"Latar Belakang: Penyakit ginjal kronis (PGK) atau chronic kidney disease (CKD) didefinisikan sebagai adanya kerusakan ginjal atau perkiraan laju filtrasi glomerolus (LFG) kurang dari 60ml/menit/1,73m2 yang berlangsung selama 3 bulan atau lebih. Tindakan akses vaskular berupa arteriovenous fistula dapat dibuat pada pasien penyakit ginjal kronis yang menjalankan terapi hemodialisis. Latihan isometrik berupa handgrip exercise dilaporkan dapat meningkatkan diameter arteri radialis dan vena cephalica, peak systolic velocity (PSV), intima media thickness (IMT), dan volume flow arteri radialis (RAVF).
Tujuan: Menganalisis pengaruh handgrip exercise sebelum dan setelah tindakan pembuatan Arteriovenous Fistula Radiocephalica terhadap perubahan diameter arteri radialis dan vena cephalica, PSV, IMT, dan RAVF.
Metode: Desain pada penelitian ini adalah controlled trial yang dilakukan di RSUPN Cipto Mangunkusumo. Setelah tindakan pembuatan Arteriovenous Fistula Radiocephalica, pasien dibagi menjadi dua kelompok, yaitu kelompok kontrol dan kelompok intervensi. Pada kelompok kontrol, pasien melakukan tatalaksana standar sedangkan pada kelompok intervensi, pasien melakukan tatalaksana standar dan handgrip exercise lalu kedua kelompok di follow up setelah 8 minggu.
Hasil: Total subjek penelitian sebanyak 53 orang, dimana terdapat 7 pasien yang masuk kriteria drop out, yang terdiri dari 2 pasien meninggal dan 5 pasien tidak kontrol. Usia median pada kelompok intervensi adalah 53 tahun dan pada kelompok kontrol adalah 56 tahun. Pada kelompok yang melakukan handgrip exercise, terdapat perbedaan bermakna antara sebelum dan sesudah latihan yaitu pada parameter diameter arteri radialis (p=0,022), diameter vena cephalica (p<0,001), PSV (p<0,001), dan volume flow arteri radialis (p<0,001). Di sisi lain, tidak terdapat perbedaan bermakna antara nilai IMT sebelum dan sesudah latihan hand grip (p=0,575). Sementara itu, pada kelompok kontrol ditemukan juga terdapat perbedaan signifikan antara sebelum dan sesudah masa follow-up terkait parameter diameter vena cephalica (p<0,001), PSV (p<0,001), dan RAVF (p<0,001). Pada parameter diameter arteri radialis (p=0,103) dan IMT (p=0,083) tidak terdapat perbedaan yang bermakna pada kelompok kontrol.
Simpulan: Handgrip exercise dapat meningkatkan perubahan diameter arteri radialis dan vena cephalica, PSV, dan RAVF. Tidak terdapat perubahan pada IMT setelah handgrip exercise.

Background: Chronic kidney disease (CKD) is defined as the presence of kidney damage or an estimated glomerular filtration rate (eGFR) less than 60ml/min per 1.73 square meters, persisting for 3 monts or more. Creating vascular access such as arteriovenous fistula can be done in hemodialysis therapy patients with chronic kidney disease. Isometric exercises in the form of handgrip exercise has been reported to increase the diameter of radial artery and cephalic vein, peak systolic velocity (PSV), intima media thickness (IMT), and radial artery volume flow (RAVF).
Objective: To analyze the effect of handgrip exercise before and after radiochepalic arteriovenous fistula creation on changes of the diameter of radial artery and cephalic vein, PSV, IMT, and radial artery volume flow.
Methods: The design of this research was controlled trial at Cipto Mangunkusumo Center National Hospital. After the creation of radiocephalic arteriovenous fistula, patients divided into two groups, control group and intervention group. The control group received usual care. Usual care and handgrip exercise was performed in the intervention group, both groups were assessed at 8 weeks post the creation of radiocephalic arteriovenous fistula.
Results: For 53 patients, 7 patients were dropped out, consist of 2 patients passed away and 5 patients lose control. Median age of this research subjects was 56 years old. A significant increase of diameter of radial artery (p=0,022) and cephalic vein (p<0,001), PSV (p<0,001), and radial artery volume flow (p<0,001) was observed in intervention group. Meanwhile there was no change of intima media thickness before and after handgrip exercise (p=0,575). Similarly, there was significant increase of cephalic vein diameter (p<0,001), PSV (p<0,001), and RAVF (p<0,001) and there was no significant change of radial artery diameter (p=0,103) and IMT in control group.
Conclusion: Handgrip exercise improved diameter of radial artery and cephalic vein PSV, and RAVF. There was no change of IMT after handgrip exercise

