PT. Kimia Farma Trading & Distribution Bekasi is a branch of PT. Kimia Farma Trading & Distribution (KFTD) which is engaged in distribution networks. Distribution is one of the spearheads of KFTD's services, where its operational activities include distribution of pharmaceutical products, supplements, cosmetics, medical devices, medical rehabilitation, and reagents. KFTD Bekasi branch itself is a Pharmaceutical Wholesaler (PBF) branch whose job is to provide pharmaceutical supplies needed by various pharmaceutical service facilities located in Bekasi City, Bekasi Regency to Karawang. PT. Kimia Farma Trading & Distribution Bekasi has a good operational management system to support efficient and effective distribution activities. The operational management system of KFTD Bekasi is guided by the principles of Good Medicine Distribution (CDOB). Good Drug Distribution Method (CDOB) is a method of distribution or distribution of drugs and/or medicinal ingredients that aims to ensure quality along the distribution or distribution channel according to the requirements and intended use (BPOM, 2012)."

"PT KFTD merupakan Pedagang Besar Farmasi yang memiliki izin untuk menyalurkan obat keras golongan narkotika. Evaluasi pemesanan obat golongan narkotika dilakukan berdasarkan ketetapan yang berlaku di PT KFTD. Evaluasi kewajaran jumlah dan frekuensi ini selanjutnya digunakan sebagai acuan bagi apoteker penanggung jawab KFTD dalam melakukan penilaian penerimaan pesanan narkotika dari outlet terkait.Penilaian awal status kewajaran pemesanan dilakukan dengan membandingkan histori pembelian aktual obat Fentanyl (PCC) dari Outlet X selama periode September 2021 – September 2022 kepada PBF KFTD Cabang Bekasi tiap bulannya dengan rata-rata pembelian obat yang didapat dari big data milik PT KFTD.Penjualan obat Fentanyl PCC menempati penjualan tertinggi untuk obat golongan narkotika di PT KFTD Cabang Bekasi, yaitu mencapai 53,265% selama periode September 2021 – September 2022. Penjualan obat Fentanyl dilakukan kepada 90 outlet mitra kerja dengan Outlet X merupakan outlet dengan frekuensi pembelian terbanyak yaitu 2.216 dus @ 5 ampul dalam setahun.Penilaian kewajaran pesanan narkotika yang diterima oleh PBF KFTD Cabang Bekasi harus dilakukan dengan membandingkan riwayat pembelian sebelumnya disertai dengan melakukan kunjungan audit langsung ke outlet mitra kerja terkait.

PT KFTD is a Pharmaceutical Distributor who has a permit to distribute narcotic hard drugs. Evaluation of orders for narcotic class drugs is carried out based on the provisions in force at PT KFTD. The evaluation of the reasonableness of the quantity and frequency is then used as a reference for the pharmacist in charge of the KFTD in assessing the acceptance of narcotics orders from related outlets. The initial assessment of the reasonableness of the order status is carried out by comparing the history of actual purchases of Fentanyl (PCC) drugs from Outlet X during the period September 2021 - September 2022 to PBF KFTD Bekasi Branch each month with an average purchase of drugs obtained from PT KFTD's big data. Sales of Fentanyl PCC drugs occupy the highest sales for narcotic drugs at PT KFTD Bekasi Branch, reaching 53.265% during the period September 2021 - September 2022. Fentanyl drug sales are carried out to 90 partner outlets with Outlet X being the outlet with the highest frequency of purchases, namely 2,216 boxes @ 5 ampoules in a year. An assessment of the fairness of narcotics orders received by PBF KFTD Bekasi Branch must be carried out by comparing the history of previous purchases accompanied by making direct audit visits to related partner outlets."

"Stress merupakan kondisi yang terjadi pada semua kalangan, baik dewasa maupun anak-anak. Stress dapat terjadi akibat adanya perubahan perilaku di lingkungan keluarga, kerja, sekolah, atau dimanapun. Stress dapat diatasi dengan terapi farmakologi golongan nartkotika, psikotropika, atau obat-obatan tertentu (OOT) dalam dosis yang sesuai. Obat-obatan tertentu (OOT) sering disalahgunakan karena memiliki efek tenang dan kesenangan berlebih. Oleh karena itu, untuk menghindari terjadinya penyalahgunaan OOT, dalam penyerahan OOT dari distributor kepada konsumen harus memerhatikan nilai kewajaran dan frekuensi pembelian sebelumnya. Tujuan dilakukan pembuatan laporan ini adalah menganalisis penjualan dan penyalahgunaan tramadol dari KFTD kepada RS. X periode Oktober 2021 – September 2022 berdasarkan nilai koefisien variasi (KV) yang diperoleh. Metode yang digunakan adaah dengan melakukan studi literature, kajian pustaka, wawancara, dan pengambilan data sekunder. Kesimpulan yang diperoleh adalah penjualan tramadol dari KFTD cabang Bekasi kepada RS.X terdistribusi secara heterogen dan memiliki risiko penyalahgunaan tinggi karena nilai koefisien variasi (KV) yang diperoleh sebesar 248,63%.

