"Praktek kerja profesi di RSPAD Gatot Soebroto bertujuan untuk memahami peran dan tanggung jawab Apoteker di Rumah Sakit. Kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan BMHP serta pelayanan farmasi klinik di RSPAD Gatot Soebroto telah sesuai dengan Permenkes No. 58 Tahun 2014 tentang Standar Pelayanan Kefarmasian di Rumah Sakit. Sedangkan tugas khusus yang dilakukan bertujuan untuk mengetahui peranan apoteker pada kegiatan pemantauan terapi obat serta dalam mengatasi masalah terkait penggunaan obat pada pasien skizofrenia.

"Pemantauan Terapi Obat (PTO) adalah salah satu kegiatan Farmasi klinis yang merupakan peran apoteker di Rumah Sakit menurut PMK no.72 tahun 2016. Peran fundamental apoteker adalah mengidentifikasi ROTD yang potensial maupun aktual, memecahkan masalah ROTD aktual, dan mencegah ROTD yang potensial terjadi. Berdasarkan Global Status Report on NCD World Health Organization (WHO) pada tahun 2010, Diabetes Melitus menduduki peringkat ke-6 penyakit yang menyebabkan kematian. Peningkatan insidensi penyakit DM diiringi dengan peningkatan komplikasi dan penyakit penyerta seperti komplikasi neuropati (63,5%), retinopati (42%), nefropati (7,3%), makrovaskuler (16%), mikrovaskuler (6%), luka kaki diabetik (15%) (Purwanti, 2013). Pengamatan pada Pasien Rawat Inap dengan diagnosa Diabetes Melitus disertai dengan penyerta Ulcer DM perlu dilakukan untuk memantau terapi obat yang diterima oleh pasien tetap sesuai dengan ketentuan yang berlaku dan mencegah terjadinya ROTD akibat komplikasi dari pemakaian obat yang banyak. Pemantauan terapi obat dilakukan dengan metode PCNE untuk memastikan akar permasalahan (cause) yang menyebabkan masalah (problem). Berdasarkan pengamatan yang dilakukan, ditemukan empat permasalahan yang selanjutnya diselesaikan dengan pemberian rekomendasi kepada dokter terkait pemilihan obat.

Medication Therapy Monitoring (MTM) is one of the clinical pharmacy activities that falls under the role of pharmacists in hospitals, as stated in Ministerial Regulation No. 72 of 2016. The fundamental role of pharmacists is to identify potential and actual Drug-Related Problems (DRPs), resolve actual DRPs, and prevent potential DRPs from occurring. According to the World Health Organization's (WHO) Global Status Report on Non-Communicable Diseases (NCD) in 2010, Diabetes Mellitus ranked 6th among the diseases causing death. The increasing incidence of diabetes is accompanied by an increase in complications and comorbidities such as neuropathy (63.5%), retinopathy (42%), nephropathy (7.3%), macrovascular (16%), microvascular (6%), and diabetic foot ulcers (15%) (Purwanti, 2013). Observations on hospitalized patients diagnosed with Diabetes Mellitus, accompanied by Diabetic Ulcers, need to be conducted to monitor the medication therapy received by patients in accordance with applicable regulations and to prevent DRPs resulting from the use of multiple medications. Medication therapy monitoring is performed using the PCNE method to ensure the identification of the root cause that leads to a problem. Based on the observations conducted, four problems were identified and subsequently addressed by providing recommendations to the relevant doctor regarding medication selection."

"Praktek kerja profesi di Rumah Sakit Pusat Angkatan Darat RSPAD Gatot Soebroto dilaksanakan selama dua bulan mulai dari tanggal 2 September 2016 sampai dengan tanggal 27 Oktober 2016. Praktek kerja profesi ini bertujuan agar mahasiswa profesi apoteker memahami tugas dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian di rumah sakit serta mendapatkan pengetahuan dan pengalaman nyata untuk melakukan praktek kefarmasian di rumah sakit. Tugas khusus yang diberikan berjudul Identifikasi Drug Related Problem pada Pasien dengan Penurunan Kesadaran pada Post VP Shunt atas Indikasi SOL dan Hidrosefalus, Diabetes Mellitus Tipe 2 serta Bronkhitis di Unit ICU RSPAD Gatot Soebroto. Tujuan penyusunan tugas khusus ini adalah untuk memonitoring, mengidentifikasi, menganalisa, dan memberikan rekomendasi solusi masalah terkait obat pada pasien. Tugas khusus ini juga bertujuan agar mahasiswa profesi apoteker mampu berkomunikasi dengan pasien, keluarga pasien, maupun tenaga kesehatan lain serta memahami tugas dan peranan farmasi klinis di ruang perawatan Intensive Care Unit ICU.

