Abstrak :
Human Reliability Assessment (HRA) merupakan metode kualitatif dan kuantitatif untuk mengukur kontribusi manusia terhadap sebuah resiko. Skripsi ini membahas mengenai Human Reliability Assessment (HRA) pada proses printing kemasan plastic fleksibel pada PT. SMPI menggunakan metode Human Error Assessment and Reduction Technique (HEART) yang bertujuan untuk mengetahui nilai Human Error Probability (HEP). Penelitian ini adalah penelitian semikuantitatif dengan observasi langsung pada operator proses printing artikel MPPS pada perusahaan kemasan menggunakan mesin GR6. Nilai HEP tertinggi terdapat pada proses mesin yang sedang menjalankan proses pencetakan, kemungkinan kesalahan yang mungkin terjadi adalah operator lalai/membiarkan mesin produksi menyala. Dalam proses ini terdapat kemungkinan 65,8% human error. Dilanjutkan dengan pengukuran dan sinkronisasi jarak guide roller diatas silinder proses dimana kemungkinan kesalahan yang terjadi adalah gagal mengukur silinder, pada proses ini terdapat kemungkinan 60% human error. Dan yang ketiga, pada proses reverse check/surface print, size check (flange hole slope, raster depth, raster angle), dan Roughness check (HV-HARDNESS VICKERS) masing-masing memiliki probabilitas error yang sama yaitu sebesar 55,6 %. ......Human Reliability Assessment (HRA) is a qualitative and quantitative method to measure human contribution to a risk. This thesis discusses the Human Reliability Assessment (HRA) in the flexible plastic packaging printing process using the Human Error Assessment and Reduction Technique (HEART) method which aims to determine the value of Human Error Probability (HEP). This research is a semi- quantitative study with direct observation of operators of the MPPS article printing process at packaging companies using GR6 machines. The highest HEP value is found in the machine process that is running the printing process, the possibility of an error that might occur is the operator neglecting/letting the production machine turn on. In this process there is a 65.8% possibility of human error. Followed by measuring and synchronizing the distance of the guide roller above the process cylinder where the possible error that occurs is failure to measure the cylinder, in this process there is a possibility of 60% human error. And thirdly, in the reverse check/surface print process, size check (flange hole slope, raster depth, raster angle), and Roughness check (HV-HARDNESS VICKERS) each have the same error probability of 55.6%.

Skripsi ini membahas mengenai Human Reliability Assessment (HRA) pada proses pembangunan Jembatan Penyeberangan Orang (JPO) pada Proyek Stasiun LRT PT X menggunakan metode Human Error Assessment and Reduction Technique (HEART) dengan tujuan untuk mengetahui Human Error Probability (HEP) yang terdapat dalam proses pekerjaan pengeboran di pembangunan JPO. Penelitian ini adalah penelitian semikuantitatif dengan desain studi deskriptif yang dilakukan dengan observasi langsung proses pekerjaan pengeboran oleh 90 orang pekerja tukang bangunan di Stasiun LRT Bekasi Barat, Ciracas, dan Harjamukti. Hasil penelitian berupa jumlah HEP, dimana nilai HEP tertinggi di Stasiun LRT Bekasi Barat terdapat pada tugas Penentuan Titik Bor dengan jumlah 71,68% kemungkinan terjadinya kesalahan manusia, di Stasiun LRT Ciracas terdapat pada tugas Pengeboran dengan jumlah 49,87%, dan di Stasiun LRT Harjamukti terdapat pada tugas Pemasangan Pipa Tremi dengan jumlah 74,68% serta menyarankan adanya pengendalian dari jenis error serta kondisi yang memungkinkan error (EPCs) untuk mencegah kejadian human error yang dapat mengakibatkan kecelakaan kerja. ......This thesis discusses the Human Reliability Assessment (HRA) in the construction process of the pedestrian bridge at PT X LRT Station Project using Human Error Assessment and Reduction Technique (HEART) with the aim of knowing the Human Error Probability (HEP) contained in the drilling work process in the construction. This research is a semi-quantitative research with a descriptive study design conducted by direct observation of the drilling work process with 90 construction workers as the sample at West Bekasi, Ciracas, and Harjamukti LRT Stations. The results of the study are the number of HEP, where the highest HEP value at the West Bekasi LRT Station is in the Drill Point Determination task with a total of 71.68% the possibility of human error, at the Ciracas LRT Station there is in the Drilling task with a total of 49.87%, and at the Ciracas LRT Station the drilling task is 49.87%. The Harjamukti LRT is found in the Tremi Pipe Installation task with a total of 74.68% and suggests controlling the types of errors and conditions that allow errors (EPCs) to prevent human error events that can lead to work accidents.

Abstrak :
Assessing the reliability of complex multi-state systems comprising numerous sub-systems and components is one of the key challenges faced by industry. The effect of degradation of individual component on other associated components plays a vital role in entire system transition from a higher performance level to a lower performance level. The proposed work focuses on the effect of dependence or functional association among various components on system reliability. There are many components in any complex system that hold higher critical order and failure of these critical components may cause the decline in the performance level of the total system. This paper takes under consideration the critical order of each component along with their dependent nature and captures the effect of their degradation on overall system reliability.
