dari penelitian ini adalah agar pihak manajemen memenuhi jumlah tenaga, mengadakan pelatihan dan seminar, membuat petunjuk teknis pengisian dokumentasi asuhan keperawatan, menerapkan supervise berjenjang serta membuat lembar catatan pasien yang terintegrasi dari seluruh tenaga kesehatan. Saran untuk Kemenkes adalah mengembangkan peraturan yang memberikan pemisahan yang jelas antara tugas dokter dan tugas perawat.

Nursing documentation is an evidence of the implementation of nursing, using the nursing process method, which is contains the report of the patients’ response to the medical and nursing care also an indicator of the nursing care quality. In order to support the nursing care quality, the nurse should applied itself with proper documentation.
The aim of this research is to analyze the completion of the nursing care document, in relation to the individual, organizational, and psychological factors by using qualitative methods such as observation and in-depth interviews. This study is allow to observed nurses challenges to complete the document, diagnose, treatment planning, implementation, evaluation and medical record.
The study revealed that nursing care document completion in Saint Antonio hospital is still below standard even the nurses realized the importance of documenting nursing care. Challenges were found are the nurses work load, no technical guideline for completing the nursing document, no trainings and no workshops for the nurses. Nurses are going to use the results of the documentation to discuss the progress of patient’s condition, but clinicians do not use it as expected.
The study suggests the management to increase the number of personnel, to held trainings and seminars for nurses, to develop technical guideline for nursing documentation, to implement head nurse’s supervision and also create an integrated record sheet based on various personal’s health. Recommendation for Ministry of Health is to provide regulation of clear duty of nurses and doctors.

ABSTRACT
Introduction: Hospital is an organization that produces a product in the form of
health services which consists of several professions and disciplines of Science,
one of which plays an important role is the nurse. Nurses in quantity become the
largest workforce, providing 24 hour service continuously and absorb more than
50% Hospital budget. High nurse turnover rates can cause disruption to services
provided to patients. Standard turn over employees, which can still be tolerated is
10% per year. Turn over nurses that occurred in RSU Prima Medika which
increased from 2015 to 2017, causing concerns from the management, especially
at the end of 2016 intention turnover rate of 22.1%.
The purpose of this study is to determine the determinants of Turn Over Intention
Nurses at Prima Medika Hospital in 2017
Method: This research is analytic with quantitative approach. This study used
cross sectional design. The number of samples used in this study was 114 nurses
who come from employees who work at Prima Medika Hospital after deducting
exclusion criteria.
Results: Salary Factor has relationship with nurse work satisfactory at RSU Prima
Medika in the year of 2017. Other factors such as: working conditions; status
within the organization; supervision; peer-partner interpersonal relationships,
superiors and subordinates; achievement and the work itself, has no significant
relationship with nurse job satisfactory. Job satisfactory has relationship with
turnover intention nurses at Prima Medika Hospital in 2017
Conclusion: nurse intention turnover is influenced by job satisfaction that lies in
the salary, so the salary in this case needs to be considered."

