"Latar belakang: Infeksi TB (TB) dan Human Immunodeficiency Virus (HIV) masih menjadi masalah penyakit menular terbanyak di negara berkembang termasuk Indonesia. Insidens nasional koinfeksi TB-HIV sebanyak 36% dan angka kematian koinfeksi TB-HIV 9,3%. Insidens nasional TB resistan obat yang masih menjadi ancaman saat ini sebesar 23% dan semakin mempersulit keadaan tersebut. Keberhasilan pengobatan koinfeksi TB resistan obat-HIV di Indonesia sejauh ini masih belum diketahui.
Tujuan: Penelitian ini bertujuan untuk memprediksi keberhasilan pengobatan pasien koinfeksi TB resistan obat HIV serta faktor yang mempengaruhi.
Metode: Penelitian ini adalah penelitian retrospektif potong lintang menggunakan data rekam medis, eTB manager dan data penunjang pasien koinfeksi TB resistan obat-HIV yang berobat di Rumah Sakit Pusat Rujukan Respirasi Nasional Persahabatan Jakarta, Indonesia sejak tahun 2013-2019 untuk evaluasi keberhasilan pengobatan dan faktor yang mempengaruhi.
Hasil: Pada penelitian ini didapatkan 63 pasien koinfeksi TB resistan obat HIV. Paisen terbanyak laki-laki (77,8%) dengan median usia 32 tahun. Pada akhir pengobatan didapatkan sebanyak 16 pasien (25,4%) berhasil menyelesaikan pengobatan. Faktor yang berkaitan terhadap keberhasilan pengobatan adalah status pekerjaan (p=0,000), status pernikahan (p=0,035), gejala akhir pengobatan (p=0,000), waktu konversi dan total lama pengobatan (p=0,013).
Kesimpulan: Pada penelitian ini didapatkan bahwa keberhasilan pengobatan pasien koinfeksi TB resistan obat HIV masih rendah (25,4%) yang dipengaruhi oleh faktor status pekerjaan, status pernikahan, gejala akhir pengobatan, waktu konversi dan total lama pengobatan.
.....Background: Tuberculosis (TB) and Human Immunodeficiency Virus (HIV) infections remain the largest communicable disease problem in developing countries, including Indonesia. The national incidence of TB-HIV co-infection is 36% and the mortality rate of TB-HIV coinfections is 9,3%. The threat of drug resistance TB which it national incidence is 23%, further complicate this situation. The success rate of drug resistant TB-HIV coinfection treatment in Indonesia, to our extent, is yet to be known.
Aim: This study aims to estimate the treatment success rate of drug resistance TB- HIV coinfection and realted factors influencing its outcomes.
Method: This retrospective cross sectional study used hospital medical records, eTB manager and additional data of TB-HIV coinfected patients treated at Persahabatan Hospital Jakarta, Indonesia, between 2013 and 2019, to observe the treatment success and its related factors.
Result: Sixty-three patients were drug resistance TB-HIV coinfected. Patients mostly men (77,8%) and were of median age of 32 years old. By the end of their treatments. 16 patients (25,4%) were completing their treatment. Factor correlated to the treatment success were employement status (p=0,046), marital status (p=0,035), final symptoms by the end of their treatment (p=0.000), convertion time and treatment total duration (p=0,013).
Conclusion: This study found that the treatment success rate of drug resistance TB-HIV coinfection was 25,4%. Its contributing factors included employment status, marital status, final symptoms by the end of their treatment, convertion time and treatment total duration."

"ABSTRAK
Pendahuluan
Faktor-faktor lingkungan seperti berbagai macam partikel dan gas dari emisi kendaraan seperti karbon dioksida, karbon monoksida, sulfur, benzen, nitrogen dioksida, nitrit oksid dan asap dipercaya memiliki peran yang bermakna dalam meningkatkan terjadinya penyakit pernapasan. Polisi lalu lintas yang bekerja di area lalu lintas yang padat selama beberapa tahun memiliki risiko terpajan polusi udara.
Tujuan
Studi ini bertujuan untuk mengetahui gambaran faal paru dan faktor-faktor yang mempengaruhi pada polisi lalu lintas yang bekerja di wilayah Jakarta Utara.
Metode
Studi ini menggunakan metode potong lintang (cross-sectional study method). Polisi lalu lintas yang bekerja di wilayah jakarta Utara diwawancara menggunakan kuesioner yang berhubungan dengan kesehatan rerpirasi, gambaran klinis, pemeriksaan CO ekspirasi, foto toraks dan pemeriksaan faal paru dengan spirometri. Pengumpulan data dilakukan antara bulan Oktober 2012 hingga November 2012.
Hasil
Pada studi ini didapatkan dari 90 subjek kelompok umur paling banyak didapatkan pada usia 41-50 tahun (37.8%), memiliki status gizi over weight ( 55.6%) sebagian besar perokok ringan (47.8%). Kami menemukan kelainan faal paru berupa restriksi ringan 6.7%, restriksi sedang 1.1% dan obstruksi ringan 1.1 %. Variabel yang berpengaruh secara bermakna dengan faal paru adalah usia dan indeks massa tubuh (BMI) sedangkan kebiasaan merokok dan penggunaan masker secara statistik tidak ditemukan hubungan yang bermakna dengan faal paru.
Kesimpulan
Studi ini menunjukkan bahwa prevalens gangguan faal paru adalah 8.9% dan variabel yang berpengaruh adalah usia dan indeks massa tubuh.

