Bioburden merupakan parameter penilaian penting dalam kualitas produk steril. Dengan mengetahui adanya beban cemaran mikro maka akan dapat diketahui gambaran mutu produk sediaan steril sebelum dan setelah melewati proses sterilisasi nantinya. PT. Harsen Laboratories sendiri merupakan industri manufaktur farmasi di Indonesia dengan produksi sediaan injeksi terbesar di Indonesia dimana untuk menjamin kualitas sediaan injeksi yang diproduksi ialah dengan menjalankan pengujian bioburden pada tiap-tiap produk injeksi. Prosedur penyusunan protap bioburden dimulai dari suprevisor Andev menyusun dan merevisi prosedur pengujian bioburden kemudian diperiksa oleh Manajer RnD sub departemen Andev dan Packdev untuk memastikan prosedur pengujian bioburden dapat berjalan dengan baik. Protap pengujian bioburden dapat dilakukan dengan 2 metode, yaitu metode membran filtrasi dan metode cawan tuang dimana kedua metode ini harus dilakukan oleh analis QC terlatih dan mengikuti prosedur yang terlampir secara jelas di dalam protap dan lembar kerja pengujian bioburden. Penyakit kardivoaskular ialah gangguan yang terjadi pada jantung dan pembuluh darah. Permasalahan yang sering timbul terkait penyakit ini ialah pengobatan yang bervariasi terutama dari sisi penggunaan obat yang beragam dimana keragaman dari efek samping dan interkasi pada peresepan obat-obatan tersebut juga meningkat. Sebagai seorang tenaga kesehatan yang profesional di bidang farmasi, apoteker berkewajiban untuk mengkaji terlebih dahulu resep yang masuk untuk menghindari kesalahan pemberian obat-obatan. Frekuensi masuknya resep untuk pengobatan penyakit kardiovaskular cukup banyak terutama di Apotek Roxy Biak yang terletak di pusat ibukota, yaitu kota Jakarta Pusat. Hasil skrining resep kardiovaskular pada penulisan laporan ini ialah kajian administratif dalam penulisan resep yang diterima di apotek Roxy Biak sudah cukup lengkap, namun ada beberapa informasi yang kurang dimana apoteker dapat menggali informasi lewat komunikasi untuk memverifikasi resep. Kesesuaian farmasetik dari obat-obatan kardiovaskular yang diberikan juga sudah cukup jelas dan pertimbangan klinik yang harus dipantau dari resep kardiovaskular yang masuk ialah harus memantau efek samping obat dan memantau terapi obat yang dikonsumsi pasien.

PT. Harsen Laboratories itself is a pharmaceutical manufacturing industry in Indonesia with the largest production of injection preparations in Indonesia where to ensure the quality of the injection preparations produced is to carry out bioburden testing on each injection product. The procedure for preparing the bioburden protocol starts with the Andev supervisor compiling and revising the bioburden testing procedure and then being checked by the Andev and Packdev sub-department RnD Managers to ensure the bioburden testing procedure runs properly. The procedure for testing bioburden can be carried out by 2 methods, which is the membrane filtration method and the pour plate method where both methods must be carried out by a trained QC analyst and follow the procedures clearly attached in the protocol and the bioburden testing worksheet. Cardiovascular disease is a disorder that occurs in the heart and blood vessels. Problems that often arise related to this disease are varied in the treatments, especially in terms of the use of various drugs where the diversity of side effects and interactions in prescribing these drugs also increases. As a professional health worker in the pharmaceutical field, the pharmacist is obliged to review the incoming prescription in the first place to avoid medication errors. The entry frequency of prescriptions for the treatment of cardiovascular disease is quite high, especially at the Roxy Biak Pharmacy, which is located in the capital city, Central Jakarta. The result of cardiovascular prescription screening in writing this report is that the administrative review in writing prescriptions received at the Roxy Biak pharmacy is quite complete, however there is some information that is lacking where pharmacists can dig up the information through communication to verify the prescriptions. The pharmaceutical suitability of the cardiovascular drugs given is also quite clear and clinical considerations that must be monitored from incoming cardiovascular prescriptions are to monitor the drug side effects and the drug therapy that the patient is taking.

