"Residensi Keperawatan Medikal Bedah merupakan aplikasi praktek pada program spesialis yang menggali ilmu sesuai dengan peminatan, dalam hal ini adalah sistem pernapasan. Kegiatan yang dilakukan selama residensi adalah melaksanakan asuhan keperawatan dengan pendekatan model teori self care deficit Dorothea Orem. Masalah keperawatan yang sering muncul pada pasien dengan gangguan sistem respirasi adalah batuk, sesak, nyeri, demam, gangguan nutrisi dan kelelahan. Kegiatan berikutnya adalah pemberian intervensi berbasis bukti (evidence based nursing) dengan memberikan latihan napas pursed lip breathing pada pasien Penyakit Paru Obstruksi Kronik (PPOK).Hasilnya adalah pursed lip breathing dapat menurunkan frekwensi pernapasan, meningkatkan arus puncak respirasi dan saturasi oksigen. Kegiatan berikutnya adalah dan melaksanakan program inovasi tentang water seal drainage 1 botol. Hasilnya adalah sangat direkomendasikan untuk digunakan pada pasien yang memerlukan pemasangan water seal drainage dilihat dari keamanan dan estetika penglihatan.Disimpulkan bahwa model Teori self care deficit Dorothea Orem dapat diterapkan dalam pelaksanaan asuhan keperawatan pada pasien gangguan sistem respirasi, pursed lip breathing pada pasien penyakit paru obstruksi kronik, dan penggunaan water seal drainage 1 botol inovasi sangat direkomendasikan dapat memberi keamanan bagi pasien dan baik secara estetika penglihatan.

The nursing problem that always appears to patient with disorder of respiratory system are coughing, tightness, pain, fever, nutrisional disorders and fatique. The next step is giving evidence based nursing by giving breathing excercise is pursed lip breathing to Chronic Obstruction Pulmonary Disease (COPD).

The result is pursed lip breathing that make be able to lower frequency breathing , increasing the current peak of respiration and oxygen saturation. The next activity is creating a new innovation program about water seal drainage one bottle. The result is highly recommended for use by patients who need the installation of a water seal drainage in the context of safety and visual aesthetics.

Concluded that the theory of model self care deficit dorothea orem can be applied in the implementation of nursing care for with disorders of respiration system, pursed lip breathing for patients chronic obstruction pulmonary disease and the user of water seal drainage one bottle of innovation is very recommended and it can give for patients and it?s better also from visual aesthetics."

"Proses praktik residensi keperawatan kekhususan respirasi yang dilakukan di RSUP Persahabatan Jakarta diharapkan dapat menerapkan proses asuhan keperawatan profesional dalam mengatasi masalah pernapasan yang dialami pasien. Sewaktu proses pelaksanaan praktik seorang residensi keperawatan dituntut memiliki kemampuan berpikir kritis dalam menerapkan intervensi keperawatan berdasarkan Evidence Base Nursing, mampu menjadi seorang Clinical Care Manajer (CCM) yang bertugas sebagai konsultan keperawatan bagi staf keperawatan maupun berkolaborasi dengan sejawat profesi lainnya dalam memberikan terapi keperawatan. Dalam memberikan asuhan keperawatan profesional pada pasien dengan Timoma Tipe A T1a N0 M0 dan 30 kasus resume keperawatan dengan kasus penyakit paru menggunakan pendekatan teori Self Care Dorothea Orem dapat dilihat bagaimana peran seorang perawat sebagai pemberi asuhan keperawatan membantu individu baik dalam keadaan sehat maupun sakit, melalui usaha individu tersebut melakukan berbagai aktivitas guna mendukung kesehatan dan proses penyembuhan penyakit. Dari hasil analisis praktik yang dilakukan, masalah keperawatan yang sering muncul berkaitan dengan pemenuhan kebutuhan pasien untuk dapat bernapas normal. Pada penerapan EBN berupa pelaksanaan Lotus Potitioning dan Orthopneic Potitioning pada pasien penyakit paru yang mengalami dyspnea diberikan untuk mengatasi masalah keperawatan gangguan pola napas. Pelaksanaan proyek inovasi berupa Bundle Trakeostomi untuk meningkatkan manajemen perawatan pada pasien yang terpasang trakeostomi akan dapat menunjang tatalaksana pencegahan risiko infeksi sehingga dapat mencegah efek samping akibat trakeostomi yang terjadi pada pasien. Hasil analisis praktik residensi keperawatan didapatkan bahwa asuhan keperawatan menggunakan pendekatan teori keperawatan Self Care Dorothea Orem bertujuan untuk membantu dalam proses kemandirian pasien. Penerapan teknik Lotus potitioning dan Orthopneic potitioning dapat membantu menurunkan derajat dyspnea pada pasien dengan penyakit paru. Proyek inovasi dalam meningkatkan kemampuan perawat dalam melakukan perawatan trakeostomi melalui penerapan bundle trakeostomi secara sistematis serta perawat dapat memberikan edukasi terhadap pasien dalam melakukan perawatan trakeostomi secara mandiri.

