Abstrak :
Resep adalah permintaan tertulis dari dokter, dokter gigi, atau dokter hewan kepada apoteker, baik dalam bentuk paper maupun elektronik untuk menyediakan dan menyerahkan obat bagi pasien sesuai peraturan yang berlaku. Pada Praktek Kerja Profesi Apoteker (PKPA) ini, calon Apoteker memperoleh kesempatan untuk melakukan analisis potensi interaksi dan penentuan waktu penggunaan obat dalam resep di apotek Kimia Farma No. 382 Kelapa Dua. Tugas khusus ini bertujuan untuk meningkatkan pemahaman calon Apoteker mengenai interaksi obat dan waktu penggunaan obat yang tepat. Pelaksanaan analisis resep dilakukan dengan cara mengambil dan memfoto resep yang telah disiapkan dan diserahkan oleh mahasiswa kepada pasien. Dari semua resep yang telah difoto kemudian dipilih resep-resep yang berisikan obat-obatan anti anti diabetes, anti hipertensi, anti kolesterol, asma, dan pengencer darah. Setelah itu dipilih 5 resep secara acak untuk kemudian dianalisis. Analisis potensi terjadinya interaksi obat dan penentuan waktu penggunaan obat yang tepat dalam resep (obat anti diabetes, anti hipertensi, anti kolesterol, asma dan pengencer darah) di Apotek Kimia Farma No. 382, telah dilakukan dengan baik. Dari kelima resep yang dianalisis terdapat tiga resep yang memiliki potensi interaksi farmakodinamik sinergis, satu resep yang memiliki interaksi farmakodinamik antagonis, dan satu resep yang memiliki interaksi farmakokinetik sinergis. Potensi interaksi tersebut dapat dicegah dengan cara meminumnya secara terpisah. ...... A prescription is a written request from a doctor, dentist or veterinarian to a pharmacist, either in paper or electronic form, to provide and deliver medicine to the patient in accordance with applicable regulations. In this Pharmacist Professional Work Practice (PKPA), prospective pharmacists have the opportunity to analyze potential interactions and determine the timing of use of drugs in prescriptions at Kimia Farma Pharmacy Number 382, Kelapa Dua. This special assignment aims to increase prospective pharmacists' understanding of drug interactions and the correct time to use drugs. Prescription analysis is carried out by taking and photographing prescriptions that have been prepared and handed over by students to patients. From all the recipes that had been photographed, recipes were selected which contained anti-diabetic, anti-hypertension, anti-cholesterol, asthma and blood thinner medicines. After that, 5 recipes were randomly selected and then analyzed. Analysis of the potential for drug interactions and determining the correct time to use prescription drugs (anti-diabetes, anti-hypertension, anti-cholesterol, asthma and blood thinners) at Kimia Farma Pharmacy Number 382, has been carried out well. Of the five recipes analyzed, there were three recipes that had potential synergistic pharmacodynamic interactions, one recipe that had antagonistic pharmacodynamic interactions, and one recipe that had synergistic pharmacokinetic interactions. This potential interaction can be prevented by drinking them separately.

Apoteker bertanggung jawab dalam pelayanan kefarmasian di apotek,  termasuk dalam pelayanan resep. Oleh karena itu perlu dilakukan pengkajian meliputi monitoring dan evaluasi resep untuk mencegah terjadinya medication error. Penelitian ini bertujuan untuk mengetahui golongan obat hipertensi pada resep, aturan pakai obat hipertensi pada resep, dan interaksi obat hipertensi pada resep. Evaluasi dan monitoring dilakukan dengan dengan cara mengamati dan melakukan kegiatan pelayanan 5 resep di Apotek Kimia Farma No. 382, Depok, Jawa Barat. Hal-hal yang dikaji meliputi cara kerja obat berdasarkan golongan, aturan pakai dan interaksi obat. Hasil evaluasi dan monitoring menunjukkan pemakaian obat sudah tepat dengan memperhatikan golongan obat, aturan pakai dan juga interaksi obat.

Apothecary are responsible for pharmaceutical services in pharmacies, including prescription services. Therefore it is necessary to conduct an assessment including monitoring and evaluating prescriptions to prevent medication errors. This study aims to determine the class of hypertension drugs on prescription, the rules for using hypertension drugs on prescriptions, and the interactions of hypertension drugs on prescriptions. Evaluation and monitoring is carried out by observing and carrying out service activities for 5 recipes at Kimia Farma Pharmacy No. 382, Depok, West Java. The things studied include how drugs work by class, rules of use and drug interactions. The results of the evaluation and monitoring show that the use of the drug is appropriate by taking into account the drug class, the rules for use and also drug interactions

