"Kenyamanan merupakan salah satu aspek penting dalam perawatan luka diabetes mellitus (DM). Penelitian ini bertujuan menganalisis ekspresi transforming growth factor beta 1 (TGF â1) dan kadar kortisol pada perawatan luka teknik modern dan konvensional pada luka DM dikaitkan dengan aspek kenyamanan. Penelitian menggunakan true experimental design dengan metode pengumpulan sampel secara stratified random sampling. Pengukuran ekspresi TGF â1 dan kadar kortisol dilakukan pada hari ke 0 (pretest) dan 4 (posttest). Sampel yang diambil berasal dari pasien luka kaki DM di Rumah Sakit Saiful Anwar Malang. Ekspresi TGF â1 diukur dengan metode imunohistokimia, sedangkan pengukuran kadar kortisol dilakukan dengan metode ELISA di laboratorium Fisiologi dan Histologi FK Universitas Brawijaya Malang. Hasil penelitian menunjukkan bahwa pada kelompok modern terjadi peningkatan ekspresi TGF â1, sedangkan pada kelompok konvensional terjadi penurunan ekspresi TGF â1. Kadar kortisol pada kelompok modern menunjukkan penurunan lebih besar dibandingkan kelompok konvensional. Hasil uji t menunjukkan ada perbedaan yang signifikan antara perawatan luka teknik modern dan konvensional terhadap ekspresi TGF â1 dan kadar kortisol pada luka DM (p value < 0,05). Hasil uji korelasi Pearson menunjukkan hubungan yang signifikan antara perubahan ekspresi TGF â 1 dengan perubahan kadar kortisol (p = 0,028). Dapat disimpulkan bahwa teknik perawatan luka secara modern mampu meningkatkan ekspresi TGF â1 dan menurunkan kadar kortisol dibandingkan teknik konvensional.

Comfort is one among several aspects that should be considered in the treatment of diabetic wounds. This study aimed to analyze the expression of TGF β1 and the level of cortisol in modern and conventional wound care techniques of diabetic wounds. TGF β1 expression and cortisol levels were measured on day 0 (pretest) and 4 (posttest). Samples were taken from patients with diabetic ulcer in the Saiful Anwar District Hospital at Malang. The expression of TGF β1 was measured by immunohistochemical methods in the Department of Physiology, Brawijaya University Faculty of Medicine. Cortisol level was measured with ELISA method. The results obtained from the modern group were increased TGF β1 expression and decreased cortisol level. The conventional group yielded decreased TGF β1 expression and decreased cortisol level. The cortisol level decrease was greater in the modern group. T test results showed no significant differences of modern wound care techniques and conventional on the expression of TGF β1 and cortisol levels in diabetic wounds (p value < 0,05). Pearson correlation test results showed a significant relationship between changes in cortisol levels with changes in expression of TGF β1 (p = 0,028). It can be concluded that the techniques of modern wound care is more able to increase the expression of TGF β1 and to decrease the cortisol levels compared with conventional techniques."

"Background: Diabetes Mellitus is a chronic disease characterised by elevated levels of blood glucose known as
hyperglycaemia. Diabetes is due to impaired insulin action in the metabolism of glucose and can result in impaired
wound healing. Excessive production of pro-inflammatory cytokines, an increased number of macrophages and
neutrophils, and decreased levels of transforming growth factor ? beta 1 (TGF-β1) serum can be characteristic of
impaired wound healing. This study aims to determine the effects of squid extract on certain wound parameters such as
levels of tumour necrosis factor ? alpha (TNF-α), and TGF-β1 serum and the number of macrophages and neutrophils.
Methods: This was a post-test only, randomized controlled group study that was conducted on male Wistar rats.
Experimental animals were divided into 6 groups; (1) normal wound with standard diet, (2) diabetic wound with
standard diet, (3) diabetic wound with chitosan supplement, (4) diabetic wound given squid extract orally once a day,
(5) diabetic wound given squid extract orally twice a day, and (6) diabetic wound given squid extract orally once every
two days. Levels of TNF-α and TGF-β1 serum were observed using Enzyme-Linked Immunosorbent Assay.
Haematocylin and eosin staining was used to observed macrophage and neutrophil counts. All data was analysed
statistically by one-way analysis of variance. Results: TNF-α serum levels showed a significant decrease (p < 0.05) in
subjects that received squid extract orally once every two days. The mean levels of TGF-β1 showed no significant
differences. The mean number of macrophage cells showed a significant decrease (p < 0.05) in all treatment groups.
The mean number of neutrophil cells also showed significant decrease (p < 0.05) in all treatment groups. Conclusions:
Squid extract is effective in lowering the TNF-α serum levels and the number of macrophages and neutrophils cells in
Wistar rats. However, there were insignificant findings on increasing levels of TGF-β1 serum. This data suggests that
squid extract is most effective during the inflammatory phase of wound healing which takes places about 2-4 days after
wound creation."

