Abstrak :
Salah satu hal yang dapat mempengaruhi mutu produk adalah terjadinya kontaminasi silang pada produk obat, maka dari itu perlu dilakukan proses validasi pembersihan (cleaning validation). Hasil perhitungan PDE dapat digunakan untuk mengetahui nilai Maximum Allowable Carryover (MACO), yaitu batas yang dapat diterima untuk residu obat dipastikan tidak adanya kontaminasi silang untuk bets berikutnya yang akan diproduksi. Pada tugas khusus ini dilakukan validasi terhadap pembersihan alat-alat sampling yang diterapkan di PT. Mahakam Beta Farma. Validasi yang dilakukan diharapkan dapat menjadi bukti yang menjamin bahwa pembersihan alat-alat sampling akan senantiasa memberikan hasil yang efektif.Validasi pembersihan alat-alat sampling di PT Mahakam Beta Farma dilakukan mengikuti protokol yang tersedia dengan nama Sampling Tools Cleaning Validation. Pengukuran yang telah dilakukan terhadap air bilasan alat-alat sampling Liquid Sampler dan Volumetric Pipette ini menunjukkan hasil yang memenuhi spesifikasi. Hasil pengukuran pH berada pada rentang 5-7 untuk semua alat, hasil pengukuran konduktivitas berada dibawah 1,3 ?S/cm dan hasil pengukuran TOC berada dibawah 500 ppb. Sedangkan untuk parameter jumlah kontaminasi mikroba, hasilnya belum diketahui ketika laporan ini ditulis karena masih berada dalam masa inkubasiMembuat prosedur pembersihan terhadap alat-alat sampling Liquid Sampler dan Volumetric Pipette terhadap senyawa Penanda Y di PT. Mahakam Beta Farma, sehingga memberikan hasil pembersihan yang memenuhi syarat dan konsisten. Industri farmasi memiliki kewajiban untuk menghasilkan obat yang memenuhi persyaratan mutu, keamanan, dan efektivitas. Pada penerapan Change Request, PT Mahakam Beta Farma (MBF) melaksanakan seluruh proses menggunakan dokumen fisik. Dalam rangka meningkatkan efektivitas dan efisiensi proses serta penyimpanan dokumen, PT. MBF melakukan perubahan menjadi sistem yang terkomputerisasi. Berdasarkan CPOB 2018, penggantian operasi manual menjadi sistem komputerisasi tidak boleh mengakibatkan penurunan kualitas produk, kendali proses, atau pemastian mutu, dan tidak meningkatkan risiko terhadap proses. Oleh karena itu dilakukan validasi sistem komputer Change Request PT. MBF. Validasi dilakukan berdasarkan protokol validasi yang telah disetujui berdasarkan User Requirement Specification dan dilaksanakan melalui dua tahap yaitu kualifikasi Intalasi software dan hardware serta kualifikasi operasional software. Pada kualifikasi instalasi terdapat 82 uji yang harus dilaksanakan, terdiri dari 74 uji software, 4 uji hardware, dan 4 uji terkait instruksi kerja. Seluruh uji hardware lolos uji, 3 aspek instruksi kerja lolos uji dan 1 aspek belum terlaksana tetapi pelaksanaannya telah dijadwalkan, 67 aspek software lolos uji sedangkan 7 aspek lainnya lolos dengan catatan dan perbaikan. Pada kualifikasi operasional terdapat 62 tahap uji terkait alur kerja software dan seluruh aspek telah lolos uji. Hal tersebut menunjukan bahwa sistem CR PT. MBF telah memenuhi kualifikasi instalasi dan operasional sehingga dapat dilanjutkan dengan kualifikasi performa. ......The pharmaceutical industry has an obligation to produce drugs that meet the quality, safety, and effectiveness requirements. MBF has passed the installation and operational qualifications and can be continued with performance qualification.

Salah satu tahapan dalam rangkaian validasi sistem komputer adalah kualifikasi instalasi dan kualifikasi operasional. Tujuan dari tugas khusus ini adalah untuk menyusun prosedur kualifikasi instalasi dan kualifikasi operasional dari sistem komputerisasi Master Change Request Online. Sebagai calon apoteker, penting untuk mempelajari mengenai peran-peran apoteker di dunia pekerjaan. Salah satu jenis pekerjaan yang dilakukan apoteker adalah pelayanan resep. Resep adalah permintaan tertulis dari seorang dokter, dokter gigi, dokter hewan yang diberi izin berdasarkan peraturan perundang-undangan yang berlaku kepada apoteker untuk menyiapkan, meracik serta menyerahkan obat kepada pasien. Kajian aspek administratif merupakan skrining awal dalam pelayanan resep di apotek, dalam mencegah kejadian medication error. Sebagai gambaran peran apoteker di dunia kerja dilakukanlah praktik kerja profesi apoteker. Praktik kerja dilakukan di Apotek Roxy sebagai gambaran pelayanan kefarmasian di apotek. Praktik Kerja dilakukan dari Bulan Juli hingga Desember 2021. Selain di bidang pelayanan, praktik kerja juga dilakukan di PT. Mahakam Beta Farma sebagai gambaran peran apoteker di Industri Obat. Pada praktik kerja di PT. Mahakam Beta Farma, pembelajaran dilakukan secara pengamatan langsung mengenai kegiatan yang dilakukan oleh industri farmasi. Selain belajar secara langsung dari mengamati, calon apoteker juga diberikan kesempatan untuk menganalisis masalah yang ada di masing-masing tempat kerja. Oleh karena itu, laporan praktik kerja ini akan membahas mengenai masalah beserta solusi yang diajukan di tiap tempat praktik kerja. ......It is critical for a future pharmacist to understand the duties of pharmacists in the workplace. The investigation of administrative elements is a first step in preventing pharmaceutical errors in pharmacies' prescription services. A pharmacist's professional work practice is demonstrated as an example of pharmacists' function in the workplace. Work methods are demonstrated at the Roxy Pharmacy as an example of the pharmacy's pharmaceutical