[ABSTRAK
BCG telah digunakan sebagai upaya mencegah infeksi tuberculosis selama lebih dari 90 tahun. Penelitian ini dilakukan untuk melihat efek pemberian vaksin BCG melalui rute subkutan di area dorsal leher dan paha pada mencit galur Swiss sekaligus menguji potensi pasak bumi (PB) sebagai imunomodulator setelah imunisasi BCG dengan mengukur IgG1 dan IgG3 menggunakan metode ELISA. Penelitian ini merupakan bagian dari proyek penelitian mengkaji potensi ekstrak akar pasak bumi pada beberapa infeksi pathogen intraseluler. Hasilnya pemberian vaksin BCG melalui rute subkutan di area leher lebih efektif (p<0.05) meningkatkan kadar IgG1 (OD kelompok air 2.162±0.231; kelompok PB 2.138±0.214) dan IgG3 (OD kelompok air 2.564±0.286; kelompok PB 2.521±0.228) pada mencit dibandingkan kadar IgG1 (OD kelompok air 0.817±0.101; kelompok PB 0.796±0.207) dan IgG3 (OD kelompok air 1.290±0.104; kelompok PB 1.260±0.093) pada vaksinasi BCG secara subkutan di area paha. Sementara itu potensi ekstrak akar pasak bumi dalam memodulasi produksi IgG1 dan IgG3 paska injeksi BCG belum terlihat pada percobaan ini.
ABSTRACT
BCG has been used as an effort to prevent tuberculosis for more than 90 years. The aim of this research were to investigate the effect of BCG vaccine administration trough subcutaneous route over the neck and on the thigh of mice of Swiss strain as well as to evaluate the potency of pasak bumi (PB) as immunomodulator in IgG1 and IgG3 productions after BCG vaccination. This research is part of a bigger project to analyse the potency of pasak bumi as an immunomodulator over intracellular pathogen infections. The result shows that BCG vaccination through subcutaneous over the neck was more effective (p<0.05) in generating IgG1 (OD group with water 2.162±0.231; group with PB 2.138±0.214) and IgG3 (OD group with water 2.564±0.286; group with PB 2.521±0.228) on mice compare to IgG1 level (OD group with water 0.817±0.101; group with PB 0.796±0.207) and IgG3 (OD group with water 1.290±0.104; group with PB 1.260±0.093) on subcutaneous vaccination on thigh area. Latar Belakang: Obesitas merupakan faktor risiko utama osteoartritis (OA). Penelitian terdahulu mendapatkan bahwa faktor mekanik saja tidak cukup untuk menjelaskan hubungan OA dengan obesitas. Saat ini faktor metabolik yang berkaitan dengan massa lemak tubuh dianggap memiliki peranan penting, tetapi lemak mana yang paling berperan masih kontroversial apakah lemak viseral atau lemak subkutan. Tujuan penelitian ini untuk mendapatkan korelasi antara distribusi lemak tubuh dengan lebar celah sendi tibiofemoral medial Metode: Penelitian ini merupakan studi potong lintang pada penderita OA lutut dengan obesitas yang berobat di poliklinik Reumatologi, Geriatri dan Penyakit Dalam RSCM periode Januari-Maret 2016. Diagnosis OA lutut berdasarkan kriteria American College of Rheumatology (ACR) 1986. Pemeriksaan distribusi lemak tubuh menggunakan bioelectrical impedance analysis (BIA). Pemeriksaan radiologi lutut menggunakan radiologi konvensional (foto polos) untuk menilai lebar celah sendi tibiofemoral medial. Analisis statistik bivariat digunakan untuk mendapatkan korelasi antara distribusi lemak tubuh dengan lebar celah sendi tibiofemoral medial. Hasil: Sebanyak 56 orang pasien yang memenuhi kriteria inklusi dan bersedia ikut dalam penelitian, mayoritas subjek berjenis kelamin perempuan (73,2%). Median kadar lemak viseral adalah 12% (7.5-16,5) median lemak subkutan adalah 30,2% (16,5-37,9) dan median rasio lemak viseral/subkutan adalah 0,40 (0,26-0,80). Rerata lebar celah sendi tibiofemoral medial adalah 2,34 mm (SB 0,78). Korelasi antara lemak viseral dengan lebar celah sendi tibiofemoral medial (r: -0,474 p: < 0,001). Tidak didapatkan korelasi antara lemak subkutan dengan lebar celah sendi tibiofemoral medial (r: -0,187 p: 0,169) serta tidak didapatkan korelasi antara rasio lemak viseral/subkutan dengan lebar celah sendi tibiofemoral medial (r: -0,225 p: 0,09) Simpulan: Lemak viseral berkorelasi negatif sedang dengan lebar celah sendi tibiofemoral medial (r: -0,474 p: < 0,001). Tidak didapatkan korelasi antara lemak subkutan dan rasio lemak viseral/subkutan dengan lebar celah sendi tibiofemoral
ABSTRACT