ABSTRACT
Introduction:
Environmental factors such as various particles and gases from vehicular emission like carbon dioxide, carbon monoxide, sulphur, benzene, nitrogen dioxide, nitric oxide and black smoke are believed to play a significant role in the development of respiratory diseases. Traffic policemen who work in the busy traffic signal areas for years together are exposed to the risk of air traffic pollution.
Purposes:
This study was conducted profiles and factors influencing lung fuction of traffic pollution on traffic police working in North Jakarta Distric.
Method:
This study used cross-sectional study method. The traffic police working in North Jakarta Distric were given a predetermined respiratory health questionnaire, their clinical profile,CO expiration, chest x ray and lung functions were measured. We collected data within October 2012 until November 2012.
Result:
This study showed from 90 subject with predominant age group between 41-50 years old (37.8%), over weight (55.6%) and mild smoker (47.8%). We found that mild restriction 6.7%, moderate restriction 1.1% and mild obstruction 1.1% subject. There were statistically significant age (p=0.019) and BMI (p=0.012) but no statistically significant associated smoking (p=0.145) ,used masker (p=1.000) with lung function.
Conclusion:
Study found that the prevalence of decreased pulmonary function is 8.9% and only age and BMI were associated with lung function."

"Latar Belakang: Aspergilosis paru kronik (APK) dapat menjadi komplikasi infeksi tuberkulosis (TB) paru yang telah diobati. Gejala klinis APK dengan TB paru sangat mirip, sehingga sulit dibedakan. Diagnosis APK ditetapkan sesuai konsensus berdasarkan gejala klinis, hasil pemeriksaan radiologi dan laboratorium mikologi. Pemeriksaan antibodi dengan imunoglobulin G (IgG) spesifik Aspergillus menggunakan metode Enzyme-linked immunosorbent assay (ELISA) dapat membantu diagnosis APK. Penelitian ini bertujuan untuk mengetahui karakteristik klinis, profil IgG spesifik Aspergillus, serta prevalensi APK pada pasien bekas TB di beberapa rumah sakit di Jakarta.
Metode: Penelitian prospektif dengan desain potong lintang ini dilakukan pada April 2019 - Februari 2020. Pemilihan subjek dilakukan dengan metode consecutive sampling. Subjek merupakan pasien bekas TB yang berasal dari poli rawat jalan dan rawat inap Rumah Sakit Umum Pusat Rujukan Respirasi Persahabatan Jakarta dan Rumah Sakit Graha Permata Ibu. Pemeriksaan IgG spesifik Aspergillus dan biakan sputum jamur dilakukan di Laboratorium Departemen Parasitologi FKUI.
Hasil: Dari 97 pasien yang sesuai dengan kriteria penerimaan, 66 pasien (68%) berjenis kelamin laki-laki dan rerata usia 51,8±13,6 tahun. Gejala klinis lebih dari 3 bulan yang dilaporkan berupa mudah lelah (38,4%), sesak napas (34,02%), batuk (30,93%), hemoptisis (27,84%), penurunan berat badan (23,71%), dan nyeri dada (19,6%). Gambaran radiologi terkait APK berupa ektasis (57,8%), kavitas (27,8%), penebalan pleura (26,8%), fibrosis parakavitas (18,6%), dan bola jamur (6,2%). Hasil pemeriksaan IgG spesifik Aspergillus positif dilaporkan pada 51 pasien (52,6%), sedangkan biakan sputum jamur Aspergillus didapatkan pada 43 pasien (44,3%). Berdasarkan analisis hasil-hasil pemeriksaan tersebut, diagnosis APK ditegakkan pada 28 pasien (28,9%).
Kesimpulan: Profil IgG spesifik Aspergillus pada 97 pasien bekas TB dalam penelitian ini menunjukkan hasil positif pada 51 pasien (52,6%). Gejala klinis lebih dari 3 bulan yang dilaporkan berupa batuk lama, hemoptisis, penurunan berat badan, mudah lelah, dan sesak napas. Gambaran radiologi terkait APK berupa ektasis, kavitas, penebalan pleura, fibrosis parakavitas, dan bola jamur. Prevalensi APK berdasarkan hasil-hasil pemeriksaan tersebut adalah 28,9%.

