"Distribusi farmasi merupakan bagian yang sangat esensial dalam pengelolaan akses obat dan alat kesehatan. Obat dan alat kesehatan harus dikelola dengan baik karena merupakan salah satu penunjang bagi pelayanan kesehatan. PBF maupun DAK merupakan fasilitas distribusi atau penyaluran sediaan farmasi yang digunakan untuk mendistribusikan sediaan farmasi maupun alat kesehatan. Obat dan alat kesehatan harus memenuhi persyaratan mutu yang telah ditentukan. Salah satu aspek yang memiliki peran penting dalam menjamin mutu dari obat dan alat kesehatan serta mencegah kemungkinan terjadinya kesalahan yaitu Manajemen Risiko Mutu (MRM). MRM merupakan hal penting dalam proses distribusi obat dan alat kesehatan karena merupakan suatu proses yang terstruktur yang dilakukan untuk menilai, mengendalikan, mengkomunikasikan dan mengkaji resiko terhadap obat dan alat kesehatan yang dilakukan secara proaktif maupun retrospektif. Penelitian ini bertujuan untuk mengkaji proses MRM di PT MJG. Pelaksanaan penelitian ini dilakukan dengan mengobservasi kegiatan MRM di PT MJG. Hasil penelitian menunjukkan proses MRM di PT MJG dilakukan untuk meminimalisasi risiko terhadap keamanan pasien dan kualitas dari produk. Proses MRM dimulai dari mengidentifikasi risiko dengan tingkat resikonya, mengidentifikasi dampak dari setiap risiko, menentukan peringkat kemungkinan dari timbulnya risiko, dan membuat strategi untuk mengurangi risiko yang mungkin timbul.

Pharmaceutical distribution is a very essential part of managing access to medicines and medical devices. Medicines and medical devices must be managed well because they are a support for health services. PBF and DAK are distribution facilities for pharmaceutical preparations that are used to distribute pharmaceutical preparations and medical devices. Medicines and medical devices must meet specified quality requirements. One aspect that has an important role in ensuring the quality of medicines and medical devices and preventing the possibility of errors is Quality Risk Management (QRM). QRM is important in the distribution process of medicines and medical devices because it is a structured process carried out to assess, control, communicate and assess risks to medicines and medical devices which is carried out proactively and retrospectively. This research aims to examine the QRM process at PT MJG. This research was carried out by observing QRM activities at PT MJG. The research results show that the QRM process at PT MJG is carried out to minimize risks to patient safety and product quality. The QRM process starts from identifying risks with their risk level, identifying the impact of each risk, determining the probability rating of the risk occurring, and creating strategies to reduce risks that may arise.

"Keandalan suatu sistem listrik pada suatu operasi kilang sangatlah penting untuk mendukung produksi. Pada kilang Tangguh LNG, peralatan esensial untuk mendukung operasi terhubung pada sistem emergency switchgear. Konfigurasi emergency switchgear saat ini merupakan single bus dan hanya disuplai dari satu sisi (couple normal at one end). Dengan kondisi seperti ini, saat terjadi kegagalan pada bus switchgear atau jika akan dilakukan pemeliharaan, maka mengharuskan sistem dipadamkan. Tetapi hal ini tidak mungkin dilakukan dengan konfigurasi yang ada, dikarenakan kilang akan mengalami shutdown serta kehilangan proses operasi emergency yaitu safe shutdown dan blackstart. Oleh karena itu diperlukan studi untuk menganalisa nilai keandalan dari konfigurasi sistem emergency switchgear yang ada. Tesis ini akan memaparkan analisa keandalan berdasarkan nilai unavailability dari konfigurasi emergency switchgear yang ada dan membandingkan dengan usulan desain yang baru dan kemudian dikaji dari sisi manajemen risiko finansial. Berdasarkan hasil analisa yang dilakukan, diperoleh bahwa dengan konfigurasi sistem emergency switchgear yang baru, waktu pemeliharaan dapat dilakukan tanpa menghentikan operasi kilang. Selain itu saat terjadi gangguan pada bus switchgear, operasi dari safe shutdown dan blackstart dapat tetap berfungsi. Nilai unavailability juga menurun dari 155.93 menit/tahun menjadi 11.6 menit/tahun. Selain itu, berdasarkan analisis manajemen risiko, nilai kerugian finansial juga dapat diturunkan.

The reliability of an electrical system in a refinery operation is very important to support production. At Tangguh LNG plant, the essential equipments to support operations is connected to the emergency switchgear system. The current emergency switchgear configuration is a single bus and is only supplied from one side (normal couple at one end). Under these conditions, when a switchgear bus fails or when maintenance is to be carried out, the system must be shutdown. However, this is not possible with the existing configuration, because the refinery will experience shutdown and lose the emergency operation process, which are safe shutdown and blackstart. Therefore, a study is needed to analyze the reliability value of the existing emergency switchgear system configuration. This thesis will describe a reliability analysis based on the unavailability value of the existing emergency switchgear configuration and compare it with the new design proposal and then be reviewed from the side of financial risk management. Based on the results of the analysis carried out, it was found that with the configuration of the new emergency switchgear system, maintenance time can be carried out without stopping the operation of the refinery. In addition, when there is a disturbance in the switchgear bus, the operation of safe shutdown and blackstart can still function. The unavailability value also decreased from 155.93 minutes/year to 11.6 minutes/year. In addition, based on risk management analysis, the value of financial losses can also be reduced."