"Penyakit Ginjal Kronik (PGK) merupakan kelainan struktur atau fungsi ginjal yang mengalami penurunan selama 3 bulan yang mengalami peningkatan prevalensi kasus. Peningkatan prevalensi kejadian PGK juga akan meningkatkan kebutuhan hemodialisis dan penggunaan arteriovenous fistula (AVF). Maturasi dan keberhasilan AVF dipengaruhi oleh faktor pasien dan struktur vaskular. Latihan isometrik dilaporkan dapat meningkatkan diameter vena, arteri, dan peak systolic velocity (PSV). Tujuan penelitian ini untuk melihat pengaruh latihan isometrik pre operatif terhadap diameter vena cephalica, diameter arteri radialis, PSV, intimal medial thickening (IMT), dan volume flow arteri radialis. Desain penelitian adalah eksperimental pre and post-test study, dilakukan di RSUPN Cipto Mangunkusumo. Penelitian dilaksanakan follow up pasien selama 8 minggu latihan isometri. Total subjek penelitian sebanyak 38 orang. Usia median subjek penelitian yaitu 56 tahun dengan rentang usia 20 sampai 71 tahun. Terdapat perbedaan yang signifikan antara diameter vena (p=0,003), PSV (p=0,032), dan volume flow (p=0,030) subjek penelitian pre dan post latihan isometrik. Terdapat perbedaan signifikan antara perubahan diameter vena terhadap komorbid diabetes melitus. Tidak terdapat perbedaan bermakna antara perubahan diameter vena, PSV, dan volume flow paska latihan ismoetrik terhadap kelompok usia, komorbid, dan jenis kelamin (p>0,005). Penggunaan latihan isometrik dapat meningkatkan perubahan diameter vena, PSV dan volume flow pada pasien sebelum pembuatan AVF radiocephalica. Tidak terdapat perubahan signifikan diameter vena pasca latihan isometrik pada penderita diabetes melitus.

Chronic Kidney Disease (CKD) is a disorder of kidney structure or function that has decreased over 3 months and has an increased prevalence of cases. The increasing prevalence of CKD will also increase the need for hemodialysis and the use of arteriovenous fistula (AVF). AVF maturation and success are influenced by patient factors and vascular structure. Isometric exercise is reported to increase the diameter of veins, arteries, and peak systolic velocity (PSV). Objective: Analyzing the effect of preoperative isometric exercise on the diameter of veins, arteries, PSV, intimal medial thickening (IMT), and volume flow. The research design was an experimental pre and post-test study, conducted at Cipto Mangunkusumo Center National Hospital. The study was conducted to follow up patients for 8 weeks of isometric exercise. The total study subjects were 38 people, with the highest prevalence being men, and comorbid hypertension. The median age of the research subjects was 56 years with an age range of 20 to 71 years. There were significant differences between venous diameter (p=0.003), PSV (p=0.032), and volume flow (p=0.030) in pre and post isometric training subjects. There was significant difference between cephalic vein diameter to diabetes mellitus group. There was no significant difference between changes in radial artery diameter, PSV, IMT, and post-isometric exercise volume flow for the age, comorbid, and sex groups (p>0.005). The use of isometric exercises can increase changes in venous diameter, PSV and volume flow in patients before the making of radiocephalic AVF. There was no significant change in venous diameter after isometric exercise in patient with diabetes mellitus.

Latar Belakang :

Jumlah kasus Pasien dengan Penyakit Ginjal Tahap Akhir (PGTA) terus meningkat, dan permintaan akan hemodialisis juga semakin melonjak. Arteriovenosa Fistula (AVF) menjadi pilihan utama dalam prosedur hemodialisis dan sering ditempatkan sedistal mungkin, namun tingkat keberhasilan cenderung rendah karena ukuran diameter yang tidak optimal. Latihan isometrik dan terapi Far Infrared (FIR) berpotensi untuk meningkatkan diameter, Peak Sistolyc Velocity (PSV), Volume flow arteri Radialis, serta diameter vena Cephalica pada pasien PGTA yang akan menjalani pembuatan AVF radiocephalica.

