"Latar belakang: GPPH merupakan neurodevelopmental disorder dengan prevalensi dan tingkat disabilitas tertinggi pada kelompok anak dan remaja. Kondisi ini umumnya ditatalaksana dengan menggunakan metilfenidat untuk meningkatkan derajat fungsionalitas pada aspek fisik, psikis, maupun sosial. Meskipun demikian, penggunaan metilfenidat secara kronis (≥1 tahun) dinilai berpotensi menimbulkan efek samping berupa peningkatan gejala ansietas. Penelitian ini bertujuan untuk mengetahui hubungan durasi penggunaan metilfenidat dengan tingkat ansietas pada anak dan remaja dengan GPPH di RSCM, serta variabel luar yang dapat berhubungan dengan tingkat ansietas pada subjek penelitian.
Metode: Penelitian ini menggunakan desain potong lintang dan dilakukan pada 64 anak dan remaja berusia 7 – 17 tahun dengan GPPH di RSCM (32 subjek pada masing-masing kelompok dengan durasi penggunaan metilfenidat < 1 tahun dan ≥1 tahun). Penelitian dilakukan dengan menggunakan lembar data responden yang dikonfirmasi dengan rekam medis elektronik pasien untuk mengetahui durasi penggunaan metilfenidat serta variabel luar yang dapat mempengaruhi tingkat ansietas pada subjek (jenis kelamin, tatalaksana nonfarmakologi, tingkat pendidikan, tipe GPPH, derajat keparahan GPPH) serta kuesioner tervalidasi CSAS-C yang telah dimodifikasi untuk menilai tingkat ansietas subjek. Analisis data dilakukan dengan menggunakan uji Mann-Whitney untuk menilai hubungan antara variabel durasi penggunaan metilfenidat dengan tingkat ansietas. Analisis hubungan antara variabel luar dengan tingkat ansietas dilakukan dengan uji Mann-Whitney (variabel jenis kelamin, variabel tatalaksana nonfarmakologi), uji Kruskal-Wallis (variabel tingkat pendidikan, variabel tipe GPPH) dan uji korelasi Spearman (variabel derajat keparahan GPPH).
Hasil: Sebagian besar subjek memiliki jenis kelamin laki-laki (78,1%) dengan median usia 10 tahun (7 – 17 tahun), rerata usia diagnosis 7 ± 3,04 tahun, tipe diagnosis GPPH-NOS (46.9%), dan mendapatkan tatalaksana nonfarmakologi berupa konseling (100%) dan psikoterapi (98,4%). Hasil uji Kolmogorov-Smirnov terhadap tingkat ansietas dibandingkan dengan faktor durasi penggunaan metilfenidat menunjukkan distribusi tidak normal (p<0,05), dengan median 26 (20 – 50). Variabel luar yang berhubungan dengan tingkat ansietas pada subjek adalah tipe diagnosis GPPH (p = 0,021). Hasil uji Mann-Whitney menunjukkan hubungan yang tidak signifikan antara durasi penggunaan metilfenidat dan tingkat ansietas pada subjek (p = 0,814).
Kesimpulan: Tidak terdapat hubungan yang signifikan antara durasi penggunaan metilfenidat dengan tingkat ansietas pada anak dan remaja dengan GPPH di RSCM.

