"Resep adalah permintaan tertulis dari dokter, dokter gigi, atau dokter hewan kepada apoteker, baik dalam bentuk paper maupun elektronik untuk menyediakan dan menyerahkan obat bagi pasien sesuai peraturan yang berlaku. Pada Praktek Kerja Profesi Apoteker (PKPA) ini, calon Apoteker memperoleh kesempatan untuk melakukan analisis potensi interaksi dan penentuan waktu penggunaan obat dalam resep di apotek Kimia Farma No. 382 Kelapa Dua. Tugas khusus ini bertujuan untuk meningkatkan pemahaman calon Apoteker mengenai interaksi obat dan waktu penggunaan obat yang tepat. Pelaksanaan analisis resep dilakukan dengan cara mengambil dan memfoto resep yang telah disiapkan dan diserahkan oleh mahasiswa kepada pasien. Dari semua resep yang telah difoto kemudian dipilih resep-resep yang berisikan obat-obatan anti anti diabetes, anti hipertensi, anti kolesterol, asma, dan pengencer darah. Setelah itu dipilih 5 resep secara acak untuk kemudian dianalisis. Analisis potensi terjadinya interaksi obat dan penentuan waktu penggunaan obat yang tepat dalam resep (obat anti diabetes, anti hipertensi, anti kolesterol, asma dan pengencer darah) di Apotek Kimia Farma No. 382, telah dilakukan dengan baik. Dari kelima resep yang dianalisis terdapat tiga resep yang memiliki potensi interaksi farmakodinamik sinergis, satu resep yang memiliki interaksi farmakodinamik antagonis, dan satu resep yang memiliki interaksi farmakokinetik sinergis. Potensi interaksi tersebut dapat dicegah dengan cara meminumnya secara terpisah.

A prescription is a written request from a doctor, dentist or veterinarian to a pharmacist, either in paper or electronic form, to provide and deliver medicine to the patient in accordance with applicable regulations. In this Pharmacist Professional Work Practice (PKPA), prospective pharmacists have the opportunity to analyze potential interactions and determine the timing of use of drugs in prescriptions at Kimia Farma Pharmacy Number 382, Kelapa Dua. This special assignment aims to increase prospective pharmacists' understanding of drug interactions and the correct time to use drugs. Prescription analysis is carried out by taking and photographing prescriptions that have been prepared and handed over by students to patients. From all the recipes that had been photographed, recipes were selected which contained anti-diabetic, anti-hypertension, anti-cholesterol, asthma and blood thinner medicines. After that, 5 recipes were randomly selected and then analyzed. Analysis of the potential for drug interactions and determining the correct time to use prescription drugs (anti-diabetes, anti-hypertension, anti-cholesterol, asthma and blood thinners) at Kimia Farma Pharmacy Number 382, has been carried out well. Of the five recipes analyzed, there were three recipes that had potential synergistic pharmacodynamic interactions, one recipe that had antagonistic pharmacodynamic interactions, and one recipe that had synergistic pharmacokinetic interactions. This potential interaction can be prevented by drinking them separately."

"Apotek merupakan salah satu sarana pelayanan kefarmasian bagi apoteker dan tenaga kefarmasian lain. Pelayanan yang dimaksud berupa pelayanan langsung dan bertanggung jawab kepada pasien terkait sediaan farmasi yang bertujuan untuk mencapai hasil yang pasti dalam meningkatkan mutu kehidupan pasien (Kemenkes RI, 2017). Tujuan penulisan tugas khusus ini adalah agar penluis dapat memahami pelaksanaan pengkajian resep yang dilaksanakan di Apotek Roxy Pitara sesuai dengan standar yang telah ditetapkan oleh pemerintah yaitu Peraturan Menteri Kesehatan Republik Indonesia Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek Berdasarkan analisis di atas dapat disimpulkan bahwa resep pertama dan kedua tidak lengkap dalam aspek administratif. Ditemukan juga interaksi obat pada resep pertama yaitu peningkatan risiko hipoglikemia pada penggunaan Diamicron DR dan Trajenta dan pada resep kedua yaitu peningkatan risiko ulkus peptik dan pendarahan pada penggunaan Pirofel dan Prednisone secara bersamaan.