Tujuan Penelitian ini bertujuan untuk mengidentifikasi korelasi antara pemberian Terapi Far Infra-Red dan latihan isometrik terhadap diameter, PSV, dan aliran volume arteri radialis serta diameter vena cephalica pada pasien-pasien dengan penyakit ginjal tahap akhir yang akan menjalani pembuatan AVF Radiocephalica.

Metode penelitian dilakukan dengan rancangan penelitian eksperimental RCT (Randomized Controlled Trial) di ruang hemodialisis RSUPN Ciptomangunkusumo. Selama 4 minggu, kelompok eksperimen diberikan latihan isometrik dan terapi FIR, dan hasilnya kemudian dibandingkan dengan kelompok kontrol yang hanya diberikan Latihan isometrik

Hasil: Jumlah total subjek penelitian adalah 46 orang, dengan mayoritas perempuan (65,2%) dan komorbiditas Diabetes Melitus (37%). Median usia subjek adalah 54 tahun dengan rentang usia antara 18 hingga 73 tahun. Tersingkap adanya perbedaan signifikan secara statistik antara diameter arteri radialis (p<0.05), PSV arteri radialis (p<0,05), Volume Flow arteri radialis (p<0,05), dan diameter vena cephalica (p<0,05) pada subjek penelitian sebelum dan setelah menjalani latihan isometrik dan Terapi Far Infrared (FIR). Faktor risiko Diabetes Melitus (p<0,05) dan obesitas (p<0,05) juga terbukti memiliki pengaruh signifikan terhadap diameter vena cephalica.

Kesimpulan: Dapat disimpulkan bahwa aplikasi latihan isometrik dan Terapi Far Infrared (FIR) efektif dalam meningkatkan parameter-parameter seperti diameter arteri radialis, PSV arteri radialis, volume flow arteri radialis, serta diameter vena cephalica pada pasien dengan Penyakit Ginjal Tahap Akhir sebelum prosedur pembuatan AVF radiocephalica.


Background :The number of patients with End Stage Kidney Disease (ESKD) continues to increase, and the need for hemodialysis is also increasing. Arterivenosa Fistula (AVF) is the main access option for hemodialysis and is performed as distally as possible, but the success rate is not very high due to the diameter that is not optimal. The utilization of isometric exercise in conjunction with Far Infrared (FIR) therapy is potentially more effective in enhancing the diameter, Peak Systolic Velocity (PSV), Volume Flow of the Radial Artery, and the cephalic vein diameter in ESKD patients before the creation of radiocephalic AVF compared to using isometric exercise alone.

 

Objective: To determine the difference between isometric exercises combined with Far Infrared therapy and isometric exercise alone on the diameter, Peak Systolic Velocity (PSV), and volume flow of the radial artery, as well as the cephalic vein diameter in End-Stage Kidney Disease patients before radiocephalic AVF creation, compared to utilizing only isometric exercises as the standard procedure..

 

Methods: This study is an RCT (Randomized Controlled Trial) experimental study, conducted in the hemodialysis room of Ciptomangunkusumo Hospital. The study was conducted for 4 weeks of isometric exercise and FIR therapy, the results were compared with the control.

 

Results: The total study subjects were 46 people, mostly female (65.2%), comorbid Diabetes mellitus (37%). Median age was 54 years with a range of 18-73 years. There were statistically significant differences between radial artery diameter (p<0.05), radial artery PSV (p<0.001), radial artery flow volume (p<0.001), cephalic vein diameter (p<0.001) pre and post isometric exercise and FIR. Risk factors of diabetes mellitus (p <0.05) and obesity (p <0.05) have a significant change on the diameter of the cephalic vein.

 

Conclusion: The use of isometric exercise and FIR can increase radial artery diameter, radial artery PSV, radial artery flow volume and cephalica vein diameter in ESKD  patients before radiocephalica AVF creation.

