Abstrak :

Tujuan: Menganalisis pengaruh paparan larutan Sodium hipoklorit (NaOCl) dengan konsentrasi 2,5% dan 5,25% pada profil MMP-9 dan struktur kolagen di dentin terhadap kekuatan ikat geser resin komposit-dentin. Metode: Seratus empat puluh empat spesimen dentin dirandom untuk analisis profil MMP-9 dengan pemeriksaan imunohistokimia (n=18) dan ELISA (n=30); analisis struktur kolagen dengan SEM di permukaan oklusal (n=18) dan proksimal (n=18) dan pewarnaan Massons trichrome; serta analisis kekuatan ikat geser resin komposit permukaan oklusal (n=30) dan proksimal (n=30). Spesimen dibagi menjadi 3 kelompok dalam tiap analisis, yaitu: kelompok kontrol, kelompok paparan NaOCl 2,5% dan 5,25%. Hasil: Pemeriksan profil MMP-9 menunjukkan bahwa peningkatan konsentrasi NaOCl dapat menekan profil MMP-9 (p<0,05). Pemeriksaan struktur kolagen menunjukkan bahwa peningkatan konsentrasi NaOCl mampu mendegradasi kolagen dentin (p<0,05). Sodium hipoklorit 5,25% paling efektif menekan jumlah profil MMP-9 (9,9+3,63 ng/mL) dan mendegradasi kolagen serta memiliki nilai kekuatan ikat geser yang paling tinggi (15,85+0,43 MPa) dari pada NaOCl 2,5% (14,51+3,66 ng/mL) dan kelompok kontrol (24,09+8,88 ng/mL; 14,41+0,96 Mpa). Kelompok NaOCl 2,5% memiliki nilai kekuatan ikat geser yang paling rendah (9,2+0,65 MPa). Kesimpulan: Larutan NaOCl 5,25% dapat menekan profil MMP-9 dan mendegradasi kolagen fibril untuk meningkatkan nilai kekuatan ikat geser resin komposit-dentin dan menciptakan suatu ikatan mikro mekanis antara resin dan permukaan anorganik dentin tanpa hybrid layer.

Objective: To analyze the effect of Sodium hypochlorite (NaOCl) 2.5% and 5.25% exposure on MMP-9 profile and dentin collagen structure toward resin composite-dentin shear bond strength. Method: One hundred and forty four dentin specimens were randomized  for MMP-9 profile analysis using immunohistochemistry staining (n=6) and ELISA (n=10); collagen structure analysis with SEM and Massons trichrome staining (n=6); and resin composite shear bond strength analysis (n=10). Then, specimens were divided into three groups: control, NaOCl 2.5% and 5.25% groups. Results: MMP-9 profile analysis showed that NaOCl concentration increase can suppress MMP-9 profile (p<0.05). Collagen structure analysis showed that NaOCl concentration increase can degrade dentin collagen (p<0.05). NaOCl 5.25% is the most effective in suppressing MMP-9 profile amount (9.9+3.63 ng/mL) and degrading collagen, it also has the highest shear bond strength (15.85 +0.43 MPa) compared to NaOCl 2.5% (14.51+3.66 ng/mL) and control group (24.09+8.88 ng/mL; 14.41+0.96 MPa). NaOCl 2.5% group has the lowest shear bond strength (9.2+0.65MPa). Conclusion: NaOCl 5.25% can suppress MMP-9 profile and degrade fibril collagen to increase the shear bond strengths value and create a micro mechanical bonding between resin and anorganic part of dentin without hybrid layer.

