Abstrak :
Skripsi ini membahas mengenai kualitas informasi yang dihasilkan oleh Puskesmas Sukmajaya Kota Depok masih rendah untuk pengambilan keputusan. Oleh karena itu, penelitian ini bertujuan untuk menghasilkan model manajemen informasi pemantauan wilayah setempat terkait kesehatan ibu yang dapat menghasilkan informasi berbasis fakta untuk pengambilan keputusan yang tepat sasaran. Penelitian ini menggunakan metode kualitatif dengan cara observasi dan wawancara, serta melakukan analisis sistem dan mengembangkan sistem yang sudah ada. Hasil analisis dari penelitian ini adalah adanya kekurangan dan kendala dari sistem yang sudah berjalan di Puskesmas Sukmajaya sehingga mempengaruhi kualitas dari informasi. Sehingga hasil dari penelitian adalah sebuah rancangan sistem informasi pencatatan dan pelaporan yang berbasis spreadsheet dan database untuk pelayanan kesehatan ibu di Puskesmas Sukmajaya Kota Depok. ......This thesis describes about quality of information that is produced by Sukmajaya Primary Health Center, Depok. The quality of information that is produced by Sukmajaya Primary Health Center, is still low to make a decision. So, this research is in order to make information management model of surveilance that concern about maternal health care. So, it can produce information based of evidence to make a decision that can intervetion to certain target. This thesis use qualitative methods with observation, interview and analysis a system and then discover a existing system. The results of this research that information system at primary health center of sukmajaya has some weakness and constraint so it can influence quality of information. Result from this research is a discovery of registry and report information system based on spreadsheet and database for maternal health care at Sukmajaya Primary Health Center, Depok.

Latar belakang/ Tujuan Pengawasan mutu terhadap suatu produk obat yang akan dipasarkan merupakan hal yang diwajibkan. Selain instrumen yang terdapat pada laboratorium, aplikasi standar seperti Excel spreadsheet juga digunakan untuk mendukung tugas ini. Validasi dari aplikasi dibutuhkan agar hasil analisis sesuai dengan spesifikasi produk yang diuji, tidak terjadi kompleksitas dalam pengolahan data, dan keamanan dari data tersebut terjamin. Metoda Hasil analisis kadar Oxx pada sediaan injeksi Oyy yang dilakukan oleh analis PT Sydna Farma diverifikasi kesesuaian rumusnya dengan spesifikasi dan dibandingkan hasil perhitungannya dengan perhitungan manual Hasil Rumus-rumus yang digunakan untuk menghitung hasil analisis telah sesuai dan data-data yang penting tidak dapat diakses oleh pihak yang tidak berwenang mengubah hasil analisis Kesimpulan Spreadsheet telah sesuai dengan spesifisikasi dan tervalidasi untuk membantu analis dalam melakukan kegiatan pengawasan mutu suatu produk. ......Background/ Aims Quality control of a pharmaceutical product intended for the market is mandatory. Besides the instruments available in the laboratories, standard applications like Excel Spreadsheets are also performed to support this task. Validation of the application is required to ensure that the analysis results conform to the product specifications being tested, prevent data processing complexities, and guarantee the security of the data. Methods The analysis results of Oxx Content in Oyy injection formulations conducted by analysts at PT Sydna Farma were verified for the appropriateness of the formula and compared with manual calculations. Results The formulas used to calculating the analysis results were found to be accurate, and crucial data cannot be accessed or altered by unauthorized parties during the analysis process Conclusion The spreadsheet has met the specifications and has been validated to assist analysys in carrying out quality control activities for a product.

Tujuan dari praktik kerja profesi apoteker di PT. Medifarma Laboratories Periode Bulan Agustus-September Tahun 2017 adalah untuk memahami peranan, tugas dan tanggung jawab serta menambah wawasan, keterampilan dan pengalaman praktis sebagai apoteker di industri farmasi. Selain itu, kegiatan ini juga bertujuan untuk memahami penerapan CPOB dan gambaran nyata tentang masalah yang terjadi di industri farmasi. Tugas khusus yang dilakukan berjudul Pembuatan MS Excel Spreadsheet Analytical Report-Stability Sample. Tugas ini bertujuan untuk membuat Microsoft Excel Spreadsheet untuk Analitycal Report stability sample dan membuat protokol validasi dari Microsoft Excel Spreadsheet tersebut. Dari kegiatan praktik ini, penulis telah mengetahui peran, tugas, tanggung jawab, wawasan dan pengalaman praktis serta solusi dari permasalahan di industri farmasi. PT. Medifarma Laboratories juga telah menerapkan semua aspek CPOB dengan benar.

