"Produksi kemasan plastik yang kualitas sangat penting bagi perusahaan untuk mendapatkan keuntungan jika produk yang dihasilkan sesuai dengan standar yang sudah ditentukan. Penelitian ini bertujuan untuk mengurangi kerugian yang terjadi karena adanya produk cacat pada produk kemasan Agar-agar. Metode Six Sigma melalui tahapan DMAIC (Define, Measure, Analyze, Improve, dan Control) dipakai untuk mencari solusi dan memperbaiki kualitas produk. Berdasarkan perhitungan terhadap data produk cacat, diketahui bahwa cacat Tonjolan Pinggir, cacat Keriput, dan cacat Pitch Unstd merupakan tiga jenis cacat dengan jumlah dan persentase tertinggi. Pada tahap measure didapatkan nilai DPMO (Defects Per Million Opportunities) sebesar 32707,271 unit defect dengan nilai sigma sebesar 3,34. Failure Mode and Effect Analysis (FMEA) digunakan untuk mengetahui risiko potensi kegagalan dari faktor yang mendapat bobot nilai tinggi.
Setelah dilakukan perbaikan, nilai DPMO turun menjadi 7272,111 unit defect dan nilai sigma naik menjadi 3,94 sigma. Perubahan nilai sigma yang terjadi sebesar 0,60. Tahap control dapat dilaksanakan dengan adanya rancangan perbaikan serta sebuah pemeriksaan setiap prosesnya agar jumlah cacat produk terus berkurang. Rancangan perbaikan pendekatan 5S berupa standar kerja check sheet maintenance pada Mesin Tandem Laminasi dan standar kerja work station di proses laminasi. Setelah dilakukan perbaikan, hasil menunjukan penurunan jumlah dan pesentase defect, dimana sebelumnya mencapai angka sebesar 5,89% menuju 1,31%.

The production of quality plastic packaging is very important for companies to gain benefits that can be obtained if the quality of the product complies with the prescribed standards. This study aims to reduce losses that occur due to the presence of defective products in Jelly packaging products. Six Sigma methods through DMAIC stages (Define, Measure, Analyze, Improve, and Control) are used to find solutions and improve product quality. Based on the calculation of defective product data, it is known that Uneven surface defects, wrinkled defects, and Pitch Unstd defects are the highest number of defects. In the measure phase, the DPMO (Defects Per Million Opportunities) value were 32707.271 defect units with a sigma value of 3.34. Failure Mode and Effect Analysis (FMEA) is used to determine the potential risk of failure from a factor that gets a high value.
After repairs, the DPMO value drops to 7272.111 defect units and the sigma value rises to 3.94 sigma. Changes in sigma values that occur amounted to 0.60. Control can be done with the inspection and checks in each process so that the number of defects can continue to decrease. Improvements use the 5S approach in the form of a work standards check sheet maintenance on the Tandem Lamination Machine and work standard of work station in the lamination process. After improvement, the results show a decrease in the number and percentage of defects, which previously reached a number of 5.89% towards 1.31%.

"Peningkatan Indeks Kualitas Produk Pada Tahap Pengembangan Produk di PT. XYZ Dengan Metode Six Sigma. Tesis ini membahas penggunaan metode six sigma (DMAIC) untuk memperbaiki indeks kualitas pertama pada tahap pengembangan produk Final Engineering Piloting (FEP). Pada fase define diketahui bahwa rata-rata indeks kualitas pertama pada tahap FEP sebesar -336, 27. Pada fase measure ditemukan bahwa rendahnya indeks kualitas diakibatkan oleh permasalahan pada torso dimana gaya untuk melepas dan memasang tangan dari dan ke lower arm berada di luar spesifikasi.
Analisis menggunakan FMEA merekomendasikan agar material lower arm diganti dari PVC 85 menjadi PVC 65. Pergantian ini bisa membawa perubahan lebih baik, dan setelah perbaikan pada fase improvement didapatkan peningkatan indeks kualitas menjadi 5,46.

This paper discuss about the use of six sigma method (DMAIC) to improve first quality index during development product phase called Final Engineering Piloting. During define phase, found that average first QI FEP is -336,27. Measure phase shows that those index mostly contributed by torso problem, in which poundage to attach and detach hands to and from lower arm are out of product specification.
Analysis phase using FMEA, recommends to change material for lower arm from PVC 85 to PVC 65. This changes can improve product and after all the improvement implemented, average first quality index improve to 5,46.