Introduction: ADHD is a neurodevelopmental disorder with the highest prevalence and disability level among children and adolescents. It is usually treated with methylphenidate to increase the degree of functionality in physical, psychological, and social aspects. However, chronic methylphenidate treatment (≥1 year) is considered to have a potential side effect of increasing anxiety levels. Therefore, this study aims to determine the association between the duration of methylphenidate treatment and anxiety levels in children and adolescents with ADHD and the associations between other extraneous variables and anxiety levels of the samples.
Method: This study used a cross-sectional design and was conducted on 64 children and adolescents aged 7-17 years old with ADHD that were treated with methylphenidate in RSCM. Equal 32 subjects were included in each group based on the duration of methylphenidate treatment (< 1 and ≥1 year of duration). This study used a respondent data sheet, confirmed with the patient’s EMR, to gain information regarding the duration of methylphenidate treatment and other extraneous variables which potentially affect anxiety levels of the samples (gender, nonpharmacological treatments, level of education, ADHD subtypes, severity of the ADHD). This study used a validated questionnaire (modified CSAS-C) to evaluate the anxiety levels of the samples. Data analysis was conducted using the Mann-Whitney test to evaluate the association between the duration of methylphenidate treatment and anxiety levels. Associations between extraneous variables and anxiety levels in samples were also analyzed using the Mann-Whitney test for gender & nonpharmacological treatments variables, Kruskal-Wallis test for the level of education & ADHD subtypes variables, and the Spearman correlation test for severity of the ADHD variable.
Result: The majority of the samples were male (78,1%) with a median age of 10 years (7 – 17), average diagnosis age of 7 ± 3,04 years, predominantly ADHD-NOS subtypes, and were majorly treated with counseling (100%) and psychotherapy (98,4%). Kolmogorov-Smirnov test for anxiety levels showed that the data is not normally distributed (p<0,05), with a median score of 26 (20 – 50). An extraneous variable that was significantly associated with anxiety levels of the samples is the ADHD subtypes (p = 0,021). The Mann-Whitney test showed no significant association between the duration of methylphenidate treatment and anxiety levels in the samples.

Anak dengan ADHD diketahui memiliki defisit dalam regulasi diri dan menampilkan perilaku impulsif. Adanya hambatan dalam meregulasi diri membuatnya kesulitan untuk secara sadar mengatur serta mengendalikan emosi, pikiran, dan tubuhnya untuk berperilaku sesuai dengan situasi yang dihadapi. Hal ini yang membuat anak dengan ADHD sulit diatur, cenderung menarik diri, menampilkan perilaku agresif, dan memiliki masalah sosial, baik dengan teman maupun keluarga. Orangtua dengan anak ADHD cenderung tidak merespon secara tepat kebutuhan anak, memiliki kontrol yang berlebihan, kurang memberikan pujian, dan kurang interaktif pada anaknya, sehingga terbentuklah insecure attachment pada anak dengan ADHD. Terbentuknya insecure attachment dapat memperparah masalah regulasi diri pada anak ADHD. Hal serupa terjadi pada N, anak ADHD berusia 6 tahun yang memiliki insecure attachment dengan orangtua. Salah satu intervensi yang dapat digunakan untuk mengatasi masalah regulasi diri pada N adalah Theraplay. Hasil dari penelitian ini menunjukkan bahwa Theraplay efektif dalam menangani masalah regulasi pada N dan merubah pola interaksi N dengan orangtua menjadi lebih positif

ABSTRACT
Children with ADHD are known to have deficits in self-regulation and shown impulsive behavior. Difficulties to do self-regulation makes it difficult to consciously manage and control emotion, mind, and body in order to behave accordingly to the situation. This things that makes children with ADHD tend to withdraw, displaying aggressive behavior, and have social problems, either with friends or family. Parents with ADHD children tend not to respond the needs of their children properly, have excessive control, failed to give appreciation, and less interactive with children, thus forming insecure attachment with ADHD children. Insecure attachment may worsening the self-regulation in children with ADHD. Something similar happened to N, 6 years old children with ADHD who have insecure attachment with the parents. One of the interventions that can be used to overcome the problem of self-regulation with N is Theraplay. The results of this study indicate that Theraplay is effective in dealing with regulatory issues at the N and also N change his patterns of interaction with his parents to become more positive."