Pharmacy is a pharmaceutical service facility for pharmacists and other pharmaceutical personnel. The service in question is in the form of direct and responsible service to patients related to pharmaceutical preparations which aim to achieve definite results in improving the quality of life of patients (RI Ministry of Health, 2017). The purpose of writing this special assignment is so that the writer can understand the implementation of the prescription review carried out at the Roxy Pitara Pharmacy in accordance with the standards set by the government, namely the Regulation of the Minister of Health of the Republic of Indonesia Number 73 of 2016 concerning Pharmaceutical Service Standards at Pharmacies. Based on the analysis above, it can be concluded that the first and second prescriptions were incomplete in the administrative aspect. Drug interactions were also found in the first prescription, namely an increased risk of hypoglycemia when using Diamicron DR and Trajenta and in the second prescription, an increased risk of peptic ulcer and bleeding when using Pirofel and Prednisone together."

People with mental disorders have the right to obtain mental health services in accordance with mental health service standards. One way to deal with these psychiatric problems is to use pharmacological therapy using antipsychotics to treat the symptoms experienced. Antipsychotics are a class of drugs to control and reduce the symptoms of psychosis that can be experienced by people with mental disorders. One of the important roles of pharmacists is to provide comprehensive pharmaceutical care by supporting the correct and rational use of drugs, identifying, preventing, and overcoming drug related problems, as well as monitoring and evaluating treatment in accordance with applicable service standards. One form of clinical pharmacy services in pharmacies is to conduct prescription assessments from the administrative, pharmaceutical, and clinical aspects. During the treatment period with antipsychotic and antidepressant drugs, monitoring is required regarding medication adherence, therapeutic benefits, and side effects."

"Hipertensi merupakan salah satu penyakit dengan prevalensi yang terus menerus mengalami peningkatan. Hal ini merupakan masalah kesehatan dunia yang perlu ditangani dan apabila terdapat penyakit penyerta dapat menimbulkan risiko terjadinya penyakit ginjal dan kardiovaskuler. Apotek adalah sarana pelayanan kefarmasian tempat dilakukan praktik kefarmasian oleh Apoteker. Apotek memiliki tujuan untuk meningkatkan kualitas pelayanan kefarmasian di Apotek, Sebagai penanggung jawab apotek, apoteker bertanggung jawab dalam melakukan kajian resep yang diterima di Apotek, tujuan pengkajian resep adalah untuk menjamin keamanan (safety) dan kemanjuran (efficacy) dari obat dalam resep tersebut ketika digunakan oleh pasien serta memaksimalkan tujuan terapi. Tujuan dari penulisan ini adalah untuk mengkaji dan mengetahui pola peresepan yang dilakukan selama Praktik Kerja Profesi Apoteker periode November 2020 di Apotek Roxy Pondok Labu. Metode pelaksanaan dilakukan dengan mengumpulkan resep yang diterima selama bulan November, kemudian memisahkan resep yang mengandung obat antihipertensi dan dipilih 2 resep secara acak pada resep tersebut. Kemudian dilakukan pengkajian dan analisis pada 2 resep terpilih. Dari total semua resep pada periode November 2020 yang diresepkan oleh dokter terdapat 10 resep yang mengandung obat antihipertensi. Obat yang sering diresepkan oleh dokter, antara lain golongan obat penghambat enzim angiotensin yakni Ramipril, golongan obat penghambat reseptor B yakni Bisoprolol, golongan obat penghambat kanal kalsium yakni Amlodipine. Pengkajian resep yang dilakukan terhadap resep periode Oktober 2020 di Apotek Roxy Pondok Labu menyatakan bahwa resep sudah rasional namun kelengkapan administratif belum lengkap.

Hypertension is a disease with a high prevalence and always increase rapidly. This is a world health problem that needs to be addressed and if there are comorbidities it can pose a risk of developing kidney and cardiovascular disease. Pharmacy is a pharmaceutical service facility where pharmacists practice pharmacy. The pharmacy has the aim to improve the quality of pharmaceutical services at the pharmacy. As the person in charge of the pharmacy, the pharmacist is responsible for conducting a review of prescriptions received at the pharmacy, the purpose of reviewing prescriptions is to ensure the safety and efficacy of the drugs in the prescription when used, as well as maximizing therapeutic goals. The purpose of this paper is to study and find out the prescribing patterns carried out during the Professional Work Practice of Pharmacists for the period November 2020 at the Roxy Pondok Labu Pharmacy. The method of implementation is by collecting prescriptions received during November, then separating prescriptions containing antihypertensive drugs and randomly selecting 2 prescriptions for those prescriptions. Then an assessment and analysis was carried out on the 2 selected recipes. Of the total of all prescriptions in the November 2020 period prescribed by doctors, there were 10 prescriptions containing antihypertensive drugs. Drugs that are often prescribed by doctors include the class of angiotensin enzyme inhibitors namely Ramipril, and the B receptor blocker namely Bisoprolol, a class of calcium channel blockers namely Amlodipine. The prescription review carried out on the recipe for the October 2020 period at the Roxy Pondok Labu Pharmacy stated that the recipe was rational but the administrative completeness was incomplete."

