"Six Sigma is a quality target of -3,4 Defect Per Million Opportunities- that allows quality characteristic being measured by perspective of total? defects compare to the total' opportunities of defect to occur. Six Sigma quality improvement methodology is coiled Six Sigma Improvement Framework which consist of 5 phases DMAIC (Define, Measure, Analyze, Improve, and Control. Sir Sigma focused on customer and oriented to the process which affect the final quality characteristic required on a product. This research analyzed the implementation of Six Sigma in a garment company PT X to reduced the number of appearance defect, and the high number of rework for the clothes product. Quantitative and qualitative data were processed by means of some Six Sigma tools for each phase ofDMA1C_ This research focused on the product with style number 148 824 which has I I critical quality characteristic (CTQ). Metric value acquired : defect per unit (DPDQ = Q603974, and the sigma value = 3, 095. Throughput yield = 39, 1026%."

Six Sigma merupakan konsep peningkatan kualitas yang b_Clff0k\lS kepada pemenuhan kebutuhan kritis pelanggan dengan cara mengurangi tingkat cacat. Pemsahaan-perusahaan kelas dunia menjadikan Six Sigma sebagai suatu standar karena kemampuannya untulc mencapai 3,4 cacat per juta peluang-. Six Sigma melakukan S fasc untuk mencapai tingkat kegagalan nol, Define - Measure -Analyze - Improve - Control (DMAIC).
Pada penelitian ini, peneliti akan mencoba menerapkan konsep Six Sigma melalui 5 fase DMAIC-padaproses bisnis di Departemen Weaving. Penelitian-bertujuan untuk mengurangi tingkat cacat pada kain Grey yang merupakan masalah utama yang sering terjadi pada Departemen ini.
Hasil penelitian yang dilakukan, didapatkan bahwa rata-rata proses di Departemen Weaving menghasilkan nilai sigma sebesar 3,71 dengan tingkat Defect Per Million Opportunity sebesar 13.655._ Hasil ini juga sebanding dengan nil i indeks kapabilitas proses sebesar 1,235. Hasil ini dapat menjadi tolak ukur untuk melakukan perbaikan hingga mencapai perusahaan kelas dunia.

In the present day, quality is the most important element in global business competition. Only company that has high quality business process and the ability to satisfy customer's needs could be compete and stay exist. One of' issue that closely related to quality is Six Sigma.
Six Sigma is a quality improvement concept that focused o_n fulfilrnent of customefs critical expectation by reducing the level of defect. Six Sigma becomes a standard for world class company, because its ability to achieve up to 3,4 non conformity per million opportunity. Six Sigma's goal is zero defect which is achieve by performing 5 phase, Define - Measure -Analyze - lm prove - Control (DMAIC).
In this research, Six Sigma will be implemented at Weaving Department by performing 5 phase of DMAIC Its goal to reduce Grey Fabric's Defect Level which are the main problem and frequently-occurred at this Department.
The results of this research pointed that process held at Weaving Department has an average sigma value of 3,71 and the average Defect Per Million Opportunity of 13.655. This result is equivalent to Process Capability Index of 1,235. Management could consider the results to be a baseline for quality improvement to achieve world class company.

Melalui penerapan Six Sigma, performa proses injection untuk menghasilkan produk yang bebas cacat dapat terukur. Setelah itu dilakukan analisa terhadap sumber variasi dan penentuan solusi potensial untuk memperbaiki performa proses.
Penelitian dibatasi pada produk injection khususnya cacat silver dan short shoot Nilai defect per million opportunity (DPMO) yang dihasilkan sebesar 11910,6 DPMO dan nilai sigma sebesar 3.8. Nilai-nilai ini menggambarkan kemampuan proses injection yang belum cukup tinggi. Dari hasil analisa identifikasi solusi-solusi yang dapat diimplementasikan unluk rneningkatkan performa proses. Implementasi dari solusi-solusi potensial yang teridentifikasi merupakan upaya unluk menghilangkan sumber atau penyebab timbulnya variasi dan akan dapat meningkatkan kemampuan proses dalam menghasilkan produk yang tidak memiliki cacat.

