"Praktek Spesialis Keperawatan merupakan sebuah proses penting dari pendidikan profesi dalam rangka mengaplikasikan peran perawat spesialis yang berimpak pada upaya peningkatan kualitas layanan keperawatan. Praktek ini dilakukan dan dikembangkan berdasarkan ilmu pengetahuan dan teknologi melalui berbagai penelitian. Tiga kompetensi yang harus dicapai dalam praktek residensi ini adalah memberikan asuhan keperawatan menggunakan pendekatan teori keperawatan, menerapkan implementasi keperawatan yang berbasis bukti ilmiah (evidence based nursing practice), dan melakukan proyek inovasi keperawatan yang berguna bagi lahan yang menjadi tempat praktek residensi. Sebagai pemberi asuhan keperawatan, mahasiswa residensi telah memberikan asuhan keperawatan menggunakan pendekatan teori adaptasi Roy pada 1 kasus kelolaan utama dan 30 kasus resume pasien HIV/AIDS dengan berbagai infeksi oportunitis. Sebagai peneliti, mahasiswa residensi telah mengaplikasi penerapan tindakan keperawatan yang berbasis bukti ilmiah ”A Brief Risk Reduction Intervention” pada ODHA LSL dimana hasilnya menunjukkan adanya peningkatan pengetahuan tentang kesehatan seksual ODHA LSL dan menurunnya frekuensi praktek anal seks tanpa kondom yang berdampak pada menurunnya resiko transmisi HIV dikalangan ODHA LSL dan pasangannya. Sedangkan sebagai innovator, telah dilakukan konseling terstruktur ”CEMARA” pada perawat dan ODHA putus ARV dan hasil penerapan proyek inovasi ini menunjukkan manfaat yang sangat signifikan bagi peningkatan self conffidence perawat dalam melakukan konseling kepatuhan dan meningkatnya kepatuhan minum ARV pasien ODHA dengan riwayat putus obat.

Nursing Specialist Practice is an important process of professional education in order to apply the role of specialist nurses who have an impact on efforts to improve the quality of nursing services. This practice is carried out and developed based on science and technology through various studies. Three competencies that must be achieved in this residency practice are providing nursing care using nursing theory approaches, implementing evidence-based nursing practice, and undertaking nursing innovation projects that are useful for the land where residency practices are based. As a provider of nursing care, residency students have provided nursing care using Roy's adaptation theory approach in 1 major case management and 30 cases of resumes of HIV / AIDS patients with various opportunitis infections. As a researcher, residency students have applied scientific evidence-based nursing actions "Brief Risk Reduction Intervention" in men who have sex with men (MSM) with HIV/AIDS, where the results show an increase in knowledge about sexual health of them, and a decrease in the frequency of unprotected anal sex practice that has an impact on decreasing the risk of HIV transmission among MSM with HIV/AIDS. While as an innovator, "CEMARA" structured counseling for nurses and PLWHA dropped out and the results of the application of this innovation project showed a very significant benefit for the increase in nurse self confidence in conducting adherence counseling and increasing adherence to ARV consumption of PLWHA patients with a history of drug withdrawal."

Program praktik residensi ners spesialis keperawatan medikal bedah peminatan endokrin, bertujuan agar para residen memiliki tingkat kompetensi profesional, berpikir kritis dan reflektif serta komunikasi interpersonal yang lebih tinggi ketika dihadapkan pada situasi klinis yang kompleks. Praktik dilakukan di ruang gawat darurat, ruang intensif, rawat inap, dan rawat jalan RSUPN Dr Cipto Mangunkusumo Jakarta dengan mengaplikasikan teori Model Adaptasi Roy pada 30 kasus resume dan 1 kasus utama pada pasien diabetes melitus tipe 2 dengan komplikasi peripheral arterial disease dan ulkus kaki diabetik. Penerapan evidence based nursing Short Form Berg balance Scale telah terbukti sebagai alat skrining yang mudah, murah, hemat waktu dan memiliki reliabilitas Intraclass Correlation Coefisien yang baik untuk pengkaji keseimbangan dan resiko jatuh pada pasien DM yang sudah lanjut usia. Penerapan inovasi New Simulation Tools terbukti efektif mengurangi rasa takut pasien terhadap jarum suntik dan meningkatkan keterampilan menyuntik pada pasien yang baru pertama kali menerima terapi insulin.