Stress can be overcome with pharmacological therapy including narcotics, psychotropics, or certain drugs (OOT) in appropriate doses. Certain drugs (OOT) are often abused because they have a calming effect and excessive pleasure. Therefore, to avoid misuse of OOT, when handing over OOT from distributors to consumers, they must pay attention to the fairness value and frequency of previous purchases. The purpose of making this report is to analyze the sale and abuse of tramadol from KFTD to hospitals. X period October 2021 – September 2022 based on the coefficient of variation (KV) value obtained. The method used is by conducting literature studies, literature reviews, interviews, and collecting secondary data. The conclusion obtained is that sales of tramadol from KFTD Bekasi branch to hospitals are distributed heterogeneously and have a high risk of abuse because the coefficient of variation (CV) value obtained is 248.63%."

"Pada distribusi produk rantai dingin, terdapat persyaratan khusus yang harus dipenuhi sebagai standar meliputi aturan yang berkaitan dengan masalah suhu pada saat penerimaan,penyimpanan dan pengiriman. Potensi produk rantai dingin mungkin dapat berkurang atau bahkan hilang bila terkena suhu di luar kisaran yang dipersyaratkan, maka dalam penyimpanannya perlu dijaga suhunya. Terjadinya perubahan suhu hingga diluar rentang yang dipersyaratkan dapat mengubah kualitas produk serta efikasinya. Dalam pengelolaannya, terdapat faktor lain yang memengaruhi seperti seperti bangunan dan peralatan (chiller, freezer, termometer) yang memadai serta personel yang memenuhi kualifikasi sesuai dengan CDOB. Inspeksi diri perlu dilakukan sebagai tindakan pemenuhan terhadap CDOB dan sebagai penjaminan konsistensi mutu terhadap kinerja PBF. Oleh karena itu, dilakukan evaluasi kesesuaian hasil inspeksi diri terhadap CDOB. Metode yang digunakan antara lain studi literatur, wawancara dan observasi. Ketiga metode tersebut dilakukan agar dokumentasi inspeksi diri yang dihasilkan sesuai dengan kondisi pedagang besar farmasi dan dapat memenuhi persyaratan CDOB. Studi literatur dilaksanakan dengan cara mencari referensi melalui pedoman teknis CDOB. Wawancara dilaksanakan untuk memperoleh data-data yang diperlukan untuk mengisi form inspeksi diri. Observasi dilaksanakan dengan cara mengamati secara langsung keberlangsungan penerapan CDOB di Kimia Farma Trading & Distribution cabang Bekasi. Kesimpulan yang bisa dihasilkan adalah Kimia Farma Trading & Distribution cabang Bekasi telah melaksanakan dan mematuhi CDOB melalui standar prosedur operasional yang berlaku berdasarkan hasil inspeksi diri untuk produk rantai dingin/cold chain product (CCP).

In the distribution of cold chain products, there are special requirements that must be met as standards, including rules relating to temperature issues at the time of receipt, storage and delivery. The potential of cold chain products may be reduced or even lost if exposed to temperatures outside the required range, so it is necessary to maintain the temperature in storage. Changes in temperature beyond the required range can change product quality and efficacy. In its management, there are other influencing factors such as adequate buildings and equipment (chillers, freezers, thermometers) as well as personnel who meet the qualifications according to the CDOB. Self-inspection needs to be carried out as an act of compliance with CDOB and as a guarantee of quality consistency towards PBF performance. Therefore, an evaluation of the suitability of the results of the self-inspection against the CDOB was carried out. The methods used include literature studies, interviews and observation. These three methods are carried out so that the resulting self-inspection documentation is in accordance with the conditions of pharmaceutical wholesalers and can meet CDOB requirements. The literature study was carried out by looking for references through the CDOB technical guidelines. Interviews were conducted to obtain the necessary data to fill out the self-inspection form. Observations were carried out by directly observing the continuity of CDOB implementation at the Bekasi branch of Kimia Farma Trading & Distribution. The conclusion that can be drawn is that the Bekasi branch of Kimia Farma Trading & Distribution has implemented and complied with the CDOB through the applicable standard operating procedures based on the results of self-inspection for cold chain products (CCP)."

