Abstrak :
[Penyakit Arteri Perifer (PAP) merupakan sumbatan aliran darah arteri selain koroner dan intrakranial. PAP dihasilkan dari proses atherosklerosis, emboli, trombus, dan inflamasi yang mengarah ke stenosis arteri. PAP asimptomatik menempati 3-10% populasi dunia, dan meningkat hingga 15-20% pada 70 tahun ke atas. Divisi kami mencatat 18,1-24,7% pasien kaki diabetik dengan PAP mengalami amputasi pada kurun waktu 3 tahun terakhir. Evaluasi ultrasonografi Doppler pada arteri utama ekstremitas bawah diharapkan mampu mendeteksi secara dini apakah pasien akan diamputasi atau tidak baik mayor maupun minor. Metode yang diambil adalah analitik komparatif kategorik independen dengan disain penelitian kohort retrospektif. Selama Januari 2010 hingga Desember 2011 didapatkan 24 pasien yang masuk kriteria inklusi. Arteri yang diperiksa a.femoralis, a.poplitea, a.tibialis posterior, dan a.dorsalis pedis dengan tampilan spektral mulai dari monofasik, bifasik, atau trifasik terutama dengan pelebaran spektral. Ultrasonografi salah satu modalitas ?operator dependent?, untuk mengurangi bias, peneliti menggunakan operator ultrasonografi adalah peneliti sendiri, trainee atau konsultan divisi kami. Hasil didapatkan spektral bifasik hingga monofasik pada a.femoralis 25,0%, a.poplitea 58,3%, a.tibialis posterior 41,6%, a.dorsalis pedis 45,8%, angka amputasi mayor dan minor masing-masing 4%. Perhitungan statistik didapatkan untuk a.femoralis p=0,054 (95% CI), a.poplitea p=0,006 (95% CI), a.tibialis posterior p=0,010 (95% CI), dan a.dorsalis pedis p=0,021 (95% CI). Secara statistik, prediksi amputasi dapat bermakna pada ultrasonografi Doppler pada a.poplitea, a.tibialis posterior, dan a.dorsalis pedis. Dapat ditarik kesimpulan pemeriksaan ultrasonografi Doppler penting dilakukan pada setiap pasien PAP untuk mengevaluasi secara khusus keadaan empat arteri utama ekstremitas bawah pasien dan untuk prediktor amputasi ...... Ultrasonografi (USG) merupakan salah satu teknologi pencitraan medis. Teknologi ini paling banyak digunakan dalam dunia kedokteran saat ini. Beberapa faktor pemilihan teknologi USG ini adalah minimalnya faktor tingkat resiko serta persiapan fisik dan waktu pasien dan operator USG-nya. Kurangnya penelitian berkaitan dengan teknologi USG ini di Indonesia menjadi ketergantungan pembelian perangkat USG secara import. Peneliti mencoba melakukan pengembangan dan penambahan fungsi antarmuka visualisasi dengan memberikan analisa sederhana untuk penelitian lebih lanjut. Dalam penelitian ini Peneliti menggunakan Matlab untuk pengolahan sinyal, pengolahan matrik, visualisasi, dan kontrol. Tujuan penelitian ini adalah merancang dan mengembangkan suatu antarmuka perangkat lunak untuk visualisasi dan analisis citra USG. Algoritma yang ada dinalisis lebih jauh sehinga bisa dilakukan pemilahan proses. Eksperimen awal dilakukan dengan mengolah hasil data dari sinyal A-mode dan kemudian divisualisasi menjadi B-mode. Selanjutnya dikembangkan suatu antarmuka visual dan analisa proses pencitraan, serta penambahan fungsi citra lainnya. Visualisasi citra USG yang dilakukan belum secara real time. Visualisasi citra hasil pengembangan antarmuka perangkat lunak ini dapat dalam bentuk citra B-mode dan juga Video. Citra bisa dikarakterisasi dengan menggunaan filter IIR dan FIR ataupun tanpa filter. Aplikasi mendukung penggunaan lowpass filter dan highpass filter dan perubahan kondisi nilai cut-off secara dinamis. Pengubahan filter order menentukan hasil citra yang divisualkan. Pada nilai filter order tertentu dengan karakteristik filter yang berbeda akan menghasikan citra yang bervariasi. Hasil visual citra scan dapat disimpan dalam format Jpeg dan dapat dicetak. Aplikasi bisa memvisualkan konstruksi proses sinyal data grafik secara optimal. Dalam hal ini pengguna dapat memilih line data pada frame citra untuk dianalisa. Pengembangan antarmuka memberikan kemudahan dalam penggunaan aplikasi, serta bisa memahami proses visualisasi dengan lebih baik. Komponen-komponen antarmuka yang jelas menjadikan solusi analisa visualisasi, dan pemahaman terhadap algoritma USG lebih jauh. ......Ultrasonography (USG) is an application of medical image technologies. It is mostly used in medical practice nowadays. Several factors in selecting USG technology are risk factor level, physical and time preparedness for patient and USG operator. The lack of research related to USG technology in Indonesia is the major cause to import USG devices. Researchers try to develop interface and enrich functions with a simple analysis to study the existing algorithms. In this study, Matlab is used for signal processing, matrix processing, visualization, and control. The purpose of this research is to design and develop a software interface for visualization and image analysis of USG. The algorithm is analyzed further so they can be processed with filtering process. Early experiments performed by processing the data results from the A-mode signal and then visualized to B-mode. Furthermore, visualization interface and imaging process analysis are contructed, and enrichment of other imaging function. USG visualization is not in real time. Visualization of application outputs can be in the B-mode image and also Video. The image can be characterized with or without IIR or FIR filter. Applications support the use of lowpass filter and highpass filter, and capable of setting up of the cut-off value dynamically. The combination of the value of Filter Order capable of determined the outcome of the visualized image. By combining different filters, various images could be obtained. The results of scanned image can be stored in Jpeg format and printed directly. Application is capable of visualizing the contruction process in the form of grapichs. In this case the user can select the data line position on the frame layer for analisis. Tool interface provides ease of use of the application and better understanding of the visualization process. Clear component of the interface is created to visually analyze the solution and for understanding USG algoritms further.