"Anemia defisiensi besi merupakan salah satu komplikasi penyakit ginjal kronis (PGK) yang sering dijumpai terutama pada pasien yang menjalani hemodialisis rutin. Inflamasi merupakan kondisi yang selalu ada pada pasien PGK yang menjalani hemodialisis, dengan salah satu markernya yaitu C-reactive protein (CRP). Marker besi yang rutin digunakan seperti feritin dan saturasi transferin dipengaruhi oleh inflamasi sehingga status besi pada pasien PGK menjadi sulit dinilai. Marker lain seperti persentase eritrosit hipokrom (%Hypo-He), reticulocyte haemoglobin content (Ret-He), soluble transferrin receptor (sTfR), indeks sTfR, dan persentase eritrosit mikrositik (%MicroR) dapat digunakan untuk menilai status besi, namun belum rutin digunakan. Tujuan penelitian ini adalah untuk mengetahui pengaruh inflamasi terhadap %Hypo-He, Ret-He, sTfR, indeks sTfR, dan %MicroR pada pasien PGK dengan hemodialisis rutin dalam menentukan status besi. Penelitian ini merupakan studi potong lintang yang melibatkan 123 pasien hemodialisis rutin berusia lebih dari 18 tahun di Unit Hemodialisis RS Cipto Mangunkusumo yang berlangsung pada bulan Agustus sampai September 2018. Setiap subjek diperiksakan parameter %Hypo-He, Ret-He, dan %MicroR menggunakan alat automated hematology analyzer Sysmex XN 3000, sedangkan sTfR, indeks sTfR, dan CRP diperiksa menggunakan alat Cobas c311. Didapatkan median CRP sebesar 3,99 (0,2- 129,97) mg/L dengan proporsi pasien PGK dengan hemodialisis rutin yang mengalami inflamasi sebanyak 45,5%. Pada penelitian ini tidak didapatkan perbedaan %Hypo-He, Ret-He, sTfR, indeks sTfR, dan %MicroR pada pasien yang menjalani hemodialisis rutin yang mengalami inflamasi dan noninflamasi sehingga marker-marker tersebut dapat digunakan untuk menentukan status besi pada pasien PGK dengan hemodialisis rutin.

Iron deficiency anemia is one of the complications seen in chronic kidney disease especially in routine hemodialysis patients. Inflammation, marked by C-reactive protein (CRP), is often found in chronic kidney disease (CKD) patients with hemodialysis. Routine iron markers, such as ferritin and transferrin saturation, are influenced by inflammation. Hence the iron status in CKD patients is difficult to interpret. Other markers like hypochromic erythrocytes percentage (%Hypo-He), reticulocyte haemoglobin content (Ret-He), soluble transferrin receptor (sTfR), sTfR index, and microcytic erythrocytes percentage (%MicroR) can be used to evaluate iron status, but these markers are not commonly used. This study aims to evaluate the influence of inflammation in %Hypo-He, Ret-He, sTfR, sTfR index, and %MicroR in routine hemodialysis patients to determine the iron status. This study was a cross sectional study comprised of 123 routine hemodialysis patients, aged over 18 years old, in Hemodialysis Unit Cipto Mangunkusumo Hospital during August to September 2018. Blood samples from all subjects were evaluated using automated hematology analyzer Sysmex XN 3000 for %Hypo-He, Ret-He, and %MicroR, and Cobas c311 for sTfR, sTfR index, and CRP. Median CRP in all patients was 3,99 (0,2-129,97) mg/L with inflammation occurred in 45,5% patients. There were no differences found in %Hypo-He, Ret-He, sTfR, sTfR index, and %MicroR in routine hemodialysis patients with inflammation and noninflammation so these markers could be used to evaluate iron status in hemodialysis patients."

"ABSTRAK
Hemodialisis merupakan tatalaksana renal replacement yang tersering pada pasien gagal ginjal
kronik stadium 5, Akses vaskular dan morbiditas sebagai akibat komplikasi akses merupakan
penyebab utama perawatan di rumah sakit. Kegagalan maturitas merupakan hambatan utama
penggunaan arteriovenous fistula. Tujuan dari penelitian ini adalah untuk mengkaji korelasi peak
sistolik velocity arteri brakialis dan volume flow draining vein intraoperatif dengan
menggunakan ultrasonografi doppler untuk memprediksi maturasi AVF. Uji statistik yang
digunakan adalah uji Mann Whitney dan uji Chi Squere. Hasil yang didapatkan tidak ditemukan
korelasi antara PSV arteri brakialis dengan maturitas. Rerata nilai titik potong volume flow
draining vein intraoperatif 259,43 ml/min dan paska operatif 679,22 ± 65,36 ml/min
dihubungkan dengan maturitas, ini dapat menjadi acuan menetukan perlu tidaknya melakukan
tindakan revisi saat intraoperatif, yang pada akhirnya diharapkan dapat menurunkan angka