"Gangguan Pemusatan Perhatian dan Hiperaktivitas (GPPH) adalah gangguan neuropsikiatri yang dapat terjadi pada anak-anak serta dapat memberikan beban dan hambatan dalam menjalankan fungsi sehari-hari. Komorbiditas psikiatrik pada GPPH dapat menambah morbiditas dan memperburuk prognosis dari GPPH. Perbaikan klinis GPPH berhubungan dengan kualitas hidup yang lebih baik, namun belum ada penelitian mengenai hubungan komorbiditas terhadap lama perbaikan klinis GPPH yang menggunakan alat ukur Abbreviated Conners Parent/Teacher Rating Scale (ACP/TRS), alat ukur yang digunakan di Indonesia. Penelitian ini bertujuan untuk mengetahui hubungan komorbiditas gangguan jiwa terhadap lama perbaikan klinis GPPH dengan menggunakan ACP/TRS. Dengan menggunakaan metode cross-sectional, penelitian ini menggunakan rekam medis pasien GPPH yang datang ke RSUPN Cipto Mangunkusumo (RSCM) dalam periode 1 Januari 2014-1 Januari 2018 sehingga didapatkan 94 sampel. Angka perbaikan klinis pasien GPPH dalam 7 minggu secara keseluruhan adalah 56,4% (n = 53), dengan komorbiditas yang paling sering adalah retardasi mental (40%, n = 16). Penelitian ini tidak menemukan hubungan komorbiditas gangguan jiwa terhadap perbaikan klinis GPPH (P = 0,85), kemungkinan karena variabel lain yang memengaruhi perbaikan klinis pasien tidak dieksklusi. Penelitian lebih lanjut perlu mempertimbangkan kepatuhan pasien dalam berobat dan kecukupan dosis obat yang diberikan.

Attention-Deficit/Hyperactivity Disorder (ADHD) is a neuropsychiatric disorder that can occur in children, which could add burden to their daily functions. Psychiatric comorbidities in ADHD may increase morbidity and worsen the prognosis of ADHD. Clinical improvement of ADHD is associated with better quality of life. However, there has not been a study of ADHD using Abbreviated Conners Parent/Teacher Rating Scale (ACP/TRS), the instrument used in Indonesia. This cross-sectional study used the medical record of ADHD patients in RSUPN Cipto Mangunkusumo (RSCM) that were in the Medical Records Unit within the period of 1 January 2014-1 January 2018. A total of 94 medical records were obtained. Within 7 weeks, 56,4% of all ADHD patients (n = 53) has improved, with the most prevalent comorbidity being mental retardation (40%, n = 16). This study found no significant relationship between psychiatric comorbidity and the clinical improvement of ADHD (P = 0,85), probably because some variables that affect clinical improvement are not excluded. Further studies that consider patients adherence to medication dan adequacy of the dosage of the drug administered are required."

Pola asuh orang tua diduga sebagai salah satu faktor risiko dari GPPH. Tujuan: Mengetahui hubungan antara pola asuh orang tua dengan GPPH pada siswa sekolah dasar di daerah Jakarta Pusat. Metode: Studi case control dilakukan terhadap 376 siswa SD Kenari 01,03, dan 05 pada periode tahun ajaran 2015-2016. Hasil: 108 28,7 siswa SD mengalami GPPH dan sebanyak 268 71,3 tidak mengalami GPPH. 314 83,5 orang tua menerapkan pola asuh demokratis dan sisanya sebanyak 62 16,5 orang tua menerapkan pola asuh lainnya pada anak. Secara statitstik tidak terdapat hubungan bermakna antara pola asuh orang tua dengan GPPH p = 0,464, uji chi square . Kesimpulan: Tidak terdapat hubungan bermakna antara pola asuh orang tua dengan GPPH pada siswa sekolah dasar.

Introduction Attention deficit hyperactivity disorder ADHD is one of neurobehavioral chronic disorders and caused by some factors, including genetic and environment cause including parenting style. The objective of this study is to find whether parenting style applied by parents is related with ADHD. Methods case control study was done involving 376 elementary students in SD Kenari 01,03,05 on school year 2015 ndash 2016 using questionnaires given to parents and teachers to determine the parenting style used and presentation of ADHD. Results 108 28,7 elementary school students suffer from ADHD. 314 83,5 of parents apply authoritative parenting style to their children. There was no correlation statistically between parenting style and ADHD p 0.464 , chi square test . Conclusion Parenting style is not related with ADHD on elementary students.