The nursing residency practice process specialising in respiration carried out at the Friendship Hospital Jakarta is expected to apply the professional nursing care process in overcoming respiratory problems experienced by patients. During the practical implementation process, a nursing residency is required to have the ability to think critically in applying nursing interventions based on Evidence Base Nursing, be able to become a Clinical Care Manager (CCM) who serves as a nursing consultant for nursing staff and collaborates with other professional colleagues in providing nursing therapy. In providing professional nursing care to patients with Thymoma Type A T1a N0 M0 and 30 cases of nursing resumes with lung disease cases using the Dorothea Orem Self Care theory approach, it can be seen how the role of a nurse as a nursing care provider helps individuals both in health and illness, through the efforts of these individuals to carry out various activities to support health and the healing process of the disease. From the results of the practice analysis carried out, nursing problems that often arise are related to fulfilling the patient's need to be able to breathe normally. In the application of EBN in the form of implementing Lotus Potitioning and Orthopneic Potitioning in lung disease patients who experience dyspnea is given to overcome the nursing problem of breathing pattern disorders. Peran pertama yaitu memberikan asuhan keperawatanprofesional pada pasien utama kanker paru dan 30 kasus resume menggunakan pendekatan teori Self Care Dorothea Orem. bertujuan untuk membantu individu kegiatan memenuhi kebutuhan dalam mempertahankan kehidupan, kesehatan dan kesejahteraan individu baik dalam keadaan sehat maupun sakit yang dilakukan oleh individu itu sendiri. Pasien dengan diagnosa kanker paru, untuk mengetahui stadium nodul kanker parunya. Maka dokter akan melakukan prosedur bronkoskopi. Pasien yang akan menjalani prosedur bronkoskopi, 57.2% pasien merasa takut/cemas. Pada penerapan Evidence Base Nursing (EBN), residen melakukan penerapan pemberian terapi musik binaural beat untuk mengurangi kecemasan pada pasien yang menjalani prosedur bronkoskopi. Hasil penerapan EBN menunjukan bahwa penerapan pemberian terapi musik binaural beat signifikan dalam menurunkan skor kecemasan pasien yang menjalani prosedur bronkoskopi. Implementasi proyek inovasi booklet bundle tracheostomy. Tujuan dari inovasi ini untuk meningkatkan kemampuan dan pengetahuan perawat dalam melakukan perawatan trakeostomi. serta perawat dapat memberikan edukasi terhadap pasien dalam melakukan perawatan trakeostomi secara mandiri.

Competencies that are required with specialized nurse is to be able do professional nursing care plan, to advocate, to act as nursing consultant in Clinical Care Manager (CCM) for other nursing staff, to give therapy for patients, to be a a researcher and implement evidence-based nursing (EBN) practice, and to be an innovator. These competencies is in line with the competencies that was achieved during this residency practical. The first role is to give professional care plan for patients with primarily lung cancer and 30 case-resumes with the Dorothea Orem Self Care Theory approach, aiming to help individuals with life-sustaining needs, health care needs, and welfare needs during health condition or during illness of the individual. To know the stage of the lung cancer nodules for lung cancer diagnostics, the doctor is required to do bronchoscopy procedure. 57.2% of patients that will undergo bronchoscopy procedure felt scared or worried. In this implementation of Evidence-Based Nursing (EBN), binaural beat music therapy was given for the patients that will be having bronchoscopy procedure to help to reduce the patient's anxiety. The results of the EBN showed that giving binaural beat musical therapy significantly reduced the anxiety score for lung cancer patients that is undergoing bronchoscopy procedure. To implement the innovation project of Tracheostomy Bundle Booklet. The aim for this innovation is to increase the knowledge and ability for nurses to be able to do tracheostomy care. In addition, nurses will be able to educate patients and their families to do tracheostomy care independently if needed for home care."
