"Apotek merupakan suatu sarana pelayanan kefarmasian tempat dilakukannya praktek kefarmasian oleh Apoteker. Pelayanan kefarmasian di Apotek terbagi kedalam dua jenis yaitu pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai serta pelayanan farmasi klinis. Mengingat banyaknya ketidaksesuaian antara resep yang diberikan oleh dokter kepada pasien, maka tugas khusus ini dilakukan untuk melakukan observasi terkait pelayanan farmasi klinis di Apotek Biak khususnya terkait ketepatan pelayanan resep serta kesesuaian data dan kelengkapan resep yang diterima di Apotek Roxy sesuai dengan ketentuan Standar Pelayanan Kefarmasian yang berlaku. Hal ini diharapkan dapat mencegah terjadinya kesalahan pemberian obat (medication error). Pembuatan laporan tugas khusus Praktik Kerja Profesi Apoteker (PKPA) di Apotek Roxy Biak diawali dengan mengumpulkan beberapa data berupa sampel resep bulan Februari 2023 dan dilakukan analisis resep berdasarkan telaah aspek administratif, farmasetik, dan klinis. Berdasarkan hasil kegiatan yang telah dilakukan, pelayanan farmasi klinis yang dilakukan di Apotek Roxy termasuk pelayanan dan pengkajian resep serta dispensing obat yang dilakukan sudah tepat dan sesuai dengan Standar Pelayanan Kefarmasian di Apotek, serta Resep obat yang diterima dan dilayani di Apotek Roxy biak sudah sesuai dengan kelengkapan data yang tepat berdasarkan pengkajian administrative, farmasetika, dan klinis.

"Pelayanan farmasi klinik merupakan bagian penting dari komitmen apotek untuk memberikan pelayanan prima. Pengkajian resep merupakan bagian penting dalam pelayanan farmasi klinik untuk menentukan masalah terkait obat dalam resep seperti obat antihipertensi. Tujuan dari pengkajian ini adalah menilai kerasionalan resep dalam aspek administratif, farmasetik, dan  klinis pada resep obat antihipertensi yang dilayani oleh Apotek Roxy Biak selama bulan April 2023. Dari seluruh resep yang masuk, dipilih dua resep untuk dikaji aspek administratif, farmasetik, dan klinisnya. Berdasarkan pengkajian dari aspek farmasetik, kedua resep tidak ada penyimpangan, namun dari aspek administratif  tidak terdapat berat badan pasien dan paraf pembuat resep dan dari aspek klinis terdapat adanya duplikasi dan interaksi antar obat yang perlu dimonitor. Hal tersebut perlu dikonfirmasi kembali untuk mencegah terjadinya kesalahan pemberian obat.

Based on the assessment from the pharmaceutical aspect, there are no irregularities in the two prescriptions, but from the administrative aspect there is no patient weight and prescription-making paraphrasing and from the clinical aspect there is duplication and interaction between drugs that need to be monitored. This needs to be reconfirmed to prevent the occurrence of medication errors."

"Kegiatan pengkajian resep meliputi kajian administrasi, kesesuaian farmasetik, dan pertimbangan klinis. Jika ditemukan adanya ketidaksesuaian dari hasil pengkajian apoteker harus menghubungi dokter penulis resep. Pada setiap tahap alur pelayanan Resep dilakukan upaya pencegahan terjadinya kesalahan pemberian obat (medication error). Penyusunan tugas khusus Praktik Kerja Profesi Apoteker di Apotek Roxy Sawangan bertujuan untuk mengidentifikasi kelengkapan resep obat terapi gangguan pencernaan berdasarkan aspek administratif, aspek farmasetik dan aspek klinis yang terdapat pada Apotek Roxy Sawangan periode April 2023.Tugas khusus ini disusun dengan metode deskriptif dan melakukan kegiatan pengkajian resep pada dua resep pasien di Apotek Roxy Sawangan. Obat yang terdapat pada kedua resep tersebut memiliki indikasi yang beririsan yaitu sebagai terapi gangguan pencernaan. Berdasarkan pengkajian resep yang dilakukan terhadap kedua resep dapat disimpulkan pada resep 1 aspek administratif dan farmasetik sudah lengkap tetapi pada aspek klinis menunjukkan ketidaktepatan dosis pemberian sirup sukralfat. Selanjutnya pada resep 2, aspek administratif kurang lengkap karena tidak ada alamat , paraf dokter dan berat badan pasien pada aspek administratif dan farmasetik sudah lengkap dan sesuai.