"Semakin ketatnya persaingan antara suatu perusahaan menjadikan kualitas sebagai alat strategi bisnis untuk bersaing terhadapkompetitornya dalam menguasai pasar. Six sigmasebagai salah satu metode untuk melakukan perbaikan kualitas pada masalah peningkatan jumlah defect . PT Phillip Morris Indonesia mengalami penurunan kualitas produk yang dapat dilihat dari Visual Quality Index (VQI) yang semakin meningkat. Hal ini terjadi karena adanya peningkatan jumlah defect pada proses cigarette making dimana Tobbaco Particel Under Tipping merupakan defect yang paling banyak terjadi. Perbaikan yang dilakukan adalah dengan membuat SOP dan sebuah alat bantu agar mesin bisa berjalan optimal.Pada saat sebelum perbaikan nilai sigma yang didapat adalah 3,6 dengan yield sebesar 98,20%. Setelah dilakukan perbaikan didapatkan nilai sigma sebesar 3,76 dengan yield sebesar 98,82%.

The increasing competition between a company makes quality as a strategic business to compete against competitors as a market leader. Six sigma as a method that can be use for quality improvment to solve a increasing defect product. Now, PT. Philip Morris Indonesia has been decrease on visual quality product. We can see on Visual Quality Index (VQI) that remainly increase. This increase is due to cigarette defect in cigarette making process where the defect Tobacco Particles under Tipping is the most contributors in VQI. The improvements that have been done are making a SOP and also make a new design for vacuum machine. At the time before improvement value is 3.6 sigma obtained at a yield of 98.23% and after improvement the value is 3.76 sigma and a yield is 98.82%."

"Penerapan konsep manajemen mutu seperti Lean Six Sigma mulai diterapkan dalam industri jasa seperti pada industri telekomunikasi. Tesis ini dilakukan untuk menganalisis penerapan konsep Lean Six Sigma pada operator telekomunikasi dengan melihat kapabilitas proses BTS melalui kinerja Call Setup Success Rate (CSSR) dan Call Drop Rate (CDR). Parameter tersebut merupakan dua parameter utama jaringan seluler. Penerapan Six Sigma pada proses perbaikan kualitas layanan seluler dilakukan agar kedua parameter tersebut dapat mencapai nilai standar yang telah ditentukan. Kinerja parameter BTS dapat dilihat berdasarkan nilai kinerja proses Ppk yang kemudian dikonversi menjadi nilai sigma. Nilai sigma terbesar yang dapat dicapai setelah dilakukan perbaikan berdasarkan penelitian ini adalah 6σ untuk parameter CSSR area Jakarta Utara. Faktor-faktor yang menentukan keberhasilan implementasi Six Sigma dalam proses perbaikan kualitas jaringan seluler juga dianalisis dan didapatkan bahwa faktor kepemimpinan merupakan faktor utama keberhasilan implementasi Six Sigma menurut para responden. Penelitian ini menggunakan data sekunder yang berupa kinerja BTS dan data primer yang didapatkan dari kuesioner kepada Divisi Optimisasi.

The application of quality management concepts such as Lean Six Sigma was implemented in the service industry such as the telecommunications industry. This thesis is conducted to analyze the application of the concept of Lean Six Sigma in the telecommunications operator to see performance capability of BTS through the performance of Call Setup Success Rate (CSSR) and Call Drop Rate (CDR). These parameters are the two main parameters of the mobile cellular network. Application of Six Sigma in mobile cellular service quality improvement process is done so that these two parameters can achieve a standard value that has been determined before. Performance parameters of BTS can be seen by Ppk process performance values which then converted into a sigma value. The largest sigma value that can be achieved after improvement process by this research is 6σ for CSSR parameter in North Jakarta area. The factors that determine the success of the implementation of Six Sigma in the process of improvement of the quality of mobile networks is also analyzed and found that leadership is a key factor for the successful implementation of Six Sigma according to the respondents. This study uses secondary data such as the performance of the BTS and primary data obtained from the questionnaire to the Division of Optimization."