Pengkajian resep termasuk kedalam bagian dari kegiatan farmasi klinis di Apotek yang merupakan bagian dari Pelayanan Kefarmasian yang langsung dan bertanggung jawab kepada pasien berkaitan dengan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai dengan maksud mencapai hasil yang pasti untuk meningkatkan kualitas hidup pasien. Kegiatan pengkajian Resep meliputi administrasi, kesesuaian farmasetik dan pertimbangan klinis. Penyakit infeksi di Indonesia masih termasuk dalam sepuluh penyakit terbanyak. Peresepan antibiotik di Indonesia yang cukup tinggi dan kurang bijak akan meningkatkan kejadian resistensi. Berdasarkan gambaran dari peresepan antibiotik yang cukup tinggi, maka diperlukan pengkajian terhadap skrining administrasi dan farmasetik resep antibiotik untuk mencegah terjadinya kesalahan pengobatan yang dapat merugikan pasien di Apotek Roxy Biak. Tujuan penulisan tugas khusus praktik kerja profesi apoteker ini yaitu pengkajian resep obat untuk dua resep antibiotik di apotek Roxy Biak pada bulan juli 2022 dari segi kajian administratif, klinis dan farmasetik. Hasil yang didapat yaitu obat yang ditebus pada Apotik Roxy Biak masih memiliki kekurangan pada kelengkapan kajian administratif, yaitu Berat Badan (BB), umur pasien, dan alamat pasien, paraf dan nomor telepon dokter. Dari segi kajian farmasetik dan klinis, resep antibiotik yang ditebus di Apotek Roxy Biak telah memenuhi persyaratan.

Prescription review activities include administration, pharmaceutical suitability and clinical considerations. Infectious diseases in Indonesia are still among the ten most common diseases. Prescription of antibiotics in Indonesia which is quite high and not wise will increase the incidence of resistance. Based on the description of the high prescribing of antibiotics, it is necessary to study the administrative and pharmaceutical screening of antibiotic prescriptions to prevent medication errors that can harm patients at the Roxy Biak Pharmacy. The purpose of writing this special assignment for the work practice of the pharmacist profession is to review drug prescriptions for two antibiotic prescriptions at the Roxy Biak pharmacy in July 2022 in terms of administrative, clinical and pharmaceutical studies. The results obtained were that the drugs that were redeemed at the Roxy Biak Pharmacy still had deficiencies in the completeness of administrative studies, namely body weight (BB), patient's age, and patient's address, initials and doctor's telephone number. Drug information services and counseling are important aspects of clinical pharmacy services to increase public knowledge about how to use drugs properly. Drug information services is an activity carried out by pharmacists in providing information about drugs that are impartial, evaluated critically and with the best evidence in all aspects of drug use to other health professionals, patients or the public. Counseling is an interactive process between pharmacists and patients/families to increase knowledge, understanding, awareness, and compliance so that a change in behavior occurs in drug usage and resolves problems faced by patients. The implementation of drug counseling and information services at the Roxy Pharmacy has not yet been implemented for all patients who meet the counseling criteria. This is caused by various factors such as the limited time that the patient can spare, the patient's willingness to be counseled, and the patient who uses a delivery service to redeem his prescription. Roxy Biak Pharmacy provides a telephone number that patients can use to contact the pharmacist on duty. However, from the archives of drug information services reports and drug-related counseling at the Roxy Biak Pharmacy, it is known that all of them are still carried out face-to-face. Therefore, drug information services and counseling are still often hampered by the patient's time availability.

