Pusat Kesehatan Masyarakat atau disebut juga Puskesmas berdasarkan Peraturan Menteri Kesehatan Nomor 30 Tahun 2014 tentang Standar Pelayanan Kefarmasian di Puskesmas merupakan unit pelaksana teknis dinas kesehatan kabupaten/kota yang bertanggung jawab menyelenggarakan pembangunan kesehatan di suatu wilayah kerja. Pembangunan kesehatan yang dilakukan di Puskesmas salah satunya ditunjang oleh pelayanan kefarmasian.Sebagai dasar untuk menjamin pelayanan kefarmasian yang bermutu di Puskesmas, pemerintah telah menetapkan standar pelayanan kefarmasian di Puskesmas berdasarkan Peraturan Menteri Kesehatan No 30 Tahun 2014 Tentang Standar Pelayanan Kefarmasian di Puskesmas, yaitu standar pengelolaan obat dan bahan medis habis pakai serta pelayanan farmasi klinik. Selain menjamin mutu pelayanan kefarmasian, standar pelayanan tersebut bertujuan untuk menjamin kepastian hukum bagi tenaga kefarmasian dan melindungi pasien dan masyarakat dari penggunaan obat yang tidak rasional dalam rangka keselamatan pasien (patient safety). Pengelolaan obat dan bahan medis habis pakai meliputi kegiatan perencanaan kebutuhan, permintaan, penerimaan, penyimpanan, pendistribusian, pengendalian, pencatatan, pelaporan, pengarsipan, dan pemantauan serta evaluasi pengelolaan. Sedangkan pelayanan farmasi klinik meliputi pengkajian resep, penyerahan obat, dan pemberian informasi obat; Pelayanan Informasi Obat (PIO); konseling; ronde/visite pasien (khusus Puskesmas rawat inap); pemantauan dan pelaporan efek samping obat; pemantauan terapi obat; dan evaluasi penggunaan obat

"Apotek Safa sebagai tempat dilakukan praktik kefarmasian oleh apoteker merupakan bagian dari saranan pelayanan kesehatan memegang peranan penting dalam hal pelayanan kefarmasian. Menurut Peraturan Menteri Kesehatan Nomor 35 Tahun 2014 Tentang Standar Pelayanan Kefarmasian di Apotek, pekerjaan kefarmasian terdiri atas kegiatan pengelolaan perbekalan farmasi dan pelayanan farmasi klinik. Kegiatan pengelolaan perbekalan farmasi di apotek meliputi kegiatan perencanaan, pengadaan, penerimaan, penyimpanan, pemusnahan, pengendalian, pencatatan dan pelaporan. Untuk kegiatan pelayanan farmasi klinik terdiri dari kegiatan pengkajian resep, dispensing, Pelayanan Informasi Obat (PIO), konseling, pelayanan kefarmasian di Rumah (Home Pharmacy Care), Pemantauan Terapi Obat (PTO), dan Monitoring Efek Samping Obat (MESO). Seiring dengan perkembangan ilmu pengetahuan dan teknologi di bidang kefarmasian dan kesehatan telah terjadi pergeseran paradigma dari pelayanan kefarmasian yang berorientasi kepada pengelolaan obat sebagai komoditi (drug oriented) menjadi pelayanan kefarmasian yang berorientasi ke pasien (patient oriented) dengan mengacu pada pelayanan kefarmasian (pharmaceutical care). Apoteker sebagai pelaku utama pelayanan kefarmasian memiliki wewenang untuk memberikan pelayanan langsung dan bertanggung jawab kepada pasien yang berkaitan dengan sediaan farmasi, sesuai dengan kompetensi pendidikan yang diperolehnya, dengan maksud mencapai hasil yang pasti untuk meningkatkan mutu kehidupan pasien.

Apotek Safa, which is a place for pharmacist to do their pharmaceutical practices, is part of the proposition of healthcare and plays an important role in terms of pharmacy services. According to the Minister of Health Regulation no. 35 on 2014 about the standards of pharmaceutical services in drugstore, pharmacy practice are consisted of pharmaceutical supply management and clinical pharmacy services. Management of pharmaceuticals supplies in drugstore includes planning, procurement, receipt, storage, destruction, control, recording and reporting. For clinical pharmacy service activities are consisted of prescription assesment, dispensing, drug information service (PIO), counseling, home pharmacy care, therapeutic drug monitoring (PTO), and drug side effects monitoring (MESO). Along with the development of science and technology in the field of pharmacy and health, there has been a shift in the paradigm of pharmaceutical care from the orientation to the management of the drug as a commodity (drug oriented) into pharmaceutical services oriented to the patient (patient-oriented) in which refer to the pharmaceutical care. Pharmacists as the main perpetrators of pharmaceutical care have the authority to provide direct services and responsible to the patient associated with a pharmaceutical preparation, according to their educational competence, with the intention of reaching a definitive result to improve the quality of life of patients."