Apoteker sebagai bagian dari tim pelayanan kesehatan memiliki peran penting dalam PTO. Peran tersebut yakni mencegah munculnya permasalahan terkait dengan obat. Pengetahuan penunjang yang harus dimiliki seorang apoteker dalam PTO adalah terkait farmakoterapi, patofisiologi penyakit, serta interpretasi hasil pemeriksaan fisik, diagnostic dan laboratorium. Selain itu, diperlukan keterampilan berkomunikasi, membina hubungan interpersonal, serta menganalisis suatu permasalahan (Kemenkes RI, 2009). Apotek Kimia Farma merupakan perusahaan farmasi dengan jaringan apotek terbesar dan penyedia layanan kesehatan yang terintegrasi melalui layanan obat dan alat kesehatan lengkap, konsultasi apoteker dan dokter, layanan laboratorium, dan optik serta menyediakan aplikasi kesehatan untuk mempermudah akses layanan. Aplikasi Kesehatan yang dimaksud yaitu aplikasi Kimia Farma Mobile yang memiliki beberapa fitur antara lain fitur layanan klinik, layanan laboratorium, dan layanan apotek. Ketiga fitur tersebut dikembangkan untuk mempermudah pengguna dalam memperoleh pelayanan kefarmasian dari Apotek Kimia Farma. Meninjau dari adanya beberapa kekurangan seperti miss communication antara pasien dan tenaga kefarmasian terkait terapi yang digunakan, sehingga pengembangan inovasi terbaru berbasis digital melalui aplikasi Kimia Farma Mobile yakni fitur PTO (Pemantauan Terapi Obat) diharapkan mampu meminimalisir terjadinya kesalahan tersebut. ......Pharmacists as part of the health care team have an important role in the PTO. This role is to prevent the emergence of problems related to drugs. The supporting knowledge that a pharmacist must have in PTO is related to pharmacotherapy, disease pathophysiology, and interpretation of physical, diagnostic and laboratory examination results. In addition, communication skills are needed, fostering interpersonal relationships, and analyzing a problem (Kemenkes RI, 2009).Kimia Farma Apotek is a pharmaceutical company with the largest pharmacy network and integrated health service provider through complete drug and medical device services, pharmacist and doctor consultation, laboratory services, and optics as well as providing health applications to facilitate access to services. The Health application in question is the Kimia Farma Mobile application which has several features including clinical service features, laboratory services, and pharmacy services. These three features were developed to make it easier for users to obtain pharmaceutical services from the Kimia Farma Pharmacy. In view of the existence of several deficiencies such as miss communication between patients and pharmacy staff regarding the therapy used, it is hoped that the development of the latest digital-based innovations through the Kimia Farma Mobile application, namely the PTO (Drug Therapy Monitoring) feature, is expected to be able to minimize the occurrence of these errors.

Praktik Kerja Profesi Apoteker di Rumah Sakit Universitas Indonesia bertujuan untuk menganalisis gambaran umum mengenai kegiatan pelayanan kefarmasian di rumah sakit, menganalisis peran dan fungsi apoteker di rumah sakit secara profesional, serta secara khusus menganalisis daftar obat dan kelas terapi dalam Formularium Rumah Sakit Universitas Indonesia Periode 2020 terhadap Formularium Nasional 2022. Praktik Kerja Profesi Apoteker di PT Anugerah Pharmindo Lestari bertujuan untuk mengamati kegiatan distribusi obat dan bahan medis habis pakai dalam ruang lingkup Pedagang Besar Farmasi (PBF), mengetahui peran dan fungsi apoteker di PBF secara profesional, serta secara khusus melakukan identifikasi permasalahan pada proses pendaftaran customer baru kemudian mengembangkan inovasi pada eZView PT Anugerah Pharmindo Lestari. Praktik Kerja Profesi Apoteker di Apotek Kimia Farma 382 bertujuan untuk menganalisis kegiatan pelayanan kefarmasian di apotek secara umum, mengetahui peran dan fungsi apoteker di apotek secara profesional, serta secara khusus menganalisis kerasionalan peresepan terhadap pasien diabetes melitus di Apotek Kimia Farma 382. ......Pharmacy Professional Internship at the Universitas Indonesia Hospital aims to analyze the general description of pharmaceutical service activities in hospitals, analyze the roles and functions of pharmacists in hospitals professionally, and specifically analyze the list of drugs and therapeutic classes in the Universitas Indonesia Hospital Formulary for the 2020 Period towards the 2022 National Formulary. The Pharmacy Professional Internship at PT Anugerah Pharmindo Lestari aims to observe the distribution of drugs and medical consumables within the scope of Major Pharmaceutical Supplier, to know the roles and functions of pharmacists in Major Pharmaceutical Supplier professionally, and specifically to identify problems in the new customer registration process then develop innovations on eZView at PT Anugerah Pharmindo Lestari. The Pharmacy Professional Internship at Kimia Farma Pharmacy 382 aims to analyze the activities of pharmaceutical services at the pharmacy in general, to know the role and function of pharmacists in the pharmacy in a professional manner, and specifically to determine the rationality of prescribing to diabetes mellitus patients at Kimia Farma 382 Pharmacy.
