"Pengkajian resep adalah bagian dari pelayanan farmasi klinik yang dilakukan oleh Apoteker untuk menganalisa adanya masalah terkait obat dan menghindari terjadinya medication error terutama pada tahap peresepan (presribing error.  Penelitian ini bertujuan untuk mengkaji resep pasien dengan diagnosis hiperlipidemia di Apotek Kimia Farma 049 dan mengetahui adanya medication error. Hiperlipidemia merupakan suatu keadaan dimana terjadi peningkatan kadar kolesterol dengan atau tanpa peningkatan kadar trigliserida dalam darah. Penelitian ini menggunakan pendekatan deskriptif dengan metode analisis data resep pasien dengan diagnosis hiperlipidemia yang diterima di apotek selama periode penelitian. Fokus utama dari kajian ini adalah untuk menilai kepatuhan resep dan jenis obat yang diresepkan. Hasil pengkajian resep yang telah dilakukan secara administratif masih terdapat beberapa aspek yang kurang lengkap, seperti; Surat Izin Praktik (SIP) dokter, alamat dokter, nomor telepon dokter, paraf dokter, umur pasien, jenis kelamin pasien, dan berat badan pasien. Berdasarkan pertimbangan klinis secara umum dapat dikatakan tidak ada permasalahan yang terlalu signifikan tetapi hanya perlu monitoring mengenai efek samping pada masing-masing obat, serta konfirmasi kepada dokter mengenai interaksi beberapa obat yang bersifat moderate.

Prescription review is part of clinical pharmacy services carried out by pharmacists to analyze drug-related problems and avoid medication errors, especially at the prescribing stage (prescribing errors).  This study aims to examine prescriptions for patients diagnosed with hyperlipidemia at Kimia Farma 049 Pharmacy and determine whether there are medication errors. Hyperlipidemia is a condition where there is an increase in cholesterol levels with or without an increase in triglyceride levels in the blood. This study used a descriptive approach with a data analysis method for prescriptions from patients diagnosed with hyperlipidemia received at the pharmacy during the research period. The main focus of this study is to assess the fulfillment of prescriptions and the type of medication promised. The results of the review of prescriptions that have been carried out administratively still contain several aspects that are incomplete, such as; Doctor's Practice License (SIP), doctor's address, doctor's telephone number, doctor's initials, patient's age, patient's gender, and patient's weight. Based on clinical considerations in general, it can be said that there are no problems that are too significant, but it is only necessary to monitor the side effects of each drug, as well as confirm with the doctor regarding moderate drug interactions."

Metode yang digunakan adalah studi literatur. Hasil yang diperoleh menunjukkan, bahwa aspek administrasi berupa data berat badan dan nomor telepon dokter perlu dilengkapi pada kedua resep dan dari aspek farmasetik perlu ditanyakan kembali kepada dokter yang meresepkan terkait bentuk sediaan (resep 1) dan data stabilitas obat (resep 1 dan 2). Dari aspek klinis pada resep 1 perlu dilakukan penambahan aturan pakai medixon dan alopurinol sementara resep 2 terdapat potensi interaksi obat.

The use of drugs in large quantities can lead to an increased risk of medication errors (drug interactions and duplication of therapy), non-compliance and side effects of drugs, so it is necessary to study prescriptions, both administrative, pharmaceutical, and clinical to minimize the risk of medication errors due to the administration of polypharmacy drugs. This report aims to conduct a prescription review which includes administrative review, pharmaceutical review, and clinical review of polypharmacy prescriptions that enter the Roxy Depok Pharmacy. The method used is literature study. The results obtained show that the administrative aspects in the form of weight data and the doctor's telephone number need to be completed on both prescriptions and from the pharmaceutical aspect it is necessary to ask the prescribing doctor regarding the dosage form (prescription 1) and drug stability data (recipes 1 and 2). From a clinical aspect, prescription 1 needs to be added to the rules for using medixon and allopurinol, while prescription 2 has the potential for drug interactions."

