"Latar Belakang: Sel kanker membutuhkan asam amino untuk metabolismenya telah ditemukan pada beberapa studi. Metabolisme ini meyebabkan profil asam amino mengalami perubahan pada kanker payudara. Profil asam amino pada kanker payudara ini dapat menentukan prognosis penyakit pada kanker payudara.
Tujuan: Mengetahui hubungan profil asam amino dengan faktor risiko kanker payudara di RS Ciptomangunkusumo, Indonesia.
Metode: Penelitian ini merupakan penelitian cross-sectional pada 19 subjek kanker payudara dan 19 subjek kontrol yang memenuhi kriteria penelitian di RSCM, Jakarta, Indonesia. Asam amino dianalisis menggunakan teknik liquid chromatography. Data tersebut dianalisis menggunakan uji independent sample t-test dan uji Mann-Whitney U.
Hasil:
Perbedaan profil asam amino pada kanker payudara yang signifikan dibandingkan dengan kontrol ditemukan pada peningkatan asam amino sistin dan penurunan asam amino valin, lisin, histidin, alanin, ornitin, tirosin, glutamin, fenilalanin, dan asam amino prolin. Terdapat signifikansi antara asam amino dengan faktor risiko usia, jumlah paritas, riwayat pemberian ASI, usia menarke, dan kebiasaan berolahraga.

Background: Cancer cell needs amino acids for its metabolism has been found in recent studies. Amino acids profile changes due to cancer cells’ metabolism. This amino acids profile in breast cancer could determine the prognosis of disease in breast cancer.
Objective: To aimed relationship between amino acids profile in breast cancer with its risk factor in Ciptomangunkusumo Hospital, Indonesia.
Method: A cross-sectional study in 19 breast cancer subjects and 19 control subjects that included into the criteria was conducted in this research. Amino acids was examined with liquid chromatography technique. The data was analyzed with independent sample t-test and Mann-Whitney U test.
Result: There are significant differences of amino acids profile between breast cancer subjects and control subjects, essentially for increased cystine profile and decreased valine, lysine, histidine, alanine, ornithine, tyrosine, glutamine, phenylalanine, and proline. There are also significance between amino acids profile in breast cancer with risk factors, including age, parity, breastfeeding history, menarche age, and workout habit.

"Latar Belakang: Sel punca mesenkimal SPM asal jaringan lemak ASCs dan tali pusat UCSCs merupakan sumber sel punca yang umum digunakan pada terapi seluler. SPM berkomunikasi dengan sel kanker diantaranya melalui berbagai faktor biologis aktif yang dinamakan sekretom. Lingkungan mikro kanker yang hipoksik dapat memberi pengaruh berbeda pada interaksi ini. Efek interaksi sekretom SPM terhadap agresivitas sel punca kanker payudara hingga kini belum banyak diketahui.
Tujuan: Menganalisis berbagai penanda agresivitas yang berkaitan dengan pertumbuhan dan ketahanan hidup viabilitas, proliferasi, sifat pluripotensi OCT4 dan SOX2, tumorigenik MFU, progresif-agresif TGF-?1 dan T?R1, penanda kepuncaan dan kemampuan detoksifikasi ALDH1A1 dan ALDH1A3, serta sifat invasif MMP2 dari sel punca kanker payudara BCSCs ALDH paska interaksi dengan sekretom dari conditioned medium CM SPM asal tali pusat dan jaringan adiposa yang diproduksi dalam kondisi normoksia maupun hipoksia.
Metode: Studi eksperimental in vitro menggunakan CM UCSCs dan ASCs normoksia dan hipoksia yang disuplementasikan pada medium asal DMEM-F12 dari sel punca kanker payudara BCSCs ALDH dengan konsentrasi 50 v/v selama 72 jam. Analisis uji viabilitas dan proliferasi, q-RT-PCR ekspresi mRNA ALDH1A1, ALDH1A3, OCT4, SOX2, MMP2, TGF-?1 dan T?R1 serta uji MFU dari BCSCs ALDH dilakukan untuk mengetahui efek dari sekretom dalam CM terhadap agresivitas BCSCs ALDH.
Hasil: Sekretom dalam CM-UCSCs dapat meningkatkan agresivitas BCSCs melalui peningkatan penanda invasif ndash;agresif dan detoksifikasi. Sekretom dalam CM-ASCs dapat meningkatkan agresivitas BCSCs melalui peningkatan penanda pluripotensi, invasif dan detoksifikasi. Prekondisi hipoksia pada CM-SPM dapat meningkatkan potensi agresivitas lebih tinggi daripada CM normoksia. Perbedaan regulasi viabilitas dan proliferasi serta turunnya penanda tumorigenik BCSCs paska suplementasi CM perlu diinterpretasikan dengan hati-hati dan masih memerlukan verifikasi.
Kesimpulan: Sekretom dalam CM UCSCs maupun ASCs dapat memberikan efek meningkatkan sifat agresif dari BCSCs ALDH. Preparasi hipoksia pada produksi CM cendrung lebih mendukung sifat agresif dari BCSCs ALDH dibandingkan CM normoksianya. Perbedaan regulasi viabilitas dan proliferasi serta turunnya penanda tumorigenik pada BCSCs paska suplementasi CM SPM masih membutuhkan penelitian lebih lanjut untuk menganalisis dasar molekuler yang menyebabkannya.