Latar belakang: Ketahanan ikatan antara resin komposit dengan dentin merupakan penentu keberhasilan restorasi resin komposit. Tujuan: Menganalisis EBA terhadap kekuatan ikat resin-dentin. Metode: 48 sampel dentin dibagi menjadi 6 kelompok. Kemudian seluruh kelompok di ukur kekuatan ikat gesernya menggunakan UTM. Data dianalisa statistik dengan uji hipotesis Kruskal Wallis yang dilanjutkan dengan uji Mann Whitney. Hasil: Median tertinggi kelompok 3, sedangkan median terendah kelompok 5. Kesimpulan: Ekstrak biji anggur yang diaplikasikan sebelum etsa dapat meningkatkan kekuatan ikat gesek namun tidak dapat menghambat biodegradasinya.ABSTRACT

Latar Belakang: Residu radikal bebas pasca perawatan internal bleaching dapat mengganggu proses polimerisasi resin komposit. Akibatnya, terjadi penurunan kekuatan ikatan restorasi. Antioksidan ekstrak teh hijau dapat digunakan untuk menghilangkan radikal bebas pasca bleaching dan meningkatkan shear bond strength resin komposit. Tujuan: mengetahui perbedaan shear bond strength resin komposit pada dentin pasca internal bleaching dengan dan tanpa aplikasi ekstrak teh hijau pada konsentrasi yang berbeda selama 2 menit. Metode: 25 gigi premolar, dipotong dalam arah mesiodistal, dan dibagi menjadi 5 kelompok: Kelompok 1 dentin normal, Kelompok 2 dentin pasca bleaching, Kelompok 3 dentin 2 minggu pasca bleaching, Kelompok 4  dentin pasca bleaching dengan teh hijau 10% 2 menit, dan Kelompok 5 dentin pasca bleaching dengan aplikasi teh hijau 35% 2 menit. Semua kelompok diaplikasikan restorasi resin komposit. Uji shear bond strength menggunakan Universal Testing Machine. Analisis statistik menggunakan one-way ANOVA dan Uji Post Hoc Bonferroni. Hasil: Uji Post Hoc Bonferroni menunjukkan perbedaan bermakna antara kelompok 1 dan 4, kelompok 2 dan 5, serta kelompok 4 dan 5. Kesimpulan: Aplikasi antioksidan ekstrak teh hijau 35% selama 2 menit lebih tinggi dalam meningkatkan shear bond strength resin komposit dibandingkan dengan aplikasi ekstrak teh hijau 10% selama 2 menit pada dentin pasca internal bleaching menggunakan hidrogen peroksida 35%. ......Introduction: Internal bleaching treatment could leave free radicals in the dentinal tubules. It can interfere with the polymerization process of the composite resin. Antioxidants from green tea extract can remove the free radicals after bleaching and increase the shear bond strength of composite resin. Objective: To determine the shear bond strength of composite resin on dentin after internal bleaching with and without green tea extract application at different concentrations in 2 minutes. Methods: 25 premolars were cut in mesiodistal direction and divided into five groups: Group 1 normal dentin, Group 2 bleached dentin, Group 3 bleached dentin, Group 4 10% green tea application in 2 minutes, and Group 5 35% green tea application in 2 minutes. All groups were restored with composite resin. All specimens' shear bond strength was tested with the Universal Testing Machine. The data were analyzed with one-way ANOVA and Bonferroni Post Hoc Test. Results: Post Hoc Bonferroni test showed that there were statistical differences  in groups 1 and 2, groups 1 and 4, groups 2 and 5, and groups 4 and 5. Conclusion: 35% green tea extract is more effective than 10% green tea extract as an antioxidant for increasing the shear bond strength of composite resin after internal bleaching.

The incidence of loose brackets during orthodontic treatment has reached 5%; therefore, loose brackets should be rebonded. Rebonding without etching might decrease chair time without compromising bond strength. The objective of the study was to determine the shear bond strength and tensile bond strength of rebonded brackets without etching, in comparison with corresponding strength when using prior enamel etching. Forty human first upper premolar brackets were used as samples, divided into two groups. Group A (20 samples) were used to assess tensile bond strength, and group B (20) samples) to assess shear bond strength. Each group was tested twice with Universal testing Machine Shimadzu AG-5000. Test i was carried out to measure tensile and shear bond strength of brackets which were bonded after enamel etching, by pulling the brackets until loose. Test II was carried out to measure tensile and shear bond strength of brackets that were bonded without prior etching of the enamel, again by pulling the brackets until loose. The results showed a significant difference (p<0.05) between the two groups. Test II demonstrated significantly different tensile bond strength when compared to test I. Tensile and shear bond strength values of the bonding agent on rebonded bracket without etching were lower than tensile and shear bond strength values of the bonding agent in case of bonding with etching. However, even though the score decreased, it was still above the minimal score required for a bonding agent.

