ABSTRAK
Keterlambatan pengobatan kanker payudara merupakan masalah serius yang dapat memengaruhi kesintasan hidup seseorang. Terutama mereka yang berobat di rumah sakit rujukan, kemungkinan besar telah mengalami keterlambatan berulang. Sehinggu tujuan dalam penelitian ini untuk mengetahui faktor-faktor yang memengaruhi keterlambatan pengobatan kanker payudara tersebut. Penelitian ini merupakan penelitian dengan menggunakan desain cross sectional dengan sampel dari rumah sakit rujukan nasional pada tahun 2013 - 2016. Pengobatan dikatakan terlambat jika membutuhkan waktu lebih dari tiga bulan untuk mendapatkan pengobatan sejak pertama kali datang ke rumah sakit. Penelitian ini menggunakan analsisi cox regresi dengan tingkat kemaknaan 0,005. Hasil penelitian ini didapatkan bahwa faktor yang paling memengaruhi keterlambatan pengobatan pada pasien yang pernah mendapatkan pengobatan adalah tempat tinggal PR 1,593 95 CI 1,031 ndash; 2,462 . Kemudian faktor yang memengaruhi keterlambatan pada pasien baru adalah tingkat pendidikan PR 1,743 95 CI 1,025 ndash; 2,997 dan riwayat pengobatan alternative PR 2,741 95 CI 1,419 ndash; 5,296.
ABSTRACT
Delayed treatment of breast cancer is a serious problem that can worsen the survival period. Then, there is patients already experiencing repeated delays. The purpose of this study is to find out what factors influences the delay of treatment. The study was done with cross sectional design taken from 564 samples of patient breast cancer in Cipto Mangunkusumo General Hospital between 2013 and 2016. Data collected based on register hospital. Time of delay treatment that measured the time from the first came to hospital to the first treatment in hospital. Tuberkulosis masih menjadi masalah kesehatan di dunia. Diperkirakan pada tahun 2021 terdapat 10,6 juta orang yang terinfeksi tuberkulosis. Indonesia merupakan salah satu negara yang masuk dalam 20 negara dengan beban TB, TB MDR/RR, dan TB HIV tertinggi di dunia berdasarkan estimasi jumlah kasus hasil modelling yang dilakukan WHO. Angka inisiasi pengobatan pasien tuberkulosis resistan obat meningkat dari tahun 2020-2022, namun pasien yang terdiagnosis tuberkulosis resistan obat tidak dapat segera mendapatkan pengobatan di fasilitas kesehatan. Penelitian ini bertujuan untuk mengetahui durasi keterlambatan pengobatan pasien tuberkulosis resistan obat, serta pengaruh faktor sistem kesehatan dan faktor pasien terhadap keterlambatan pengobatan pasien tuberkulosis resistan obat di Indonesia tahun 2020-2022. Penelitian ini menggunakan desain cross sectional dengan sampel pasien tuberkulosis resistan obat yang memulai pengobatan tahun 2020-2022 dan dilaporkan ke sistem informasi tuberkulosis. Penelitian ini menggunakan metode regresi logistik multilevel dengan sumber data sekunder dari Sistem Informasi Tuberkulosis dan Profil Kesehatan Indonesia. Hasil penelitian menunjukkan rerata durasi keterlambatan pengobatan pasien tuberkulosis resistan obat meningkat dari tahun 2020-2022; faktor sistem kesehatan yang mempengaruhi keterlambatan pengobatan pasien tuberkulosis resistan obat antara lain rasio rumah sakit, metode diagnosis baseline, dan wilayah pendampingan komunitas; sedangkan faktor pasien yang mempengaruhi keterlambatan pengobatan pasien tuberkulosis resistan obat yaitu jenis kelamin, domisili pasien, riwayat pengobatan OAT suntik, jenis fasilitas kesehatan pertama yang dikunjungi, dan jumlah kunjungan ke fasilitas kesehatan. Perluasan penggunaan cartridge XDR pada alat TCM diperlukan untuk mengetahui resistansi fluorokuinolon sehingga pasien yang terdiagnosis resistan obat dapat segera diobati dan perlunya penguatan kolaborasi antara fasilitas kesehatan, dinas kesehatan, dan organisasi komunitas dalam mendukung pengobatan pasien tuberkulosis resistan obat. ......Tuberculosis is still a health problem in the world. It is estimated that in 2021 there will be 10.6 million people infected with tuberculosis. Indonesia is one of the 20 