"Penyakit ginjal kronis (PGK) merupakan masalah kesehatan masyarakat global yang menyumbang kenaikan angka morbiditas, mortalitas, beban biaya kesehatan, dan masalah kesehatan lainnya. Menurut Riskesdas 2018, prevalensi PGK di Indonesia mencapai 0,38% dan mengalami peningkatan 0,2% dibandingkan tahun 2013. PGK juga merupakan penyebab kematian tertinggi ketiga di Indonesia berdasarkan data Global Burden of Disease tahun 2019. Meningkatnya insiden penyakit ginjal kronis turut mempengaruhi peningkatan jumlah pasien yang menjalani hemodialisis sebagai terapi pengganti fungsi ginjal untuk bertahan hidup. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang mempengaruhi ketahanan hidup pasien penyakit ginjal kronis yang menjalani hemodialisis di RS Krakatau Medika tahun 2019-2021. Desain penelitian ini adalah kohort retrospektif dengan menggunakan data sekunder dari rekam medis pasien PGK yang menjalani hemodialisis di RS Krakatau Medika Tahun 2019-2021. Data pasien yang diambil meliputi usia, jenis kelamin, riwayat keluarga PGK, komorbid hipertensi, komorbid diabetes melitus, dan komorbid kardiovaskular. Analisis data menggunakan analisis survival dengan metode Kaplan-Meier dan Regresi Cox. Dari studi ini diketahui sebanyak 216 sampel yang memenuhi kriteria inklusi dan eksklusi dengan 84 pasien telah meninggal dan 132 pasien adalah sensor. Probabilitas ketahanan hidup satu, dua, dan tiga tahun pasien PGK yang menjalani hemodialisis sebesar 58%, 43%, dan 36%. Terdapat perbedaan yang signifikan pada ketahanan hidup pasien berdasarkan usia, komorbid hipertensi, dan komorbid diabetes melitus (log rank test, p<0,05). Hasil analisis regresi cox menunjukkan usia (HR=2,28, 95% CI 1,444—3,588, p<0,001) dan komorbid hipertensi (HR=0,40, 95% CI 0,245—0,668 p<0,001) mempengaruhi ketahanan hidup pasien. Usia dan komorbid hipertensi merupakan faktor yang paling berpengaruh terhadap ketahanan hidup pasien. Pasien PGK yang menjalani hemodialisis pada usia ≥ 55 tahun dan tidak terdapat komorbid hipertensi memiliki ketahanan hidup yang lebih rendah dibandingkan pasien dengan usia <55 tahun dan terdapat komorbid hipertensi. Diharapkan dapat meningkatkan peran keluarga dan petugas kesehatan dalam memberikan dukungan moril serta pengawasan pada pasien selama menjalani manajemen perawatan hemodialisis khususnya pada pasien berusia tua, memiliki komorbid hipertensi, dan komorbid diabetes melitus

Chronic kidney disease (CKD) is a global public health issue that contributes to rising morbidity, mortality, health costs, and other health issues. According to Riskedas 2018, the prevalence of CKD in Indonesia was 0.38% and increase by 0.2% compared to 2013. CKD is also the third leading cause of death in Indonesia based on Global Burden of Disease data 2019. The rising insidences of chronic kidney disease also affects the increasing number of patients undergoing hemodialysis as a replacement therapy for kidney function to survive. This study aims to identify the factors that affect the survival of patients with chronic kidney disease undergoing hemodialysis at Krakatau Medika Hospital in 2019-2021. The design of this study was a retrospective cohort using secondary data from the medical records of CKD patients undergoing hemodialysis at Krakatau Medika Hospital in 2019–2021. Age, gender, family history of CKD, comorbidities for hypertension, diabetes mellitus, and cardiovascular were among patient data collected. Data analysis used survival analysis with the Kaplan-Meier method and Cox regression. From this study, there were 216 samples met the inclusion and exclusion criteria with 84 patients had died and 132 patients were censored. The probability of one, two, and three-year survival of CKD patients undergoing hemodialysis were 58%, 43%, and 36%, respectively. There were significant differences in patient survival based on age, comorbid hypertension, and comorbid diabetes mellitus (log-rank test, p<0.05). The results of the Cox regression analysis showed that age (HR = 2.28, 95% CI: 1.444–3.588, p<0.001) and comorbid hypertension (HR = 0.40, 95% CI: 0.245–0.668, p<0.001) affected patient survival. The most significant factors affecting patient survival are age and comorbid hypertension. Patients with CKD undergoing hemodialysis at the age of ≥ 55 years old and no comorbid hypertension have lower survival rates than patients with age < 55 years old and comorbid hypertension. It is expected to increase the role of family and health workers in providing emotional support and monitoring of patients during hemodialysis care management especially in patients who are elderly, have comorbid hypertension, and comorbid diabetes melitus."

