Abstrak :
Sistem Pencatatan dan Pelaporan Terpadu Puskesmas (SP2TP) telah diberlakukan sejak tahun 1981. SP2TP secara potensial dapat berperan banyak dalam menunjang proses manajemen Puskesmas, baik untuk memenuhi aspek Perencanaan, Penggerakan, Pelaksanaan maupun Pengawasan, Pengendalian dan Penilaian. Tujuan pelaksanaan SP2TP adalah didapatnya semua data hasil kegiatan Puskesmas dan data yang berkaitan serta dilaporkannya data tersebut kepada jenjang administrasi di atasnya sesuai kebutuhan secara benar, berkala, teratur guna menunjang pengelolaan upaya kesehatan masyarakat. Penelitian ini bertujuan untuk mengetahui pelaksanaan SP2TP di Puskesmas Pembina DKI Jakarta tahun 1997 dengan pendekatan sistem dan dilakukan dengan studi kualitatif yang didukung oleh data primer dan data sekunder. Hasil penelitian ini menunjukkan bahwa Pelaksanaan SP2TP di Puskesmas Pembina DKI Jakarta tahun 1997 belum berjalan secara optimal. Berdasarkan hasil penelitian, penulis menyarankan agar dilakukan perbaikan dari segi kualilas maupun kuantitas pada input , proses , output . Perbaikan ini harus dilakukan pada semua tingkatan mulai dari Departemen Kesehatan sampai dengan Puskesmas Kelurahan, agar pelaksanaan SP2TP disetiap jenjang administrasi kesehatan dapat berjalan dengan optimal.

The Implementation of SP2TP at Puskesmas Pembina DKI Jakarta in the Year of 1997The registration system and Integrated Report of Puskesmas ( SP2TP) have been put into effect since 1981. Potentially SP2TP has many roles in supporting the process of Puskesmas's management, both to fulfill the planning aspect, movement execution or supervision, control and judgment. The objective of SP2TP is to obtain data of all Puskesmas activities and data in relation there to also to report them to the superior administrative level suitable with the needs, in proper manner periodically and regularly order to support the public health management. The objective of research is to establish the SP2TP implementation at Puskesmas Pembina DKI Jakarta in 1997 by System-approach and qualitative study supported by primary and secodary data. This research shows that the implementation of SP2TP in Puskesmas Pembina DKI Jakarta in 1997 has not been optimized. By virtue of this research the writer suggests improvement on quality and quantity of input, process, output. The improvements should be implementation in every level starting from the Ministry of Health to Puskesmas Kelurahan, so that the implementation of SP2TP in every health administration level can be optimized.

Organisasi Dinas Kesehatan Kota Pontianak dimasa depan dituntut untuk lebih profesional bukan saja karena desakan dan tuntutan dari masyarakat pengguna, akan tetapi juga karena adanya perubahan -kewenangan pemerintahan daerah. Untuk menghadapi tuntutan tersebut perlu diantisipasi dengan baik agar Dinas Kesehatan Kota Pontianak mampu secara cepat dan tepat mengakomodir tuntutan tersebut. Keadaan yang dihadapi saat ini, dalam rangka mengantisipasi otonomi daerah tersebut adalah rendahnya kinerja organisasi. Untuk memperbaiki kinerja tersebut maka dilakukan suatu kegiatan intervensi budaya mutu dengan model kalakarya, berupa pembinaan dan pembimbingan Total Quality Management terhadap kinerja organisasi Dinas Kesehatan Kota Pontianak. Masalah kinerja yang diteliti disini, dibatasi dalam lingkup Kinerja Sistem Pencatatan dan Pelaporan Terpadu Puskesmas (SP2TP) yang dalam pelaksanaannya dikoordinasikan oleh Kepala Seksi Pemulihan Kesehatan selaku Koordinator Organisasi Tim SP2TP Dinas Kesehatan Kota Pontianak. Penelitian ini bertujuan untuk mendapatkan informasi yang mendalam tentang pengaruh intervensi budaya mutu terhadap kinerja SP2TP serta diketahuinya komponen-komponen yang penting sebagai karakteristik budaya organisasi yang berorientasi Total Quality Management untuk peningkatan kinerja SP2TP. Kegiatan intervensi ini merupakan action reseach, dengan jenis penelitian Quasi Experiment Design dan bentuk desain penelitiannya Non Randomized Pretest Posttest (Self} Control Group Design, yang dilakukan oleh Tim dari FKM-UI bekerja sama dengan Kanwil Depkes Propinsi Kalimantan Barat. Peneliti membatasi diri pada penelitian kualitatif untuk menganalisa pengaruh intervensi tersebut terhadap Kinerja SP2TP Dinas Kesehatan Kota Pontianak, dengan analisis thematic approach. Pengumpulan data dilakukan dengan metode pengamatan dan wawancara mendalam. Jumlah sampel untuk wawancara mendalam sebanyak 10 informan. Hasil penelitian memperlihatkan bahwa adanya perubahan peningkatan kinerja SP2TP secara bertahap, namun hasilnya belum sesuai dengan yang diharapkan. Hal ini disebabkan karena: Pertama; penghayatan visi dan misi serta tugas pokok dan fungsi, baru sampai pada tahap "awareness" untuk revitalisasi visi dan misi organisasi Dinas Kesehatan Kota Pontianak. Kedua; koreksi hasil entri data laporan SP2TP serta pengolahan dan analisanya tidak dilakukan oleh para pengelola program selaku anggota