countries with the highest burden of TB, MDR/RR TB and HIV TB in the world based on the estimated number of cases resulting from modeling conducted by WHO. The rate of initiation of treatment for drug-resistant tuberculosis patients increased from 2020-2022, however, patients diagnosed with drug-resistant tuberculosis cannot immediately receive treatment at health facilities. This study aims to determine the duration of delays in treatment of drug-resistant tuberculosis patients, as well as the influence of health system factors and patient factors on delays in treatment of drug-resistant tuberculosis patients in Indonesia in 2020-2022. This study used a cross-sectional design with a sample of drug- resistant tuberculosis patients who started treatment in 2020-2022 and reported to the tuberculosis information system. This research uses a multilevel logistic regression method with secondary data sources from the Tuberculosis Information System and the Indonesian Health Profile. The results of the study show that the average duration of delay in treatment for drug-resistant tuberculosis patients increased from 2020-2022; health system factors that influence delays in treatment of drug-resistant tuberculosis include hospital ratios, baseline diagnosis methods, and community assistance areas; Meanwhile, patient factors that influence delays in treatment for drug-resistant tuberculosis patients are gender, patient domicile, history of injectable drugs, type of first health facility visited, and number of visits to health facilities. Expanding the use of XDR cartridges in GenExpert is needed to determine fluoroquinolone resistance so that patients diagnosed with drug resistance can be treated immediately and there is a need to strengthen collaboration between health facilities, health services and community organizations in supporting the treatment of drug-resistant tuberculosis patients.

Peningkatan angka morbiditas dan mortalitas akibat penyakit kanker di Indonesia menunjukkan bahwa permasalahan kanker ini semakin besar dan kompleks Salah satunya adalah adanya keterlambatan pasien kanker untuk mendapatkan terapi definitif yang disebabkan oleh faktor faktor yang berasal dari pasien sendiri Penelitian ini merupakan studi analisis deskriptif menggunakan metode campuran kuantitatif dan kualitatif untuk mengetahui data insidens keterlambatan terapi karena keterlambatan pasien pada pasien kanker yang dirujuk ke Departemen Radioterapi RSUPN Dr Cipto Mangunkusmo pada bulan Mei Agustus 2015 serta mengevaluasi faktor faktor apa saja yang mempengaruhi keterlambatan pasien tersebut Terdapat 294 orang pasien yang diikutsertakan dalam penelitian ini setelah mendapatkan persetujuan tertulis Sebagian besar pasien 71 4 adalah perempuan dan 141 orang 48 bekerja sebagai ibu rumah tangga Rentang umur terbanyak adalah 36 50 tahun yaitu sebanyak 132 pasien 44 9 dan hampir seluruh pasien 91 8 telah menikah Keterlambatan terapi didapatkan pada 153 orang pasien 52 dan 67 orang di antaranya 43 8 memiliki riwayat pengobatan alternatif yang dilakukan dalam interval waktu setelah timbul keluhan pertama kali dan atau setelah pasien melakukan konsultasi medis pertama kali Analisis statistik menunjukkan adanya hubungan yang signifikan antara faktor usia p 0 047 pendidikan p 0 047 dan riwayat pengobatan alternatif p 0 0001 dengan keterlambatan terapi Adanya rasa takut untuk berobat secara medis atau menjalani tindakan medis menjadi alasan 51 orang pasien untuk memilih pengobatan alternatif Oleh karena itu diperlukan pengawasan dan evaluasi terhadap pengobatan alternatif terutama yang menyangkut kualitas efikasi dan keamanannya ABSTRACT