Profession internship at Gatot Soebroto Army Central Hospital RSPAD was held for two months started from September, 2nd 2016 until October, 27th 2016. This profession internship was intended to make apothecary students understand roles and responsibilities of pharmacists in pharmaceutical practice at hospital and also to obtain knowledge and practical experience to do pharmaceutical practice at hospital. The internship given a special assignment titled Identification of Drug Related Problem in Patient with Decreased Consciousness on Post VP Shunt on indications of SOL and Hydrocephalus, Diabetes Mellitus Type 2 and Bronchitis at RSPAD Gatot Soebroto ICU unit. The purpose of the special assignment was to monitor, identify, analyze drug related problem in patient and to give recommendations solution. This special assignment was also intended to make apothecary students to be able to communicate with patient, patient rsquo s family, and other health professionals as well as understand the role of clinical pharmacists in Intensive Care Unit ICU ."

The aims of internship at Presidential Hospital RSPAD Gatot Soebroto Ditkesad period July September 2016 are to understand the roles, duties and responsibilities of pharmacist in hospital in accordance with the provisions of laws and ethics of pharmaceutical care in particular and healthcare in general to have insights, knowledges, skills, and practical experience for doing pharmacy practice in the hospital and to realize the problems in pharmacy practice as well as studying the strategies and activities can be done to develop pharmacy practice in the hospital. "Apoteker sebagai salah satu tenaga kesehatan yang profesional memiliki peranan yang sangat dibutuhkan di berbagai sektor, antara lain industri farmasi, distribusi dan pelayanan. Apoteker memastikan mutu obat yang dihasilkan sesuai dengan persyaratan dan tujuan penggunaan. Tugas seorang Apoteker di distribusi adalah bertanggung jawab terhadap pelaksanaan ketentuan pengadaan, penyimpanan dan penyaluran obat atau bahan obat sesuai dengan CDOB. Apoteker harus menjamin aspek keamanan, pencegahan masuknya obat dan/atau bahan obat palsu ke dalam rantai distribusi, menjamin penyimpanan obat aman dan sesuai, termasuk selama transportasi. Apoteker juga memiliki peranan penting di tempat pelayanan kesehatan seperti di Apotek, Rumah Sakit dan Puskesmas. Pelayanan kesehatan sangat diperlukan bagi masyarakat untuk meningkatkan derajat kesehatan demi kualitas hidup masyarakat yang lebih baik. Pelayanan kefarmasian meliputi dua kegiatan, yaitu kegiatan yang bersifat manajerial berupa pengelolaan Sediaan Farmasi, Alat Kesehatan, Bahan Medis Habis Pakai dan kegiatan pelayanan farmasi klinik. Apoteker menjamin seluruh rangkaian kegiatan perbekalan Sediaan Farmasi, Alat Kesehatan, dan BMHP sesuai dengan ketentuan yang berlaku. Pelayanan farmasi klinik diberikan Apoteker kepada pasien untuk tujuan keselamatan pasien sehingga kualitas hidup pasien terjamin. Dalam upaya membekali mahasiswa calon apoteker dengan pengetahuan dan kemampuan tentang peran, fungsi dan tanggung jawab apoteker di industri farmasi, distribusi, Rumah Sakit, apotek dan puskesmas, Program Studi Apoteker Universitas Indonesia bekerja sama dengan instansi terkait dalam penyelenggaraan Praktik Kerja Profesi Apoteker (PKPA) bagi calon apoteker periode Maret-Oktober 2022. Setelah pelaksanaan PKPA diharapkan mahasiswa memiliki wawasan, keterampilan dan pengalaman untuk melakukan pekerjaan kefarmasian sehingga siap memasuki dunia kerja sebagai tenaga farmasi yang profesional.

Pharmacists as professional health workers have a role that is needed in various sectors, including the pharmaceutical industry, pharmaceutical supplier and public health center. Pharmacists in the pharmaceutical industry ensure that the quality of the drugs produced is in accordance with the requirements and intended use. The duty of a pharmacist in pharmaceutical supplier is to be responsible for implementing the provisions for the procurement, storage and distribution of drugs or medicinal ingredients in accordance with the CDOB. Pharmacists must guarantee security aspects, prevent the entry of counterfeit drugs and/or medicinal ingredients into the distribution chain, ensure safe and appropriate storage of drugs, including during transportation. Pharmacists also have an important role in health care settings such as pharmacies, hospitals and health centers. Health services are needed for the community to improve health status for a better quality of life for the community. Pharmaceutical services include two activities, namely managerial activities in the form of management of Pharmaceutical Preparations, Medical Devices, Medical Consumables and activities of clinical pharmacy services. Pharmacists guarantee that the entire series of activities for the supply of Pharmaceuticals, Medical Devices, and BMHP are in accordance with applicable regulations. Pharmacists provide clinical pharmacy services to patients for the purpose of patient safety so that the patient's quality of life is guaranteed. In an effort to equip prospective pharmacist students with knowledge and skills about the roles, functions and responsibilities of pharmacists in the pharmaceutical industry, pharmaceutical supplier, hospitals, pharmacies and health centers, the University of Indonesia Pharmacist Study Program works closely with relevant agencies in implementing internship for prospective pharmacists for the period March- October 2022. After the implementation of internship, students are expected to have the insight, skills and experience to do pharmaceutical work so that they are ready to enter the world of work as professional pharmacists."