The nursing profession always develops following the latest science. This is realized through a process of continuing education after the master of nursing. The residency practice program for medical surgical nursing specialists specializing in endocrine, aims to make residents have a higher level of professional competence, critical and reflective thinking and interpersonal communication when faced with complex clinical conditions. The practice was carried out in the emergency room, intensive care unit, inpatient and outpatient care at RSUPN Dr Cipto Mangunkusumo Jakarta by applying Roy's Adaptation Model theory to 30 resume cases and 1 main case in patients with type 2 diabetes melitus with complications of peripheral arterial disease and diabetic foot ulcers. . The application of evidence-based nursing Short Form Berg balance Scale has been proven as a screening tool that is easy, inexpensive, time-saving and has good Intraclass Correlation Coefficient reliability for assessing balance and risk of falling in elderly DM patients. The application of the innovative New Simulation Tools has been proven to be effective in reducing patients' fear of needles and improving injecting skills in patients who are receiving insulin therapy for the first time."

"Praktik residensi keperawatan medikal bedah peminatan keperawatan endokrin merupakan kelanjutan dari pendidikan magister keperawatan yang dilaksanakan dalam rangka memberikan kesempatan dan pengalaman kepada residen perawat untuk menerapkan ilmu dan teknologi keperawatan khususnya memberi asuhan keperawatan lanjut dan kompleks dalam menyelesaikan permasalahan keperawatan endokrin pada fasilitas pelayanan kesehatan. Praktik klinik dilaksanakan di setting gawat darurat, perawatan intensif, rawat inap, dan rawat jalan di RSUPN Dr Cipto Mangunkusumo Jakarta. Penerapan teori Model Adaptasi Roy dilakukan oleh residen pada 30 kasus resume dan 1 kasus utama yaitu pada pasien SIDA on ARV, Diabetes mellitus tipe 2, Selulitis regio colli, Hepatitis C, Sirosis hepatis. Asuhan keperawatan berbasis bukti diterapkan berupa latihan Passive Stretching untuk menurunkan gula darah pada pasien yang tidak mampu melakukan latihan fisik mandiri. Inovasi pelayanan keperawatan dilakukan dengan menyusun media edukasi simulasi suntik insulin yang terdiri dari alat New Simulation Tools (alat peraga suntik insulin) video dan buku panduan edukasi suntik insulin (buklet) yang digunakan sebagai panduan dan media edukasi bagi perawat dalam memberikan edukasi suntik insulin, mengurangi rasa takut pasien terhadap penyuntikan, memberikan lebih banyak latihan, dan meningkatkan keterampilan penyuntikan insulin pasien.

The medical surgical nursing residency program specializing in endocrine nursing is a continuation of nursing master education which is carried out in order to provide opportunities and experience to nurse residents to apply nursing science and technology, especially providing advanced and complex nursing care in solving endocrine nursing problems in health care facilities. Clinical practice is carried out in emergency, intensive care, inpatient, and outpatient settings at Dr. Cipto Mangunkusumo Hospital Jakarta. The application of the Roy Adaptation Model theory was carried out by residents in 30 resume cases and 1 main case, namely in patients with SIDA on ARV, Diabetes mellitus type 2, Cellulitis regio colli, Hepatitis C, Cirrhosis hepatis. Evidence-based nursing care is applied in the form of Passive Stretching exercises to reduce blood sugar in patients who are unable to do independent physical exercise. Nursing service innovation is carried out by compiling educational media consisting of insulin injection simulation tools consisting of New Simulation Tools (insulin injection tool) videos and insulin injection education guidebooks (booklets) which are used as guidelines and educational media for nurses in providing insulin injection education, reducing patient fear of injections, providing more practice, and improving patient insulin injection skills."

"Praktik Spesialis Keperawatan Medikal Bedah peminatan respirasi memiliki tujuan untuk mencapai kompetensi Ners Spesialis Respirasi. Pencapaian kompetensi dilakukan melalui 3 bentuk kegiatan, antara lain: pengelolaan 30 pasien gangguan sistem respirasi dengan menggunakan pendekatan teori model Adaptasi Roy, pengelolaan kasus kompleks dengan pendekatan teori model Adaptasi Roy, penerapan Tehnik Pernapasan Buteyko pada pasien asma sebagai suatu praktik keperawatan berbasis bukti (Evidence Based Practice Nursing), dan pelaksanaan inovasi keperawatan: pembuatan WSD Pionir dengan sistem 1 botol sebagai upaya peningkatan kualitas pelayanan dan keamanan pasien (patient safety). Praktik dilaksanakan selama 9 bulan di RSUP Persahabatan sebagai rumah sakit pusat rujukan nasional penyakit paru. Ners spesialis respirasi mencapai kompetensi sebagai expert practice, educator, innovator/agent of change, researcher, dan consultant selama praktik di rumah sakit. Hasil dari pencapaian kompetensi didapatkan kesimpulan bahwa teori model Adaptasi Roy dapat diterapkan dengan efektif pada kasus-kasus gangguan respirasi terutama kasus TB Paru, Kanker Paru, dan pasien Pneumonia. Penerapan teori Adaptasi Roy pada pasien dengan ventilator sulit diterapkan terutama pada mode pangkajian konsep diri, fungsi peran, dan interdependensi. Pelaksanaan EBNP tehnik pernapasan Buteyko selama 1 bulan memberikan pengaruh secara signifikan pada peningkatan kualitas hidup pasien asma, nilai Arus Puncak Respirasi (APE), dan nilai control pause (CP). Pembuktian keefektifan botol WSD Pionir terhadap kualitas dan kemananan pasien akan diuji secara ilmiah melalui proses penelitian.

