Abstrak :
Kegiatan pelayanan farmasi klinik yang dilaksanakan di apotek salah satunya mencakup pengkajian dan pelayanan resep. Resep yang dilayani di apotek cukup beragam, mulai dari resep untuk penyakit akut maupun kronis seperti diabetes.Tujuan dari skrining dan analisis resep obat antidiabetes adalah untuk mengetahui obat antidiabetes yang diresepkan oleh dokter, mengetahui jumlah resep yang mengandung obat antidiabetes, serta menilai kerasionalan penggunaan obat antidiabetes pada resep yang dilayani di Apotek Roxy Biak selama periode Juli 2022. Dari seluruh resep yang masuk selama periode tersebut, resep yang mengandung obat antidiabetes dicatat dan dikumpulkan data, kemudian dipilih dua resep untuk dikaji kelengkapan resep berdasarkan aspek administratif, farmasetik, serta klinis. Pengkajian 2 (dua) resep antidiabetes di Apotek Roxy Biak dilihat dari aspek administratif, farmasetika dan klinis sudah sesuai. Namun pada aspek administratif penulisan umur dan berat badan pasien tidak dituliskan. Aspek administratif terkait umur dan berat badan perlu dikonfirmasi kembali kepada pasien atau keluarga pasien yang mengambil obat di apotek, hal ini penting untuk mengkaji ketepatan pengobatan yang akan diterima oleh pasien. ......One of the clinical pharmacy service activities carried out in pharmacies includes assessment and prescription services. Praktek kerja profesi di Apotek Kimia Farma No. 295 Jatiwaringin Periode April 2018 bertujuan untuk memahami dan mendapatkan pengalaman praktis mengenai tugas dan tanggung jawab apoteker dalam pengelolaan apotek dan praktik pelayanan kefarmasian yang sesuai dengan ketentuan perundang-undangan dan etika yang berlaku. Praktek kerja profesi juga bertujuan untuk memberikan gambaran nyata tentang permasalahan praktik kefarmasian serta mempelajari strategi dan kegiatan yang dapat dilakukan dalam rangka pengembangan praktik kefarmasian. Praktik kerja profesi dilakukan selama satu bulan dengan tugas khusus yaitu ldquo;Pengkajian Resep Penyakit Ginjal rdquo; yang dilakukan terhadap resep penyakit ginjal yang diterima di Apotek Kimia Farma No. 295 Jatiwaringin selama tiga bulan terakhir yang bertujuan agar calon apoteker dapat mengetahui kelengkapan persyaratan resep dan dapat mengkaji resep baik dari segi administratif, farmasetik, dan klinis, serta dapat memberikan informasi mengenai obat pada resep dengan baik.

ABSTRACT
Internship at Apotek Kimia Farma No. 295 Jatiwaringin Period April 2018 aims to understand and gives experiences of duites and responsibilites of pharmacist in pharmacy management and gives practical experiences on practical pharmaceutical services in accordance to applicable laws and ethics. Internship also gives an insight of real problems on pharmaceutical practices. Internship was conducted for one month with special assignment ldquo;Screening of Kidney Disease Prescription rdquo; that was done on kidney disease prescriptions that was received by Apotek Kimia Farma No. 295 Jatiwaringin at the last 3 months, w hich purpose is so that the prospective pharmacist may know the completeness of a prescriptions and be able to screening prescriptions well from administrative, pharmaceutical, and clinical aspects, and gives good informations about drugs that was prescribed well.

Selain itu, diberikan tugas khusus yang berjudul lsquo;Analisis Resep Penyakit Kardiovaskular rsquo; bertujuan untuk menganalisis kerasionalan terapi kardiovaskular berdasarkan resep di Apotek.

Apothecary Intership at Apotek Kimia Farma is conducted for 1 month. The aims of this internship is apothecary student understands the duties and responsibilities of the pharmacist in the management, as well as perform pharmaceutical practices in accordance with applicable laws and ethics; and having the insight, knowledge, skill, and practical experience to perform pharmaceutical practices in Pharmacy. In addition, a special assignment entitled lsquo;Cardiovascular Disease Prescription Analysis rsquo; was aimed at analyzing the rationale of cardiovascular therapy based on a prescription in pharmacy.