Introduction: Chronic pulmonary aspergillosis (CPA) might become a complication of pulmonary tuberculosis (TB) that has been treated. The clinical symptoms of CPA can resemble with PTB, making it difficult to distinguish. The diagnosis of CPA is determined by the consensus based on clinical symptoms, radiological features, and mycological results. Antibody detection with Aspergillus- specific immunoglobulin G (IgG) using the Enzyme-linked immunosorbent assay (ELISA) method can contribute to CPA diagnosis. This study aims to determine the clinical characteristics, Aspergillus-specific IgG detection, and the prevalence of CPA in prior TB patients at several hospitals in Jakarta.
Method: This prospective cross-sectional study was conducted in April 2019 - February 2020. Patients recruitment was carried out by consecutive sampling method. Subjects were prior TB patients at Persahabatan National Respiratory Referral Hospital and Graha Permata Ibu Hospital. Detection of Aspergillus- specific IgG and fungal cultures from sputum were carried out in the Laboratory of the Parasitology Department, FMUI.
Results: Of 97 patients recruited according to inclusion criteria, 66 patients (68%) were male and the mean age was 51.8 ± 13.6 years. The clinical symptoms of more than 3 months were fatigue (38.4%), shortness of breath (34.02%), cough (30.93%), hemoptysis (27.84%), weight loss (23, 71%), and chest pain (19,6%). Radiological features associated with CPA were ectasis (57.8%), cavity (27.8%), pleural thickening (26.8%), para-cavitary fibrosis (18.6%), and fungal ball (6.2%). The Aspergillus-specific IgG positive were reported in 51 patients (52.6%), whereas Aspergillus sputum cultures were found in 43 patients (44.3%). Based on the analysis of those examinations, the diagnosis of CPA was determined in 28 patients (28.9%).
Conclusion: The detection of Aspergillus-specific IgG in 97 prior TB patients showed the positive results in 51 patients (52.6%). The clinical symptoms more than three months were fatigue, shortness of breath, cough, hemoptysis, and weight loss. The radiological features related to CPA were ectasis, cavitary lesions, pleural thickening, paracavitary fibrosis, and fungal ball. The prevalence of CPA based on those examinations was 28.9%.

"Pendahuluan: Depresi mempengaruhi 45,19% pasien tuberkulosis paru (TB) dalam kepatuhan terhadap pengobatan, yang menyebabkan peningkatan morbiditas dan kematian, resistensi obat yang meningkat, serta penularan penyakit yang terus berlanjut. Usia, jenis kelamin, pendidikan, pendapatan, status gizi, komorbiditas, fase terapi, dan status HIV adalah faktor-faktor yang dapat berkontribusi terhadap depresi pada pasien TB. Infeksi Mycobacterium tuberculosis menyebabkan peradangan sistemik, mengubah respons pusat sistem kekebalan tubuh di sistem saraf pusat, mengaktifkan sumsum tulang belakang-hipotalamus-kelenjar adrenal (HPA) dan saraf simpatis, serta berkontribusi terhadap masalah psikiatri. Komposisi asam lemak, termasuk jumlah tinggi EPA dan DHA, mempengaruhi fungsi sel dengan memodifikasi pola produksi eikosanoid, resolvin, dan protektin. Selain itu, fluiditas membran sel yang meningkat dengan peningkatan asam lemak omega-3 dibandingkan dengan asam lemak omega-6 mempengaruhi kejadian depresi.
Metode: Studi ini merupakan studi potong lintang terhadap 99 orang dengan TB paru. Data dikumpulkan menggunakan Semi-Kuantitatif Food Frequency Questionnaire (SQ-FFQ), pengukuran antropometri, dan Beck Depression Inventory-II (BDI-II).
Hasil: Analisis korelasi menggunakan uji Spearman menunjukkan rasio asupan omega-6/omega-3 PUFA sebesar 7,78 ± 1,13, dengan nilai median skor depresi sebesar 9 (10-36). Tidak ada korelasi antara asupan omega-6/omega-3 PUFA dan skor depresi (r=0,063; p = 0,534).
Kesimpulan: Tidak ada korelasi antara rasio asupan omega-6/omega-3 PUFA dan skor depresi pada pasien TB paru. 

Introduction: Depression affects 45.19% of pulmonary tuberculosis (TB) patients in their adherence to treatment, leading to increased morbidity, mortality, drug resistance, and disease transmission. Factors like age, gender, education, income, nutrition, comorbidities, therapy phase, and HIV status contribute to TB-related depression. Mycobacterium tuberculosis infection induces systemic inflammation, alters the immune response in the central nervous system, activates the hypothalamic-pituitary-adrenal (HPA) axis and sympathetic nerves, and influences psychiatric issues. Fatty acid composition, particularly high levels of EPA and DHA, modifies cellular function by affecting eicosanoid, resolvin, and protectin production. The greater cell membrane fluidity with omega-3 fatty acids compared to omega-6 fatty acids affects depression occurrence.
Methods: A cross-sectional study of 99 individuals with pulmonary TB was conducted. Data was collected using the Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ), anthropometric measurements, and Beck Depression Inventory-II (BDI-II).
Results: Spearman correlation analysis revealed an omega-6/omega-3 PUFA intake ratio of 7.78 ± 1.13, with a median depression score of 9 (10-36). No correlation was found between omega-6/omega-3 PUFA intake and depression score (r=0.063; p = 0.534).
Conclusion: No correlation exists between the omega-6/omega-3 PUFA intake ratio and depression scores in pulmonary TB patients.

