"ABSTRAKLatar Belakang. Penegakan diagnosis DVT akut ekstremitas bawah dengan sarana diagnostik yang ada termasuk venous duplex ultrasonography VDUS masih terkendala biaya, waktu dan tenaga operator. Penelitian ini bertujuan menilai efektivitas skor Wells, kadar D-Dimer atau skor kombinasi keduanya dibandingkan dengan VDUS dalam mendeteksi kasus-kasus DVT akut ekstremitas bawah.Metode. Dilakukan studi diagnostik dengan desain cross-sectional yang dilakukan di Divisi Bedah Vaskular dan Endovaskular Departemen Ilmu Bedah FKUI-RSCM pada semua pasien dewasa dengan dugaan DVT akut ekstremitas bawah yang dikonsultasikan ke Divisi Bedah Vaskular RSCM yang dikonfirmasi dengan pemeriksaan VDUS pada periode Januari 2014 ndash; Desember 2015 yang memenuhi kriteria inklusi dan eksklusi. Sumber data diambil dari rekam medik data sekunder . Analisis data berupa uji diagnostik dan diuji dengan SPSS version 17.0 for WindowsHasil. Studi melibatkan 85 orang. Didapatkan kejadian DVT akut ekstremitas bawah sebesar 65,88 . Sensitivitas dan nilai duga negatif tertinggi 100.00 didapatkan pada skor kombinasi II dan IV. Sedangkan spesifisitas tertinggi 89,66 dan nilai duga positif tertinggi 92,68 diperoleh pada skor kombinasi III. Skor dengan nilai diagnostik berimbang sensitivitas 87,50 ; spesifisitas 72,41 dijumpai pada skor Wells 3 level I.Simpulan. Skor Wells dan VDUS memiliki efektivitas sebanding dalam deteksi kasus-kasus kecurigaan DVT akut ekstremitas bawah.

ABSTRACTIntroduction. The diagnosis of acute lower extremity DVT with available resources including VDUS was still costly, time consuming and power consumng. The purpose of this study was to evaluate effectivity of Wells score, D Dimer and combination of both compared with VDUS in detecting lower extremity DVT. Methods. This was a cross sectional diagnostic study that was carried out at Vascular and Endovascular Surgery Division of Surgery Department of FKUI RSCM between by enrolling all adult patients with suspected acute DVT of lower extremity that was consulted to Vascular Surgery Division and confirmed with VDUS study between January 2014 and December 2015 that meet inclusion and exclusion criteria. Data source was taken from medical record. Data analysis in the form of diagnostic study was performed with SPSS version 17.0 for WindowsResult. The study enrolled 85 subjects, of which 65,88 was found to have acute lower extremity DVT. The highest sensitivity and negative predictive value 100,00 was discovered in combination score II and IV. The combination score III had the highest specificity 89,66 and positive predictive value 92,68 . Whereas the most balanced diagnostic study score was the Wells score 3 Level I with sensitivity 87,50 and specificity 72,41 . Conclusion. Wells score and VDUS were equally effective in detection of lower extremity acute DVT cases. "

"Praktik klinik residensi keperawatan medikal bedah pada pasien gangguan sistem muskuloskeletal bertujuan untuk mengimplementasikan peran perawat spesialis sebagai pemberi asuhan keperawatan, edukator, peneliti, dan inovator. Peran pemberi asuhan keperawatan dilakukan dengan menggunakan pendekatan Teori Self Care Orem pada pasien neglected hip dislocation dan 30 pasien lainnya dengan berbagai gangguan sistem muskuloskeletal. Peran sebagai edukator dilakukan oleh perawat spesialis terhadap pasien dan juga rekan sejawat di tatanan pelayanan rumah sakit. Penerapan EBN (Evidence Based Nursing) yaitu dengan mendengarkan musik untuk menurunkan nyeri pada pasien post operasi joint arthroplasty dilakukan untuk mengaplikasikan peran sebagai peneliti. Proyek inovasi juga dilakukan sebagai perwujudan peran sebagai inovator yaitu melalui pelaksanaan proyek inovasi berupa format pengkajian risiko DVT (Deep Vein Trombosis).

Medical surgical nursing residency clinical practice in patients with musculoskeletal system disorders aims to implement the role of specialist nurses as nursing care providers, educators, researchers, and innovators. The role of the nursing care provider was carried out using Orem's Self Care Theory approach to neglected hip dislocation patients and 30 other patients with various musculoskeletal system disorders. The role as an educator is carried out by specialist nurses for patients and also colleagues in the hospital service setting. The application of EBN (Evidence Based Nursing), namely by listening to music to reduce pain in postoperative joint arthroplasty patients, is carried out to apply the role of a researcher. Innovation projects are also carried out as an embodiment of the role as an innovator, namely through the implementation of an innovation project in the form of a DVT (Deep Vein Thrombosis) risk assessment format."

Proyek inovasi juga dilakukan sebagai perwujudan peran sebagai pembaharu atau inovator yaitu melalui pelaksanaan berupa Pengkajian Risiko DVT skala Autar dalam rangka meningkatkan kualitas asuhan keperawatan profesional di rumah sakit.

"Professionalime ilmu keperawatan adalah bagian disiplin ilmu keperawatan sampai dengan keperawatan sub spesialisasi dimana perawat spesialis sebagai seorang perawat yang dipersiapkan melampaui level seorang perawat generalis yang berwenang untuk praktek sebagai spesialis dengan keahlian dalam cabang bidang keperawatan. Ners spesialis keperawatan medikal bedah dengan peminatan muskuloskletal berperan dalam perawatan kekhususan masalah dan kasus kelolaan yang berkaitan dengan orthopedi atau muskuloskletal. Tujuan utama memberikan asuhan keperawatan secara komprehensif pada pasien kasus Spinal Cord Injury dengan trauma melalui pendekatan keperawatan model teori self-care Orem. Asuhan berupa pengelolaan 1 kasus kelolaan uatam dan 30 resume yang terdiri 3 bagian kasus besar yaitu kasus Spine, ekstremitas atas, dan ekstremitas atas dengan menggunakan teori model keperawatan Self-Care Orem yang menekankan tentang kebutuhan perawatan diri. Pencegahan terjadinya luka tekan dengan pemberian Aloe vera gel sebagai bagian dari tindakan mandiri keperawatan berdasarkan bukti penelitian ilmiah. Proyek inovasi berupa pengkajian resiko DVT dengan menggunakan Autar Scale adalah bentuk pengembangan dari intervensi keperawatan untuk pencegahan resiko terjadinya DVT pada pasien dengan kasus muskuloskletal. Tujuan seluruh kegiatan yang dilakukan selama kegiatan residensi adalah mewujudkan asuhan keperawatan yang komprehensif berdasarkan bukti ilmiah terkini.

The aim of all activities carried out during the residency activities is to realize comprehensive group care based on the latest scientific evidence."