Six Sigma was a target, that is 3.4 Defect Per Million Opportunities, which the characteristic of the quality measured from a perspective of error or defect by the opportunity of error or defect itself. The method of six sigma improvement which focusing on problem solving of a system is called Six Sigma Improvement Framework. This framework contain 5 phase called DMIAC (Define, Measure, Analyze, Improve, dan Control). Six sigma focusing on customer satisfaction and process oriented.
This research is analyzing six sigma implementation in a plastic part manufacturing company for reducing silver and short shoot reject at injection process. The processing of data using six sigma tools at each phase of DMIAC. Through the six sigma implementation, injection process performance can be calculated. After the calculation, variation source can be analyze and the potential solution is determined to improve the process performance.
This research is only for the injection product specially silver and short shoot reject. Defect per million opportunity (DPMO) value produced in injection process is 11910.6 DPMO and 3.8 sigma value. This value was not good enough. Potential solution and implementation need to be identiiied for reducing source of variation and problem. Good improvement will increase the process capability and performance and hopefully the company can produce only good product.

In this observation, SixSigma is used in assembly process because many defecs are founded in this Process. The product that being observed are running Nike shoes which will be export to whole continent in this world. The aim of Six Sigma implemention in this Subcontractors Nike are decreasing the amount of defects with knowing the value of sigma and yields as a result of processing the quantitative data. "Penerapan konsep manajemen mutu seperti Lean Six Sigma mulai diterapkan dalam industri jasa seperti pada industri telekomunikasi. Tesis ini dilakukan untuk menganalisis penerapan konsep Lean Six Sigma pada operator telekomunikasi dengan melihat kapabilitas proses BTS melalui kinerja Call Setup Success Rate (CSSR) dan Call Drop Rate (CDR). Parameter tersebut merupakan dua parameter utama jaringan seluler. Penerapan Six Sigma pada proses perbaikan kualitas layanan seluler dilakukan agar kedua parameter tersebut dapat mencapai nilai standar yang telah ditentukan. Kinerja parameter BTS dapat dilihat berdasarkan nilai kinerja proses Ppk yang kemudian dikonversi menjadi nilai sigma. Nilai sigma terbesar yang dapat dicapai setelah dilakukan perbaikan berdasarkan penelitian ini adalah 6σ untuk parameter CSSR area Jakarta Utara. Faktor-faktor yang menentukan keberhasilan implementasi Six Sigma dalam proses perbaikan kualitas jaringan seluler juga dianalisis dan didapatkan bahwa faktor kepemimpinan merupakan faktor utama keberhasilan implementasi Six Sigma menurut para responden. Penelitian ini menggunakan data sekunder yang berupa kinerja BTS dan data primer yang didapatkan dari kuesioner kepada Divisi Optimisasi.

The application of quality management concepts such as Lean Six Sigma was implemented in the service industry such as the telecommunications industry. This thesis is conducted to analyze the application of the concept of Lean Six Sigma in the telecommunications operator to see performance capability of BTS through the performance of Call Setup Success Rate (CSSR) and Call Drop Rate (CDR). These parameters are the two main parameters of the mobile cellular network. Application of Six Sigma in mobile cellular service quality improvement process is done so that these two parameters can achieve a standard value that has been determined before. Performance parameters of BTS can be seen by Ppk process performance values which then converted into a sigma value. The largest sigma value that can be achieved after improvement process by this research is 6σ for CSSR parameter in North Jakarta area. The factors that determine the success of the implementation of Six Sigma in the process of improvement of the quality of mobile networks is also analyzed and found that leadership is a key factor for the successful implementation of Six Sigma according to the respondents. This study uses secondary data such as the performance of the BTS and primary data obtained from the questionnaire to the Division of Optimization."

Aneka Star could reach the sigma capability as 3.8 sigma as a baseline capability in output level, with the analysis start from the beginning process pretreatment rmtil the end process cooling, the main problem could be identified. The main problem are pretreatment process and curing process. As the conclusion, PT. Aneka Star has to make an accurate work standard for curing and pretreatment and also PL Aneka Star has to make a schedule of preventive maintenance."

"[ABSTRAK
Tesis ini bertujuan untuk menjelaskan bagaimana penerapan Six Sigma di PT SKF Indonesia serta melakukan perbaikan proses bisnis di area produksi PT SKF Indonesia dalam menurunkan waktu resetting di CH 2, 3 & 4 menggunakan metodologi Six Sigma. Penelitian dilakukan dengan mengikuti tahapan yang ada dalam DMAIC serta menggunakan tools Six Sigma untuk memperbaiki proses resetting. Untuk menguji perbaikan yang telah dilakukan, digunakan metode statistik untuk membandingkan data sebelum dan sesudah menggunakan Normality Test dan Two Sample T-Test. Penelitian ini dilakukan dalam kurun waktu Januari hingga Juni 2015 dimana data baseline diambil dari Januari ? Maret 2015 dan improvement dilakukan mulai April 2015 dimana pengukuran waktu resetting setelah perbaikan dilakukan dari April ? Juni 2015. Pada akhirnya, penerapan Six Sigma ternyata mampu mengurangi waktu resetting di CH 2, 3 & 4 dari 4,7 jam menjadi 3 jam. Pengurangan 1,7 jam ini jika dikalikan dengan jumlah resetting di tahun 2014 dan dikalikan dengan rate per hour production line maka akan memberikan pemasukan sebesar 4,2 Miliar dan sebagian dari pemasukan tersebut dapat dijadikan sebagai insentif bagi operator pelaksana resetting untuk memberi semangat sehingga improvement yang telah dilakukan akan terus berjalan;