Latar Belakang : Di Indonesia, Kanker Nasofaring (KNF) merupakan insiden terbanyak ke lima yang mencapai 5.2% dari seluruh kasus kanker, dan merupakan kanker terbanyak ke tiga pada laki-laki serta penyebab kematian ke tujuh akibat kanker. Tatalaksana kanker menjadi semakin kompleks sehingga meningkatkan risiko terjadinya keterlambatan pada penanganan kanker, dan kanker nasofaring (KNF) adalah salah satu diantaranya. Penelitian ini dilakukan untuk mengidentifikasi faktor-faktor keterlambatan penatalaksanaan kanker dan mengetahui peran dan hubungan case manager terhadap keterlambatan diagnosis dan terapi pada pasien KNF. Metode : Penelitian potong lintang retrospektif terhadap 110 pasien kanker nasofaring yang dirujuk ke RSCM periode Juli 2018-Maret 2019, dilihat karakteristik pasien, peran case manager terhadap faktor keterlambatan diagnosis dan terapi yang didapat. Analisis univariat, bivariate, chi square, kolmogorov smirnov dan kaplan Meier dilakukan pada pasien yang memenuhi kriteria inklusi. Hasil : Didapatkan 64,6% pasien adalah laki-laki, rerata usia 44 (12-66) tahun, rujukan internal RSCM 83,3% dengan perujuk 90% dari Departemen THT-KL. Sebagian besar pasien datang dengan stadium IV A (64,6%) dengan domisili terbanyak dari luar DKI Jakarta (81,3%). Secara umum didapatkan perbedaan yang signifikan pada kelompok tanpa pendampingan case manager lebih banyak mengalami keterlambatan diagnosis dibandingkan pada kelompok yang dengan pendampingan case manager (73,3% versus 24,2%; P = 0,001). Demikian pula pada keterlambatan tindakan pengobatan atau terapi (86,7% versus 54,5%; P = 0,031), namun tidak berbeda signifikan pada waktu tunggu keseluruhan pelayanan pasien, walaupun secara proporsi tetap lebih tinggi waktu tunggu pelayanan pasien pada yang tanpa pendampingan case manager (60% versus 54,5%; P = 0,724). Untuk faktor yang mempengaruhi keterlambatan tatalaksana kanker lainnya didapatkan tidak ada hubungan yang bermakna antara usia, tingkat pendidikan dan tempat tinggal terhadap keterlambatan waktu diagnosis ataupun terapi. Kesimpulan : Case manager terbukti dapat mengoptimalkan pelayanan kesehatan sehingga dapat memperbaiki waktu tatalaksana pada pasien kanker Nasofaring di RSCM. ......Background: In Indonesia, Nasopharyngeal Cancer (NPC) is the fifth highest incidence which reaches 5.2% of all cancer cases, and is the third most cancer in men after lung cancer and liver cancer and the seventh cause of cancer death. The management of cancer became more complex which increasing the risk of delays in cancer treatment, and nasopharyngeal cancer (NPC) is one of them. This study was conducted to identified the delay factors in cancer management and to determine the role and relation of case manager on factors from diagnosis delay and treatment delay for NPC patients. Methods: A retrospective cross sectional study of 110 nasopharyngeal cancer patients reffered to Cipto Mangunkusumo General Hospital (RSCM) for the period of July 2018-March 2019, analyzed the characteristics of patients and the role of case managers on the factors of diagnosis and treatment delay obtained. Bivariate, univariate, chi square, kolmogorov smirnov and kaplan Meier analyzes were performed on patients who met the inclusion criteria. Results: Obtained 67.1% of patients were men, mean age of patients 44 (12-66) years old, RSCM internal majority of patients came with stage IV A (64.6%) and staying mostly outside from DKI Jakarta area (81.3%). In general, there were significant differences in the group without case manager had a higher delay in diagnosis than in the group with the case manager (73.3% versus 24.2%; P = 0.001). Same as in treatment delay (86.7% versus 54.5%; P = 0.031), but not significantly different in awaiting time overall patient care, however in a proportion, the waiting time for patient care was higher in group without case manager (60% versus 54.5%; P = 0.724). For the other factors that influence the delay treatment of cancers showed, there is no relations which related to age, education level and place of residence to delay the time of diagnosis or treatment. Conclusion: Case manager are proven to optimize health services that can improve management time and decrease treatment delay in Nasopharyngeal cancer patients at RSCM.