Background: Obesity is a major risk factor for knee osteoarthritis. The relationship between obesity and OA may not simply due to mechanical factor. Evidence suggests that metabolic factors related to body fat play important roles, but the specific type of fat that contributes to OA is unclear. The objective of this study was to examine the possible correlation between body fat distributions with knee OA Method: This study was a cross sectional study in OA patients with obesity visiting Rheumatology, Geriatric, Internal Medicine clinics in Cipto Mangunkusumo Hospital between January-March 2016. Samples were collected using consecutive sampling method. Knee OA was diagnosed from clinical and radiologic evaluation based on American College of Rheumatology 1986 criteria. Body fat distribution was measured by bioelectrical impedance analysis (BIA). Radiographs of the knee was measured by conventional radiography to evaluate joint space narrowing (JSN). The correlation between body fat distributions with joint space width was analyzed by bivariate analysis Result: A total of 56 subjects were recruited, with majority of subjects were women (73,2%). Median of visceral fat was 12% (7.5-16,5), median of subcutaneous fat was 30,2% (16,5-37,9) and median of visceral to subcutaneous fat ratio was 0,40 (0,26-0,80). Mean of medial tibiofemoral joint space width was 2,34 mm (SB 0,78). In bivariate analysis we found correlation between visceral fat and medial tibiofemoral joint space width (r: -0,474 p: < 0,001). There is no correlation between subcutaneous fat and medial tibiofemoral joint space width (r: -0,187 p: 0,169) and also visceral to subcutaneous fat ratio and medial tibiofemoral joint space width (r: -0,225 p: 0,09). Conclusion: Visceral fat is correlated with medial tibiofemoral joint space width (r: -0,474 p: < 0,001). There is no correlation between neither subcutaneous fat nor visceral to subcutaneous fat ratio and medial tibiofemoral joint space width.

ABSTRAK
Medroksiprogesteron asetat merupakan progestin sintetik yang digunakan sebagai kontrasepsi hormonal jangka panjang. Tujuan dari penelitian ini adalah meningkatkan penetrasi subkutan dari medroksiprogesteron asetat. Pada penelitian ini telah dilakukan optimasi formula transfersom yaitu F1, F2 dan F3 dengan perbandingan fosfatidilkolin : Tween 80  berturut-turut adalah 90:10; 85:15; dan 75:25. Hasil karakterisasi menunjukkan F2 adalah formula terbaik dengan efisiensi penjerapan 81,20 ± 0,42 %, Zaverage 81,35 ±0,78 nm, indeks polidispersitas 0,198 ± 0,012 dan potensial zeta -34,83±0,64 mVsehingga digunakan pada formulasi sediaan gel. Sediaan gel yang dibuat terdiri dari dua formula, yaitu gel transfersom (FGT) dan gel non transfersom (FG). Terhadap kedua gel tersebut dilakukan uji penetrasi in vitro menggunakan sel difusi Franz dan uji in vivo pada tikus betina Sprague Dawley. Hasil uji penetrasi in vitro menunjukkan jumlah kumulatif medroksiprogesteron asetat terpenetrasi dari FGT lebih tinggi daripada FG selama 24  jam, dengan nilai fluks untuk FGT dan FG masing-masing adalah 112,77 ± 6,47 ng.cm-2.jam-1 dan 17,99 ± 4,81 ng.cm-2.jam-1. Pada uji in vivo, medroksiprogesteron asetat dalam sediaan FGT memberikan hasil area under the curve (AUC) yang lebih tinggi dari pada dalam sediaan FG. 
ABSTRACT
Medroxyprogesterone acetate is a synthetic progestin which is used as long-acting hormonal contraceptive. The aim of this research was to increase subcutaneous penetration of medroxyprogesterone acetate. In this research three transferosomes formulas were prepared and optimized,e.g.F1,F2 and F3 with posphatidylcoline : tween 80 concentration were 90:10; 85:15; and 75:25, respectively. F2 was the best formula with highest entrapment eficiency 81.20 ± 0.42 %, Zaverage 81.35 ±0.78 nm, indeks polidispersity 0.198±0.012 and zeta potensial was -34.83±0.64 mV, so it was incorporated into gel dosage form. There were two gels prepared, transferosomal (FGT) and non transferosomal gel (FG). Both of them were evaluated, in vitro penetration test using Franz Diffusion cells and in vivo study in Sprague Dawley female rats were also conducted to each dosage forms. According to in vitro penetration study, cummulative penetration of medroxyprogesterone acetate from FGT was higher than FG, which flux value for FGT and FG were 112.77 ± 6,47 ng.cm-2.hr-1 and 17.99 ± 4.81 ng.cm-2.hr-1, respectively. The result of in vivo study showed that medroxyprogesterone acetate in FGT dosage form had higher area under the curve of medroxyprogesterone acetate than in FGdosage form.