"Apotek adalah sarana pelayanan kefarmasian tempat dilakukan praktik kefarmasian oleh Apoteker. Pengkajian resep merupakan salah satu standar pelayanan kefarmasian yang dilakukan di Apotek. Pengkajian resep dilakukan untuk melakukan analisa adanya permasalahan terkait obat dan apabila ditemukan masalah terkait obat harus segera dikonsultasikan kepada dokter penulis resep. Tujuan dilakukannya pengkajian resep untuk mencegah permasalahan terkait obat sebelum obat diserahkan kepada pasien sehingga tidak terjadi medication error yang berdampak pada kegagalan terapi dan efek obat yang tidak diharapkan yang merugikan pasien. Penelitian ini bertujuan untuk mengetahui kelengkapan administrasi, kesesuaian farmasetik dan kesesuaian klinis pada peresepan di Apotek Kimia Farma Menteng Huis. Dalam penelitian ini, dilakukan pengkajian resep Pasien Rujuk Balik (PRB) pasien asma dengan mengkaji secara administratif, farmasetik dan klinis. Pada persyaratan administratif, didapatkan ketidaklengkapan berat badan pasien dan paraf dokter. Pada pengkajian secara farmasetik, resep yang tertulis sudah sesuai bentuk dan kekuatan sediaannya. Pada pengkajian secara klinis, terdapat duplikasi dan interaksi pada beberapa obat. Dapat disimpulkan bahwa masih banyak ketidaksesuaian dalam penulisan resep yang diamati sehingga harus dikonsultasikan kepada dokter penulis resep.

Pharmacy is a pharmaceutical service facility where pharmacists practice pharmacy. Prescription review is one of the standard pharmaceutical services carried out at pharmacies. Prescription review is carried out to analyze drug-related problems and if a drug-related problem is found, a doctor who prescribes it must be consulted immediately. The purpose of reviewing prescriptions is to prevent drug-related problems before the drugs are dispensed to patients so that medication errors do not occur which result in therapy failure and unexpected drug effects that harm the patient. This study aims to determine the completeness of administration, pharmaceutical suitability and clinical suitability for prescriptions at the Kimia Farma Pharmacy, Menteng Huis. In this study, the prescription review of Back-Referral Program (PRB) was carried out in asthma patient by examining administrative, pharmaceutical and clinical aspects. In administrative requirements, incomplete patient weight and doctor's initials were obtained. In the pharmaceutical study, the written prescription have appropriate dosage forms and dosage strengths. In clinical studies, there are duplications and interactions with several drugs. It can be concluded that there are still many observed discrepancies in prescription writing, so a doctor who prescribes it must be consulted."

"Berdasarkan Pedoman tentang Standar Pelayanan Kefarmasian di Apotek, pengkajian resep merupakan salah satu aspek dari kegiatan farmasi klinis. Risiko terjadinya kesalahan atau medication error yang paling besar adalah prescribing error dengan persentase sebesar 99,12% yang dapat terjadi baik pada aspek administratif resep, aspek farmasetik, dan aspek klinis seperti polifarmasi atau interaksi obat. Adanya polifarmasi pada resep akan meningkatkan risiko terjadinya interaksi obat. Laporan ini disusun untuk menganalisa adanya masalah terkait obat pada resep polifarmasi baik dari aspek administratif, farmasetik, dan klinis. Pengkajian resep dilakukan terhadap 2 resep dengan polifarmasi mayor dan minor yang masuk ke Apotek Roxy Depok pada periode November 2022. Terdapat beberapa hal yang harus dilengkapi kembali terkait data berat badan pasien, nomor telepon dokter, dan paraf dokter untuk aspek administratif. Pada aspek farmasetik resep 1 perlu ditanyakan kembali kepada dokter yang meresepkan terkait bentuk sediaan ramipril dan kekuatan sediaan gliquidon. Pada aspek klinis resep 1 perlu ditambahkan terkait aturan penggunaan allopurinol, serta melakukan konfirmasi kembali kepada dokter yang meresepkan bahwa terdapat interaksi mayor yang perlu diwaspadai pada resep 2.