Abstrak :
Pelayanan resep merupakan salah satu kegiatan pelayanan kefarmasian yang berupa kegiatan menyiapkan permintaan tertulis (resep) dari dokter, dokter gigi dan dokter hewan kepada apoteker untuk menyediakan dan menyerahkan obat yang tepat kepada pasien dan sesuai dengan peraturan yang berlaku. Akan tetapi, terkadang masih terdapat beberapa resep yang kurang tepat (resep irasional) yang disebabkan oleh pemilihan obat, dosis, dan cara penggunaan yang kurang tepat. Terdapat beberapa dampak yang mungkin diakibatkan karena kesalahan peresepan diatas, mulai dari biaya pengobatan yang lebih mahal hingga kematian. Salah satu pelayanan resep yang sering diberikan kepada pasien di apotek Kimia Farma cabang Pos Pengumben merupakan pelayanan resep PRB (pasien rujuk balik). Pasien rujuk balik (PRB) merupakan pemberian obat-obatan untuk pasien penyakit kronis dengan kondisi stabil untuk kebutuhan 1 bulan dan dapat diulang sebanyak 3 kali. Salah satu jenis penyakit kronik yang dilayani pada program pasien rujuk balik adalah penyakit diabetes. Diabetes melitus merupakan penyakit gangguan metabolisme yang menyumbangkan angka kematian tertinggi ketiga di Indonesia. Diabetes Melitus (DM) merupakan penyakit tidak menular yang memerlukan pengobatan yang kompleks. Banyaknya risiko komplikasi pada pasien DM menyebabkan peningkatan jumlah obat-obat yang harus dikonsumsi oleh pasien yang mengarah pada polifarmasi. Pemberian obat polifarmasi memiliki risiko yang tinggi terhadap terjadinya Drug Related Problems (DRPs) yang dapat membahayakan pasien. Laporan ini bertujuan untuk mengkaji resep obat pada pasien DM di Apotek Kimia Farma Kejayaan untuk mengetahui ada tidaknya DRPs. Pengumpulan informasi dilakukan dengan studi literatur terhadap setiap obat yang diresepkan. Pada hasil pengkajian resep, diperoleh masih banyaknya aspek yang belum terpenuhi, yaitu aspek administratif, berat badan, usia, nomor telepon dan SIP dokter perlu dicantukan. Pada aspek farmasetik bentuk sediaan dan data stabilitas perlu ditanyakan kembali kepada dokter penulis resep. Sementara pada aspek klinis, perlu dicek kembali dosis obat (resep 1), penambahan aturan pakai dan risiko interaksi obat yang dapat mengarah pada efek samping yang serius terhadap pasien. ......Diabetes Mellitus (DM) is a non-communicable disease that requires complex treatment. This report aimed to assest drug prescriptions in DM patients at Kimia Farma Kejayaan Pharmacy to determine whether DRPs were present. Information collection was carried out by studying the literature on each drug prescribed. Based on the results of the review of prescriptions, it was found that there were still many aspects that had not been fulfilled, namely administrative aspects, weight, age, telephone number and doctor's SIP that needed to be included. Regarding the pharmaceutical aspects of dosage forms and stability data, it is necessary to ask the prescribing doctor again. While on the clinical aspect, it is necessary to re-check the dosage of the drug (recipe 1), add the rules for use and the risk of drug interactions which can lead to serious side effects in patients.

Pengkajian resep termasuk kedalam bagian dari kegiatan farmasi klinis di Apotek yang merupakan bagian dari Pelayanan Kefarmasian yang langsung dan bertanggung jawab kepada pasien berkaitan dengan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai dengan maksud mencapai hasil yang pasti untuk meningkatkan kualitas hidup pasien. Kegiatan pengkajian Resep meliputi administrasi, kesesuaian farmasetik dan pertimbangan klinis. Penyakit infeksi di Indonesia masih termasuk dalam sepuluh penyakit terbanyak. Peresepan antibiotik di Indonesia yang cukup tinggi dan kurang bijak akan meningkatkan kejadian resistensi. Berdasarkan gambaran dari peresepan antibiotik yang cukup tinggi, maka diperlukan pengkajian terhadap skrining administrasi dan farmasetik resep antibiotik untuk mencegah terjadinya kesalahan pengobatan yang dapat merugikan pasien di Apotek Roxy Biak. Tujuan penulisan tugas khusus praktik kerja profesi apoteker ini yaitu pengkajian resep obat untuk dua resep antibiotik di apotek Roxy Biak pada bulan juli 2022 dari segi kajian administratif, klinis dan farmasetik. Hasil yang didapat yaitu obat yang ditebus pada Apotik Roxy Biak masih memiliki kekurangan pada kelengkapan kajian administratif, yaitu Berat Badan (BB), umur pasien, dan alamat pasien, paraf dan nomor telepon dokter. Pengkajian resep dan simulasi konseling dilakukan terhadap lima resep obat. Pembahasan ini dilakukan untuk memberikan gambaran pelaksanaan kegiatan pengkajian resep dan konseling di Apotek Kimia Farma 456 Cinere. Metode yang dilakukan dimulai dari pemilihan resep sebanyak 5 (lima) resep dengan penyakit yang berbeda antara satu dengan yang lainnya, melakukan pengkajian resep yang meliputi pengkajian administratif, farmasetik, dan klinis, serta membuat dialog konseling dari masing- masing resep tersebut sebagai gambaran pelaksanaan konseling di apotek. ......This report discusses the review of drug prescriptions and the creation of counseling simulations conducted at the Kimia Farma 456 Cinere Pharmacy. Praktik Kerja Profesi Apoteker di Rumah Sakit Umum Pusat (RSUP) Fatmawati dan Apotek Roxy Klender Periode Bulan Agustus – November Tahun 2020 bertujuan untuk mengetahui tugas dan tanggung jawab apoteker, menambah wawasan, pengetahuan dan keterampilan, serta pengalaman praktis untuk melakukan praktik kefarmasian di pelayanan kefarmasian seperti rumah sakit dan apotek. Selain itu, peserta praktik kerja profesi juga mendapat gambaran nyata tentang permasalahan praktik kefarmasian, serta dapat mempelajari strategi dan kegiatankegiatan yang dapat dilakukan dalam rangka pengembangan praktik. Tujuan pengerjaan tugas khusus ini adalah untuk melakukan Pemantauan Terapi Obat (PTO) pada pasien thalassemia anak dan menganalisis pola penyakit berdasarkan resep yang masuk di Apotek Roxy Klender.