Telah dilakukan Praktek Kerja Profesi Apoteker (PKPA) di PT Mahakam Beta Farma, Jakarta Timur yang berlangsung pada tanggal 3 Januari – 25 Februari 2022 dan di Apotek Roxy Biak, Jakarta Pusat yang berlangsung pada tanggal 9 – 31 Mei 2022. Kegiatan ini merupakan sarana penulis untuk mempelajari dan memahami tugas dan tanggung jawab seorang aporeker. Hal ini diharapkan dapat menjadi bekal penulis untuk melaksanakan peran apoteker di ranah professional serta dapat bermanfaat bagi diri sendiri, rekan sejawat, serta masyarakat. Kegiatan PKPA di PT Mahakam Beta Farma dan Apotek Roxy Biak meliputi tinjauan tugas managerial di bagian produksi sediaan solid industri farmasi, tugas farmasi klinis di apotek, serta tugas khusus baik di industri farmasi maupun apotek.

The Pharmacist Professional Work Practice (PKPA) has been carried out at PT Mahakam Beta Farma, East Jakarta which took place on January 3 – February 25, 2022 and at the Roxy Biak Pharmacy, Central Jakarta which took place on May 9 – 31 2022. This activity is the author's means to learn and understand the duties and responsibilities of an apothecary. This is expected to be the author's provision to carry out the role of pharmacists in the professional realm and can be useful for themselves, colleagues, and society. PKPA activities at PT Mahakam Beta Farma and Roxy Biak Pharmacy include reviewing managerial duties in the solid dosage production section of the pharmaceutical industry, clinical pharmacy duties in pharmacies, as well as special tasks in both the pharmaceutical and pharmacy industries.

Diabetes melitus adalah penyakit gangguan metabolisme yang ditandai dengan terjadinya hiperglikemia dan kelainan metabolisme seperti pada karbohidrat, lemak, dan protein. Obat oral antidiabetes termasuk ke dalam lima penyakit terbanyak yang ditebus di apotek. Dalam mengelola apotek, dibutuhkan kemampuan untuk manajemen persediaan obat yaitu memastikan perputaran obat agar permintaan obat tetap terpenuhi. Selain itu, salah satu kegiatan pelayanan farmasi klinik yang dilakukan di apotek yaitu pengkajian resep. Pengkajian resep bertujuan untuk meningkatkan kualitas hidup pasien dengan cara menganalisa masalah terkait obat dan mencegah terjadinya medication error. Tujuan dari laporan ini yaitu untuk mengetahui obat antidiabetes yang termasuk dalam kategori fast moving dengan penjualan tertinggi pada bulan Februari 2023 dan untuk mengetahui apakah terdapat masalah terkait resep obat antidiabetes oral yang ditebus di Apotek Roxy Biak. Metode yang digunakan yaitu mengumpulkan data penjualan obat antidiabetes oral pada bulan Februari 2023 untuk menentukan obat yang tergolong dalam kategori fast moving dan pemilihan 2 resep yang digunakan sebagai perwakilan bulan Februari untuk dilakukan analisis kajian resep. Berdasarkan pengkajian resep obat antidiabetes oral fast moving di Apotek Roxy Biak, didapatkan 10 obat dengan jumlah penjualan terbanyak yaitu Metformin 500 mg Glucophage XR 500 mg, Glucophage 500 mg, Diamicron MR 60 mg, Glibenklamida 5 mg, Glimepiride 2 mg, Forxiga 10 mg, Jardiance 25 mg, Gliquidone 30 mg, dan Janumet 50/1000 mg. Penjualan terbanyak yaitu Metformin 500 mg dengan jumlah 4.065 tablet. Hasil pengkajian resep berdasarkan kajian administrasi, kajian kesesuaian farmasetik, dan pertimbangan klinis tidak ditemukan masalah terkait obat.