Telah dilakukan pembuatan dan pengujian Fantom buatan yang sesuai standar AIUM untuk control kualitas Transduser USG dengan bahan akrilik (PMMA) setebal 5 mm. Pengukuran yang dapat dilakukan meliputi Resolusi Lateral, Resolusi Axial, Dead Zone dan Depth Resolution. Pengujian awal pada 8 pesawat USG dengan tahun pembuatan dan merek yang berbeda memperlihatkan kemampuan fantom untuk mengasses USG dengan baik untuk semua tipe pengujian. Pengujian mampu mengidentifikasikan tiga pesawat USG yang bekerja di bawah standard.
A test Phantom based on AIUM standard for the Quality Control of USG Transducer has been fabricated using 5 mm thick PMMA slabs. This phantom can be used to assess Lateral Resolution, Axial Resolution, Dead Zone and Depth Resolution. Preliminary testing involving 8 different USG machines has shown its possibility to be used as an effective quality control test tool. Testing has also been able to identify 3 USG machines that were operating below standard.

Latar Belakang : Penggunaan terapi hormon memiliki efek samping pada payudara yang dapat terdeteksi pada ultrasonografi (USG) berupa peningkatan ketebalan jaringan fibroglandular yang berhubungan dengan peningkatan risiko kanker payudara. Hingga saat ini masih terdapat kontroversi mengenai efek samping terapi hormon tibolone terhadap payudara. Tujuan Penelitian : Mengetahui perubahan ketebalan jaringan fibroglandular payudara setelah terapi hormon tibolone. Metode : Penelitian ini berlangsung dari bulan Desember 2013 hingga Agustus 2015 di Departemen Radiologi RSUPN dr. Cipto Mangunkusumo dengan desain penelitian berupa studi kuasi eksperimental dengan amandemen berupa pengambilan data USG payudara sebelum dan sesudah terapi hormone tibolon. Pengukuran dengan USG payudara dilakukan di empat regio (superior, inferior, medial dan lateral) yang kemudian dilakukan analisis terhadap rerata perubahan ketebalan jaringan fibroglandular sebelum dan sesudah terapi hormon tibolone. Hasil : Dari 40 subjek yang dianalisis, terdapat 36 subjek yang mengalami peningkatan ketebalan jaringan fibroglandular. Terdapat perbedaan bermakna (p<0,001) pada ketebalan jaringan fibroglandular payudara sebelum dan sesudah 3 bulan pemberian tibolon. Indeks massa tubuh (p = 0,020) dan riwayat kontrasepsi hormonal (p=0,015) merupakan faktor yang memengaruhi peningkatan ketebalan jaringan fibroglandular payudara sesudah terapi hormon tibolon. Sedangkan paritas, usia melahirkan anak pertama, riwayat menyusui, usia menarke, usia menopause dan riwayat kontrasepsi hormonal tidak berhubungan. Kesimpulan : Terdapat peningkatan ketebalan jaringan fibroglandular berdasarkan pemeriksaan USG payudara perempuan menopause yang mendapat terapi hormon tibolone. ......Background : The use of hormone therapy has side effects in the breast which detected on ultrasound by increasing the thickness of breast fibroglandular tissue associated with increased risk of breast cancer. Until now there is still controversy regarding the side effects of tibolone on the breast tissue. Purpose : This study aims to determine the thickness changes of the fibroglandular tissue due to tibolone hormone therapy. Methods : This research was done between December 2013 untill August 2015 in Department of Radiology, dr. Cipto Mangunkusumo Hospital. Design of this research is a quasi-experimental study with the amendment in the form of data retrieval breast ultrasound before and after tibolon hormone therapy. Measurements with breast ultrasound was done in four regions (superior, inferior, medial and lateral) then the average change in fibroglandular thickness was analyzed. Results : Of the 40 subjects were analyzed, there were 36 subjects showed significant increase (p <0.001) of breast fibroglandular tissue thickness after 3 months of tibolone hormone therapy. The body mass index (p = 0.02) and a history of hormonal contraception (p = 0,01) were factors that influence the increase of fibroglandular thickness post tibolone hormone therapy. While parity, age of first child, history of breastfeeding, age of menarche, age of menopause and history of hormonal contraception were not related. Conclusions : There is an increase of fibroglandular tissue thickness by breast ultrasound examination of menopausal woman who received tibolone hormone theraphy.