"Tulisan ini merupakan suatu pentaburan penerapan metodologi Six Sigma dalam upaya perbaikan proses dengan studi kasus proses pemotongan material dengan mesin Eye Tracer di lini persiapan bahan PT. United Tractors Pandu. Pada psrbaikan proses ini dilakukan penggunaan tahapan Six Sigma yang dikenal sebagai DMAIC (Define-Measure-Analyze-Improve-Control), dimana dalam tiap tahapannya digunakan berhagai kombinasi metode ataupun alat (tools) baik yang bersifat kuantitatif maupun kualitatif secara feksibel dan koutekstual. Dengan penggunaan metodologi Six Sigma, dihatapkan akan terjadi perbaikan proses k arah proses tampa cacat (zero defect) atau secara realistis mencapai 3,4 cacat per juta kemuugkinan (DPMO) pada saat suatu proses telah mencapai tingkatan enam sigma (Six Sigma).
Usaha peningkatan kualitas produk ataupun jasa yang ditawarkan kepada konsumen dalam era yang semakin kompelitif merupakan sesuatu yang mutlak perlu untuk dilakukan oleh setiap perusahaan. Demikian pula halnya dengan kuntitas produk yang dihasilkan oleh PT. United Tractors Pandu Produk setengah jadi yang dihasilkan oleh proses pemotongan dengan mesin Eye Tracer pada Lini Persiapan Bahan di perusahaan ini baru mencapai 3 sigma, yang menandakan masih terdapat sekitar 66.800 kejadian cacat dalam satu juta kemungkinan yang ada (DPMO). Banyaknya jumlah kejadian cacat yang ditemukan pads proses pemotongan tersebut, umumnya disebabkan oleh permasalahan metode dan permasalahan permesinan.
Melalui penerapan melodologi Six Sigma, sepedi dalam penelitian ini, diharapkan akan mampu meningkatkan nilai sigma proses pemotongan menjadi 3,5 sigma, yaitu 22-800 kejadian cacat dalam satu juta kCHl H. Peningkatan tersebut dapal diwujudkan melalui pengaplikasian usulan-usulan perbaikan yang berupa pembuatan prosedur standar opcrasi (SOP), perbaikan meja polong, maupun berbagai perbaikan lain yang digabarkan dalam tulisan ini.

This writing will explain in details about the implementation of Six Sigma methodology in order to make an improvement of cutting process by using Eye Tracer cutting machine in material preparation section, PT. United Tractors Pandu. This irnprovement method are using Six Sigma improvement stem which known as DMAIC (Define-Measure-Analyze-Improve-Control). In every those step, there would be a different methods and tools flexibly and eontextually used, which differ into quantitative or qualitative characteristics. By using Six Sigma methodology, we will go to achieve zero defect process or realistically 3,4 defect per million opportunity (DPMO) when the process has gain six sigma level.
All effort to increase a quality of products or services offered to customers was something really need to be done by many companies in this fast growing competitive market, including PT. United Tractors Pandu. As this far, semi finish product produced by Eye Tracer cutting machine which located in material preparation section., still only have 3 sigma in their process capability. This 3 sigma shows that there is a possibility of 66.800 defect was found in a million opportunity (DPMO) of their semi Enish product. The large number of defect found in that cutting process was identiiied mostly caused by cutting method and machinery problem.
By using Six Sigma method in this research, hopefully the company could eliminate those problems, and could increase the sigma capability into 3,5 sigma, which means there would be 22.800 defect occur in a million opportunity- This target of improvement could be realize by implementing the solutions given, like establishing a standard operating procedure (SOP) for cutting process, cutting table changing, and many other solutions proposed.

"Penyediaan produk yang berkualitas merupakan hal penting yang perlu menjadi fokus dan tujuan perusahaan di industri mana pun, termasuk industri produk Fast-Moving Consumer Goods (FMCG). Terlebih lagi, industri FMCG sedang mengalami kenaikan volume penjualan secara global. Namun, sebuah perusahaan FMCG di wilayah Tangerang sedang mengalami masalah mengenai penyediaan produk berkualitas yang memenuhi kepuasan pelanggan. Salah satu produknya, yaitu sabun cuci piring, mengalami peningkatan biaya rework produk cacat secara drastis dibandingkan bulan-bulan sebelumnya. Oleh karena itu, identifikasi akar permasalahan cacat produk diperlukan agar tingkat kecacatan pada proses produksi dapat dikurangi. Penelitian ini menerapkan konsep six sigma dengan mengikuti alur metode Define, Measure, Analyze, Improve, dan Control (DMAIC) untuk membantu mencari akar masalah dan merancang perbaikan proses produksi yang relevan. Hasil penelitian menunjukkan bahwa akar dari masalah yang ada yaitu kurang telitinya operator saat mengoperasikan panel HMI untuk melakukan aktivitas pencampuran bahan baku cairan sabun, kelalaian operator dalam mengikuti alur kerja, dan mesin penunjang aktivitas yang tidak beroperasi dengan benar. Berdasarkan hal tersebut, diberikan rekomendasi berupa modifikasi tampilan panel Human-Machine Interface (HMI), pelatihan operator, serta pengecekan mesin secara berkala. Melalui rekomendasi yang diberikan, diharapkan tingkat produksi barang cacat dapat berkurang.