ABSTRACT
This thesis aims to explain how the application of Six Sigma in PT SKF Indonesia as well as the conduct of business process improvement in the production area of PT SKF Indonesia in reducing the time resetting in CH 2, 3 & 4 using Six Sigma methodology. The study was conducted by following stages in DMAIC and using Six Sigma tools to improve the process of resetting. To test the improvements made, statistik method was used to compare data before and after using normality Test and Two Sample T-Test. This research was conducted in the period January to June 2015 where baseline data taken from January to March 2015 and improvement conducted from April 2015 in which the measurement time resetting after the repair is done from April to June 2015. In the end, the application of Six Sigma was able to reduce the time resetting in CH 2, 3 & 4 from 4.7 hours to 3 hours. Instalasi Radiologi, yang merupakan Salah satu lnstalasi dalarn RS X, juga beroperasi dengan kerangka pikiran kualitas.
Tingkat kegagalan dan pengulangan foto rontgen, yang berkaitan langsung dengan jumlah penggunaan film, merupakan salah satu ukuran kinerja dalam Instalasi Radiologi, karena selain memiliki pengaruh langsung dengan kepuasan pasien, juga pada biaya yang harus dikeluarkan. Untuk mengukur kinerja Instalasi Radiologi RS X secara obyektif dan mengidentifikasikan usaha dan solusi peningkatan kinerja yang efektif digunakan metode Sir Sigma. Sb: Sigma merupakan suatu pendekatan terhadap kualitas yang disiplin terhadap fakta dan data. Saat ini Six Sigma sudah banyak diaplikasikan di industri manufaktur dan mulai populer di industri jasa, seperti rumah sakit.
Hasil dari penelitian ini meliputi berbagai hal, di antaranya analisa kapabilitas proses dan usulan solusi peningkatan kualitas dan pengendalian kinerja.

The high level of competition and patients’ demands has forced hospitals to continuously strive on improving their quality of service. As one of the hospitals operating in South Jakarta, Hospital X is always in search of ways to improve its quality of service. Radiology Installation, being one ofthe installations operating in Hospital X, is thus working in a quality mindset.
X-ray photo reject and repeat rate, directly related to the usage of Ehns, is one of the measures of perfomiance in the Radiology Installation, for it has a direct effect both to the installation’s customers’ satisfaction level and the expenditure level. To obtain an objective performance measurement and to identify potential Solutions to the existing problems, a method called Six Si grna is deployed- Six Sigma is an eifective approach to quality that grotmds its deployment in a most disciplined fashion on hard facts and data. Today, Six Sigma is already a popular approach in the manufacturing industry, and it is gaining prominence in the services industry, such as hospitals.
The results that can be expected from this research will include a process capability analysis and recommendations for quality improvements and performance control.

"PT Nabisco Foods is a food industry which is facing quality problem, that is product overweight. Overweight percentage existing reaches 5,44 percent meanwhile company?s maximum taret for overweight is only 3 percent. Six Sigma is a comprehensive methodology for quality improvement which is directly eliminating the main special sources of problem with DMAIC (De_firie-MeaszIre-Analyze-improve-Coniro/) approach. Six Sigma concept is aimed to increase process capability, reduce variation between outputs and adjust them with the target.
Determining product O with Reguler type as the main focuss, research scope is limited in mixing, forming dan baking area. Then the actual process capability is evaluated by measuring weight and thickness parameter of base cake. For weight parameter, level sigma is 5,04 dan 99,93 percent yield. These metric value indicates that the process capability for weight parameter is quite good. On the other hand, for thickness parameter, level sigma 3,45 and 97,42 percent yield are received which points that improvement focuss on process related to thickness parameter is needed. By analyzing actual process, the root causes with highest risk level are identiiied, one of them is the leak of correlation of temperature between oven zone. The outcome of using Taguchi method is the optimum setting that are I baking time in 6 menit, suhu zone 1,2 dan 3 subsequently 500, 610 and 650 F.For the last phase of Six Sigma., author recommend utilization of control chart form for monitoring process capability continuously with thickness of base cake as the parameter."