"Standar pelayanan kefarmasian merupakan tolak ukur yang dipergunakan sebagai pedoman bagi tenaga kefarmasian dalam menyelenggarakan pelayanan kefarmasian. Pengaturan standar pelayanan kefarmasian di apotek bertujuan untuk meningkatkan mutu pelayanan kefarmasian, menjamin kepastian hukum bagi tenaga kefarmasian, dan melindungi pasien dan masyarakat dari penggunaan obat yang tidak rasional dalam rangka keselamatan pasien. Mengingat pentingnya pelayanan kefarmasian yang harus diberikan oleh tenaga farmasi di apotek, maka tugas khusus ini dilakukan yang untuk mengamati pelaksanaan kegiatan pelayanan kefarmasian oleh petugas apotek di Aotek Kimia Farma 382 dan Pusat Pelayanan Obat RS Bhayangkara Brimob Depok kepada pasien. Data dalam tugas khusus ini dikumpulkan secara deskriptif berupa standar operasional prosedur (SOP) sistem penerimaan resep umum dan kredit. Dari hasil analisis mengenai sistem pelayanan kefarmasian pada Apotek Kimia Farma 382 dan Pusat Pelayanan Obat RS Bhayangkara Depok telah melakukan kegiatan pelayanan kefarmasian sesuai dengan Permenkes no. 73 Tahun 2016 tentang Sandar Pelayanan Kefarmasian dengan baik. Sistem pelayanan kefarmasian yang dilakukan yaitu pengkajian resep, dispensing, pelayanan informasi obat, dan pelayanan kefarmasian di rumah.

Pharmaceutical service standards are benchmarks used as guidelines for pharmaceutical personnel in administering pharmaceutical services. Setting pharmaceutical service standards in pharmacies aims to improve the quality of pharmaceutical services, guarantee legal certainty for pharmaceutical staff, and protect patients and the public from irrational drug use in the framework of patient safety. Given the importance of pharmaceutical services that must be provided by pharmacists in pharmacies, this special task was carried out to observe the implementation of pharmaceutical service activities by pharmacists at Kimia Farma 382 Pharmacy and Drug Service Center at Bhayangkara Brimob Hospital, Depok, to patients. Data in this special assignment were collected descriptively in the form of standard operational procedures (SOP) for general prescription and credit acceptance systems. From the results of an analysis of the pharmaceutical service system at the Kimia Farma 382 Pharmacy and the Drug Service Center at the Bhayangkara Hospital, Depok, pharmaceutical service activities have been carried out in accordance with Permenkes no. 73 Tahun 2016 about Good Pharmaceutical Service Standards. The pharmaceutical service system that is carried out is prescription review, dispensing, drug information services, and home pharmacy services."

"Menurut Peraturan Menteri Kesehatan No. 74 Tahun 2016 mengenai Standar Pelayanan Kefarmasian di Puskesmas, beberapa tugas apoteker dalam pelayanan farmasi klinik yang dapat meningkatkan kepatuhan pasien dalam meminum obat adalah pelaksanaan pelayanan informasi obat dan konseling. Pemberian konseling juga penting terutama pada pasien dengan penyakit kronis agar pasien memahami terkait pengobatan yang dilakukan, memiliki pengetahuan mengenai penyakitnya, dan mengetahui resiko jika tidak meminum obat secara tepat. Pengamatan kepatuhan dilakukan di Puskesmas Matraman di bagian Instalasi Farmasi dari pukul 07.30 – 16.00 dengan menggunakan kuisioner MMAS-8 dan wawancara terhadap pasien yang diresepkan obat Diabetes Mellitus dengan teknik accidental sampling. Kepatuhan pasien diabetes mellitus di Puskesmas Matraman dalam menggunakan obat adalah sebanyak 18 responden memiliki kepatuhan yang rendah, 13 responden dengan kepatuhan sedang, dan 2 responden dengan kepatuhan yang tinggi. Karakteristik yang memiliki hubungan signifikan dengan kepatuhan adalah umur dan lama pengobatan. Hal ini disebabkan karena semakin lama responden telah melakukan pengobatan, semakin menunjukkan pemahamannya dalam tujuan dari pengobatan yang dilakukan serta umur responden mempengaruhi proses degenerasi dari organ tubuh manusia, salah satunya penurunan memori yang menyebabkan meningkatnya resiko kelupaan pasien dalam meminum obat. Peran apoteker dalam meningkatkan dan mempertahankan kepatuhan pasien dalam minum obat diabetes melitus adalah dengan melakukan pelayanan informasi obat secara tepat dan konseling dengan baik agar tersampaikannya informasi dengan baik mengenai pengobatan yang sedang dilaksanakan pasien.