services. From July to December 2021, work practices will be carried out. Work practices are also carried out at PT. Mahakam Beta Farma as an indication of the function of pharmacists in the drug industry, in addition to the service sector. Learning takes place in the work environment at PT. Mahakam Beta Farma through direct observation of pharmaceutical business activities. Asma merupakan penyakit gangguan nafas dengan intensitas dan durasi yang bervariasi. Penyakit ini menjadi penyakit yang diderita oleh 334 juta jiwa pada 2017. Pengobatan asma perlu dilakukan dengan tepat melalui pengkajian resep yang sesuai dengan standar peraturan yang berlaku. Laporan ini bertujuan agar calon apoteker dapat memahami pelaksanaan pengkajian resep asma yang dilaksanakan di Apotek Roxy Pondok Labu. Pada laporan ini terdapat dua resep yang dikaji dengan masing-masing resep terdiri lebih dari satu obat. Resep pertama tertuliskan obat berupa Bricasma dan Lansoprazole. Hasil kajian resep pertama ini menunjukkan aspek administratif resep belum lengkap, aspek farmasetis yang sesuai, dan pada aspek klinis perlu komunikasi lebih lanjut kepada dokter terkait dosis yang diberikan. Resep kedua terdiri dari Ventolin dan Pulmicort yang diracik menjadi nebulizer dengan larutan tambahan berupa NaCl. Kajian yang dilakukan pada resep ini juga menunjukkan aspek administrative yang belum lengkap, aspek farmasetis sudah sesuai, dan pada aspek klinis dibutuhkan konfirmasi kepada dokter yang menulis resep. Kedua resep ini membutuhkan konfirmasi kepada dokter terkait aspek administratif dan aspek klinis. Selain itu, untuk menunjang keberhasilan terapi, seorang apoteker juga harus membekali pasien dengan pengetahuan terkait cara penggunaan obat. Oleh karena itu pengetahuan yang baik akan ilmu kefarmasian dan keterampilan dalam berkomunikasi dengan pasien diperlukan oleh seorang apoteker untuk dapat mengkaji resep dari dokter dan mengkomunikasikan penggunaannya kepada pasien untuk keberhasilan terapi. ......Asthma is a respiratory disorder with varying intensity and duration. This disease affected 334 million people in 2017. Asthma treatment needs to be carried out properly through a review of prescriptions in accordance with applicable regulatory standards. This report aims to enable prospective pharmacists to understand the implementation of the asthma prescription assessment carried out at Apotek Roxy Pondok Labu. In this report, there are two recipes that were reviewed with each recipe consisting of more than one drug. The first prescription listed drugs in the form of Bricasma and Lansoprazole. The results of this first prescription review showed that the administrative aspect of the prescription is incomplete, the pharmaceutical aspect was compatible, and on the clinical aspect, further communication doctor was needed regarding the dose given. The second recipe consisted of Ventolin and Pulmicort which were mixed into a nebulizer with additional NaCl solution. The study conducted on this prescription also showed that the administrative aspects were incomplete, the pharmaceutical aspects were compatible, and the clinical aspects required confirmation to the doctor regarding the dose used. Both of these prescriptions required confirmation from the doctor regarding administrative and clinical aspects. In addition, to support the success of therapy, a pharmacist must also provide patients with knowledge related to how to use drugs. Therefore, a good knowledge of pharmacy science and skills in communicating with patients are needed by a pharmacist to be able to review prescriptions from doctors and communicate how to use the drug to patients for success therapy.

Profesi apoteker menjadi profesi memiliki peran dan tanggung jawab inti dalam terlaksananya kegiatan kefarmasian dengan baik. Latar belakang pendidikan apoteker membuatnya menjadi tenaga yang memiliki pengetahuan spesifik mengenai kefarmasian untuk mengisi peran dan memenuhi tanggung jawab tersebut. Selain dibekali dengan ilmu kefarmasian, apoteker juga dibekali dengan praktik kerja profesi untuk memberikan pengalaman langsung dalam proses kegiatan praktik kefarmasian, baik pada farmasi klinis, industri farmasi, dan pemerintahan. Pada laporan ini dibahas mengenai Praktik Kerja Profesi Apoteker di Apotek Roxy Pondok Labu, Periode Februari 2022 dan PT Mahakam Beta Farma, Periode Maret-April 2022. Adapun topik yang menjadi pembahasan dalam laporan ini yaitu pengkajian resep obat asma yang terdapat pada apotek tersebut dan validasi pemberihan alat-alat sampling di industri farmasi. Pengalaman praktik kerja ini diharapkan dapat menjadi bekal bagi apoteker untuk menjalankan kegiatan profesinya nanti. ...... In addition to being equipped with pharmaceutical knowledge, pharmacists are also equipped with professional work practices to provide direct experience in the process of pharmaceutical practice activities, not only in clinical pharmacy but also in pharmaceutical industry and government. This report discusses the Professional Internship of Pharmacists at Apotek Roxy Pondok Labu on February 2022 and PT Mahakam Beta Farma on March-April 2022. The topics discussed in this report are the review of drug prescription for asthmam in the pharmacy and sampling tools cleaning validation in the pharmaceutical industry. This practical work experience is expected to be a provision for pharmacists to carry out their professional activities in the future.