Oral antidiabetic drugs are included in the five most common diseases redeemed at the Roxy Biak Pharmacy. In managing a pharmacy, the ability to manage drug supplies is needed for drug circulation to ensure that drug demands are fulfilled. In addition, one of the clinical pharmacy service activities carried out at the pharmacy is prescription review. The prescription review aims to improve patient’s quality of life by analyzing drug-related problems and preventing medication errors. This report aims to find out which antidiabetic drugs are included in the fast-moving category with the highest sales in February 2023 and to investigate whether there are any problems with prescriptions for oral antidiabetic drugs that are redeemed at the Roxy Biak Pharmacy. The method used was through data collection on sales of oral antidiabetic drugs in February 2023 to determine which drugs belong to the fast- moving category and to select two recipes to be used as representatives for February to conduct a prescription review analysis. Based on a review of fast-moving oral antidiabetic drug prescriptions at the Roxy Biak Pharmacy, the ten drugs with the highest sales were found, namely Metformin 500 mg, Glucophage XR 500 mg, Glucophage 500 mg, Diamicron MR 60 mg, Glibenclamide 5 mg, Glimepiride 2 mg, Forxiga 10 mg, Jardiance 25 mg, Gliquidone 30 mg, and Janumet 50/1000 mg. The biggest seller was Metformin 500 mg with a total of 4,065 tablets. The results of the prescription review based on administrative reviews, pharmaceutical suitability studies, and clinical considerations found no drug-related problems.

Kegiatan pelayanan farmasi klinik yang dilaksanakan di apotek salah satunya mencakup pengkajian dan pelayanan resep. Resep yang dilayani di apotek cukup beragam, mulai dari resep untuk penyakit akut maupun kronis seperti diabetes.Tujuan dari skrining dan analisis resep obat antidiabetes adalah untuk mengetahui obat antidiabetes yang diresepkan oleh dokter, mengetahui jumlah resep yang mengandung obat antidiabetes, serta menilai kerasionalan penggunaan obat antidiabetes pada resep yang dilayani di Apotek Roxy Biak selama periode Juli 2022. Dari seluruh resep yang masuk selama periode tersebut, resep yang mengandung obat antidiabetes dicatat dan dikumpulkan data, kemudian dipilih dua resep untuk dikaji kelengkapan resep berdasarkan aspek administratif, farmasetik, serta klinis. Pengkajian 2 (dua) resep antidiabetes di Apotek Roxy Biak dilihat dari aspek administratif, farmasetika dan klinis sudah sesuai. Namun pada aspek administratif penulisan umur dan berat badan pasien tidak dituliskan. Aspek administratif terkait umur dan berat badan perlu dikonfirmasi kembali kepada pasien atau keluarga pasien yang mengambil obat di apotek, hal ini penting untuk mengkaji ketepatan pengobatan yang akan diterima oleh pasien.

Of all prescriptions received during this period, prescriptions containing antidiabetic drugs were recorded and data collected, then two recipes were selected to review the completeness of the prescription based on administrative, pharmaceutical, and clinical aspects. The review of 2 (two) antidiabetic prescriptions at the Roxy Biak Pharmacy from an administrative, pharmaceutical and clinical perspective was appropriate. However, in the administrative aspect, the patient's age and weight were not written down. Administrative aspects related to age and weight need to be confirmed again with the patient or the patient's family who picks up the drug at the pharmacy, this is important to assess the accuracy of the treatment that the patient will receive.

Evaluasi kesesuaian pelayanan kefarmasian di apotek mulai dari penerimaan, pemeriksaan ketersediaan, penyiapan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai termasuk peracikan obat, pemeriksaan, penyerahan disertai pemberian informasi. Pada setiap tahap alur pelayanan resep dilakukan upaya pencegahan terjadinya kesalahan pemberian obat (medication error). Pelayanan dan pengkajian resep di Apotek Roxy Biak aspek administratif, farmasetika, dan klinis serta pengelolaan sediaan farmasi di Apotek Roxy Biak mulai dari perencanaan, pengadaan, penerimaan, penyimpanan disertai stok opname yang dilakukan setiap hari, pemusnahan, pencatatan dan pelaporan telah memenuhi persyaratan Peraturan Menteri Kesehatan No. 73 Tahun 2016.

