Abstrak :
Skripsi ini membahas mengenai prosedur pelayanan kesehatan rawat jalan di RS PMI Bogor, mulai dari proses pendaftaran pasien, pengambilan dokumen rekam medis, pengisian resume medis, hingga pembuatan laporan yang digunakan untuk monitoring dan evaluasi kegiatan. Analisis menggunakan prosedur penilaian cepat dalam pengumpulan data primer, serta pendekatan sistem dan metode prototipe. Hasil analisis memberikan informasi yang dapat diartikan tidak efisien dan efektif sistem rekam medis yang berjalan di RS PMI Bogor, baik dalam input maupun pembuatan laporan. Penggunaan dua sistem yang berbeda antara Poliklinik Reguler dan Poliklinik Afiat menciptakan dua database yang tidak terhubung sehingga dokumen rekam medis pasien menjadi terpisah diantara kedua sistem. Selain itu, dalam pembuatan laporan mengharuskan melakukan input ulang terhadap laporan-laporan dari Poliklinik Reguler dan Poliklinik Afiat sehingga memakan waktu lama dalam pembuatan laporan. Hasil akhir penelitian ini berupa rancangan prototipe pelaksanaan rekam medis untuk pelayanan rawat jalan terintegrasi yang dapat menjadi solusi dan diterapkan di RS PMI Bogor. ......This thesis describe about outpatient health care procedures in PMI Bogor hospital, starting from patient registration, medical record document retrieval, writing medical resume, and creating reports used for monitoring and evaluation activities. The Analysis using rapid assessment procedures in primary data collection, as well as the approach and methods of the prototype system. The analysis results give an information that the existing medical record system on PMI Bogor hospital is not efficient and effective, both in input and creating report. Using of two different systems between the Reguler Outpatient Department and Afiat Outpatient Departmen create two databases of patient’s medical records that are not connected to each other. In addition, in creating reports required to input again from the reports of the Reguler Outpatient Department and Afiat Outpatient Department, it takes more time in creating the report. The result of this research is design of a prototype of the integrated medical records for outpatient services that can be used as solution and applied in PMI Bogor hospital.

Abstrak :
Indikator persentase keterlambatan pengiriman berkas rekam medis pasien rawat jalan tahun 2018 di RS Hermina Ciputat masih belum mencapai target. Hal ini ditunjukkan dengan output yang dihasilkan selama 4 triwulan. Penelitian ini bertujuan untuk mendapatkan hasil analisis keterlambatan pengiriman berkas rekam medis pasien rawat jalan di Instalasi Rekam Medis RS Hermina Ciputat tahun 2018 dengan pendekatan sistem. Jenis penelitian ini adalah deskriptif dengan desain penelitian kualitatif. Hasil penelitian menunjukkan beberapa faktor penyebab terjadinya keterlambatan pengiriman berkas rekam medis rawat jalan yaitu pengembalian berkas rekam medis tidak tepat waktu, berkas rekam medis terletak tidak sesuai dengan nomor rekam medis, dan perangkat elektronik serta sistem yang mengalami eror. Berdasarkan hasil penelitian tersebut maka saran yang diberikan berupa meningkatkan pengawasan pada kinerja pekerja pelaksana, meningkatkan komunikasi efektif dalam bekerja, dan melakukan perencanaan penggunaan EMR.

Indicator of delivery delayed percentage of outpatients medical record files in 2018 have not yet reached the target. This is indicated by the result of output in 4 quarters. The aims of this research is to get the analysis results of the outpatients medical record files delivery delayed in Medical Record Installation at Hermina Ciputat Hospital in 2018 with an approach to the system. The type of this research is descriptive with qualitative research design. The results showed several factors causing the delayed in sending outpatient medical record files, including the return of medical record files not on time, the medical record file was located not based on the medical record number, and electronic devices and systems were errored. Thus, it was necessary to increase the supervision on the performance of workers, improving the effective communication at work, and planning the use of EMR.