"Pengkajian dan pelayanan resep merupakan salah satu kegiatan yang termasuk dalam pelayanan kefarmasian secara klinis. Pada pengkajian resep apoteker diharapkan dapat memastikan keabsahan resep dan mencegah terjadinya kesalahan pengobatan. Salah satu penyakit yang memiliki prevalensi tinggi di Indonesia dan sering diresepkan adalah resep berisi obat antihipertensi. Tujuan dari pengkajian dan analisis resep antihipertensi ini yaitu mengetahui jumlah resep antihipertensi yang diterima, mengetahui golongan obat antihipertensi yang paling banyak diresepkan, mengetahui kelengkapan penulisan resep dari segi administratif, farmasetik, dan klinis. Pengkajian dan analisa dilakukan selama penulis melakukan Praktek Kerja Profesi Apoteker (PKPA) yang bertempat di Apotek Roxy Jagakarsa, Jalan Jagakarsa Raya No. 54, Jakarta Selatan. Data diperoleh dengan mengumpulkan resep bulan Juni hingga Agustus 2020, kemudian dilakukan pencatatan terhadap resep yang mengandung obat untuk penyakit hipertensi selama periode tersebut dan dipilih 2 resep yang akan dikaji dan dianalisa kesesuaiannya dengan pedoman pengkajian resep pada Peraturan Menteri Kesehatan nomor 73 tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Dari analisa yang dilakukan ditemukan jumlah resep resep yang mengandung obat anthipertensi yang diterima oleh Apotek Roxy Jagakarsa selama bulan Juni-Agustus 2020 yaitu sebanyak 265 resep berisi obat antihipertensi dari total resep 3.571 resep atau sebanyak 7,42%, dengan obat antihipertensi yang paling banyak diresepkan yaitu Amlodipin yang merupakan golongan penghambat kanal kalsium sebanyak 42 resep, Ramipril golongan obat penghambat enzim angiotensin sebanyak 38 resep, dan Bisoprolol golongan obat penghambat reseptor B sebanyak 33 resep, yang diresepkan baik dalam terapi kombinasi maupun terapi tunggal. Dari contoh resep yang dianalisa, ditemukan beberapa kekurangan pada kelengkapan dalam aspek administratif dan interaksi obat dalam resep.

Assessment and prescription analytics is one of the activities included in clinical pharmacy services. In the pharmacist prescription assessment, it is hoped that it can ensure the validity of the prescription and prevent medication errors. One disease that has a high prevalence in Indonesia and is often prescribed is a prescription containing antihypertensive drugs. The objectives of the study and analysis of antihypertensive prescriptions were to determine the number of antihypertensive prescriptions received, to know which antihypertensive drug classes were most commonly prescribed, to determine the completeness of prescribing from an administrative, pharmaceutical, and clinical perspective. The assessment and analysis were carried out when the author carried out the Pharmacist Professional Work Practice (PKPA) which was located at the Roxy Jagakarsa Pharmacy, Jagakarsa Raya Street No. 54, South Jakarta. Data was obtained by collecting prescriptions from June to August 2020, then recording prescriptions containing drugs for hypertension during that period and selecting 2 prescriptions to be reviewed and analyzed for their conformity with the prescription review guidelines in Minister of Health Regulation number 73 years 2016 about Pharmacy Service Standards at the Pharmacy. From the analysis conducted, it was found that the number of prescription containing anthypertensive drugs received by the Roxy Jagakarsa Pharmacy during June-August 2020 was 265 from a total of 3,571 prescriptions or as much as 7.42%, with the most widely prescribed antihypertensive drugs namely Amlodipine which is a calcium channel blocker class for 42 prescriptions, Ramipril for an angiotensin enzyme inhibitor group for 38 prescriptions, and Bisoprolol for a B receptor blocker class for 33 prescriptions, these drugs are prescribed both in combination therapy and single therapy. From the sample prescriptions analyzed, there were some flaws of the administrative and drug interactions aspects of the prescription.