Delivery of quality products is an important matter that needs to be the focus and goal of companies in any industry, including the Fast-Moving Consumer Goods (FMCG) industry. Especially, FMCG industry is experiencing an increase in sales volume globally. However, an FMCG company in the Tangerang area is experiencing problems regarding providing quality products that meet customer satisfaction. One of its products, namely dishwashing liquid, experienced a drastic increase in the cost of reworking defective products compared to previous months. Therefore, identification of the root causes of product defects is necessary so that the level of defects in the production process can be reduced. This study applies the six sigma concept by following the Define, Measure, Analyze, Improve, and Control (DMAIC) method to help identify root causes and design relevant production process improvements. The results showed that the root of the problem was the operator's uncarefulness when operating the HMI panel to carry out the activity of mixing the raw materials for dishwashing liquid, the operator's negligence in following the workflow, and the supporting machines that did not operate properly. Based on this, recommendations are given in the form of modifying the appearance of the Human-Machine Interface (HMI) panel, operator training, and periodic machine checks. Through the recommendations given, it is expected that the level of production of defective goods can be reduced."

"ABSTRAK
Penerapan manajemen kualitas yang baik dan benar akan meminimalisir cacat pada hasil pekerjaan. Cacat ini akan menimbulkan pekerjaan perbaikan atau pekerjaan ulang, sehingga akan mempengaruhi besar waktu dan biaya. Penelitian ini bertujuan untuk mengetahui kondisi eksisting manajemen mutu pada perusahaan-perusahaan beton pracetak di Indonesia dan mengetahui proses-proses dalam metode Six Sigma yang dapat diimplementasikan pada perusahaan-perusahaan beton pracetak di Indonesia dan pengaruhnya terhadap kinerja waktu produksi. Hasil penelitian menunjukkan bahwa proses yang paling berpengaruh terhadap kinerja waktu adalah manajer produksi telah mengenali kompetensi tim produksi yang dibutuhkan sehingga proses perekrutan menjadi efektif.

ABSTRACT
A well and right application of quality management will minimize defects in the work. Defects will cause the repair work or re-employment, so as to affect the time and cost. This study aims to determine the existing condition of quality management in precast concrete companies in Indonesia and knowing the processes in the Six Sigma method that can be implemented in precast concrete companies in Indonesia and its impact on the performance of production time. The results showed that the process that affect time performance the most is the production manager has recognized the competence of the production team are needed so that the recruitment process to be effective."

"Penelitian ini membahas tentang losses material pada proses pembuatan Open Top Can (OTC). Tujuan dari penelitian ini adalah untuk mengidentifikasi penyebab dan mengurangi losses material. Objek penelitian ini adalah perusahaan yang bergerak dalam bidang makanan dan minuman, sedangkan fokus penelitian pada proses pembuatan OTC yang akan dipakai dalam proses pengalengan produk tersebut. Penelitian ini dianalisis menggunakan metode Six Sigma dengan tahapan Define, Measure, Analyze, Improve, Control (DMAIC). Metode Six Sigma merupakan tahapan peningkatan kualitas untuk mencapai 3,4 kecacatan per satu juta kesempatan. Hasil penelitian ini yaitu 4 penyebab terjadinya losses material yaitu defect OTC, defect lid, defect body can dan juga material sisa. Pengurangan defect OTC sebesar 25.838,50 Defect Per Million Opprtunities (DPMO) dan kenaikan level Sigma sebesar 0,35.

The focus of this research is the material loss in the process of making Open Top Can (OTC). This research aims to identify the cause and to reduce material loss. The object of this research is a company working in food and beverage field, while the focus of the research is the making process of Open Top Can (OTC) that will be used in the canning process of the product. This research is analyzed using Six Sigma method with Define, Measure, Analyze, Improve, Control (DMAIC) phase. Six Sigma methods are step to increase quality to reach 3.4 defects per million opportunities. The research reveals four causes of material loss - defect OTC, defect lid, defect body can and scraps of material. Reduce the defect OTC up to 25,838.50 Defect Per Million Opportunities (DPMO) and increase the Sigma level up to 0.35."