"Tingkat penyalahgunaan narkotika di Indonesia dinilai cukup tinggi dengan nilai persentase sebesar 1,80% berdasarkan hasil survei Badan Narkotika Nasional dan Pusat Penelitian Masyarakat dan Budaya – Lembaga Pengetahuan Ilmu Indonesia (PMB-LIPI) pada tahun 2019. Dalam peraturan Badan Pengawas Obat dan Makanan Nomor 9 Tahun 2019, penyaluran obat narkotika wajib memenuhi ketentuan peraturan dan perundang-undangan dan Cara Distribusi Obat yang Baik (CDOB). Namun hanya Pedagang Besar Farmasi (PBF) yang telah mendapatkan izin khusus dapat menyalurkan narkotika ke apotek, puskesmas, rumah sakit, dan klinik. Sehingga, sebagai salah satu langkah antisipasi untuk mencegah penyimpangan dalam jalur distribusi narkotika, apoteker dalam PBF dengan izin khusus tersebut wajib melakukan evaluasi secara berkala terhadap rasionalitas jumlah dan frekuensi pesanan narkotika dari klien PBF terkait. Tujuan dilakukan penelitian dalam tugas ini adalah untuk menilai kewajaran pesanan narkotika dari apotek-apotek yang merupakan klien dari KFTD cabang Bekasi. Penilaian kewajaran dilakukan berdasarkan jumlah dan frekuensi pesanan suatu apotek serta lokasi apotek yang berdekatan dengan suatu fasilitas kesehatan tertentu atau praktik dokter perorangan. Dari 65 apotek, ditemukan sebanyak 7 apotek yang membuat pesanan narkotika dengan jumlah dan frekuensi diatas rata-rata. Namun, berdasarkan penilaian kewajaran terhadap lokasi ketujuh apotek tersebut, semua apotek berada dengan dengan klinik-klinik kesehatan dan rumah sakit.

The level of narcotics abuse in Indonesia is considered quite high with a percentage value of 1.80% based on the results of a survey by the National Narcotics Agency and the Center for Research on Society and Culture - Indonesian Institute of Science (PMB-LIPI) in 2019. Based on the Food and Drug Supervisory Agency regulation Number 9 of 2019, the distribution of narcotic drugs must comply with the provisions of regulations and legislation and Good Drug Distribution Methods (CDOB). However, only Pharmaceutical Wholesalers (PBF) who have obtained the special permits can distribute narcotics to pharmacies, health centers, hospitals and clinics. Thus, as one of the anticipatory steps to prevent irregularities in the narcotics distribution channel, pharmacists in PBF with special permits are required to carry out regular evaluations of the rationality of the number and frequency of narcotics orders from relevant PBF clients. The aim of the research in this assignment was to assess the fairness of narcotics orders from pharmacies which are the clients of the Bekasi branch of KFTD. The fairness assessment is carried out based on the number and frequency of orders from a pharmacy as well as the location of the pharmacy which is close to a particular health facility or individual doctor's practice. Of the 65 pharmacies, it was found that 7 pharmacies made orders for narcotics with quantities and frequencies above the average. However, based on a reasonable assessment of the locations of the seven pharmacies, all pharmacies are located near health clinics and hospitals."

During the period from September 2021 to September 2022, the sales level of Diazepam reached 48.31% of the total quantity of psychotropic drugs sold. This Diazepam was sold to 61 partner outlets, one of which is Outlet X. The analysis of Diazepam sales is necessary to evaluate and determine the appropriateness of purchases by a specific outlet. From the analysis results, it was found that during the specified period, Outlet X made 5 purchases of Diazepam with a total quantity of 570 boxes. These purchases increased compared to Outlet X's previous purchases, but the increase in drug purchases may be influenced by a rise in anxiety cases by 6.8%, safety stock inventory, and consideration of lead time during ordering due to temporary stockouts at KFTD Bekasi. Therefore, it cannot be said that Outlet X's purchase of Diazepam was unreasonable.