Based on the Guidelines on Standard for Pharmaceutical Services in Pharmacies, prescription screening is an aspect of clinical pharmacy activities. The greatest risk of medication error is prescribing error with a percentage of 99.12% which can occur both in the administrative aspects of the prescription, pharmaceutical aspects, and clinical aspects such as polypharmacy or drug interactions. The presence of polypharmacy in the prescription will increase the risk of drug interactions. This report was prepared to analyze drug-related problems in polypharmacy prescriptions in administrative, pharmaceutical, and clinical aspects. The prescription review was conducted on 2 prescriptions with major and minor polypharmacy that entered Apotek Roxy Depok in the November 2022 period. There are several things that must be completed again regarding patient’s weight data, doctor's telephone number, and doctor's initials for administrative aspects. In the pharmaceutical aspect, prescription 1 needs to be asked again to the prescribing doctor regarding the dosage form of ramipril and the strength of the gliquidon preparation. In the clinical aspect, prescription 1 needs to be added regarding the rules for using allopurinol, as well as confirming again to the prescribing doctor that there are major interactions that need to be watched out for in prescription 2.

"Apotek sebagai sarana pelayanan praktik kefarmasian bertanggung jawab dalam memberikan pelayanan farmasi klinik, termasuk pengkajian dan pelayanan resep. Peran apoteker penting dalam pengkajian resep agar pasien mendapatkan obat yang tepat dengan dosis dan tujuan terapi yang sesuai. Pengkajian resep perlu dilakukan berdasarkan ketentuan administratif, kesesuaian farmasetik, ketentuan terapi dan farmakologis. Resep rasional meliputi tepat indikasi, pemilihan obat, dosis, waktu pemberian, rute dan cara pemberiannya untuk mengurangi kejadian medication error seperti efek samping yang tidak dihendaki. Diabetes merupakan penyakit kronis yang terjadi ketika pankreas tidak dapat memproduksi insulin yang cukup atau tidak dapat menggunakan insulin yang diproduksi secara efektif. Tatalaksana utama diabetes adalah pengendalian kadar glukosa untuk mencapai kadar glikemik yang optimal dengan obat antidiabetes. Pada tugas khusus ini dilakukan analisa kelengkapan dan rasionalitas penggunaan obat antidiabetes pada resep yang dilayani Apotek Roxy Biak selama Periode Januari 2023. Sebanyak tiga resep yang memiliki jumlah obat minimal tiga dan terdapat pengobatan diabetes pada periode bulan Januari 2023 dan melakukan studi literatur untuk menilai rasionalitas peresepan agen antidiabetes. Seluruh resep dianalisis berdasarkan aspek administratif, farmasetik dan farmakologis. Dari ketiga resep yang ditelaah secara administratif, farmasetik dan klinis, ditemukan beberapa kekurangan aspek administratif seperti usia pasien dan paraf dokter. Secara farmasetik dan klinis, seluruh obat yang diresepkan rasional, dosis dan aturan pakai tepat. Formulir pengkajian resep sebaiknya digunakan untuk mendokumentasikan kelengkapan aspek administratif, farmasetik maupun klinis dari resep-resep yang dilayani. Peran aktif apoteker untuk menyampaikan informasi terkait obat dan saran non farmakologis juga penting untuk meningkatkan kepatuhan pasien dalam penggunaan obat.

The pharmacy is responsible to serve its purpose to provide clinical pharmaceutical care including prescription screening and service to provide patients with the right drug with the appropriate dosage and therapeutic goals. Prescription review is analyzed through the pharmaceutical, clinical and pharmacological aspects. Rational prescribing includes the right indication, drug selection, dosage, time of administration, route and method of administration to reduce the incidence of medication errors such as adverse effects. Diabetes is a chronic disease that occurs when the pancreas does not produce enough insulin or cannot use the insulin it produces effectively. The main management of diabetes is controlling glucose levels to achieve optimal glycemic levels with antidiabetic drugs. In this report, analysis on prescriptions of diabetic drugs was carried out on prescriptions received by Roxy Biak Pharmacy in January 2023. Three prescriptions containing a minimum of three antidiabetic agents were sampled, followed by literature study to assess rational drug prescription. All prescriptions were analyzed based on administrative, pharmaceutical and pharmacological aspects. Of the three prescriptions that were reviewed several administrative aspects were found to be lacking such as the patient's age and doctor's initials. All prescriptions fulfilled the pharmaceutical and clinical aspects whereby all the drugs prescribed with rational dosages. A prescription review form should be used to document the completeness of the administrative, pharmaceutical and clinical aspects of the prescriptions. The active role of pharmacists in providing drug-related information and non-pharmacological treatments is also pivotal to increase patient adherence in drug use."