"Salah satu karakteristik klinis yang sering diamati pada TB paru adalah adanya kavitas paru pada pemeriksaan radiologis dada. Kavitas paru akan menyebabkan prognosis lebih buruk akibat keterlambatan konversi kultur sputum, hasil klinis yang buruk, dan penularan infeksi yang lebih tinggi. Beberapa faktor yang telah ditemukan berkaitan dengan kavitas paru adalah usia tua, jenis kelamin laki-laki, penyakit penyerta diabetes mellitus, dan malnutrisi. Prevalensi malnutrisi pada pasien dengan TB diperkirakan berkisar antara 50% sampai 57%, dan malnutrisi dikaitkan dengan dua kali lipat risiko kematian. Telah lama diketahui bahwa terdapat hubungan antara TB dan malnutrisi, tetapi dampak malnutrisi terhadap derajat keparahan TB, yang dilihat dari adanya kaviats paru, masih kurang diketahui dan data yang telah ada masih saling bertentangan. Penelitian ini bertujuan untuk mengetahui hubungan antara status gizi dan kavitas paru pada pasien tuberkulosis paru di Rumah Sakit Umum Pusat Persahabatan. Penelitian ini merupakan penelitian potong lintang. Sebanyak 134 pasien yang memenuhi kriteria menjadi subjek penelitian di Instalasi Rawat Jalan dan Rawat Inap Rumah Sakit Umum Pusat Persahabatan. Pasien pada penelitian ini umumnya berjenis kelamin laki-laki (61,9%) dan berusia 18-59 tahun (92,5%). Mayoritas subjek penelitian termasuk dalam kategori status gizi SGA B (malnutrisi ringan-sedang) sebanyak 77 orang (57,5%), SGA A (status gizi baik) sebanyak 35 orang (26,1%), dan SGA C (malnutrisi berat) sebesar 22 orang (16,4%). Proporsi kavitas paru pada pasien TB paru dalam penelitian ini sebanyak 42 orang (31,3%). Penelitian ini mendapatkan hubungan bermakna secara statistik antara status gizi berdasarkan SGA dan kavitas paru (OR=6,933; 95%CI=1,986-24,205; p=0,002; aOR=7,303 (95%CI=2,060-25,890; p=0,002). Variabel lain yang mempengaruhi terbentuknya kavitas paru adalah pemeriksaan bakteriologis (p=0,016), TB resisten obat (p<0,001), dan perubahan BB (p=0,033). Analisis multivariat mendapatkan bahwa pemodelan dapat memenuhi 29,3% faktor prediktor kejadian kolonisasi dan setelah dimasukkan ke dalam perhitungan, maka probabilitas seorang pasien yang mengalami TB resisten obat dan malnutrisi untuk pembentukan kavitas paru adalah sebesar 95,16%. Kesimpulan: Terdapat hubungan antara status gizi dan kavitas paru pada pasien tuberkulosis paru di Rumah Sakit Umum Pusat Persahabatan.

One of the clinical characteristics that is often found in pulmonary TB is the presence of lung cavities on chest radiological examination. Lung cavities will lead to a worse prognosis due to delayed sputum culture conversion, poor clinical outcome, and higher transmission of infection. Several factors that have been found to be related to the lung cavity are elder age, male gender, comorbid diabetes mellitus, and malnutrition. The prevalence of malnutrition itself in patients with TB is estimated to range from 50% to 57%, and malnutrition is associated with a twofold risk of death. It has long been known that there is a relationship between TB and malnutrition, but the impact of malnutrition on the severity of TB, which is observed from lung cavity presence, is still poorly understood and the available data are conflicting. This study aims to determine the relationship between nutritional status and lung cavity in pulmonary tuberculosis patients at Persahabatan General Hospital. This research is a cross-sectional study. A total of 134 patients who met the criteria became research subjects at the Outpatient and Inpatient Department at the Persahabatan General Hospital. Patients in this study were generally male (61.9%) and aged 18-59 years (92.5%). The majority of research subjects were included in the SGA B (mild-moderate malnutrition) category of 77 people (57.5%), SGA A (good nutritional status) of 35 people (26.1%), and SGA C (severe malnutrition). by 22 people (16.4%). The proportion of lung cavities in pulmonary TB patients in this study were 42 people (31.3%). This study found a statistically significant relationship between nutritional status based on SGA and lung cavities (OR=6.933; 95%CI=1.986-24.205; p=0.002; aOR=7.303 (95%CI=2.060-25.890; p=0.002). Variables Other factors that influenced the formation of lung cavities were bacteriological examination (p=0.016), drug-resistant TB (p<0.001), and changes in weight (p=0.033). Multivariate analysis found that modeling could fulfill 29.3% of the predictors of colonization and after taken into account, the probability of a patient with drug-resistant TB and malnutrition for lung cavity formation is 95.16%. Conclusion: There is a relationship between nutritional status and lung cavity in pulmonary tuberculosis patients at Persahabatan General Hospital."

