"Penggunaan obat dalam jumlah banyak atau yang disebut polifarmasi dapat meningkatkan risiko interaksi antar obat atau obat dengan penyakit. Komplikasi umum biasa terjadi pada pasien geriatri, oleh sebab itu pasien geriatri dengan gangguan penyakit kronis, seperti hipertensi, gangguan jantung, osteoarthritis, diabetes melitus dan sebagainya pada umumnya akan memperoleh lebih dari satu obat dalam sekali konsumsi. Maka dari itu akan dilakukan pengkajian rasionalitas dan masalah terkait obat terhadap resep dengan polifarmasi. Data yang dapat digunakan untuk pengkajian berupa resep polifarmasi yang berasal dari Apotek Roxy Depok pada Bulan September 2023. Melalui pengkajian ini diharapkan dapat memberikan gambaran terkait metode yang tepat dalam mengkaji rasionalitas dari resep polifarmasi. Diperoleh hasil pada aspek klinis, resep 1 ditemukan ketidaksesuaian dosis obat Xarelto, sehingga perlu ditanyakan pada dokter penulis resep dan juga terdapat interaksi anatara aspirin dengan Rivaroxaban yang mana dapat meningkatkan risiko pendarahan. Resep kedua sudah sesuai. dan pada resep ketiga terdapat interaksi obat yang memerlukan monitoring dalam penggunaannya yaitu, obat Theobron dengan Lasal, Desloratadine dengan Zitromax dan obat Lasal dengan Zitromax.

Using drugs in large quantities or what is called polypharmacy can increase the risk of interactions between drugs or drugs and disease. Through this study it is hoped that it can provide an overview of the appropriate method for assessing the rationality of polypharmacy prescriptions. Based on the results obtained from the clinical aspect, prescription 1 found a discrepancy in the dosage of the drug Xarelto, so it was necessary to ask the doctor who wrote the prescription and there was also an interaction between aspirin and Rivaroxaban which could increase the risk of bleeding. The second recipe is suitable. and in the third prescription there is a drug interaction that requires monitoring in its use, namely, the drug Theobron with Lasal, Desloratadine with Zitromax and the drug Lasal with Zitromax."

"Apotek merupakan sarana pelayanan kefarmasian dimana dilakukannya praktik kefarmasian oleh Apoteker. Pengkajian dan pelayanan resep merupakan kegiatan pertama dalam memberikan pelayanan farmasi klinik. Tujuan penulisan ini adalah untuk memperoleh gambaran terkait kelengkapan resep di Apotek Roxy Depok berdasarkan pengkajian administratif, farmasetika, dan klinik periode Januari 2023. Metode pelaksanaan dilakukan secara retrospektif dengan melakukan pengkajian pada beberapa resep. Berdasarkan pengkajian, diperoleh bahwa resep telah memenuhi aspek administratif, farmasetik, dan klinik. Namun, masih ditemukan ketidaklengkapan terkait aspek administratif pada identitas dokter penulis resep.

A pharmacy is a pharmaceutical service facility where pharmaceutical practice is carried out by pharmacists. Reviewing and providing prescription services is the first activity in providing clinical pharmacy services. The purpose of this writing is to obtain an overview regarding the completeness of prescriptions at Apotek Roxy Depok based on administrative, pharmaceutical and clinical studies for the period January 2023. The implementation method is carried out retrospectively by reviewing several prescriptions. Based on the assessment, it was found that the prescription met administrative, pharmaceutical and clinical aspects. However,incompleteness was still found regarding the administrative aspects of the identity of the prescribing doctor."

The use of drugs in large quantities can lead to an increased risk of medication errors (drug interactions and duplication of therapy), non-compliance and side effects of drugs, so it is necessary to study prescriptions, both administrative, pharmaceutical, and clinical to minimize the risk of medication errors due to the administration of polypharmacy drugs. This report aims to conduct a prescription review which includes administrative review, pharmaceutical review, and clinical review of polypharmacy prescriptions that enter the Roxy Depok Pharmacy. The method used is literature study. The results obtained show that the administrative aspects in the form of weight data and the doctor's telephone number need to be completed on both prescriptions and from the pharmaceutical aspect it is necessary to ask the prescribing doctor regarding the dosage form (prescription 1) and drug stability data (recipes 1 and 2). From a clinical aspect, prescription 1 needs to be added to the rules for using medixon and allopurinol, while prescription 2 has the potential for drug interactions."