[Resistensi obat antimalaria mendorong pengembangan obat antimalaria baru. Salah satu alternatif pengembangan obat antimalaria adalah mengombinasikan klorokuin dengan komponen lain, contohnya ekstrak akar pasak bumi (Eurycoma longifolia jack.). Berbagai penelitian in vitro maupun in vivo telah membuktikan potensi pasak bumi sebagai antimalaria. Penelitian ini bertujuan untuk mengetahui efek pemberian kombinasi ekstrak akar pasak bumi dan klorokuin secara injeksi subkutan pada mencit (Mus musculus) yang diinfeksi Plasmodium berghei. Perlakuan yang diberikan adalah pemberian ekstrak akar pasak bumi dengan dosis 10 mg/kgBB, 20 mg/kgBB, serta kombinasi ekstrak akar pasak bumi dengan dua dosis tersebut dan klorokuin. Berdasarkan Peters 4-days suppressive test, pertumbuhan parasitemia mencit yang diberikan kombinasi obat memiliki nilai yang mendekati kontrol positif (0,60%), yaitu 0,60% pada dosis kombinasi pasak bumi 10 mg/kgBB dan 0,50% pada dosis 20 mg/kgBB namun analisis statistik menunjukkan perbedaan tersebut tidak bermakna (p>0,05). Hasil penghitungan penghambatan pertumbuhan parasit menunjukkan kecenderungan yang sama. Penghambatan pertumbuhan parasit kontrol positif menunjukkan angka 97,9% sementara pemberian kombinasi obat menunjukkan angka 97,7% (dosis pasak bumi10 mg/kgBB) dan 98,2% (dosis pasak bumi 20 mg/kgBB). Hal ini menunjukkan bahwa tidak ada perbedaan bermakna antara efek pemberian kombinasi ekstrak akar pasak bumi dan klorokuin secara injeksi subkutan dibandingkan dengan terapi klorokuin saja;Antimalarial drug resisstance demand us to develop new antimalarial drug. One of the alternative is combining chloroquine with new compund, for example pasak bumi root extract (Eurycoma longifolia jack). Many studies have shown the potency of pasak bumi root extract as a antimalarial drug. This study is aim to investigate the effect of combination of pasak bumi root extract and chloroquine which is administrated by subcoutaneous injection to Plasmodium berghei-infected mice. Mice were given pasak bumi root extract only with 10 mg/kgBW and 20 mg/kgBW dose , also combination therapy of pasak bumi root extract on same dose and choloquine. Based on Peters 4-days suppressive test, parasite growth in mice with combination therapy was nearing the postive control value (0.60%), 0.60% for 10 mg/kgBW dose and 0.50% for 20 mg/kgBW dose. However, the statistic analysis showed the difference was not significant (p>0.05). Growth inhibition counting showed the same trend. Latar Belakang: Kontrol tekanan darah lebih buruk pada pasien hipertensi dengan sindrom metabolik. Lingkar leher telah diperkenalkan sebagai salah satu indikator obesitas sentral dan adipositas tubuh bagian atas. Tidak seperti lingkar pinggang, lingkar leher tidak dipengaruhi oleh pergerakan pernapasan dan distensi abdominal postpandrial. Penelitian ini bertujuan untuk meneliti lebih lanjut hubungan lingkar leher dengan tekanan darah pada populasi dewasa. Metode: Studi potong lintang ini dilakukan di Fakultas Kedokteran Universitas Indonesia pada 94 subjek berusia > 18 tahun. Dilakukan pengambilan data lingkar leher dan tekanan darah bersamaan dengan kadar trigliserida, kolesterol-HDL, dan glukosa darah puasa. Hasil: Rerata lingkar leher subjek 33.89 cm. Tekanan darah sistolik dan diastolik subjek didapatkan retata 131.63 + 13.16 mmHg dan 84.26 + 8.01 mmHg. Pada analisis korelasi Pearson, ditemukan korelasi positif yang signifikan antara lingkar leher dengan tekanan darah sistolik (r = 0.438, p < 0.001), dan tekanan darah dastolik (r = 0.385, p < 0.001). Kesimpulan: Lingkar leher secara signifikan berkorelasi dengan tekanan darah sistolik dan diastolik pada populasi dewasa. ......Background: Blood pressure control is worse in hypertensive patients with metabolic syndrome. Neck circumference has been introduced as an indicator of central obesity and upper body adiposity. Unlike waist circumference, neck circumference neck circumference is not affected by respiratory movements and postprandial abdominal distension. This study aimed to investigate the association between neck circumference and blood pressure among adult population. Methods: This cross-sectional study was conducted in Faculty of Medicine Universitas Indonesia involving 94 subjects aged > 18 years. Anthropometric, neck circumference and blood pressure data were collected together with triglyceride levels, HDL-Cholesterol, and fasting blood glucose. Results: The mean of the neck circumference was 33.89 cm. Subject’s systolic and diastolic blood pressure were 131.63 + 13.16 mmHg dan 84.26 + 8.01 mmHg. In a Pearson’s correlation analysis, there were positive significant correlation between neck circumference with both systolic blood pressure (r = 0.438, p < 0.001) and diastolic blood pressure (r = 0.385, p < 0,001). Conclusion: The neck circumference is significantly correlated with systolic and diastolic blood pressure in adult population.