"Latar Belakang: Tuberkulosis resistan obat (TB-RO) masih menjadi masalah kesehatan global, penyakit utama penyebab kematian kasus infeksi tunggal terbanyak setelah Corona virus disease 2019 (COVID-19). Tahun 2021 secara global terdapat 167.000 orang dengan TB Multi Drug Resistant/Rifampicin Resistant (MDR/RR) terkonfirmasi, 162.000 memulai terapi. Angka keberhasilan global pengobatan TB RR/MDR sebesar 60%, sedangkan di Indonesia sebesar 47%. WHO merekomendasikan paduan bebas injeksi untuk TB-RO tahun 2019 meningkatkan angka keberhasilan 60% menjadi 73% dan menurunkan angka putus berobat 17,3% menjadi 9,9%. Pengobatan OAT all-oral direkomendasikan di Indonesia sejak 2020 termasuk paduan individual. Penelitian ini bertujuan untuk mengetahui luaran pengobatan TB-RO dengan paduan individual all-oral serta faktor apa saja yang memengaruhi keberhasilan pengobatan TB-RO di RSUP Persahabatan.
Metode: Desain penelitian ini adalah kohort retrospektif menggunakan data rekam medis fisik dan elektronik, dilakukan di Poli TB-RO RSUP Persahabatan Oktober 2022, dengan teknik total sampling. Subjek penelitian adalah pasien TB-RO yang mendapatkan paduan individual all-oral dan memulai terapi Januari2020-Desember 2021 di poli TB-RO RSUP Persahabatan yang memenuhi kriteria penelitian.
Hasil: Didapatkan 162 subjek penelitian dengan karakteristik median usia 44 (18−74) tahun, 61,7% laki-laki dan 52,5% subjek dengan IMT<18,5 kg/m2. Komorbid terbanyak adalah DM tipe 2 dengan proporsi 37%. Jenis resistansi terbanyak TB-MDR 45,1% dengan proporsi bacterial load terbanyak BTA negatif (25,9%). Proporsi konversi biakan sputum ≤ 2 bulan 56,2%. Terdapat 86,4% subjek mengalami intoleransi obat dengan proporsi terbanyak gastrointestinal 66,2%. Luaran berhasil sebesar 43,2%, meninggal 40,1%, putus berobat 13,6% dan gagal 3,1%. Analisis multivariat mendapatkan bacterial load dan kecepatan konversi merupakan faktor-faktor yang memengaruhi keberhasilan pengobatan TB-RO.
Kesimpulan: Luaran berhasil pasien TB-RO yang menggunakan paduan individual all-oral 43,2%, meninggal 40,12%, gagal 3,09% dan putus berobat 13,6%. Bacterial load,BTA ≤1+ dan kecepatan konversi ≤ 2 bulan merupakan faktor yang memengaruhi keberhasilan pengobatan TB-RO dengan paduan individual all-oral. Nilai odds masing-masing faktor berturut-turut aOR:2,35 (95%IK: 1,12-4,90) dan aOR: 2,2 (95%IK: 1,05-4,61).

Background: Drug-resistant tuberculosis (DR-TB) is still the global health problem, the leading cause of death in the single infectious case after Corona virus disease 2019 (COVID-19). Globally, there were 167.000 confirmed cases of Multi Drug Resistant/Rifampicin Resistant tuberculosis (MDR/RR-TB) in 2021 and 162.000 started treatment. Global success rate for MDR/RR-TB treatment reach 60% while in Indonesia 47%. In 2019 WHO recommended injection-free regimen that increase treatment success rate of DR-TB treatment from 60% to 73% and lower lost to follow up (LTFU) cases 17,3% to 9,9%. Indonesia started all-oral regimen in 2020 consist of shorter and longer regimen. Aim of this study is to observe outcomes of drug-resistant tuberculosis (DR-TB) treated with long term all-oral regimen and factors that influence the success rate of DR-TB treatment at Persahabatan Hospital Jakarta.
Metods: Design of the study was retrospective cohort using secondary data, physical and electronic medical records. It carried out in October 2022 at DR-TB clinic Persahabatan Hospital with total sampling technique. Subject of this study were medical records of DR-TB patients enrolled with long-term all-oral regimen treatment by January 2020-December 2021 which met the inclusion criteria.
Results: There were 162 subjects with median of age 44 (18-74) years old, 61,7% were male, and 52,5% subject were malnutrition. Proportion of DM type 2 comorbid was 37%. MDR-TB were found in 45,1% of subject with the high proportion of negative smear bacterial load (25,9%) as baseline. Amount of sputum Mtb culture conversion within 2 months reached 56,2%. There were 86,4% subjects experiencing drug intolerance with the highest proportion being gastrointestinal 66,2%. Outcomes of the long-term DR-TB treatment were successful 43,2%, died 40,1%, dropped-out 13,6% and failed 3,1%. Multivariate analysis found that bacterial load and time of sputum conversion were factors that associated with DR-TB success rate treatment.
Conclusion: Outcomes of drug-resistant tuberculosis (DR-TB) treated with long term all-oral regimen 43,2% cured, 40,12% died, dropped-out from treatment 13,6% and failed 3,1%. Bacterial load ≤1+ and time of sputum conversion ≤2 months were factors that associated with DR-TB success rate treatment with adjusted aOR:2,35 (CI95% :1,12-4,90) and aOR:2,2 (CI95% : 1,05-4,61) respectively.