"Apotek Kimia Farma Unit 389 merupakan salah satu apotek yang melayani resep pasien BPJS di Depok. Laporan ini disusun untuk mengkaji dan menganalisis resep di Apotek Kimia Farma Unit 389 Depok sehingga dapat memberikan informasi mengenai kelengkapan resep dan kejadian 'drug related problems' (DRPs) pada resep. Sampel yang digunakan berjumlah lima resep pasien BPJS kronis poli penyakit dalam di Apotek Kimia Farma Unit 389 periode April 2023. Hasil pengkajian dan analisis resep menunjukkan bahwa semua resep tidak lengkap ditinjau dari aspek administratif, aspek farmasetik, dan aspek klinis berdasarkan Peraturan Menteri Kesehatan Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Kejadian DRPs terjadi pada semua resep. DRPs yang berkaitan dengan masalah efektivitas terapi terjadi pada resep 1 dan 3, sedangkan yang berkaitan dengan masalah keamanan terapi terjadi pada resep 1, 2, 4, dan 5.

Kimia Farma Pharmacy Unit 389 is one of the pharmacies that serve prescriptions for BPJS patients in Depok. This report was prepared to assess and analyze prescriptions at the Kimia Farma Pharmacy Unit 389 Depok so that it can provide information about the completeness of prescriptions and the incidence of drug related problems (DRPs) in prescriptions. The sample used consisted of five prescriptions for chronic BPJS patients of internal medicine poly at Kimia Farma Pharmacy Unit 389 for the period of April 2023. The results of the assessment and analysis of prescriptions showed that all prescriptions were incomplete in terms of administrative aspects, pharmaceutical aspects, and clinical aspects based on the Regulation of the Minister of Health Number 73 of 2016 concerning Pharmaceutical Service Standards in Pharmacies. DRPs occurred in all prescriptions. DRPs related to therapeutic effectiveness issues occurred in prescriptions 1 and 3, while those related to therapeutic safety issues occurred in prescriptions 1, 2, 4, and 5."

"Batuk pilek atau common cold merupakan penyakit yang sering terjadi dan menginfeksi saluran pernapasan atas (ISPA). Batuk pilek dapat diatasi dengan memberikan terapi farmakologi dan non famakologi. Terapi farmakologi batuk pilek dapat menggunakan kombinasi obat-obatan over the counter (OTC) dengan obat yang mengandung prekursor. Penggunaan obat yang mengandung prekusor harus disertai dengan resep dokter karena obat ini memiliki efek sedatif yang mirip dengan senyawa metafetamin. Tujuan dalam melakukan penelitian ini adalah mengkaji resep obat batuk dan pilek yang diterima di Apotek Roxy Galaxy periode Januari tahun 2023 dan menganalisis potensi interaksi obat batuk dan pilek pada resep tersebut. Metode yang digunakan dalam laporan ini adalah studi literature dan pengambilan data sekunder. Kedua resep obat batuk dan pilek yang diterima di Apotek Roxy Cabang Galaxy menggunakan obat golongan bronkodilator, antihitamin, kortikosteroid, mukolitik, dan prekursor farmasi. Kesimpulan yang dapat diambil berdasarkan hasil telaah pada kedua resep tersebut adalah kedua resep tidak memenuhi syarat pengkajian administratif, farmasetik, dan klinis serta resep pertama tidak berpotensi terjadi interaksi obat sedangkan resep kedua berpotensi terjadi interaksi obat.

Common cold is a disease that ofter occurs and infects the upper respiratory tract. Cough and cold can by treated by pharmalogical and non-pharmacological therapy. Pharmacological therapy for cough and cold can be use a combination of over the counter (OTC) drug and drug containing precursors. The use of drugs containing precursors must be accompanied by a doctor’s prescription because this drug has sedative effect similar to metaphetamine compounds. The aim of this research is to analyse prescription for cough and cold drugs received at the Apotek Roxy Galaxt for the period Januari 2023 and analyze potential interaction for cough and cold medicines in this prescriptions. The menthos used in this reports area literature study and secondary data collection. The two prescription for cough and cold medicine received at Apotek Roxy Galaxy used bronchodilator, antihistamine, corticosteroid, mucolytic, and pharmaceutical precursor drugs. The conclusion in this research are both two prescriptions do not meet administrative, pharmaceutical, and clinical assessment requirments and the firs prescription does not have a potential drug interaction while the second prescription has potential drug interactions"