Tujuan dari observasi ini adalah untuk mengetahui kesesuaian pelayanan kefarmasian di Apotek Kimia Farma 007 Djuanda Bogor dengan standar pelayanan kefarmasian di Apotek, serta melakukan pengkajian resep pasien diabetes melitus Apotek Kimia Farma 007 Djuanda. Pengkajian resep dilakukan berdasarkan kelengkapan pada aspek administratif, farmasetik dan klinis, kemudian ditentukan apakah pemberian resep tersebut telah memenuhi persyaratan aspek atau belum. Berdasarkan hasil observasi dapat disimpulkan bahwa pelayanan kefarmasian yang diterapkan di Apotek Kimia Farma 007 Djuanda meliputi pengkajian resep, dispensing obat, PIO, konseling, pelayanan apotek digital sebagai pelayanan home pharmacy care, PTO, dan MESO serta hasil pengkajian resep terdapat belum memenuhi kelengkapan pada aspek administrasi dan aspek klinis berupa data diri pasien, alergi pasien, cara pemberian obat, duplikasi obat, dan interaksi obat.

Prescription review is carried out based on completeness in administrative, pharmaceutical and clinical aspects, then it is determined whether the prescription has fulfilled the required aspects or not. Based on the results of observations, it can be concluded that the pharmaceutical services applied at the Kimia Farma 007 Djuanda Pharmacy include prescription review, drug dispensing, PIO, counseling, digital pharmacy services as home pharmacy care services, PTO, and MESO and the results of the prescription review do not yet complete of the administration and clinical aspects in the form of patient personal data, patient allergies, how to administer drugs, drug duplication, and drug interactions."

"Pelayanan kefarmasian di apotek merupakan pelayanan yang bersentuhan langsung dengan masyarakat, sehingga dituntut untuk mampu menjamin ketersediaan obat yang aman, bermutu, dan berkhasiat. Diabetes mellitus (DM) merupakan gangguan metabolisme kronis yang ditandai dengan terjadinya hiperglikemia persisten. Terapi pasien DM tipe dua bertujuan untuk mempertahankan tingkat konsentrasi glukosa dalam darah dalam batas normal, dan untuk mencegah berkembangnya komplikasi jangka panjang dari kondisi diabetes. Penulisan Tugas Khusus ini dilakukan agar penulis lebih memahami secara mendalam terkait analisis dan pengkajian resep di apotek, khususnya resep pasien diabetes. Metode pelaksanaan analisis resep dilakukan dengan pendekatan pengkajian, pelayanan, dan screening resep terhadap resep pasien diabetes di KFA Matraman sesuai dengan yang tertera pada buku Petunjuk Teknis Standar Pelayanan Kefarmasian di Apotek. Kesimpulan yang didapat dari analisis resep pasien diabetes kali ini yaitu secara umum seringkali terdapat kombinasi pengunaan obat pada pasien (polifarmasi), sehingga potensi terjadi interaksi obat pada pasien dapat meningkat. Menyikapi hal ini, apoteker perlu memastikan setiap kombinasi terapi yang diterima pasien merupakan kombinasi terapi yang rasional, sehingga efektif untuk menunjang kesembuhan pasien.

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by persistent hyperglycemia. Therapy for type 2 DM patients aims to maintain blood glucose concentration levels within normal limits, and to prevent the development of long-term complications from diabetes. This Special Assignment was written so that the author could get better understanding in the analysis and assessment of prescriptions in pharmacies, especially prescriptions for diabetes patients. The analysis of diabetes prescription in this assignment was carried out using the approach of assessment, service, and screening of prescriptions for diabetes patients at KFA Matraman in accordance with that stated in the Technical Instructions for Standard Pharmaceutical Services in Pharmacies. The conclusion obtained from the analysis of diabetes patient prescriptions this time is that in general there is often a combination of drug use in patients (polypharmacy), so that the potential for drug interactions in patients can increase. In response to this, pharmacists need to ensure that every combination of therapy received by patients is a rational combination of therapy, so that it could be effective in supporting patient recovery."

"Peresepan obat yang ada di rumah sakit harus sesuai dengan Fornas dan Formularium Rumah Sakit (FRS) dengan target 100% sesuai dengan FRS dan memenuhi standar minimal kesesuaian peresepan sebesar ≥80%. Kesesuaian peresepan berdasarkan formularium dapat meningkatkan kualitas layanan dan ketersediaan obat bagi pasien serta dapat meningkatkan mutu pelayanan kesehatan melalui peningkatan efektivitas dan efisiensi pengobatan sehingga tercapai penggunaan obat yang rasional. Berdasarkan hasil analisis di RSUI, terdapat 189 jumlah item obat yang diresepkan dari 50 resep pasien selama periode September-Oktober 2023 dengan rincian 171 item obat sesuai dengan Fornas dan 18 item obat tidak sesuai dengan Fornas. Oleh karena itu, persentase kesesuaian peresepan obat pasien BPJS dengan Fornas sudah melampaui standar indikator mutu pelayanan kesehatan RS karena menunjukkan kesesuaian item obat yang diresepkan terhadap Fornas 2021 sebesar 90,48%. Diharapkan instalasi farmasi RSUI dapat memonitoring secara berkala minimal satu bulan sekali terkait peresepan obat di seluruh departemen di RS serta mengingatkan kepada dokter untuk meresepkan obat sesuai dengan daftar obat yang ada di Fornas. Selain itu, peran apoteker sangat diperlukan untuk meningkatkan pelayanan kefarmasian di RSUI dengan aktif memberikan edukasi serta pengenalan kepada pasien maupun dokter terkait obat-obatan yang terdapat dalam Fornas.