At each stage of the prescription service flow, efforts are made to prevent medication errors. The service and review of prescriptions at the Roxy Biak Pharmacy in administrative, pharmaceutical and clinical aspects also management of pharmaceutical preparations at the Roxy Biak Pharmacy starting from planning, procurement, receipt, storage with stock taking which is carried out every day, destruction, recording and reporting has met the requirements of Minister of Health Regulation No. 73 of 2016. 

Narkotika, psikotropika, prekursor, dan obat-obat tertentu (OOT) merupakan golongan obat yang dapat dijual oleh apotek tertentu dan memerlukan perhatian khusus dalam proses pengelolaannya. Apotek Roxy Biak memiliki persediaan yang lengkap hingga mampu memenuhi kebutuhan obat-obat narkotika, psikotropika, prekursor dan OOT sesuai permintaan pada resep. Adanya resep narkotika, psikotropika, prekursor dan OOT yang masuk ke apotek ini setiap harinya, membuat penulis terpicu untuk menelaah profil peresepan obat-obat tersebut guna mengetahui jenis obat yang banyak dibutuhkan dan memiliki frekuensi penggunaan yang tinggi. Penulis juga perlu melakukan analisis pareto untuk menentukan kelompok obat yang butuh diprioritaskan persediaannya. Penulisan laporan ini bertujuan untuk menentukan obat narkotika, psikotropika, prekursor, dan OOT yang paling banyak diresepkan dan memiliki penjualan tertinggi selama tiga puluh satu hari terakhir di periode November hingga Desember 2022 serta memprediksi kebutuhan pengadaan untuk periode berikutnya. Metode yang digunakan adalah studi literatur, pengamatan lapangan, pencatatan resep selama tiga puluh satu hari terakhir, dan penarikan laporan pembelian dan penjualan obat. Hasil menunjukkan bahwa peresepan obat narkotika memiliki persentase 9,41%, psikotropika 44,62%, prekursor 31,68%, dan OOT 14,29%. Obat narkotika yang paling banyak diresepkan adalah Codikaf 20 mg, psikotropika adalah Braxidin, prekursor adalah Rhinos SR, dan OOT adalah Hexymer 2 mg. Dapat disimpulkan bahwa golongan obat yang paling banyak diresepkan adalah obat psikotropika sebanyak 8.269 obat dan persentase 44,62%. Prioritas tertinggi dalam perencanaan kebutuhan obat untuk diadakan di periode berikutnya jatuh pada kelompok obat psikotropia khususnya obat-obat kelas A.

Roxy Biak Pharmacy has a complete inventory to meet the needs of narcotics, psychotropics, precursors, and OOT according to prescription requests. The prescriptions for narcotics, psychotropics, precursors, and OOT that enter this pharmacy every day has motivated the author to examine the prescriptions profiles for these drugs to find out the types of drugs that were much needed and frequently used. A pareto analysis to determine which drug need to be prioritized for supplies. This report aims to determine which narcotics, psychotropics, precursors, and OOT drugs were most prescribed and had the highest sales during the last thirty-one days from November to December 2022 and predict procurement needs for the following period. The methods used were literature studies, observations, recording prescriptions for the last thirty-one days, and withdrawing reports on drug purchases and sales. The results showed the percentage of prescribing narcotics was 9.41%, psychotropics 44.62%, precursors 31.68%, and OOT 14.29%. The most widely prescribed narcotic was Codikaf 20 mg, the psychotropic was Braxidin, the precursor was Rhinos SR, and the OOT was Hexymer 2 mg. In conclusion, the most widely prescribed class of drugs was psychotropic with 8,269 drugs and a percentage of 44.62%. The highest priority in planning the need for drugs in the next period falls on the psychotropic group, especially class A drugs.