Temuan menunjukkan bahwa skrining resep, khususnya untuk polifarmasi, sangat penting dalam mencegah masalah terkait obat dan kesalahan pengobatan. Proses skrining membantu menghindari salah tafsir, kegagalan komunikasi, dan kesalahan pengobatan di antara para profesional layanan kesehatan, yang pada akhirnya meningkatkan keselamatan pasien dan kualitas hidup. The aim of the study is to investigate the impact of prescription screening on reducing medication errors and enhancing patient safety in a pharmacy setting.Methodology: The research design involves conducting prescription screening, including administrative, pharmaceutical, and clinical screening, to ensure the legality of prescriptions and minimize treatment errors. The study participants consist of pharmacists working at Apotek Roxy Beji, and the context is within the period of July during a Professional Work Practice of a Pharmacist (PKPA) program. "Polifarmasi merupakan suatu kejadian penggunaan lima obat atau lebih oleh seorang pasien secara bersamaan. Kondisi polifarmasi umumnya ditemukan pada populasi lansia dengan penyakit kronis. Dari beberapa penyakit kronis, hipertensi merupakan penyakit kronis yang paling banyak ditemukan di seluruh fasilitas pelayanan kesehatan. Hipertensi adalah sindrom atau kumpulan gejala kardiovaskuler yang progresif dan ditandai dengan adanya peningkatan tekanan darah diatas 140/90 mmHg. Polifarmasi yang tepat membutuhkan peresepan obat secara tepat dan cermat, sehingga pasien dapat menunjukkan outcome klinis yang baik. Oleh karena itu, diperlukan sebuah proses identifikasi resep polifarmasi melalui kegiatan pengkajian resep di fasilitas pelayanan kesehatan. Pengkajian resep merupakan salah satu bentuk pelayanan kefarmasian di apotek. Kegiatan pengkajian resep meliputi kajian administratif, kesesuaian farmasetik dan pertimbangan klinis. Pengkajian atau skrining resep merupakan salah satu kegiatan yang bertujuan untuk mengidentifikasi, mencegah, dan mengatasi kesalahan pengobatan (medication error) atau Drug Related Problem (DRP). Adapun tujuan dari penelitian ini adalah untuk mengidentifikasi dan mengkaji resep polifarmasi pada pasien dengan di Apotek Roxy Poltangan September 2023. Metode yang digunakan dalam penelitian ini adalah dengan melakukan observasi dan analisa resep polifarmasi yang didalamnya tercantum obat hipertensi. Hasil penelitian ini menunjukkan bahwa secara aspek pengkajian administratif, farmasetik, dan klinis pada resep sudah cukup baik akan tetapi ditemukan DRP pada aspek keamanan terapi dikarenakan terdapat kombinasi pengobatan yang kurang tepat yakni simvastatin dan amlodipin.

Hypertension is a syndrome or a progressive cardiovascular condition characterized by elevated blood pressure above 140/90 mmHg. Appropriate polypharmacy requires precise and careful medication prescription so that patients can achieve good clinical outcomes. Therefore, a process of identifying polypharmacy prescriptions through prescription assessment activities in healthcare facilities is necessary. Prescription assessment is a form of pharmaceutical service in pharmacies. The activities involved in prescription assessment include administrative review, pharmaceutical suitability, and clinical considerations. Prescription assessment or screening aims to identify, prevent, and address medication errors (medication errors) or Drug Related Problems (DRP). The purpose of this study is to identify and assess polypharmacy prescriptions in patients with hypertension at Roxy Poltangan Pharmacy in September 2023. The method used in this study involves observation and analysis of polypharmacy prescriptions that include hypertension medications. The results of this study indicate that in terms of administrative, pharmaceutical, and clinical aspects, the prescriptions were quite good; however, a DRP was found in the therapeutic safety aspect due to an inappropriate drug combination of simvastatin and amlodipine."

"Apotek merupakan salah satu sarana pelayanan kefarmasian bagi apoteker dan tenaga kefarmasian lain. Pelayanan yang dimaksud berupa pelayanan langsung dan bertanggung jawab kepada pasien terkait sediaan farmasi yang bertujuan untuk mencapai hasil yang pasti dalam meningkatkan mutu kehidupan pasien (Kemenkes RI, 2017). Tujuan penulisan tugas khusus ini adalah agar penluis dapat memahami pelaksanaan pengkajian resep yang dilaksanakan di Apotek Roxy Pitara sesuai dengan standar yang telah ditetapkan oleh pemerintah yaitu Peraturan Menteri Kesehatan Republik Indonesia Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek Berdasarkan analisis di atas dapat disimpulkan bahwa resep pertama dan kedua tidak lengkap dalam aspek administratif. Ditemukan juga interaksi obat pada resep pertama yaitu peningkatan risiko hipoglikemia pada penggunaan Diamicron DR dan Trajenta dan pada resep kedua yaitu peningkatan risiko ulkus peptik dan pendarahan pada penggunaan Pirofel dan Prednisone secara bersamaan.

The service in question is in the form of direct and responsible service to patients related to pharmaceutical preparations which aim to achieve definite results in improving the quality of life of patients (RI Ministry of Health, 2017). The purpose of writing this special assignment is so that the writer can understand the implementation of the prescription review carried out at the Roxy Pitara Pharmacy in accordance with the standards set by the government, namely the Regulation of the Minister of Health of the Republic of Indonesia Number 73 of 2016 concerning Pharmaceutical Service Standards at Pharmacies. Based on the analysis above, it can be concluded that the first and second prescriptions were incomplete in the administrative aspect. Drug interactions were also found in the first prescription, namely an increased risk of hypoglycemia when using Diamicron DR and Trajenta and in the second prescription, an increased risk of peptic ulcer and bleeding when using Pirofel and Prednisone together."