Latar Belakang: Prosedur bonding braket dengan teknik transiluminasi menjadi salah satu alternatif teknik penyinaran pada segmen posterior gigi yang dinilai memiliki akses dan pandangan yang terbatas. Hingga saat ini, masih terdapat perbedaan pendapat mengenai tingkat keberhasilan bonding braket dengan teknik ini. Belum ada standarisasi mengenai arah penyinaran, waktu penyinaran dan besar intensitas light-curing unit dalam bonding braket dengan teknik transiluminasi. Penelitian ini bertujuan untuk mengetahui perbedaan nilai kuat rekat geser pada polimerisasi bahan adhesif dengan teknik transiluminasi pada prosedur bonding braket logam gigi premolar terhadap berbagai intensitas light-curing unit LED. Metode: Dua puluh empat gigi premolar dibagi secara acak kedalam 3 kelompok menggunakan light-curing unit LED dengan intensitas 1000, 1300 dan 3200 mW/cm2. Waktu penyinaran disesuaikan dengan petunjuk teknis alat. Penyinaran braket dilakukan dari arah oklusal dan lingual gigi premolar pada semua kelompok. Spesimen direndam dalam larutan akuades setelah bonding kemudian disimpan didalam inkubator dengan suhu 37°C selama 24 jam. Uji kuat rekat geser dilakukan menggunakan mesin uji universal dengan beban automatic 5kN dan crosshead speed 0.5 mm/menit. Hasil: Nilai rerata kuat rekat geser braket logam pada kelompok LED intensitas 1000, 1300 dan 3200 mW/cm2 secara berurutan adalah 9,33±2.12 MPa, 13,34±2.66 MPa, dan 9,82±2,13 MPa. Kelompok 2 (intensitas 1300 mW/cm2) memiliki nilai kuat rekat tertinggi dibandingkan kelompok lainnya. Uji statistik menunjukan terdapat perbedaan bermakna antara nilai kuat rekat geser braket logam pada LED intensitas 1300 mW/cm2 dengan intensitas 1000 mW/cm2 dan 3200 mW/cm2. Rendahnya nilai kuat rekat geser pada kelompok 3 (intensitas 3200 mW/cm2) pada penelitian ini disebabkan oleh kurangnya pemeliharaan light-curing unit yang digunakan sehingga mempengaruhi output intensitas cahaya. Kesimpulan: Teknik transiluminasi dengan kombinasi arah penyinaran merupakan teknik penyinaran yang efektif dan dapat diterima secara klinis pada prosedur bonding braket premolar. Ketiga intensitas yang digunakan memenuhi nilai kuat geser klinis sehingga klinisi dapat menggunakan ketiga pilihan intensitas LED tersebut. Pemeriksaan dan perawatan berkala pada light-curing unit diperlukan untuk menjaga kualitas dari perangkat yang digunakan. ......Introduction: Transillumination is an alternative technique to light-curing the posterior segment of the tooth which is considered to have limited access and vision in bracket bonding procedure. However, the effectiveness of this technique is still arguable. There is still no standardization regarding the curing direction, times and light intensity of the curing unit in the bonding bracket with the transillumination technique. This study aims to determine the differences of shear bond strength of orthodontic stainless steel braket using transillumination technique with various LED curing intensity in posterior teeth. Methods: Twenty four premolars were divided into 3 groups using a LED curing unit with various light intensity of 1000, 1300 and 3200 mW/cm2. The curing time was adjusted to the technical instructions of the curing unit. The curing direction was performed from the occlusal and lingual surface of the premolars in all groups. The specimens were placed in distilled water after bonding and then stored in an incubator at 37°C for 24 hours prior to testing. The shear bond strength test was carried out with an universal testing machine at an automatic load 5kN and crosshead speed of 0.5 mm/min. Results: The average value of the shear bond strength of stainless steel brackets in the LED groups with intensities of 1000, 1300 and 3200 mW/cm2 respectively were 9.33±2.12 MPa, 13.34±2.66 MPa, and 9.82±2.13 MPa. The group 2 (intensity of 1300 mW/cm2) had the highest shear bond strength value compared to other groups. There was a significant difference statistically between the shear bond strength of stainless steel brackets on LED intensity of 1300 mW/cm2 with an intensity of 1000 mW/cm2 and 3200 mW/cm2. The lower shear bond strength value of the group 3 (intensity of 3200 mW/cm2) in this study occured due to the lack of maintenance of the curing unit used which affects the light intensity output. Conslusion: The transillumination technique with a combination of curing directions from occlusal and lingual surface of posterior teeth is an effective and clinically acceptable light curing technique for orthodontic bracket bonding procedures. The three light intensities of LED curing unit used meet the clinical shear bond strength values, therefore clinicians can use all the options above. Regular inspection and maintenance of the light-curing unit is necessary to maintain the quality of the device used.