"Petugas kamar bedah merupakan salah satu jenis tenaga kesehatan yang mempunyai resiko tinggi terhadap kemungkinan terpapar oleh berbagai kuman penyakit, terutama HIV/AIDS yang saat ini sangat ditakuti. Salah satu upaya perlindungan diri adalah dengan menerapkan kewaspadaan universal melalui tindakan cuci tangan secara benar, penggunaan alat pelindung, desinfektan, mencegah tusukan alat/benda tajam. Adapun konsep yang dianut adalah, bahwa semua darah dan cairan tubuh tertentu harus dikelola sebagai sumber yang dapat menularkan HIV, Hepatitis dan berbagai penyakit lain melalui darah.
Departemen Kesehatan telah menetapkan upaya peningkatan mutu layanan kesehatan dengan memprioritaskan upaya pengendalian infeksi nosokomial bagi seluruh rumah sakit di seluruh Indonesia, antara lain dengan menerapkan kewaspadaan universal. Sejalan dengan ketetapan Departemen Kesehatan tersebut, maka Rumah Sakit Umum Pusat Persahabatan berupaya meningkatkan mutu layanan antara lain dengan upaya pengendalian infeksi nosokomial, salah satu upaya yaitu dengan menerapkan kewaspadaan universal secara baik dan benar di seluruh ruang termasuk kamar bedah.
Penelitian ini bertujuan untuk mengetahui tingkat kepatuhan petugas kamar bedah dalam menerapkan kewaspadaan universal dan untuk mengetahui seberapa jauh faktor predisposisi, faktor pemungkin, dan faktor penguat berpengaruh terhadap kepatuhan.
Responden adalah seluruh petugas kamar bedah (IGD dan IBS) berjumlah 78, yang terdiri dari dokter dan perawat yang bersifat tenaga tetap, berhubungan langsung dalam penanganan pasien, dan telah bekerja minimal 1 (satu) bulan.
Metoda penelitian menggunakan rancangan kros seksional. Data diperoleh melalui penyebaran kuesioner dan pengamatan langsung di rumah sakit. Pengumpulan data dilakukan selama periode Maret sampai Mei 2001 (satu bulan lima belas hari) di Rumah Sakit Umum Pusat Persahabatan.
Hasil penelitian menunjukkan, bahwa sebagian besar (73,1%) responden memiliki tingkat kepatuhan sedang. Ketidak patuhan lebih banyak ditemukan di IGD dibandingkan dengan IBS, terutama dalam hal tidak mencuci tangan saat datang di kamar bedah (9,5%), saat meninggalkan kamar bedah (15,4%), sebelum melakukan tindakan (12,8%), setelah melakukan tindakan (7,7%), tidak menggunakan alas kaki (12,8%), masih melakukan pemasangan kembali tutup jarum suntik (recapping =87,1%). Pengetahuan responden tentang kewaspadaan universal. sebagian besar masih kurang (73,1) dengan nilai rata-rata 65,995 (p-wald = 0,0448 OR 1,7744).
Hasil analisis ditemukan ada hubungan antara variabel pengetahuan, tempat kerja, dan lama kerja di kamar bedah dengan kepatuhan dalam menerapkan kewaspadaan universal. Pengetahuan dan lama kerja merupakan variabel yang paling menentukan kepatuhan dalam menerapkan kewaspadaan universal. Responden yang memiliki pengetahuan kurang berpeluang untuk tidak patuh hampir 2 kali dibandingkan yang memiliki pengetahuan baik setelah variabel lama dan tempat kerja dikendalikan.
Berdasarkan hasil penelitian tersebut peneliti menyarankan agar dilakukan upaya meningkatkan pengetahuan petugas melalui sosialisasi kewaspadaan universal dengan pelatihan, penyebaran leaflet, atau mengadakan pekan kewaspadaan universal.
Di samping itu mungkin perlu diadakan evaluasi terhadap kcpatuhan secara berkala, bila memungkinkan adakan reward dan punishment. Untuk menjaga kesehatan petugas agar tetap terpelihara sesuai standar kesehatan yang dibutuhkan, perlu adanya pemeriksaan kesehatan secara berkala bagi seluruh petugas kamar bedah.

Factors Related to Obedience Level of Surgery Room Staff in Applying Universal Precautions on Persababatan General HospitalStaff on surgery room is one of health personal who has high risk to possibly contaminate by any of germ disease, especially the frightening HIV/AIDS. One effort of self protection is applying universal precautions through real action such as washing hands properly, using protection tools, disinfectants, prevented any stab from sharp-pointed tools. The adherent concept is all blood and particular body liquid has to be managed as a source, with can infect HIV, hepatitis and other disease through blood.
Department of Health has determined effort to increase health service quality with prioritizing effort of restrained nosocomial infection for all hospital in Indonesia, like applying universal precautions. Along with department of Health's determination, Persahabatan General Hospital tries to increase service quality on restraining nosocomial infection with applying universal precaution properly in all room including surgery room.
The objective of this research is to understand the obedience level of surgery room's staff in applying universal precautions and to understand how far predisposition factors, enabling factors, and reinforcement factors is influencing the obedience. The cross sectional design is used for research method. Data was obtained by distributing questionnaires and direct observation in the hospital. Data was collected during March to May 2001 (one month and 15 days) on Persahabatan General Hospital.
The result indicated that most of respondent (73, 1%) has medium level of obedience. The obedient staff was found on not washing their hands when they arrived at surgery room (9, 5%), when they left surgery room (15, 4%), before they did actions (12, 8%), after they did action (7, 7%), not using fear footwear (12, 8%), still recapped the injection needle (90, 8%). Most respondent have less knowledge on Universal Precautions (73, 1%), mean 65,995.
Statistical analyst result indicated that p-wald =0, 0448 OR 1,7744 wich means that there is a meaningful relations between knowledge with obedience level. Respondent who has less knowledge is likelihood to be disobedient more than twice if compare to those who has better knowledge after controlling old working variable.
Based on those result, research recommend doing more effort in increasing knowledge of staff through socializing the universal precautions with training, leaflet or Universal Precautions week.
The obedience level should be evaluated periodically, present reward and punishment if possible. Health examination has to be done periodically to all surgery room staff."