According to Minister of Health Regulation No. 74 of 2016 concerning Standards for Pharmaceutical Services at Community Health Centers, some of the duties of pharmacists in clinical pharmacy services that can increase patient compliance in taking medication are the implementation of drug information and counseling services. Counseling is also important, especially for patients with chronic diseases so that patients understand the treatment being carried out, have knowledge about their disease, and know the risks if they do not take the medicine properly. Observation of compliance was carried out at the Matraman Health Center in the Pharmacy Installation section from 07.30 – 16.00 using the MMAS-8 questionnaire and interviewing patients prescribed Diabetes Mellitus drugs using accidental sampling techniques. Compliance with diabetes mellitus patients at the Matraman Health Center using the drug was 18 respondents who had low adherence, 13 respondents who had moderate adherence, and 2 respondents who had high adherence. Characteristics that have a significant relationship with adherence are age and duration of treatment. This is because the longer the respondent has been taking treatment, the more he shows his understanding of the purpose of the treatment being carried out, and the age of the respondent affects the degeneration process of the human body's organs, one of which is memory loss which causes an increased risk of patient forgetfulness in taking medication. The role of pharmacists in improving and maintaining patient adherence to taking diabetes mellitus medication is to provide appropriate drug information services and good counseling so that good information is conveyed regarding the treatment being carried out by the patient."

"Apotek adalah tempat dimana pasien mencari obat. Pelayanan kefarmasian yang terdapat di apotek tidak hanya mencakup penyiapan dan penyerahan obat, hal lain yang perlu diperhatikan yaitu seperti fasilitas apotek dan ketersediaan obat yang ada di apotek. Agar pelayanan kefarmasian dapat mencukupi kebutuhan pasien, maka diperlukan survei pada pasien apotek terkait fasilitas yang perlu ditambahkan, layanan yang perlu ditingkatkan, dan obat yang harus selalu tersedia di apotek. Survei dilakukan pada pasien Apotek Kimia Farma 628 Sukahati yang membeli produk di apotek dan bersedia menjawab kuesioner. Adapun kuesioner berisikan pertanyaan mengenai data demografis pasien (nama, jenis kelamin, usia, jumlah berkunjung, penyakit, dan obat yang digunakan/dibeli) serta data kebutuhan pasien (fasilitas yang perlu ditambahkan, layanan yang perlu ditingkatkan, dan obat yang harus selalu tersedia di Apotek Kimia Farma 628 Sukahati). Kebutuhan pasien yang paling diinginkan ada di apotek dari hasil survei pada 65 pasien yaitu adanya praktek dokter umum dan terjaganya ketersediaan obat di apotek, terutama untuk obat swamedikasi yang menjadi obat yang paling sering dicari di Apotek Kimia Farma 628 Sukahati. Apotek Kimia Farma 628 Sukahati dapat meninjau kembali pengadaan obat di apotek untuk menghindari kekosongan obat dan membuka layanan praktek dokter umum di apotek.

A pharmacy is a place where patients look for medicine. Pharmaceutical services in the pharmacy do not only cover the preparation and delivery of drugs, there are other things that need to be considered such as pharmacy facilities and the availability of drugs in the pharmacy. In order for pharmacutical services to be sufficient for patient needs, a survey is needed on pharmacy patients regarding facilities that need to be added, services that need to be improved, and medicines that must always be available in the pharmacy. The survey was conducted on Kimia Farma 628 Sukahati Pharmacy patients who bought products at the pharmacy and were willing to answer the questionnaire. The questionnaire contains questions regarding patient demographic data (name, gender, age, number of visits, disease, and medicines used/purchased) as well as patient needs data (facilities that need to be added, services that need to be improved, and medicines that must always be available at Kimia Farma 628 Sukahati Pharmacy). According to the results of a survey of 65 patients, the most desired patient needs were the opening of general practitioner practice in the pharmacy and the sure availability of medicines in the pharmacy, especially for self-medication which is the most frequently sought after drug at Kimia Farma 628 Sukahati Pharmacy. Kimia Farma 628 Sukahati Pharmacy can review medicines procurement at pharmacies to avoid medicine shortages and open general practitioner practice services at the pharmacy."

Tugas khusus yang diberikan berjudul Kegiatan Pelayanan Kefarmasian Di Rumah Home Pharmacy Care Pada Pasien Hipertensi Di Apotek Kimia Farma No. 352 Depok. Professional Practices for Pharmacists in Apotek Kimia Farma No. 352 was performed so that pharmacists students understood the pharmacists rsquo duties and responsibilities in pharmacy management, also to performed pharmaceutical services according to the applied regulations and ethical standards, had knowledge, information, ability, and practical experience to performed pharmaceutical practices in a pharmacy, and had an actual picture about pharmaceutical problems usually occur and also to learned the strategies and activities which could be performed to develop the pharmaceutical practices. The additional task given was written and titled Pharmaceutical Services Activities at Home Home Pharmacy Care in Hypertension Patients in Apotek Kimia Farma No. 352 Depok. The purpose of this additional task was so that pharmacist students understood pharmacists rsquo role in pharmaceutical practices, especially in home pharmacy care, had knowledge, information, ability, and practical experience to perform home pharmacy care, and had an actual picture about problems usually occur in home pharmacy care, and also to learned the strategies and activities could be performed to develop home pharmacy care."