Latar Belakang: Prosedur internal bleaching dengan hidrogen peroksida 35% dapat meninggalkan residu radikal bebas pada struktur gigi yang akan mengganggu kekuatan ikatan restorasi resin komposit. Natrium askorbat sebagai antioksidan dapat menghilangkan residu radikal bebas dan meningkatkan kekuatan ikatan geser resin komposit pada gigi.Tujuan: mengetahui kekuatan ikatan geser resin komposit pada gigi pasca internal bleaching dengan hidrogen peroksida yang diaplikasikan natrium askorbat 10% dan 35% selama 2 menit dan 10 menit Metode: Prosedur internal bleaching dilakukan pada 25 gigi premolar yang dipotong menjadi dua bagian buko-palatal, kemudian sampel dibagi menjadi 5 kelompok. Kelompok 1 tanpa aplikasi natrium askorbat, kelompok 2 aplikasi natrium askorbat 35% selama 2 menit, kelompok 3 aplikasi natrium askorbat 35% selama 10 menit, kelompok 4 aplikasi natrium askorbat 10% selama 2 menit, dan kelompok 5 aplikasi natrium askorbat 10% selama 10 menit. Semua kelompok dilakukan restorasi resin komposit kemudian dilakukan uji kekuatan ikatan geser dengan alat Universal Testing Machine (UTM). Data yang diperoleh dianalisa secara statistik dengan ANOVA satu jalur dan Uji Post Hoc Bonferroni. Hasil: Hasil uji Bonferroni menunjukkan perbedaan bermakna kekuatan ikatan geser pada kelompok 1 dan 2; kelompok 1 dan 3; kelompok 2 dan 4, dan kelompok 3 dan 5. Sedangkan antara kelompok 2 dan 3; dan kelompok 4 dan 5 tidak terdapat perbedaan bermakna. Kesimpulan: Aplikasi natrium askorbat 35% selama 2 menit cukup untuk meningkatkan kekuatan ikatan geser resin komposit pada gigi pasca internal bleaching dengan hidrogen peroksida 35%. ......Background: Internal bleaching with 35% hydrogen peroxide will produce free radical residues within the tooth structure and disrupt the bond strength of composite resin restorations. Sodium ascorbate as an antioxidant can remove free radical residues and increase the shear bond strength of composite resins. Objective: To examine the effect of 10% and 35% sodium ascorbate application on the shear bond strength of composite resin after internal bleaching with 35% hydrogen peroxide. Methods: Internal bleaching procedure was performed on 25 premolars, then the samples were divided into 5 groups. Group 1 without sodium ascorbate, group 2 application of 35% sodium ascorbate for 2 minutes, group 3 application of 35% sodium ascorbate for 10 minutes, group 4 application of 10% sodium ascorbate for 2 minutes, and group 5 application of 10% sodium ascorbate for 10 minutes. All groups were restored with composite resin and tested for shear bond strength with Universal Testing Machine (UTM). The data were statistically analyzed with one-way ANOVA and Bonferroni Post Hoc Test. Results: The results showed that there were statistical differences between groups 1 and 2; groups 1 and 3; groups 2 and 4, and groups 3 and 5. Meanwhile between groups 2 and 3; and groups 4 and 5 there was no significant difference. Conclusion: Application of 35% sodium ascorbate for 2 minutes increased the shear bond strength of the composite resin after internal bleaching with 35% hydrogen peroxide.