Background: Adipose and umbilical cord tissue derived mesenchymal stem cells MSCs are the most common sources that are used in various cellular therapies. MSCs are known to communicate with cancer cells through their secretomes. The hypoxic microenvironment of cancer may cause different effects on this interaction. Effects of MSC secretomes against the aggressiveness of breast cancer stem cells BCSCs ALDH have not been widely investigated.
Aim: To analyze various markers of aggressiveness that are associated with growth and survival viability, proliferation, pluripotency OCT4 and SOX2, tumorigenic MFU, progressive aggressive TGF 1 and T R1, stemness and detoxification ALDH1A1 and ALDH1A3, as well as the invasive nature MMP2 of BCSCs ALDH post interaction with both normoxic and hypoxic MSC secretomes.
Methods: The in vitro experimental study using conditioned medium CM of MSCs produced in normoxic and hypoxic condition that were supplemented in medium of BCSCs ALDH with concentrations of 50 v v for 72 hours. Analysis of viability, proliferation, and q RT PCR of ALDH1A1, ALDH1A3, OCT4, SOX2, MMP2, TGF 1 and T R1 mRNA as well as MFU assay were performed to determine the effect of secretomes on the aggressiveness of BCSCs ALDH.
Results: Secretomes of UCSCs supported the aggressiveness by increasing invasive aggressive and detoxification markers, while secretomes of ASCs supported the aggressiveness by increased pluripotency, invasive and detoxification markers. Hypoxic preconditioning of MSC secretomes increased the potential for aggressiveness higher than normoxic secretomes. Differences in viability and proliferation regulation and the decrease in BCSCs tumorigenic post secretomes supplementation need to be interpreted carefully and further verification.
Conclusion: Supplementation of MSC secretomes increased the aggressive properties of BCSCs ALDH. The hypoxic secretomes tend to favor the aggressive nature of BCSCs ALDH compared to its counterpart. Differences in viability and proliferation regulation as well as the decrease in tumorigenic markers in BCSCs after MSC secretomes supplementation still need further research to analyze the molecular underlying basis.

"ABSTRAK
Pasien kanker payudara di Indonesia sering mengalami ketidaknyamanan fisik dan psikologis, namun perawat belum dapat secara optimal membantunya. Hal ini terjadi antara lain karena sulitnya mengkaji kondisi/tingkat kenyamanan dan mengidentifikasi faktor-faktor yang mempengaruhi kenyamanannya. Penelitian ini bertujuan untuk mengembangkan instrumen pengkajian kenyamanan dan mengidentifikasi faktor-faktor yang mempengaruhi kenyamanan melalui suatu pemodelan teoritis kenyamanan dalam pendekatan Kolcaba. Penelitian ini diawali dengan studi literatur, wawancara mendalam dengan 10 pasien, dan konsultasikan dengan 10 orang pakar dari berbagai keilmuan terkait, dilanjutkan dengan menggunakan metode cross sectional untuk uji construct validity dan uji model teoritis dengan menggunakan Structural Equation Modelling pada 308 pasien kanker payudara. Hasil penelitian ini adalah instrumen pengkajian kenyamanan kanker payudara yang diberi nama ldquo;PKKP rdquo; dan pemodelan teoritis kenyamanan pada pasien kanker payudara di Indonesia dengan pendekatan Kolcaba. Hasil penelitian menjelaskan bahwa dukungan emosi, perawatan paliatif, spiritual, dan usia pasien mempengaruhi kenyamanan pasien melalui mediator kondisi fisik dan emosi. Pengembangan perawatan paliatif yang memperhatikan aspek psikologis, spiritual, dan karakteristik usia pasien, diperlukan di Indonesia untuk meningkatkan kenyamanan pada pasien kanker, khususnya pada pasien kanker payudara.

ABSTRACT
Breast cancer rsquo patients in Indonesia frequently experience physical and psychologist discomforts. Nevertheless, it is difficult for nurses, to precisely measure comforts and identify factors affecting comforts among cancer patients. This study aimed to develop a comfort assessment tool and generate a theoretical comfort model for breast cancer patients in Kolcaba approach. The first stage of this study used literature reviews, in depth interviews with 10 cancer patients, and consulted with 10 experts, followed by cross sectional method for construct validity test and theoretical model tests using Structural Equation Modelling to 308 breast cancer patients. Results of our study were the comfort assessment breast cancer instrument and a theoretical comfort model for breast cancer patients in Indonesia. The results of this research concluded emotional support, palliative care, spiritual, and patients rsquo age affects patients 39 comfort through the mediator of physical and emotional conditions. Our study suggested the development of palliative care that takes into account psychologist, spiritual and patient rsquo s characteristic aspect to promote comforts among breast cancer patients."

ABSTRACT
Purpose
Geriatric surgery poses specific challenges due to patient vulnerability in relation to aging. We analyzed perioperative challenges concerning super-elderly patients with breast cancer.
Methods
Between 2013 and 2018, 908 patients with breast cancer were treated surgically. Of these, two patient groups were compared: Group A (≥ 85 years old, n = 34, 3,7%) and Group B (75-84 years old, n = 136, 15%).
Results
In Groups A and B, 26,4% and 36,8% of patients lived alone, respectively. Group A patients had higher rates of psychiatric and cardiovascular disease (32,4% and 41,2%) than Group B (8,8% and 16,2%) (p = 0,0009 and p = 0,0031, respectively). There was no marked difference in the type of surgery or length of hospital stay between groups, and most complications involved surgical site disorders. Postoperatively, Group A had a higher rate of delirium (29,4%) than Group B (3,7%) (p < 0,0001). The 30-day postoperative mortality rate was 0, and 76,5% of Group A and 45,6% of Group B patients received no adjuvant therapy (p = 0,0024).
Conclusions
Age alone does not constitute a contraindication for appropriate surgery, although there are some challenges necessary to consider for super-elderly patients."
