Abstrak :
Hiperlipidemia merupakan kelainan metabolisme lipid dan salah satu penyakit kronis yang sering dialami oleh masyarakat Indonesia dengan prevalensi 72,8%. Selain hiperlipidemia, gout adalah penyakit kronis yang sering dijumpai di Indonesia. Pasien dengan penyakit kronis terutama penyakit yang menimbulkan nyeri, umumnya harus mengkonsumsi obat dalam jangka panjang dan termasuk dalam kategori obat keras yang dapat diperoleh melalui resep dokter. Oleh karena itu, salah satu peran apoteker adalah melakukan pengkajian resep untuk memastikan pasien mendapatkan pengobatan yang tepat dan rasional serta menghindari terjadinya kesalahan dalam pelayanan kefarmasian dan medication error. Berdasarkan hasil pengkajian resep yang masuk di Apotek Roxy Ciledug mengenai terapi OAINS dan pengaruh OAINS terhadap penyakit hiperlipidemia dan gout, pada kajian administratif, resep memenuhi hampir semua aspek administratif, kecuali SIP dokter, alamat praktik dokter, dan nomor telepon dokter. Pada kajian farmasetik, resep memenuhi seluruh aspek dan persyaratan farmasetik. Pada kajian pertimbangan klinis, resep hampir memenuhi kriteria, namun terdapat masalah terkait obat, yaitu pemilihan dosis dan pemilihan obat yang dapat menyebabkan peningkatan risiko efek samping, yaitu terdapat interaksi mayor antara meloxicam dengan methylprednisolone dan pemilihan dosis meloxicam yang melebihi dosis maksimal per hari. ......Hyperlipidemia is a disorder of lipid metabolism and one of the chronic diseases often experienced by Indonesians with a prevalence of 72.8%. In addition to hyperlipidemia, gout is a chronic disease that is often found in Indonesia. Patients with chronic diseases, especially diseases that cause pain, generally have to take drugs in the long term and are included in the category of hard drugs that can be obtained through a doctor's prescription. Therefore, one of the roles of pharmacists is to review prescriptions to ensure that patients receive appropriate and rational treatment and avoid errors in pharmaceutical services and medication errors. Based on the results of the review of incoming prescriptions at Roxy Ciledug Pharmacy regarding NSAID therapy and the effect of NSAIDs on hyperlipidemia and gout, in the administrative review, the prescription fulfilled almost all administrative aspects, except for the doctor's SIP, doctor's practice address, and doctor's telephone number. In the pharmaceutical review, the prescription fulfilled all pharmaceutical aspects and requirements. In the clinical consideration review, the prescription almost met the criteria, but there were drug-related problems, namely dose selection and drug selection that could lead to an increased risk of side effects, namely there was a major interaction between meloxicam and methylprednisolone and meloxicam dose selection that exceeded the maximum dose per day.

Rumah sakit diselenggarakan dengan tujuan untuk meningkatkan mutu, mempertahankan standar pelayanan, dan mengutamakan keselamatan pasien. Medication error merupakan kesalahan penggunaan obat atau kegagalan dalam terapi obat ketika masih dalam pengawasan profesional kesehatan dan pasien yang dapat mengakibatkan efek berbahaya pada pasien. Kejadian ini dapat disebabkan oleh komunikasi yang buruk, seperti tidak lengkapnya informasi pasien, tulisantangan tidak terbaca, atau kesalahan komunikasi terkait pesanan obat, ambiguitas dalam nama produk, unit dosis, atau singkatan medis, kurangnya pelabelan yang sesuai, penyalahgunaan pasien karena pemahaman yang buruk tentang petunjuk penggunaan obat, serta faktor lingkungan, seperti pencahayaan, panas, dan kebisingan. Laporan ini dibuat dengan tujuan memperoleh data mengenai kejadian medication error pada fase prescribing dari segi administrasi, farmasetik, dan klinis pada resep Dinas Instalasi Farmasi RSPAD Gatot Soebroto. Teknik pengambilan data dengan purposive sampling dimana data populasi diperoleh dari resep Dinas yang dikumpulkan oleh Instalasi Farmasi Pusat. Sampel yang diambil merupakan sampel yang memenuhi kriteria dan dihitung menggunakan rumus Slovin. Populasi yang telah dihitung berjumlah 1067 resep sehingga sampel yang perlu diambil adalah sebanyak 291 resep. Medication error pada resep dari segi administrasi pada BPJS Dinas RSPAD Gatot Soebroto periode 12 hingga 16 Desember ditemukan terjadi dalam parameter nama pasien sebanyak 4.124%, umur pasien sebanyak 56.357%, jenis kelamin sebanyak 32.302%, berat badan dan tinggi badan pasien sebanyak 100%, nama dokter sebanyak 0.344%, nomor izin dokter sebanyak 0.344%, paraf dokter sebanyak 0.344%, serta tanggal peresepan sebanyak 20.619%. Sedangkan pada segi farmasetik dan klinis tidak ditemukan kejadian medication error pada resep. ...... Medication error is an error in the use of drugs or failure in drug therapy while still under the supervision of health professionals and patients which can result in harmful effects on patients. These incidents can be caused by poor communication, such as incomplete patient information, illegible handwriting, or miscommunication regarding drug orders, ambiguity in product names, dosage units, or medical abbreviations, lack of appropriate labeling, patient misuse due to poor understanding of instructions for using the drug, as well as environmental factors, such as lighting, heat, and noise. This report was prepared with the aim of obtaining data regarding the incidence of medication errors during the prescribing phase from an administrative, pharmaceutical, and clinical perspective on prescriptions from the Pharmacy Installation Service of the Gatot Soebroto Army Hospital. The data collection technique was purposive sampling where population data were obtained from official prescriptions collected by the Central Pharmacy Installation. The sample taken is a sample that meets the criteria and is calculated using the Slovin formula. The population that has been calculated is 1067 recipes so that the sample that needs to be taken is 291 recipes. Medication errors in prescriptions from an administrative point of view at the BPJS Dinas RSPAD Gatot Soebroto for the period December 12 to 16 were found to occur in the parameters of the patient's name by 4,124%, patient's age by 56,357%, gender by 32,302%, patient's weight and height by 100%, 0.344% doctor's name, 0.344% doctor's license number, 0.344% doctor's initials, and 20.619% prescription date. Meanwhile, from the pharmaceutical and clinical perspectives, there were no medication errors found in prescriptions.