Errors in prescription writing are common and can lead to medication errors, which can be fatal for patients. Screening prescriptions, especially for polypharmacy, is essential to prevent drug-related problems and medication errors.Research Problem and Aim: The research problem focuses on the importance of screening prescriptions, particularly for polypharmacy, to prevent medication errors and improve patient outcomes. The aim of the study is to investigate the impact of prescription screening on reducing medication errors and enhancing patient safety in a pharmacy setting.Methodology: The research design involves conducting prescription screening, including administrative, pharmaceutical, and clinical screening, to ensure the legality of prescriptions and minimize treatment errors. The study participants consist of pharmacists working at Apotek Roxy Beji, and the context is within the period of July during a Professional Work Practice of a Pharmacist (PKPA) program. The findings indicate that prescription screening, especially for polypharmacy, is essential in preventing drug-related problems and medication errors. The screening process helps in avoiding misinterpretations, communication failures, and medication errors between healthcare professionals, ultimately improving patient safety and quality of life. The study highlights the significance of prescription screening in reducing medication errors and enhancing patient safety. Implementing rigorous screening protocols in pharmacies can help prevent drug-related problems and improve treatment outcomes for patients."

"Terapi Sulih Hormon (HRT) estrogen dan progesteron digunakan untuk mengatasi gejala menopause dan untuk pencegahan osteoporosis. Terapi ini bermanfaat signifikan jika digunakan pada wanita dengan gejala dalam 10 tahun pertama setelah menopause atau pada usia di bawah 60 tahun. Penggunaannya dalam dosis tinggi dan jangka panjang dapat menimbulkan risiko kanker endometrium dan payudara. Apoteker dalam farmasi klinik bertanggung jawab memastikan resep hormon diatur dengan tepat untuk mencegah kesalahan pengobatan dengan melakukan pengkajian resep. Pengkajian resep dilakukan pada resep hormon dan kontrasepsi pasien yang masuk di Apotek Roxy Sawangan pada periode April 2023 dengan menelaah aspek administratif, aspek farmasetik dan aspek klinis. Kajian aspek administratif  pada resep 1: tidak ada SIP, paraf dokter dan berat badan pasien; dan pada resep 2 : tidak ada alamat dan berat badan pasien. Analisis farmasetik pada resep 1 menunjukkan kekeliruan penulisan cara pemberian untuk terapi cyclo progynova dan tidak ditemukan penyimpangan pada resep 2. Pada pengkajian aspek klinis resep 1 ditemukan ketidaksesuaian aturan pakai pada resep 1 dan cara pemakaian belum lengkap pada resep 2. Pengkajian lebih lanjut terhadap resep-resep hormon untuk indikasi lainnya perlu dilakukan untuk pencegahan masalah terkait obat dan kesalah pengobatan yang lebih baik.

Hormone Replacement Therapy (HRT) estrogen and progesterone is used to treat menopausal symptoms and for the prevention of osteoporosis. This therapy is of significant benefit if used in women with symptoms in the first 10 years after menopause or under the age of 60 years. Its use in high doses and long term can increase the risk of endometrial and breast cancer. Pharmacists in clinical pharmacy are responsible for ensuring hormone prescriptions are managed appropriately to prevent medication errors by reviewing prescriptions. Prescription reviews were carried out on hormone and contraceptive prescriptions for patients admitted to the Roxy Sawangan Pharmacy in the April 2023 period by examining administrative aspects, pharmaceutical aspects and clinical aspects. Study of administrative aspects of prescription 1: there is no SIP, doctor's initials and patient's weight; and in prescription 2: no patient's address and weight. Pharmaceutical analysis on prescription 1 showed an error in writing the administration method for cyclo progynova therapy and no irregularities were found in prescription 2. In assessing the clinical aspects of prescription 1, it was found that the instructions for use in prescription 1 were inconsistent and the instructions for use were incomplete in prescription 2.