"Peningkatan gula darah pada pasien COVID-19 menyebabkan terganggunya fungsi metabolik, tingkat keparahan penyakit serta komplikasi pada pasien. Peningkatan gula darah pada pasien COVID-19 dapat dicegah melalui langkah yang tepat dalam mengontrol glikemik dengan baik. Penelitian ini bertujuan untuk mengenali faktor yang berhubungan dengan peningkatan gula darah pada pasien COVID-19 tanpa riwayat Diabetes Melitus (DM). Metode penelitan adalah deskriptif dengan desain analitik retrospektif, melibatkan 45 responden, analisa data menggunakan rank spearman, chi square dan regresi logistik. Hasil penelitian menunjukkan, mayoritas responden berumur 46-65 tahun, berjenis kelamin perempuan, memiliki stres dan aktivitas ringan. Secara klinis memiliki keparahan COVID-19 derajat sedang serta tidak memiliki komorbid. Sebagian besar responden memiliki pola diet yang tidak tepat, tidak mempunyai riwayat keluarga dengan DM, dan tidak mempunyai riwayat pengobatan yang dapat meningkatkan gula darah. Terdapat hubungan yang signifikan antara stres (p 0,003), aktivitas (p 0,017), derajat keparahan (p 0,016), komorbid (p 0,037), dan riwayat keluarga (p 0,007) dengan peningkatan gula darah pada pasien COVID-19 tanpa riwayat DM, dimana aktivitas merupakan faktor yang paling dominan (p 0.020:OR 8,465). Perlunya dilakukan intervensi keperawatan mandiri dalam meningkatkan asuhan keperawatan pada pasien COVID-19. Deteksi dini dengan tepat, manajemen stres dan aktivitas yang rutin diharapkan dapat menurunkan risiko disregulasi glukosa dan menstabilkan kadar glikemik untuk mencegah komplikasi lebih lanjut.
......ncreased blood sugar in COVID-19 patients causes disruption of metabolic function, disease severity and complications in patients. Elevated c in COVID-19 patients can be prevented through proper steps in controlling glycemic well. This study aims to identify factors associated with increased blood sugar in COVID-19 patients without a history of Diabetes Mellitus (DM). The research method is descriptive with retrospective analytic design, involving 45 respondents, data analysis using Spearman rank, chi square and logistic regression. The results showed that the majority of respondents were 46-65 years old, female, had stress and had light activities. Clinically, he has moderate severity of COVID-19 and has no comorbidities. Most of the respondents have inappropriate diet patterns, do not have a family history of DM, and do not have a history of medication that can increase blood sugar. There was a significant relationship between stress (p 0.003), activity (p 0.017), severity (p 0.016), comorbidities (p 0.037), and family history (p 0.007) with elevated blood sugar in COVID-19 patients without a history of DM, where activity is the most dominant factor (p 0.020:OR 8.465). The need for independent nursing interventions in improving nursing care for COVID-19 patients. Appropriate early detection, stress management and routine activities are expected to reduce the risk of glucose dysregulation and stabilize glycemic levels to prevent further complications.

"Gangguan sirkulasi kaki pada penderita DM mengakibatkan proses penyembuhan ulkus diabetik tertunda. Tujuan dari penelitian ini yaitu mengidentifikasi pengaruh Ankle Range of Motion (ROM) Exercise terhadap penyembuhan luka pada pasien ulkus diabetik. Sampel dalam penelitian ini berjumlah 28 responden yang dibagi menjadi 14 responden kelompok intervensi dan 14 responden kelompok kontrol. Metode penelitan ini adalah quasi eksperimental design dengan pendekatan Pretest and Posttest With Control Group Design. Kelompok intervensi diberikan perlakuan ankle Range of Motion (ROM) exercise dan perawatan luka modern dressing, sedangkan kelompok kontrol hanya diberikan perawatan luka modern dressing saja. Ankle Range of Motion (ROM) exercise terdiri dari empat gerakan yaitu plantarfleksi, dorsofleksi, inversi dan eversi. Latihan ini termasuk dalam latihan tidak menahan beban dan aman dilakukan pada penderita ulkus diabetik. Hasil penelitian menunjukan bahwa ada perbedaan yang signifikan rerata selisih skor penyembuhan luka antara kelompok intervensi dengan kontrol dengan p value 0,000 (< 0,05). Hasil uji variabel confounding menunjukkan bahwa tidak ada hubungan antara riwayat merokok terhadap skor penyembuhan luka pada pasien ulkus diabetik dengan p value 0,143 (> 0,05). Ankle Range of Motion (ROM) Exercise diharapkan dapat dijadikan terapi tambahan dalam manajemen luka pada pasien ulkus diabetik untuk mempercepat proses penyembuhan luka
......Impaired foot circulation in diabetic patients causes the healing process of diabetic ulcers to be delayed. The purpose of this study was to identify the effect of Ankle Range of Motion (ROM) Exercise on wound healing in diabetic ulcer patients. The sample in this study amounted to 28 respondents who were divided into 14 respondents in the intervention group and 14 respondents in the control group. This research method is a quasi-experimental design with a Pretest and Posttest approach with Control Group Design. The intervention group was treated with ankle Range of Motion (ROM) exercise and modern wound dressings, while the control group was only given modern wound dressings. Ankle Range of Motion (ROM) exercise consists of four movements, namely plantarflexion, dorsiflexion, inversion and eversion. This exercise is included in non-weight-bearing exercises and is safe for diabetic ulcer sufferers. The results showed that there was a significant difference in the mean difference in wound healing scores between the intervention group and the control group with a p value of 0.000 (<0.05). The results of the confounding variable test showed that there was no relationship between smoking history and wound healing scores in diabetic ulcer patients with a p value of 0.143 (> 0.05). Ankle Range of Motion (ROM) Exercise is expected to be used as additional therapy in wound management in diabetic ulcer patients to accelerate the wound healing process."