The purpose of the Internship at PT. Medifarma Laboratories Period August-September 2017 was to understand the role, duties and responsibilities and to add insight, skills and practical experience as pharmacists in the pharmaceutical industry. In addition, this activity also aimed to understand the application of the CPOB and a real picture of the problems occurring in the pharmaceutical industry. The special assignment was done entitled Making MS Excel Spreadsheet Analytical Report-Stability Sample. This assignment aimed to create a Microsoft Excel Spreadsheet for the Analitycal Report stability sample and to create a validation protocol of the Microsoft Excel Spreadsheet. Selain itu calon apoteker juga dapat memiliki gambaran nyata tentang permasalahan pekerjaan kefarmasian di industri farmasi, memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan pekerjaan kefarmasian di industri farmasi. Tugas khusus yang diberikan yaitu berjudul ldquo;Report Excel Spreadsheet Validation Tablet X Batch 1234 rdquo;. Departemen QC bertugas untuk menghasilkan, memvalidasi, dan menerapkan semua prosedur pemastian mutu.Seluruh kegiatan departemen QC hendaknya dilakukan berdasarkan prosedur tertulis dan bila perlu didokumentasikan.Pencatatan data hasil pengujian hendaknya didokumentasikan sedemikian rupa sehingga dapat dilakukan evaluasi dan investigasi apabila ditemukan hasil di luar spesifikasi. Dokumentasi proses dapat dilakukan dengan menggunakan sistem komputerisasi yang memenuhi standar keamanan tertentu.Salah satu perangkat lunak yang dapat digunakan dalam dokumentasi adalah spreadsheet pada aplikasi Microsoft Excel. Spreadsheet yang digunakan hendaknya divalidasi untuk memastikan akurasi, kepercayaan, konsistensi performa, serta kemampuannya untuk membedakan data yang valid dan invalid. Kegiatan validasi diawali dengan perencanaan dan hasilnya didokumentasikan dalam laporan. Perencanaan validasi dapat dicantumkan dalam bentuk protokol validasi yang merupakan dokumen terstruktur untuk kegiatan inspeksi. All activities of the QC department should be carried out according to written procedures and documented when necessary. Recording of test results data should be documented in such a way in which evaluation and investigation can be carried out if results are found outside the specifications. Process documentation can be done using a computerized system that meets certain security standards. One of the software that can be used in documentation is a spreadsheet in the Microsoft Excel application. The spreadsheet used should be validated to ensure accuracy, trustworthiness, consistency of performance, and its ability to distinguish valid and invalid data. Validation activities begin with planning and the results are documented in a report. The validation plan can be included in the form of a validation protocol which is a structured document for inspection activities. This document can clearly explain the validation process and a working report as a summary of the results and validation status of the spreadsheet.

Pertumbuhan bisnis e-commerce meningkatkan jumlah pengiriman barang ke rumah pelanggan. Permasalahan peningkatan volume barang yang akan dikirim ke rumah pelanggan adalah sistem distribusi last-mile delivery yang tidak efisien menyebabkan peningkatan kendaraan pengiriman yang beroperasi di perkotaan, dimana dapat menambah eksternalitas negatif seperti kemacetan, kebisingan dan polusi. Permasalahan lain yang sering muncul dari kegiatan last-mile delivery adalah kegagalan pengiriman, dimana akan menambah biaya karena perlu dilakukan pengiriman kembali ke rumah pelanggan. Penerapan strategi konsolidasi dalam sistem distribusi last-mile delivery merupakan solusi yang efisien untuk meminimalisir biaya distribusi. Konsolidasi dilakukan dengan menambahkan hub ke sistem distribusi. Tujuan dari penelitian ini adalah untuk menganalisis efisiensi sistem distribusi last-mile delivery dengan strategi konsolidasi untuk meminimalisir total biaya distribusi dengan mempertimbangkan biaya internal, biaya eksternal dan biaya peluang akibat penambahan hub pada sistem distribusi last-mile. Perencanaan sistem distribusi dengan beberapa skenario yang diselesaikan oleh Vehicle Routing Problem (VRP) dengan mempertimbangkan jenis kendaraan dan jendela waktu yang berbeda atau dikenal sebagai HFVRPTW (Heterogeneous Fleet Vehicle Routing Problem with Time Windows). Masalah VRP dalam penelitian ini diselesaikan dengan menggunakan software VRP Spreadsheet Solver. Selain itu, kendaraan listrik juga dinilai dapat digunakan sebagai kendaraan pengiriman diharapkan dapat mengurangi polusi dan meningkatkan kualitas lingkungan. Hasil penelitian berhasil mengurangi biaya distribusi dalam hal penurunan total jarak yang ditempuh dan total biaya distribusi. ......The growth of the e-commerce business increased the number of deliveries of goods to customers' homes. The problem of increasing the volume of goods to be delivered to customers' homes is the inefficient distribution system of last-mile delivery causes an increase in delivery vehicles operating in urban areas, which can add negative externalities such as congestion, noise and pollution. Another problem that often arises from last-mile delivery activities is a failure of delivery, where which will increase costs because it needs to be re-delivered to the customer's house. The implementation of a consolidation strategy in the last-mile delivery distribution system is an efficient solution to minimize distribution costs. Consolidation did by adding a hub to the distribution system. The purpose of this study is to analyze the efficiency of the last-mile delivery distribution system with a consolidation strategy to minimize the total distribution costs by considering internal cost, external cost and opportunity costs due to the addition of a hub in the last-mile distribution system. Distribution system planning with several scenarios solved by vehicle routing problems considering different vehicle modes and time windows are known as HFVRPTW (Heterogeneous Fleet Vehicle Routing Problem with Time Windows). The VRP problem in this study was solved using VRP Spreadsheet Solver software. In addition, electric vehicles are also considered to be used as delivery vehicles is expected to reduce pollution and improve environmental quality. The result of the study succeeded in reducing distribution costs in terms of the decrease in total distance travelled and total distribution costs.