Apotek merupakan suatu sarana pelayanan kefarmasian tempat dilakukannya praktek kefarmasian oleh Apoteker. Pelayanan kefarmasian di Apotek terbagi kedalam dua jenis yaitu pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai serta pelayanan farmasi klinis. Mengingat banyaknya ketidaksesuaian antara resep yang diberikan oleh dokter kepada pasien, maka tugas khusus ini dilakukan untuk melakukan observasi terkait pelayanan farmasi klinis di Apotek Biak khususnya terkait ketepatan pelayanan resep serta kesesuaian data dan kelengkapan resep yang diterima di Apotek Roxy sesuai dengan ketentuan Standar Pelayanan Kefarmasian yang berlaku. Hal ini diharapkan dapat mencegah terjadinya kesalahan pemberian obat (medication error). Pembuatan laporan tugas khusus Praktik Kerja Profesi Apoteker (PKPA) di Apotek Roxy Biak diawali dengan mengumpulkan beberapa data berupa sampel resep bulan Februari 2023 dan dilakukan analisis resep berdasarkan telaah aspek administratif, farmasetik, dan klinis. Berdasarkan hasil kegiatan yang telah dilakukan, pelayanan farmasi klinis yang dilakukan di Apotek Roxy termasuk pelayanan dan pengkajian resep serta dispensing obat yang dilakukan sudah tepat dan sesuai dengan Standar Pelayanan Kefarmasian di Apotek, serta Resep obat yang diterima dan dilayani di Apotek Roxy biak sudah sesuai dengan kelengkapan data yang tepat berdasarkan pengkajian administrative, farmasetika, dan klinis.

Pelayanan farmasi klinik di Apotek merupakan bagian dari pelayanan kefarmasian yang langsung dan bertanggung jawab kepada pasien yang berkaitan dengan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai untuk meningkatkan kualitas hidup pasien. Salah satu upaya agar mahasiswa apoteker mampu melakukan praktik kefarmasian secara profesional, legal, dan etik di apotek, yakni melalui kegiatan PKPA (Praktik Kerja Program Apoteker). Pelaksanaan praktik kerja profesi ini berlangsung selama satu bulan dengan tugas khusus, yakni Poster Edukasi untuk Swamedikasi Batuk. Tujuan dari pembuatan poster adalah sebagai media edukasi bagi masyarakat untuk swamedikasi penyakit batuk. Metode yang digunakan dalam pembuatan poster edukasi untuk swamedikasi batuk,  yakni melakukan studi literatur mengenai pembuatan poster, swamedikasi batuk beserta pilihan obat yang dapat digunakan, dan terapi non farmakologi untuk meredakan gejala batuk, kemudian selanjutnya informasi yang telah diperoleh dituangkan menjadi sebuah poster.  Pembuatan poster edukasi dapat berguna sebagai sarana edukasi masyarakat untuk swamedikasi penyakit batuk sehingga masyarakat dapat menggunakan obat secara aman dan rasional serta dapat mencegah terjadinya kesalahan medikasi.

Clinical pharmacy services at the drug store are part of the pharmaceutical services which are direct and responsible to patients related to improve the quality of patients life. One of the efforts to be able to practice pharmacy in a professional, legal and ethical manner at drugs store is through internship. The internship duration is for one month with a special assignment, namely Educational Poster for Cough Self-Medication. The purpose of making poster is as an educational media for the community to self-medicate cough. The method used in making educational poster for cough self-medication by conducting a literature study about cough self-medication along with the choice of medicines that can be used for cough, and non-pharmacological therapy to relieve cough symptom. Making educational poster can be useful as a means of educating people for cough self-medication so that people can use medicines safely and rationally as well as can prevent medication errors.