The residency practicum of medical surgical nursing with a specialization in respiratory aimed to facilitate residents in achieving the competences as Respiratory Nurse Specialists. After the completion of the residency practicum, students would be able to achieve the competences as clinical experts, educators, innovators/ change agents, researchers, and consultants in respiratory care. Three major activities during the practicum were: 1) the application of Roy adaptation model- based nursing care and complex case management to 30 patients with respiratory disorders, 2) the application of Buteyko breathing method to asthmatic patients as an evidence- based nursing practice, and 3) the implementation of innovative project: the creation of water- seal drainage- pioneer with one- bottle system as an effort to improve patient safety and quality care. The practicum was completed in 9 months at Persahabatan Hospital as a top referral hospital for respiratory care. Results of the practicum activities revealed that Roy adaptation model was effective to improve patient safety as well as quality care to patients with respiratory disorders, particularly patients with pulmonary tuberculosis, lung cancer, and pneumonia. In addition, the results showed that the application of Buteyko breathing method for a month significantly enhanced the quality life of asthmatic patients, increased the respiratory peak flow and control pause values. Further studies, nonetheless, are required to examine the effectiveness of WSD pioneer to the improvement of patient safety and quality care."

"Praktik spesialis keperawatan medikal bedah bertujuan menerapkan peran perawat spesialis dalam memberikan asuhan keperawatan medikal bedah lanjut dalam menangani pasien sesuai dengan spesialisasinya untuk meningkatkan mutu pelayanan keperawatan. Praktik spesialisasi yang dilaksanakan menggunakan pendekatan teori model adaptasi Roy. Residen dalam pelasanaan praktik spesialisasi ini berperan dalam memberikan asuhan keperawatan pada satu kasus kelolaan dan 30 kasus resume pada pasien dengan gangguan sistem perkemihan. Peran residen yang berperan sebagai peneliti yaitu menerapkan tindakan keperawatan berdasarkan pada pembuktian ilmiah (evidence based nursing ) yaitu intervensi Auricular Acupressure terhadap xerostomia pada pasien gagal ginjal yang menjalani hemodialisis. Peran residen sebagai agen perubahan dalam pengembangan keperawatan yaitu melakukan kegiatan inovasi yaitu untuk meningkatkan kepatuhan pasien dalam membatasi cairan dengan judul sehat dengan pembatasan cairan (“SE-GARAN”).

Practice of Medical surgical nursing specialist aims to implement the role of specialist nurses in providing advanced medical surgical nursing care in treating patients based on the suitable specialization in order to improve the quality of nursing services. Specialization practice is carried out by using theoretical approach of Roy's adaptation model. Residents in this specialization practice give the role in providing nursing care in one managed case and 30 resume cases in patients with urinary system disorders. The resident role who acts as a researcher is to implement nursing actions based on scientific evidence (evidence based nursing), namely the Auricular Acupressure intervention for xerostomia in kidney failure patients that undergoing hemodialysis. The resident's role as a change agent in nursing development is to carry out innovative activities, namely to increase patient compliance in limiting fluids by using the healthy term with fluid restrictions ("SE-GARAN")."

The clinical practice of medical surgical nursing residency in patients with cardiovascular system disorders aims to implement the role of specialist nurse as care provider, educator, researcher, and innovator. The role of nursing care providers is have been done using the Roy Adaptation Model Theory approach in ADHF Acute Decompensated Heart Failure patients as well as 30 other patients with various other cardiovascular system disorders. The role of educator is performed by specialist nurses on patients as well as colleagues in hospital service arrangements. The implementation of EBN Evidence Based Nursing is the application of Ice Bag in the femoral region for pain in patients undergoing PCI Percutaneous Coronary Intervention performed to apply the role as a researcher. The innovation project is also carried out as a manifestation of the role of innovator through the implementation of innovation projects in the formulation of patient safety culture guidance using HSOPSC Hospital Survey On Patient Safety Culture questionnaire."

