"Isu medication error sendiri telah menjadi perhatian dunia kesehatan semenjak November 1999 setelah Institute of Medicine (IOM) memberikan laporan yang menyatakan bahwa sekitar 44.000 – 98.000 pasien meninggal dalam tiap tahunnya di rumah sakit Amerika karena medication error yang sebetulnya dapat dicegah. Salah satu contoh kasus medication error, yaitu terjadi di proses peresepan (prescribing) karena ketidaklengkapan data resep yang diperlukan. Untuk mengendalikan terjadinya kasus medication error pada proses peresepan, salah satu upayanya adalah dengan melakukan telaah resep. Studi ini bertujuan untuk mengetahui aspek yang harus ditelaah atau dikaji dari suatu resep, menerapkan telaah resep pada saat praktik dan mencegah terjadinya medication error. Dari hasil telaah resep obat disuria di Apotek Atrika diketahui bahwa pada aspek administratif, dalam resep yang ditelaah masih terdapat informasi yang tidak lengkap, yaitu SIP dokter, umur pasien, berat badan pasien, tinggi badan pasien dan alamat pasien. Kemudian pada aspek farmasetik, terdapat obat yang dalam resep tidak ditulis kekuatan dan bentuk sediaannya sehingga dapat menimbulkan ambigu atau ketidakjelasan, dan terakhir pada aspek klinis, ditemukan masalah dalam hal duplikasi (resep 1), yang dimana obatnya berbeda namun manfaat dari masing – masing sediaan obat sejenis.

Then in the pharmaceutical aspect, there are drugs whose strength and dosage form are not written in the prescription which can lead to ambiguity, and finally, in the clinical aspect, problems are found in terms of duplication (recipe 1), where the drugs are different, but the benefits of each drug were same."

"Apotek merupakan suatu sarana pelayanan kefarmasian tempat dilakukannya praktek kefarmasian oleh Apoteker. Pelayanan kefarmasian di Apotek terbagi kedalam dua jenis yaitu pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai serta pelayanan farmasi klinis. Mengingat banyaknya ketidaksesuaian antara resep yang diberikan oleh dokter kepada pasien, maka tugas khusus ini dilakukan untuk melakukan observasi terkait pelayanan farmasi klinis di Apotek Biak khususnya terkait ketepatan pelayanan resep serta kesesuaian data dan kelengkapan resep yang diterima di Apotek Roxy sesuai dengan ketentuan Standar Pelayanan Kefarmasian yang berlaku. Hal ini diharapkan dapat mencegah terjadinya kesalahan pemberian obat (medication error). Pembuatan laporan tugas khusus Praktik Kerja Profesi Apoteker (PKPA) di Apotek Roxy Biak diawali dengan mengumpulkan beberapa data berupa sampel resep bulan Februari 2023 dan dilakukan analisis resep berdasarkan telaah aspek administratif, farmasetik, dan klinis. Berdasarkan hasil kegiatan yang telah dilakukan, pelayanan farmasi klinis yang dilakukan di Apotek Roxy termasuk pelayanan dan pengkajian resep serta dispensing obat yang dilakukan sudah tepat dan sesuai dengan Standar Pelayanan Kefarmasian di Apotek, serta Resep obat yang diterima dan dilayani di Apotek Roxy biak sudah sesuai dengan kelengkapan data yang tepat berdasarkan pengkajian administrative, farmasetika, dan klinis.

"Seorang apoteker memiliki peran krusial dalam menjaga keselamatan pasien dengan memastikan pengelolaan maupun penggunaan obat yang aman. Adapun,  kesalahan pengobatan yang dapat membahayakan keselamatan pasien dapat terjadi  di berbagai tahap pelayanan kefarmasian, baik pada proses penulisan resep,  pembacaan resep, peracikan resep, maupun saat penggunaan obat. Sehingga, untuk menghindari terjadinya kesalahan pengobatan,  maka peran seorang apoteker sangat penting dalam melakukan pengkajian resep yang merupakan bagian dari pelayanan kefarmasian. Kegiatan pengkajian ini setidaknya mencakup tiga aspek yakni aspek  administrasi,  farmasetik serta klinis. Pada tugas khusus Praktek Kerja Profesi Apoteker (PKPA) di apotek Atrika, dilakukan pengkajian dua resep obat jantung. Adapun, dari telaah dan pengkajian resep yang telah dilakukan, diketahui bahwa dua resep obat jantung tersebut tergolong resep yang baik dari segi administrasi, farmasetis serta klinis.