tim SP2TP karena koordinator SP2TP tidak melaksanakan koordinasi dalam proses manajerial SP2TP Dinas Kesehatan Kota Pontianak. Kesimpulan dari hasil penelitian ini adalah Kinerja SP2TP Dinas Kesehatan Kota Pontianak masih rendah. Rendahnya Kinerja SP2TP ini mengakibatkan keputusan yang diambil oleh jenjang administrasi kesehatan yang lebih tinggi berdasarkan informasi yang bersumber dari data Laporan Triwulanan SP2TP yang kualitas datanya kurang terjamin/datanya tidak valid. Hal ini disebabkan karena tidak diberdayakannya para pengelola program selaku anggota tim SP2TP dalam proses manajerial SP2TP melalui koordinasi lintas program serta kurangnya penghayatan visi dan misi serta tugas pokok dan fungsi para pelaksana dan pengelola program yang merupakan komponen penting dalam peningkatan kinerja disamping komponen kepemimpinan dan ketrampilan manajerial dari Koordinator Tim SP2TP Dinas Kesehatan Kota Pontianak yang kurang mendukung. Untuk meningkatkan kinerja SP2TP Dinas Kesehatan, disarankan untuk menghayati tugas pokok dan fungsi Organisasi Tim SP2TP dan menggerakan pelaksanaan koordinasi lintas program untuk memberdayakan Anggota Tim SP2TP yang dipimpin oleh Koordinator SP2TP, sehingga adanya keterpaduan pencatatan dan pelaporan antar program, yang pada akhirnya akan meningkatkan kinerja SP2TP khususnya dan kinerja Dinas Kesehatan Kota Pontianak pada umumnya. The Health Department of Pontianak City is expected to become more professional in the near future. Such expectation arises from not only the people but the change of local government administration. To be able to meet the expectation immediately and appropriately, the Health Department of Pontianak City should have a sound anticipatory measure. The existing problem in anticipating local autonomy is inadequacy in the performance of the Health Department. To improve the performance, therefore, a quality culture intervention is conducted. The intervention takes a periodical workshop model in which training and coaching concerning Total Quality Management on the performance of the Health Department are provided. The problem of performance under this study was focused on Kinerja Sistem Pencatatan dan Pelaporan Terpadu Puskesmas (SP2TP) or Performance of Puskesmas Integrated Recording and Reporting System. The implementation of SP2TP was under the coordination of Chief of Health Recovery Section who acted as the coordinator of SP2TP team of the Health Department. This study was aimed at collecting in-depth information on the effect of the quality culture intervention on the performance of SP2TP as well as examining significant characteristics of the organization culture that are Total Quality Management oriented in the improvement of SP2TP performance. The intervention was conducted by means of action research. The research employed a Quasi Experiment Design and the research design was Non Randomized Pretest and Posttest (Self) Control Group Design conducted by a team of FKM-UI in cooperation with the Provincial Office of the Health Department of West Kalimantan. The researcher limited the study to a qualitative one attempting to analyze the effect of such intervention on the performance of SP2TP of the Health Department of Pontianak City. The analysis used a thematic approach. Data collection was conducted by means of observation and in-depth interview. The number of respondents involved in the interview was 10. The study result shows that there is a gradual improvement in the performance of SP2TP although the output has not met the set goal, yet. There are some reasons underlying such output. First, in terms of vision, mission, main tasks and functions, the organization has reached an "awareness" phase to revitalize its vision and mission. Second, correction, analysis and use of data entry results of SP2TP reports have not been carried out by the program operatives as members of SP2TP team. Such members' performance is caused by lack of coordination by the SP2TP coordinator in its managerial process. The study concludes that the performance of SP2TP of the Health Department of Pontianak City is still low. The low performance affects the quality of decisions made by higher health administration because the SP2TP three-monthly reports by which the decisions are made may be less reliable and valid. Such low performance is caused by less empowerment of the program operatives as members of SP2TP team in its managerial process through cross program coordination. Another reason is that the vision, mission, main tasks and functions of the program operatives. which are significant components in the performance improvement, are not fully comprehended. In addition, the low performance is due to inadequacy of leadership and managerial skills of the team coordinator. To improve the performance of SP2TP team, main tasks and functions of the SP2TP team have to be fully understood, the coordinator of SP2TP team should conduct cross program coordination and team member empowerment so that the recording and reporting among programs can be integrated and the performance of SP2TP in particular or the Health Department of Pontianak City in general is eventually improved. Abstrak :