"Analisis resep pada pasien program rujuk balik memainkan peran krusial dalam memastikan keselamatan dan keberhasilan perawatan pasien. Analisis resep dilakukan guna memverifikasi kesesuaian obat dengan diagnosis pasien, mencegah kesalahan atau kontraindikasi dalam pengobatan, serta memantau interaksi obat. Penelitian ini bertujuan untuk menelaah jumlah pemenuhan obat antihipertensi PRB di Apotek Kimia Farma Pos Pengumben pada Bulan Februari 2023 serta mengkaji resep obat antihipertensi pasien PRB. Penelitian ini dilakukan dengan mengambil data resep pasien PRB pada bulan Februari 2023, lalu diolah untuk mengetahui pola peresepan obat. Selanjutnya, dilakukan pemilihan 3 resep untuk dikaji menurut aspek administrasi, farmasetis, dan klinis. Jumlah obat antihipertensi pasien PRB untuk kebutuhan 30 hari sudah dapat dipenuhi oleh Apotek Kimia Farma Kemayoran dengan total 30486 obat antihipertensi dari 456 resep pasien PRB. Pada resep pasien PRB yang mengandung obat antihipertensi ditemukan beberapa masalah terkait obat seperti efek samping dan interaksi antara obat yang perlu diperhatikan. Masalah tersebut dapat diatasi dengan mengubah jadwal konsumsi obat pasien atau dengan memberikan obat tambahan untuk mengatasi efek samping yang muncul pada pasien...... The analysis of prescriptions in Back-Referral Program (BRP) plays a crucial role in ensuring the safety and success of patient treatment. Prescription analysis aims to verify the suitability of prescribed medications with the patient's diagnosis, prevent errors or contraindications in treatment, and monitor drug interactions. This study aims to examine the fulfillment of antihypertensive medication prescriptions for PRB patients at Apotek Kimia Farma Pos Pengumben in February 2023 and assess the prescriptions for antihypertensive drugs. The research involved collecting prescription data from PRB patients in February 2023 and analyzing the prescribing patterns. Subsequently, three prescriptions were selected for evaluation based on administrative, pharmaceutical, and clinical aspects. The quantity of antihypertensive drugs for PRB for 30 days use was met by Apotek Kimia Farma Kemayoran, totaling 30,486 antihypertensive drugs from 456 patient prescriptions. Several drug-related issues were identified in the prescriptions containing antihypertensive drugs, such as side effects and drug interactions that need attention. These issues can be addressed by adjusting the patient's medication schedule or providing additional medication to manage the observed side effects."

"Apotek adalah saranan praktek pelayanan kefarmasian yang memfasilitasi pasien untuk mendapatkan obat baik dengan resep ataupun tidak. Obat yang termasuk dalam golongan obat keras seperti obat antihipertensi tentunya tidak dapat dibeli tanpa resep dokter. Pengkajian atau skrining resep dilakukan untuk menjamin legalitas suatu resep, meminimalkan kesalahan pengobatan, dan untuk mencegah ketidakrasionalan dalam pemberian obat sesuai dengan peraturan yang berlaku. Skrining resep dilakukan dengan mengkaji kesesuaian aspek administrasi, farmasetika, dan klinis dari daftar tilik pengkajian resep. Dari beberapa resep yang dikaji, didapatkan hasil bahwa daftar tilik dari aspek administrasi tidak terpenuhi karena tidak mencantumkan umur, berat badan pasien, ruang/unit asal resep, dan tanda tangan dokter sementara aspek farmasetik dan klinis terpenuhi dan tidak ada obat yang saling berinteraksi dalam satu resep. Selain pengkajian resep, apotek juga berperan proaktif dalam menyampaikan informasi untuk mengedukasi para pasien. Hal tersebut dilakukan dengan membuat poster edukasi terkait hipertensi yang kemudian disampaikan kepada beberapa pasien.

A pharmacy is a pharmaceutical service practice that facilitates patients in obtaining medication, both with or without a prescription. Medications classified as 'obat keras' (hard drugs), such as antihypertensive drugs, cannot be purchased without a doctor's prescription. Prescription assessment or screening is conducted to ensure the legality of a prescription, minimize medication errors, and prevent irrational drug use in accordance with applicable regulations. The prescription screening involves evaluating the administrative, pharmaceutical, and clinical aspects based on a checklist. Among the prescriptions evaluated, it was found that the administrative aspect checklist was not fulfilled due to the lack of patient age, weight, prescription origin location/unit, and doctor's signature, while the pharmaceutical and clinical aspects were met, and there were no drug interactions within a single prescription. Besides prescription assessment, the pharmacy also plays a proactive role in providing information to educate patients.
