"Aktifitas kerja di PT XYZ Indonesia dapat menyebabkan kecelakaan kerja dan faktor manusia yang mendominasinya. Penelitian ini bertujuan untuk menganalisis penyebab dan karakteristik dari kegagalan manusia. Metode penelitian ini adalah pengisian kuesioner dan wawancara mendalam dengan mengadopsi swiss cheese model. Karakteristik human failure yang terjadi di PT XYZ Indonesia tahun 2015 adalah knowledge based error sebesar 43,53%, rule based error sebesar 28,82%, slip sebesar 12,94%, lapse sebesar 8,82% dan violation sebesar 5,88%. Kondisi laten merupakan hal yang mempengaruhi terjadinya human failure di tempat kerja. Pengawasan yang tidak efektif dan manajemen pemeliharaan yang kurang menjadi faktor terbesar dalam terjadinya human failure. Dari hasil penelitian disimpulkan bahwa karyawan cenderung belum mendapatkan informasi yang tepat mengenai prosedur dan standar yang dipengaruhi oleh faktor lingkungan kerja dan organisasi. Pengembangan organisasi yang berfokus pada pengembangan manusia perlu dilakukan.

"ABSTRACTHydroelectric power is an important renewable resource of energy is an indispensable contributor to the countrywide power network. The hydroelectric plant is expected to supply power with the highest scale of quality and reliability. Sensitivity analysis is used to determine the impact on the outcomes due to the changes in the parameters and structural changes. This research work aims to evaluate the sensitivity of the hydroelectric system, taking the human failure into consideration using fuzzy analytic hierarchy process (AHP) approach. Six different failure rates are taken into consideration. The fuzzy approach incorporates all the vagueness and uncertainty existing in the power system due to different failure and repair rates. The main contribution of this research is to evaluate the performance of any hydroelectric power plant under the failure of its various components using fuzzy approach."