"Apoteker harus memahami dan menyadari kemungkinan terjadinya kesalahan pengobatan (medication error) dalam proses pelayanan dan mengidentifikasi, mencegah, serta mengatasi masalah terkait obat , masalah farmakoekonomi, dan farmasi sosial (socio-pharmacoeconomy). Untuk menghindari hal tersebut, Apoteker harus menjalankan praktik sesuai standar pelayanan. Pengkajian resep adalah salah satu bagian dari layanan farmasi klinik yang dilakukan oleh apoteker untuk menganalisa adanya masalah terkait obat dan menghindari terjadinya medication error terutama pada tahap peresepan (presribing error). Penelitian ini bertujuan untuk mengkaji beberapa contoh resep pada Apotek Kimia Farma 494 Beji. Pelaksanaan dilakukan secara deskriptif dengan melakukan pengkajian resep berdasarakan aspek administratif, aspek farmasetik, dan aspek klinis sesuai dengan Permenkes No. 73 Tahun 2016. Pengkajian dari aspek administratif menunjukkan bahwa resep yang telah dikaji masih terdapat resep tidak memiliki data berat badan pasien, SIP dokter, paraf dokter, jenis kelamin pasien, tanggal penulisan resep, umur pasien, dan tanpa nama dokter. Pengkajian dari aspek farmasetik menunjukkan bahwa terdapat resep yang tidak memenuhi kriteria dari segi bentuk sediaan dan ketercampuran obat. Pengkajian dari aspek klinis menunjukkan bahwa terdapat resep yang tidak sesuai dengan dosis penggunaan obat, aturan penggunaan obat, lama penggunaan obat, duplikasi, dan adanya interaksi.

Pharmacists must understand and be aware of the possibility of medication errors during the service process and identify, prevent, and address issues related to drugs, pharmacoeconomics, and social pharmacy. To avoid this, pharmacists must carry out practices according to service standards. Prescription review is one part of clinical pharmacy services performed by pharmacists to analyze drug-related problems and prevent medication errors, especially at the prescribing stage. This study aims to assess several prescriptions at Kimia Farma 494 Beji Pharmacy. Implementation is carried out descriptively by reviewing prescriptions based on administrative aspects, pharmaceutical aspects and clinical aspects in accordance with Ministry of Health Regulation No. 73 of 2016. Administrative aspect assessments show that the prescriptions reviewed still lack patient weight data, doctor's SIP, doctor's signature, patient gender, prescription writing date, patient age, and doctor's name. Pharmaceutical aspect assessments indicate that there are prescriptions that do not meet criteria in terms of dosage form and drug compatibility. Clinical aspect assessments reveal that there are prescriptions that do not comply with drug dosage, drug use rules, duration of drug use, duplication, and presence of interactions."

"Pengkajian resep merupakan kegiatan farmasi klinik yang meliputi pengkajian admininstratif, kesesuaian farmasetis, dan kesesuaian klinis untuk meminimalkan terjadinya kesalahan pengobatan. Penelitian ini bertujuan untuk mengetahui gambaran kepatuhan dari petugas farmasi dan menganalisa hubungan karakteristik individu, motivasi, kepemimpinan, imbalan dan suber daya terhadap kepatuhan dari petugas farmasi dalam melaksanakan pengkajian resep di Rumah Sakit Umum Ganesha Tahun 2018. Penelitian ini adalah penelitian kuantitatif dengan desain penelitian cross sectional dengan menggunakan kuisioner yang disebar ke seluruh petugas farmasi. Total jumlah sampel sebanyak 31 orang petugas farmasi. Hasil penelitian menyatakan lebih dari setengah petugas farmasi memiliki kepatuhan kurang terhadap pelaksanaan pengkajian resep. Variabel yang mempengaruhi kepatuhan petugas farmasi dalam melaksanakan pengkajian resep adalah masa kerja p=0,003, kepemimpinan p=0,017 dan imbalan p=0,001. Perlu dilakukan manajemen SDM yang baik, evaluasi pemberian imbalan dan membuat kebijakan terkait reward serta punishment yang adil bagi petugas farmasi. Selain itu, rutin dilaksanakannya pelatihan dan pengawasan untuk mengevaluasi hasil kerja dari petugas farmasi dalam melaksanakan pengkajian resep.Kata Kunci :Kepatuhan, pengkajian resep, petugas farmasi.