"Indonesia merupakan negara dengan jumlah kasus TB tertinggi kedua di dunia dengan insidensi 354 per 100.000 penduduk dan 969.000 kasus pada tahun 2021. Salah satu pemeriksaan pada  TB yaitu dengan pemeriksaan darah seperti hitung jumlah leukosit. Peningkatan jumlah leukosit dan netrofil merupakan tanda reaksi inflamasi terutama bila disebabkan oleh infeksi bakteri. Mikronutrien seperti vitamin A dan seng berperan penting dalam pengobatan TB. Studi di Indonesia dan Ethiopia sebelumnya menunjukkan rendahnya asupan vitamin A dan seng serta sebagian besar mengalami defisiensi. Vitamin A dan metabolit aktifnya berperan dalam pertumbuhan dan diferensiasi sel, terutama sel epitel yang berhubungan dengan mukosa, limfosit T dan B, makrofag, dan pembentukan antibodi. Seng juga merupakan elemen penting dalam berbagai fungsi fisiologis dan metabolisme seperti menjaga integritas imunologis, imunitas seluler, dan aktivitas antioksidan. Saat ini, belum terdapat studi yang menghubungkan asupan vitamin A dan seng dengan penanda inflamasi dengan parameter NLR dan PLR pada pasien TB paru. Penelitian ini bertujuan untuk mengetahui hubungan asupan vitamin A dan seng dengan penanda inflamasi pada pasien TB paru di RSUP Persahabatan. Penelitian ini merupakan penelitian potong lintang. Sebanyak 133 subjek direkrut. Mayoritas subjek memiliki usia paling rendah 19 tahun dan paling tinggi 74 tahun. Sebagian besar subjek berjenis kelamin laki-laki, berada dalam jenjang pendidikan sedang (tamat SMA, tidak tamat perguruan tinggi), berpendapatan kurang dari UMP DKI Jakarta. Sebanyak 42,1 % memiliki berat badan normal. Sebagian besar subjek berada dalam fase pengobatan intensif, memiliki status bakteriologis BTA/TCM/kultur positif, tidak ada komorbid, dan tidak pernah merokok. Penelitian ini tidak menemukan adanya korelasi antara asupan vitamin A dan seng dengan nilai NLR dan PLR pada pasien tuberkulosis paru di RSUP Persahabatan.

Indonesia is the country with the second highest number of TB cases in the world with an incidence of 354 per 100,000 population and 969,000 cases in 2021. One of the tests for TB is blood tests such as counting leukocytes. An increase in the number of leukocytes and neutrophils is a sign of an inflammatory reaction, especially if caused by a bacterial infection. Micronutrients such as vitamin A and zinc play an important role in TB treatment. Previous studies in Indonesia and Ethiopia showed low intakes of vitamin A and zinc and that most were deficient. Vitamin A and its active metabolites play a role in cell growth and differentiation, especially epithelial cells associated with mucosa, T and B lymphocytes, macrophages, and antibody formation. Zinc is also an important element in various physiological and metabolic functions such as maintaining immunological integrity, cellular immunity and antioxidant activity. Currently, there are no studies that link vitamin A and zinc intake with inflammatory markers with NLR and PLR parameters in pulmonary TB patients. This study aims to determine the relationship between vitamin A and zinc intake with inflammatory markers in pulmonary TB patients at Persahabatan Hospital. This research is a cross-sectional study. A total of 133 subjects were recruited. The majority of subjects had a minimum age of 19 years and a maximum of 74 years. Most of the subjects were male, had a moderate level of education (graduated from high school, not graduated from college), and earned less than the DKI Jakarta UMP. As many as 42.1% had normal body weight. Most of the subjects were in the intensive treatment phase, had positive BTA/TCM/culture bacteriological status, had no comorbidities, and had never smoked. This study did not find a correlation between vitamin A and zinc intake and NLR and PLR values ​​in pulmonary tuberculosis patients at Persahabatan Hospital."