"ABSTRAK
Pasien gagal ginjal kronis rentan mengalami stress dan ansietas karena hospitalisasi dan perubahan kehidupan selanjutnya. Kondisi tersebut berkontribusi terhadap peningkatan tekanan darah. Target tekanan darah bagi pasien dengan gagal ginjal kronis adalah <130/80 mmHg untuk mencegah morbiditas terhadap penyakit kardiovaskular. Penelitian sebelumnya telah menunjukan bahwa penggunaan teknik relaksasi otot progresif dan nafas dalam dapat menurunkan tekanan darah secara efektif. Studi kasus ini bertujuan untuk mengidentifikasi pengaruh pemberian kombinasi relaksasi otot progresif dan napas dalam pada pasien gagal ginjal kronis untuk mengontrol tekanan darah melalui penurunan tingkat stres dan ansietas. Hasil analisis pada kasus kelolaan di ruang rawat penyakit dalam Dr RSUPN Cipto Mangunkusumo menunjukan bahwa tekanan darah sistolik menurun 4-6 mmHg, namun tekanan diastolik tidak menurun. Skor DASS 21 menunjukan tingkat stres dan ansietas menurun setelah 7 hari implementasi relaksasi. Sosialisasi mengenai penggunaan teknik relaksasi otot progresif dan nafas dalam sebagai terapi tambahan selain medikasi dan mengatasi ansietas dan stress diperlukan untuk membantu pasien gagal ginjal kronis untuk menurunkan tekanan darah.

ABSTRACT
Patients with chronic kidney failure are susceptible to stress and anxiety due to hospitalization and subsequent life changes. This condition contribute in increasing blood pressure. The blood pressure target for patients with chronic renal failure is <130/80 mmHg to prevent morbidity against cardiovascular disease. Previous research has demonstrated the use of progressive muscle relaxation and deep breathing techniques are effective to reduce blood pressure. The purpose of this case study was to identify the effect of combination of progressive muscle and deep breathing relaxation in patients with chronic kidney failure to control blood pressure by reducing stress and anxiety levels. The results of case study of a patient in Internal Medicine Ward of RSUPN Dr Cipto Mangunkusumo showed that systolic blood pressure decreased 4-6 mmHg, but the diastolic pressure did not decrease. Beside that, DASS 21 score shows that stress and anxiety levels decreased after 7 days of relaxation. Socialization on the use of relaxation and deep breathing techniques as adjuvant therapy of medication and to decrease stress and anxiety that related with increased blood pressure, were needed to control blood pressure of CKD patients."

"ABSTRAK
Penyakit gagal ginjal terminal mengharuskan pasien menjalani hemodialisis dengan sejumlah penyesuaian gaya hidup seperti diet dan pembatasan asupan cairan yang memunculkan berbagai pikiran dan perasaan negatif. Kebanyakan pasien berusaha menghindari ataupun denial dengan perilaku melanggar diet yang mengarahkan berbagai komplikasi medis. Untuk meningkatkan kepatuhan diet, diusulkan teknik intervensi Acceptance and Commitment Therapy (ACT). Kekuatan dari intervensi ini adalah tujuannya yang membuat individu menyadari segala pengalaman negatif tanpa berusaha menghindari/menghilangkan. Hal ini membuat individu lebih menyadari hal yang penting untuk mengarahkan pada hidup yang bermakna dan penggunaan strategi yang efektif dalam masalah diet. Desain penelitian yang digunakan adalah single subject design dengan purposive sampling pada tiga pasien gagal ginjal. Pada pelaksanaannya, satu partisipan yang dapat menyelesaikan sesi. Hasilnya menunjukkan peningkatan partisipan dalam menyadari pengalaman negatif dari diet dan tampak usaha mengatur asupan minum. Walaupun demikian, pengukuran kuantitatif tidak menunjukkan perubahan signifikan pada perilaku kepatuhan diet dan asupan cairan.