"Pengkajian resep termasuk kedalam bagian dari kegiatan farmasi klinis di Apotek yang merupakan bagian dari Pelayanan Kefarmasian yang langsung dan bertanggung jawab kepada pasien berkaitan dengan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai dengan maksud mencapai hasil yang pasti untuk meningkatkan kualitas hidup pasien. Tujuan penulisan tugas khusus praktik kerja profesi apoteker ini yaitu pengkajian resep obat untuk dua resep antibiotik di apotek Roxy Biak pada bulan juli 2022 dari segi kajian administratif, klinis dan farmasetik. Hasil yang didapat yaitu obat yang ditebus pada Apotik Roxy Biak masih memiliki kekurangan pada kelengkapan kajian administratif, yaitu Berat Badan (BB), umur pasien, dan alamat pasien, paraf dan nomor telepon dokter. Dari segi kajian farmasetik dan klinis, resep antibiotik yang ditebus di Apotek Roxy Biak telah memenuhi persyaratan.

Prescription of antibiotics in Indonesia which is quite high and not wise will increase the incidence of resistance. Based on the description of the high prescribing of antibiotics, it is necessary to study the administrative and pharmaceutical screening of antibiotic prescriptions to prevent medication errors that can harm patients at the Roxy Biak Pharmacy. The purpose of writing this special assignment for the work practice of the pharmacist profession is to review drug prescriptions for two antibiotic prescriptions at the Roxy Biak pharmacy in July 2022 in terms of administrative, clinical and pharmaceutical studies. The results obtained were that the drugs that were redeemed at the Roxy Biak Pharmacy still had deficiencies in the completeness of administrative studies, namely body weight (BB), patient's age, and patient's address, initials and doctor's telephone number. The author analyzes the problem of prescription completeness including the completeness of administrative aspects, clinical aspects, and pharmaceutical aspects. Recipes were reviewed using references to the Indonesian Pharmacopoeia, Edition III, Drug Specialist Info, PIONAS, and MIMS. Melalui tugas khusus ini diketahui pada beberapa resep geriatri, pediatri, narkotika, dan psikotropika secara klinis untuk indikasi, dosis, waktu dan cara pemberian sudah sesuai, namun terdapat beberapa interaksi obat sehingga perlu dilakukan monitoring dan penyesuaian dosis, dan jika terdapat antibiotik pada resep disarankan tidak diracik bersama dengan obat lainnya. Untuk resep polifarmasi sebaiknya diperhatikan kembali karena polifarmasi dapat meningkatkan risiko terjadinya interaksi obat dan mungkin juga terdapat duplikasi obat.

This clinical study needs to be carried out so that the treatment for the patient is accurate so that the patient's therapeutic target can be achieved. Therefore, this research was conducted to obtain a clinical review of prescriptions at the Kimia Farma Siliwangi Pharmacy Cirebon in September 2022, including geriatric, pediatric, narcotics, psychotropic, and polypharmacy prescriptions. Through this special assignment it is known that several geriatric, pediatric, narcotics and psychotropic prescriptions are clinically appropriate for indications, dosage, time and method of administration, but there are several drug interactions so it is necessary to do monitoring and adjust doses, and if there is an antibiotic in the prescription it is recommended not mixed with other drugs. For polypharmacy prescriptions, must be reconsidered because polypharmacy can increase the risk of drug interactions and there may also be drug duplication.

Tujuan dilakukannya pengkajian resep untuk mencegah permasalahan terkait obat sebelum obat diserahkan kepada pasien sehingga tidak terjadi medication error yang berdampak pada kegagalan terapi dan efek obat yang tidak diharapkan yang merugikan pasien. Penelitian ini bertujuan untuk mengetahui kelengkapan administrasi, kesesuaian farmasetik dan kesesuaian klinis pada peresepan di Apotek Kimia Farma Menteng Huis. Dalam penelitian ini, dilakukan pengkajian resep Pasien Rujuk Balik (PRB) pasien asma dengan mengkaji secara administratif, farmasetik dan klinis. Pada persyaratan administratif, didapatkan ketidaklengkapan berat badan pasien dan paraf dokter. Pada pengkajian secara farmasetik, resep yang tertulis sudah sesuai bentuk dan kekuatan sediaannya. Pada pengkajian secara klinis, terdapat duplikasi dan interaksi pada beberapa obat. Dapat disimpulkan bahwa masih banyak ketidaksesuaian dalam penulisan resep yang diamati sehingga harus dikonsultasikan kepada dokter penulis resep.

In this study, the prescription review of Back-Referral Program (PRB) was carried out in asthma patient by examining administrative, pharmaceutical and clinical aspects. In administrative requirements, incomplete patient weight and doctor's initials were obtained. In the pharmaceutical study, the written prescription have appropriate dosage forms and dosage strengths. In clinical studies, there are duplications and interactions with several drugs. It can be concluded that there are still many observed discrepancies in prescription writing, so a doctor who prescribes it must be consulted."