Latar belakang: Keberadaan pelikel mengurangi kualitas dan kuantitas pengetsaan enamel, yang mengurangi kuat rekat geser adhesif. Deproteinisasi enamel dengan sodium hipoklorit (NaOCl) sebelum pengetsaan diharapkan meningkatkan kuat rekat geser bahan adhesif. P

Tujuan: membandingkan nilai kuat rekat geser braket yang direkatkan dengan adhesif Unite (UN) and Xihu-BIOM (XB) pada enamel tanpa dan dengan deproteinisasi enamel menggunakan NaOCl 5.25%.

Metode: 52 gigi premolar pertama rahang atas dibagi menjadi 4 kelompok: A1) UN tanpa deproteinisasi; A2) XB tanpa deproteinisasi; B1) UN dengan deproteinisasi; dan B2) XB dengan deproteinisasi. Spesimen direkatkan braket dan diuji menggunakan Universal Testing Machine Shimazu AG-5000.

Hasil: Rerata kuat rekat geser pada kelompok A1 adalah 12.82 + 2.187 MPa, A2 adalah 14.49 + 2.986 MPa, B1 adalah 15.82 + 3.251 MPa, dan B2 adalah 12.56 + 2.726 MPa. Pada uji statististik terdapat perbedaan bermakna rerata kuat rekat geser adhesif UN antara kelompok tanpa dan dengan deproteinisasi. Selain itu terdapat perbedaan bermakna kuat rekat geser kelompok dengan deproteinisasi antara adhesif UN dan XB. Pada semua kelompok adhesif terdapat peningkatan tidak bermakna nilai ARI pada kelompok dengan deproteinisasi.

Kesimpulan: Deproteinisasi enamel dengan NaOCl 5.25% sebelum pengetsaan memberikan peningkatan bermakna kuat rekat geser adhesif Unite. ......Background: Pellicle’s protein in enamel surface reduces the quality and quantity of acid etching, therefore it reduces shear bond strength. Enamel deproteinization with sodium hypochlorite (NaOCl) before etching was expected to enhance orthodontic adhesives’ shear bond strength.

Aim: to assess the effect of enamel deproteinization with 5.25% NaOCl before etching on shear bond strength of Unite (UN) and Xihu-BIOM (XB) adhesive.

Methods: Fifty-two maxillary first premolars were divided into four groups: A1) UN without deproteinization; A2) XB without deproteinization; B1) UN with deproteinization; and B2) XB with deproteinization.  Brackets were bonded and mechanical test were performed in Universal Testing Machine.

Results: The mean shear bond strength value for Group A1 was 13.51 + 2.552 MPa, A2 was 14.36 + 2.902, B1 was 16.43 + 2.615 and B2 was 13.05 + 2.348 MPa. There’s significant effect of sodium hypochlorite enamel deproteinization before acid etching on shear bond strength of Unite adhesive. Significant difference on shear bond strength of Xihu-BIOM and Unite adhesive within enamel deproteinization group was observed in this study. Within both adhesive groups, there were non-significant increase on ARI score in deproteinization group.

Conclusion: Enamel deproteinization with 5.25% NaOCl before etching increased shear bond strength of Unite significantly.