Prescription screening is a clinical pharmacy activity contain of an administrative screening, pharmaceutics compatibility and clinic compatibility to minimalize medication errors. The aims of this study is to understand compliance overview from the pharmacy officer and analyze the relationship of individual characteristic, motivation, leadership, reward and resources with compliance from pharmacy officer in order to do screening prescription at Ganesha General Hospital in 2018. The method of this study was a cross sectional quantitative study using questionnaire to 31 pharmacy officer as a sample.

The result of the study showed that more than half of the pharmacy officer have less compliance to prescription screening activity. The variables that affected pharmacy officer compliance in doing prescription screening was working period p 0,003, leadership p 0,017 and reward p 0,001. An effective human resource management, reward evaluation, a fair reward and punishment policyfor pharmacy officer are needed. In addition, routine training and supervision are also important to evaluate the performance of pharmacy officer in doing prescription screening.Key Words Compliance, prescription screening, Pharmacy officer."

"Pengkajian resep adalah hal yang perlu dilakukan untuk melihat apakah pengobatan yang dilakukan bersifat rasional. Hal ini perlu diperhatikan khususnya pada pasien jantung koroner dan hipertensi karena pasien umumnya mengkonsumsi jumlah obat yang cukup banyak. Pengkajian resep dilakukan di Apotek Kimia Farma 348 Dramaga pada periode bulan Januari – Maret 2023. Telaah dilakukan dengan memperhatikan tiga aspek, yaitu telaah administratif, farmasetik, dan klinis pada 20 resep. Berdasarkan hasil telaah yang dilakukan, didapatkan hasil, yaitu pada aspek telaah administratif, terdapat beberapa syarat yang belum terpenuhi, seperti kurangnya informasi mengenai jenis kelamin, berat badan, dan usia pasien yang dapat diatasi dengan melakukan konfirmasi kembali pada rumah sakit dan dokter atau pasien yang bersangkutan; pada aspek telaah farmasetik memenuhi seluruh syarat; dan pada aspek telaah klinis terdapat beberapa syarat yang belum terpenuhi, seperti adanya polifarmasi dan kemungkinan interaksi obat. Permasalahan ini dapat diatasi dengan memberikan edukasi pada pasien untuk memberikan jeda konsumsi obat serta memberikan informasi mengenai pentingnya ketepatan waktu konsumsi obat untuk mendapatkan efek terapi yang maksimal.

Prescription review needs to be done to see whether patient’s treatment is rational. This needs to be considered especially in patients with coronary heart disease and hypertension because patients generally consume a large number of drugs. A prescription review was carried out at Kimia Farma 348 Dramaga Pharmacy in the period January - March 2023. The review was carried out by paying attention to three aspects, namely administrative, pharmaceutical and clinical reviews on 20 recipes. Based on the results of the review carried out, the results were obtained, namely the aspect of administrative review, there were several conditions that had not been fulfilled, such as a lack of information regarding the patient's gender, weight, and age which could be overcome by confirming to the hospital, doctor, or patient; pharmaceutical review aspect meets all requirements; and in clinical aspect there are several requirements that have not been fulfilled, such as the existence of polypharmacy and the possibility of drug interactions. This problem can be overcome by educating patients to pause in taking drugs and providing information about the importance of taking medication on time to get the maximum therapeutic effect."