ABSTRACT
End-Stage Renal Disease (ESRD) patients have to follow hemodialysis with some life changes as dietary and fluid restriction that emerges some negative thoughts and emotions. Patients used to solve this problems by avoid or denial in non-adherence behaviors. These behaviors cause some health problem and make problem getting worse. To overcome this problem, it is used Acceptance and Commitment therapy. Strength of this intervention is accepts and aware all of experience (thoughts, emotions, memories, bodily sensations) without avoid it. So, patient can aware values that compass to do meaningful life and choose better strategies for dietary and fluid restriction problems. This study used single subject design and purposive sampling with 3 ESRD patients. Only one participant that completing the sessions and reported increased awareness to dietary negative experiences without avoidance it. He also showed efforts to controlled fluid consumptions. Nevertheless, quantitative measurement showed no significant changes in dietary and fluid adherence behavior."

"Background: chronic kidney disease (CKD) increases the severity and risk of mortality in acute coronary syndrome (ACS) patients. The role of β2-M as a filtration and inflammation marker and FGF23 as a CKD-MBD process marker might be significant in the pathophysiology in ACS with CKD patients. This study aims to determine the association of β2-M and FGF23 with major adverse cardiac event (MACE) in ACS patients with CKD. Methods: we used cross sectional and retrospective cohort analysis for MACE. We collected ACS patients with CKD consecutively from January until October 2018 at Dr. Cipto Mangunkusumo General Hospital. Data were analyzed using logistic regression and Cox's Proportional Hazard Regression. Results: a total of 117 patients were selected according to the study criteria. In bivariate analysis, β2-M, FGF23, and stage of CKD had significant association with MACE (p = 0.014, p = 0.026, p = 0.014, respectively). In multivariate analysis, β2-M - but not FGF 23- was significantly associated with MACE (adjusted HR 2.16; CI95% 1.15-4.05; p = 0.017). Conclusion: β2-M was significantly associated with MACE, while FGF23 was not so. This finding supports the role of inflammation in cardiovascular outcomes in ACS with CKD patient through acute on chronic effect."

"Gagal Ginjal Kronik pada anak saat ini berkembang pesat kasusnya terutama daerah perkotaan. Klien dengan GGK stadium akhir memerlukan terapi pengganti ginjal salah satunya hemodialisis. Komplikasi umum dari GGK adalah kelebihan volume cairan yang berkembang menjadi edema perifer atau anasarka, edema paru, dan hipertensi. Oleh karena terapi hemodialisis perlu disertai pembatasan cairan dan rendah garam. Pembatasan cairan dan garam ini bertujuan mencegah komplikasi akibat penambahan beban ginjal.
Karya Ilmiah Akhir Ners ini bertujuan menganalisis efektivitas intervensi pemantauan ketat pembatasan cairan dan garam untuk mengurangi gejala serta mencegah komplikasi pada pasien gagal ginjal kronik. Metodologi yang digunakan adalah metode studi kasus.
Hasil analisis menunjukkan pemantauan pembatasan cairan dan garam yang ketat terbukti menurunkan tanda dan gejala kelebihan volume cairan diantara dua waktu dialysis ditandai berkurangnya edema dan tidak muncul komplikasi Rekomendasi dari analisis ini adalah penting untuk perawat memonitoring dan mengedukasi anak dan keluarga dengan GGK untuk melakukan restriksi cairan dan diet rendah garam untuk menghindari masalah kelebihan volume cairan diantara waktu dialisis.

CKD in children now is a major health problem especially in urban city because of the increase of morbidity.. Children with CKD in end stage or End Stage Renal Disease ERSD needs renal replacement therapy RRT to help maintenance kidney function, and one of the RRT therapy is hemodyalisis. Moreover, although HD helping so much, the risk of complications still high, like lung oedem, anasarka or peripheral oedem,and hypertension. Dietary and fluid restrictions in CKD purposely needed to reduce the risk of complications.
This Final Scientific Work of Ners aims to analyze the interventions of the effectiveness fluid restriction and dietary as thight as much to reduce the risk fluid excess volume complication of CKD in children. The methodology used is the case study method and the existing research analisis.
The results showed that daily fluid restriction and diet monitoring is effective to reduce the risk of fluid excess inter dialisis time in children. It rsquo s important for nurses to monitor and educate patient about restriction to prevent fluid excess of CKD in children.