"Pendahuluan: Tuberkulosis (TB) adalah penyebab utama kematian akibat infeksi di dunia. Sejak tahun 2008 - 2017 terdapat penurunan angka keberhasilan pengobatan TB di Indonesia (< 90%). Rekomendasi pengobatan TB di Indonesia adalah paduan obat antituberkulosis (OAT) dosis berselang sebagian (2RHZE/4R3H3) atau harian (2RHZE/4RH). Menurut WHO, paduan OAT RHZE/R3H3 mempunyai angka kegagalan dan kekambuhan yang lebih tinggi. Namun, penelitian meta-analisis RCT menyatakan bahwa kedua paduan OAT mempunyai angka kegagalan dan kekambuhan yang sama. Oleh karena itu, dilakukan penelitian untuk membandingkan hasil pengobatan dan efek samping antara paduan OAT 2RHZE/2RH dengan 2RHZE/4R3H3.
Metode: Penelitian ini merupakan studi analitik observasional dengan desain cross sectional yang membandingkan hasil pengobatan dan efek samping antara paduan OAT 2RHZE/4RH dengan 2RHZE/4R3H3 pada pasien TB paru kategori I di RSUP Persahabatan periode Januari 2015 sampai Juni 2018. Data sekunder diambil dari rekam medik. Hasil pengobatan dinilai sesuai definisi dalam pedoman nasional penanggulangan TB di Indonesia dan WHO. Efek samping dinilai dari seluruh efek samping terkait OAT yang tercatat dalam rekam medik.
Hasil: Terdapat 175 pasien pada masing-masing kelompok. Pada kelompok paduan OAT 2RHZE/4RH terdapat 89.1% pasien berhasil, 13.1% sembuh,76.0% pengobatan lengkap, 10.6% putus berobat, 0.6% gagal, dan tidak ada yang meninggal. Pada kelompok paduan OAT 2RHZE/4R3H3 terdapat 91.4% pasien berhasil, 39.4% sembuh, 52.0% lengkap, 8% putus berobat, tidak ada yang gagal, dan 0.6% meninggal. Tidak ada perbedaan bermakna untuk keberhasilan pengobatan (p=0.470, OR=1.299, IK95%;0.637-2.648), putus berobat (p=0.659 ,OR=0.758, IK95%;0.365-1.577), gagal (p=1.000), dan meninggal (p=1.000) di antara kedua kelompok. Namun, terdapat perbedaan bermakna untuk kesembuhan (p=0.003, OR=2.358, IK95%;1.375-5.206) dan pengobatan lengkap (p=<0.001, OR=0.342, IK95%;0.217-0.540). Sebagian besar pasien mengalami efek samping pengobatan (51.1%) terutama di tahap intensif (73.2%). Pada tahap lanjutan tidak ada perbedaan bermakna kejadian efek samping antara kedua kelompok (p= 0.324, OR=1.386, IK95%; 0.723-2.657).
Kesimpulan: Kesembuhan kelompok paduan OAT 2RHZE/4R3H3 lebih baik daripada 2RHZE/4RH, sedangkan pengobatan lengkap sebaliknya. Tidak ada perbedaan bermakna untuk keberhasilan pengobatan, putus berobat, kegagalan, meninggal, dan kejadian efek samping pada tahap lanjutan di antara kedua kelompok.

Introduction: Tuberculosis (TB) is the main cause of death for infectious disease in the world. Since 2008 - 2017, there was a decline of TB success rate (< 90%) in Indonesia. Treatment of TB in Indonesia are using antituberculosis drugs with part daily dose combination (2RHZE/4R3H3) or daily dose combination (2RHZE/4RH). WHO concluded that 2RHZE/4R3H3 combination had higher failure and recurrence rate. However, a meta-analysis study showed that both combinations had same failure and recurrence rate. Therefore, this study is conducted to compare treatment outcomes and adverse effects between 2RHZE/4RH combination and 2RHZE/4R3H3 combination.
Method: This was an observational analytic study with cross sectional design which compared treatment outcomes and adverse effects between 2RHZE/4RH combination and 2RHZE/4R3H3 combination in pulmonary tuberculosis patient at RSUP Persahabatan period January 2015 until June 2018. Secondary data was taken from medical record. Treatment outcomes were assessed using definition in Indonesia National Guideline of TB and WHO. Adverse effects were assessed from all adverse effects that written in medical record.
Result: There are 175 patients in each group. In 2RHZE/4RH combination group, there were 89.1% patients succeed, 13.1% cured, 76.0% completed treatment, 10.6% lost to follow up, 0.6% failed and no one died. In 2RHZE/4R3H3 combination group, there were 91.4% patients succeed, 39.4% cured, 52.0% completed treatment, 8% lost to follow up, no one failed, and 0.6% died. There was no significant difference for success (p=0.470, OR=1.299, IK95%;0.637-2.648), loss to follow up (p=0.659, OR=0.758, IK95%;0.365-1.577), failure (p=1.000), and death rate (p=1.000) between two groups. However, there was a significant difference for cure (p=0.003, OR=2.358, IK95%;1.375-5.206) and complete treatment rate (p=<0.001, OR=0.342, IK95%;0.217-0.540) between two groups. Most patients had adverse effects (51,5%), especially in intensive phase (73,2%). In continuation phase, there was no significant difference of adverse effects event between two groups (p = 0.324, OR= 1.386, IK95%; 0.723-2.657).
Conclusion: Cure rate was better in 2RHZE/4R3H3 group than 2RHZE/4RH group, for completed treatment on the contrary. There was no significant difference for success rate, loss to follow up rate, failure rate, death rate, and adverse effects event in continuation phase between two groups.