"Pada pelaksanaan beberapa pelayanan di Puskesmas Kecamatan Ciracas, kegiatan penyerahan obat yang merupakan salah satu pelayanan kefarmasian juga dilakukan dengan bantuan dari tenaga kesehatan non farmasi. Meskipun begitu, demi memberikan pelayanan kefarmasian yang sesuai dengan standar yang berlaku, serta untuk meningkatkan pengetahuan dan keterampilan bagi tenaga kesehatan non farmasi yang melaksanakan pelayanan kefarmasian tersebut, maka perlu dilakukan Sosialisasi Pelayanan Kefarmasian. Oleh karena itu, tugas khusus ini bertujuan untuk memastikan terlaksananya kegiatan dan mengetahui capaian pengetahuan dari peserta kegiatan sosialisasi pelayanan kefarmasian kepada tenaga medis lain yang didelegasikan. Tugas khusus ini disusun dengan mengumpulkan dokumen dari kegiatan sosialisasi pelayanan kefarmasian yang telah dilakukan sejak tahun 2017 hingga tahun 2022, kemudian diperiksa kelengkapan dan bukti pelaksanaan kegiatan. Setelahnya, dilakukan perekapan pada nilai post test yang telah dikerjakan oleh peserta sebagai parameter keberhasilan penyampaian sosialisasi pelayanan kefarmasian apakah telah dipahami atau tidak. Sosialisasi pelayanan kefarmasian di Puskesmas Kecamatan Ciracas sudah berjalan dengan baik setiap tahunnya dari 2017 sampai 2022. Rata-rata persentase peserta yang terdiri atas Dokter gigi, Bidan, dan Perawat, untuk lulus sebesar 98,75% dalam lima tahun (2017, 2018, 2019, 2021, dan 2022) dan telah memahami materi terkait pelayanan kefarmasian serta didelegasikan sebagai tenaga kesehatan non farmasi yang dapat melaksanakan pelayanan kefarmasian.

In the implementation of several services at the Ciracas District Health Center, drug delivery activities, which are one of the pharmaceutical services, are also carried out with the assistance of non-pharmaceutical health workers. However, in order to provide pharmaceutical services that comply with applicable standards, as well as to increase the knowledge and skills of non-pharmaceutical health workers who carry out these pharmaceutical services, it is necessary to carry out Socialization of Pharmaceutical Services. Therefore, this special task aims to ensure the implementation of activities and determine the knowledge achievements of participants in socialization activities on pharmaceutical services to other delegated medical personnel. This special task was prepared by collecting documents from socialization activities for pharmaceutical services that had been carried out from 2017 to 2022, then checking for completeness and proof of implementation of the activities. Afterwards, a recap of the post test scores that had been completed by the participants was carried out as a parameter for the success of delivering socialization on pharmaceutical services, whether they had been understood or not. Socialization of pharmaceutical services at the Ciracas District Health Center has been going well every year from 2017 to 2022. The average percentage of participants consisting of dentists, midwives and nurses to graduate is 98.75% in five years and have understood the material related to pharmaceutical services and have been delegated as non-pharmaceutical health workers who can carry out pharmaceutical services."

"Praktik Kerja Profesi Apoteker di Apotek Kimia Farma No. 389 bertujuan untuk memahami peranan, tugas, dan tanggung jawab apoteker di apotek serta melihat secara langsung proses pengelolaan apotek, dan melakukan praktik pelayanan kefarmasian sesuai dengan ketentuan peraturan perundang-undangan. Tugas khusus yang diberikan selama melaksanakan praktik kerja profesi apoteker berjudul Pemberian Informasi Obat dan Pelayanan Kefarmasian ke Rumah Pasien Hipertensi di Apotek Kimia Farma No. 389. Tugas khusus ini diberikan agar mahasiswa memahami dan menerapkan pelayanan kefarmasian di apotek.

Profesional field work of apothecary at Apotek Kimia Farma No. 389 aims to understand the roles, duties, and responsibilities of pharmacists in the pharmacies and see the process of pharmacy management, and to practice of pharmaceutical care according the provisions of the legislation. Special assignment given during the profesional field work of apothecary entitled Giving Information and Home Pharmacy Care in Patients with Hypertension at Kimia Farma No. 389. Special assignment is given to make students understand and practice pharmaceutical care of apotechary.