Based on letter of the director general of community health No. 590/BM/DJ/Info/V/96 intend the utilization the SP2TP for strengthening the quality of management effectively and efficiency at the health center level. The purpose of this research is to obtain information regarding the performance utility of SP2TP to provide support for the management process at the health center level in municipality in the year 2000, which be focused of user side and availability of data to provide support for strengthening the management process at the health center level, and factors related with the utilization of SP2TP. The factors are Knowledge, Perception, Habitual to use data in working, Needs of information, Availability of general information, Availability in program information, and the Processing of data. This research used the cross sectional design, to identify the relationship between variables with transitory measurement, which quantitative and qualitative approach. Sample population and informant of this research are staff which responsibility of program, the chief of the health center, namely 114 workers. Test of statistic is chi square and logistic regression of multivariate, meanwhile qualitative data is analyzed with content analysis. The result of this research shows that the utilization of SP2TP to provide support the management process at the health center level is 2, 6%. There are 7 variables studied, three variables have related significantly with the utilization of SP2TP, they are Knowledge about SP2TP, Need of information on SP2TP and availability of program information in the SP2TP. It is concluded that the utilization of SP2TP to provide support the management process at health center level have relationship with knowledge, need information and availability of program information in the SP2TP. The effort of the utilization of SP2TP for strengthening the management process at the health center level, we should have to consider three factors which have been mentioned above. To overcome these problems, makes recommended 1) Training of the health center worker about the management process, 2) Strengthening the quality of supervision, 3). Coordination meetings are held between those responsible for SP2TP, programmer and other health decision maker for improvement of SP2TP. 4) Dissemination of monitoring and evaluation involved unit be held in periodic meeting.

Berdasarkan Keputusan Dirjen Binkesmas No. 590/BM/DJ/Info/V/96 diharapkan Puskesmas dapat meningkatkan kualitas manajemen Puskesmas secara lebih berhasil guna dan berdaya guna melalui pemanfaatan secara optimal informasi SP2TP. Penelitian ini bertujuan untuk melihat gambaran tingkat pemanfaatan SP2TP dalam menunjang manajemen Puskesmas di Kota Padang Tahun 2000. yang dititik beratkan dari sisi pengguna dan ketersediaan data dalam menunjang manajemen Puskesmas serta faktor-faktor yang berhubungan dengan pemanfaatan SP2TP, yaitu faktor pengetahuan, persepsi, kebiasaan menggunakan data dalam bekerja, kebutuhan informasi, ketersediaan informasi umum, ketersediaan informasi program, proses pengolahan data SP2TP. Penelitian ini merupakan penelitian potong lintang (cross sectional) dengan pendekatan kuantitatif dan kualitatif. Responden dan informan penelitian ini adalah para penanggung jawab program, dan kepala Puskesmas terpilih. Analisis data dilakukan dengan uji chi square dan analisis multivanat regresi logistik, sedangkan analisis secara kualitatif dilakukan dengan analisis isi. Dari hasil pengolahan data diperoleh informasi bahwa pemanfaatan SP2TP untuk menunjang manajemen Puskesmas adalah sebesar 2,6 %. Dari 7 (tujuh) varibel yang dipelajari terdapat hanya 3 (tiga) variabel yang mempunyai hubungan secara bermakna terhadap pemanfaatan SP2TP. Ketiga variabel tersebut adalah pengetahuan petugas tentang data yang terkandung dalam SP2TP, kebutuhan infonnasi SP2TP dan ketersediaan informasi program di dalam SP2TP. Disimpulkan bahwa pemanfaatan SP2TP dalam menunjang manajemen Puskesmasberhubungan erat dengan faktor pengetahuan petugas, faktor kebutuhan informasi dan faktor ketersediaan informasi program di dalam SP2TP itu sendiri. Dalam upaya meningkatkan pemanfaatan informasi SP2TP oleh Puskesmas, perlu kiranya memperhatikan ketiga faktor tersebut. Upaya yang dilakukan dalam mengatasi kendala yang timbul di dalam pemanfaatan SP2TP adalah mengadakan 1). Pelatihan petugas Puskesmas, 2). Meningkatkan kualitas pembinaan (supervisi), 3). Melakukan kordinasi untuk mengadakan perbaikan terhadap item SP2TP agar datanya sesuai dengan kebutuhan program dan 4) Menyebarluaskan hasil pemantauan dan evaluasi kepada lintas program maupun lintas sektoral pada pertemuan-pertemuan secara berkala.