Kompetensi yang harus dicapai selama praktek residensi keperawatan medikal bedah adalah memberikan asuhan keperawatan dengan menggunakan teori model keperawatan, penerapan tindakan keperawatan berbasis bukti ilmiah (evidence based nursing practice) dan melakukan proyek inovasi. Target kompetensi dalam hal penerapan tindakan keperawatan berbasis bukti ilmiah adalah penerapan terapi akupresur untuk mengurangi mual muntah pada pasien kanker paru yang menjalani kemoterapi.Hasil penerapan menunjukkan bahwa akupresur efektif mengurangi mual muntah pada pasien kanker paru yang menjalani kemoterapi. Peran selanjutnya sebagai innovator, proyek inovasi yang dilakukan berupa penggunaan mobile phone application untuk pengkajian dan manajemen keperawatan mandiri pada pasien kanker paru. Hasil penerapan menunjukkan bahwa mobile phone application bermanfaat bagi pasien dalam manajemen gejala secara mandiri.

Competency that must be achieved during the residency practice of surgical medical nursing is to provide nursing care using the theory of nursing models, application of scientific evidence-based nursing practice, and do innovation projects. Role as a provider of nursing care, resident carries out nursing care using the Roy Adaptation Model theory approach in 30 patients with respiratory system disorders. The competency target in the application of scientific evidence-based nursing actions is the application of acupressure therapy to reduce nausea and vomiting in lung cancer patients undergoing chemotherapy.

The results of the application showed that acupressure effectively reduced nausea and vomiting on patients with lung cancer undergoing chemotherapy. The next role as an innovator, innovation projects carried out in the form of the use of mobile phone applications for the assessment and management of independent nursing on patients with lung cancer. The results of the application show that mobile phone applications are beneficial for patients in symptom management independently."

"Pelaksanaan praktek spesialis keperawatan merupakan bagian penting dalam meningkatkan kualitas pelayanan serta pendidikan berkelanjutan. Praktek spesialis keperawatan dilakukan dapat dikembangkan berdasarkan ilmu dan teknologi. Beberapa kompetensi yang harus dicapai oleh residen selama praktek spesialis keperawatan ini adalah mampu berperan sebagaipemberi asuhan keperawatan care provider manager, mampu menerapkan hasil penelitian berbasis hasil pembuktian ilmiah evidence based nursing practice, mampu melakukan inovasi Inovator. Dalam melakukan peran perawat sebagai pemberi asuhan keperawatan, residen melaksanakan asuhan keperawatan dengan menerapkan teori Model Adaptasi Roy pada pasien dengan gangguan sistem perkemihan sebanyak 30 pasien di Rumah Sakit Umum Pusat Fatmawati Jakarta dan RSPAD Gatot Soebroto Jakarta. Intervensi yang dilakukan untuk mengurangi pruritus yang dialami oleh pasien yaitu dengan melakukan terapi akupresur pada titik Li-11 berdasarkan hasil penelitian ilmiah pada 19 pasien hemodialisis di RSPAD Gatot Soebroto. Selain itu, residen bersama kelompok melakukan program inovasi keperawatan berupa edukasi mengenai intervensi untuk menangani komplikasi akut selama hemodialisis menggunakan video pada 30 orang pasien hemodialisis. Secara umum, penerapan Teori Model Adaptasi Roy dapat dilakukan oleh perawat dan diterapkan pada pasien dengan gangguan sistem perkemihan. Kualitas asuhan keperawatan yang baik, akan meningkatkan kualitas hidup pasien.

Several competencies has to be achieved and practiced during this residential practice are competencies to be care provider and manager, competencies to implement result of research which based on scientific evidence evidence based nursing practice and being innovative innovator. As. nursing care provider, practitioners implementing nursing care based on the implementation of Roy Adaptation Model on 30 patients with urinary disorder in Fatmawati Hospital and RSPAD Gatot Soebroto, Jakarta. Practitioners also do intervention to reduce pruritus in hemodialysis patient based on evidence based nursing The practitioners do acupressure in Li-11 accupoint based on result of scientific research on 19 patient with hemodialysis in Gatot Soebroto Hospital. Practitioners also create nursing innovation program related with the education about intervention for acute complication on 30 patient with hemodialysis in form of video. Generally, Roy Model of Adaptation theory is done by nurse and implemented on patient with urinary system problems. Good quality of nursing care will improve patient quality of life."