Latar belakang: Kasus baru dan kematian kanker paru semakin meningkat. Rokok tembakau sangat berperan tetapi hanya 15% yang menderita kanker paru. Oleh karena itu faktor genetik diduga berperan pada kanker paru. Penelitian-penelitian kohort selama ini menunjukan hubungan bermakna risiko kanker paru dengan riwayat kanker di keluarga.
Tujuan: Tujuan utama dari penelitian ini adalah untuk menentukan proporsi subjek kanker paru dengan riwayat kanker pada keluarga.
Metode: Penelitian ini adalah studi observasi deskriptif potong lintang dengan subjek kanker paru yang berobat jalan maupun inap di RSUP Persahabatan Jakarta 1 Januari 2013 sampai 31 Mei 2015.
Hasil: Subjek penelitian 380 dengan komposisi laki-laki lebih banyak dibandingkan perempuan (72,9% banding 27,1%) dengan median 56 tahun dan nilai minimum dan maksimum 20-86 (66) tahun. Sebanyak 65.3% subjek adalah perokok. Dari total subjek yang merokok, 33,2% termasuk perokok dengan Indeks Brickman (IB) sedang. Jenis sel kanker terbanyak adalah masuk kelompok Kanker Paru Karsinoma Bukan Sel Kecil (KPKBSK) yaitu adenokarsinoma 73,4%. Sebagian besar subjek terdiagnosis pada stage lanjut baik pada kelompok KPKBSK maupun Kanker Paru Karsinoma Sel Kecil (KPKSK). Proporsi subjek dengan riwayat kanker keluarga sebesar 8,2% dengan subjek laki-laki lebih besar dibandingkan perempuan (5,8% dibandingkan 2,4%). Nilai minimum-maksimum usia 35-72 tahun, median 55 tahun. Subjek yang merokok hanya ditemukan pada laki-laki sebanyak 71% dan jenis kanker terbanyak adenokarsinoma 71%. Hubungan keluarga 1 orang lebih banyak ditemukan dibandingkan lebih 1 orang (64,4% banding 35,6%) dengan dominasi ayah (25,8%). Jenis kanker keluarga paling banyak bukan kanker paru dibandingkan kanker paru (85,4% banding 14,6%).
Kesimpulan: Proporsi subjek kanker paru dengan riwayat kanker pada keluarga adalah 8,2%. Subjek dengan 1 anggota keluarga yang memiliki kanker paling banyak 64,4% dengan dominasi ayah 25,8%. Jenis kanker paru dengan riwayat kanker keluarga terbanyak adenokarsinoma 71%. Jenis kanker keluarga lebih banyak adalah bukan kanker paru 85,4%.

Pendahuluan: Polimorfisme gen N-asetiltransferase 2 NAT2 dapat mempengaruhi hasil pengobatan infeksi tuberkulosis TB dan risiko terjadinya efek samping kerusakan hati imbas obat anti tuberkulosis. Penelitian ini bertujuan untuk mengetahui distribusi polimorfisme gen NAT2 pada pasien TB naif yang mengalami kegagalan konversi sputum pada pengobatan dengan rejimen OAT standar dan untuk mengetahui apakah polimorfisme gen NAT2 berpengaruh terhadap toleransi dan keberhasilan terapi TB paru dengan INH..Metode: Analisis polimorfisme gen NAT2 dilakukan pada 54 pasien TB naif Puskesmas Kecamatan Pademangan, Jakarta Utara, menggunakan teknik polymerase chain reactions PCR dan dilanjutkan dengan sekuensing. Lima puluh empat pasien TB dikelompokkan menjadi dua kelompok observasi; 26 kelompok gagal konversi dan 28 pasien berhasil konversi.Hasil: Diantara 54 pasien TB, 11,2 adalah gen prediktor asetilator lambat membawa dua alel mutan; NAT2 5, NAT2 6, NAT2 dan 88,8 adalah gen prediktor asetilator cepat membawa satu atau dua alel NAT2 4, NAT2 12, NAT2 13 . Frekuensi asetilator cepat lebih banyak ditemukan pada penelitian ini. Sebanyak 92 pasien yang gagal konversi dan 83 pasien yang gagal pengobatan akhir mempunyai gen asetilator cepat. Tidak ditemukan hubungan yang bermakna antara gen NAT2 dengan kegagalan konversi p=0,423 dan kegagalan pengobatan p=0,415 . Tidak ditemukan satupun kejadian kerusakan hati imbas obat antituberkulosis AT-DILI pada 6 pasien asetilator lambat. Kesimpulan: Tidak ditemukan hubungan antara jenis asetilator dengan kegagalan konversi sputum, kegagalan pengobatan dan terjadinya AT-DILI.

ABSTRACT
Introduction Polymorphisms of N acetyltransferase 2 NAT2 gene might affect the outcome of TB infection treatment and the risks of developing hepatic adverse event, known as antituberculosis drug induced liver injury AT DILI . The purpose of this study was to find the distribution of NAT2 gene polymorphisms in the naive TB patients, who experienced sputum conversion failure after conventional standard regimen and to clarify whether the NAT2 gene polymorphisms could affect the tolerability and the efficacy of multidrug therapy with INH for pulmonary tuberculosis.Methods Polymorphisms of NAT2 gene were studied in 54 TB naive patients in Pademangan District Primary Health Care, North Jakarta. Twenty six patients were assigned to the sputum conversion failure group and 28 to the conversion group. Analysis was done using polymerase chain reactions PCR followed by direct sequencing. Results Fifty four TB patients slow acetylators, 11,2 rapid acetylators, 88,8 were enrolled in this study. Two mutant alleles NAT2 5, NAT2 6, NAT2 7 were found in slow acetylator genes and NAT2 4, NAT2 12, NAT2 13 in fast acetylator genes. Among 92 patients who experienced sputum conversion failure and 83 patients who undergone treatment failure have fast acetylator genotypes. There were no significance association between NAT2 genotypes analysis associated to sputum conversion failure p 0,423 and treatment failure p 0,415 . None of the six slow acetylators experienced AT DILI.Conclusion Among patients with sputum conversion failure and treatment failure evaluated in this study, fast acetylator genotypes were more common and no relationship was found between the acetylator genotypes with antituberculosis treatment failure and the occurrence of antituberculosis drug induced liver injury."
