"Kegiatan pemantauan terapi obat (PTO) dilakukan secara rutin dan teratur untuk memastikan terapi obat yang aman, efektif, dan rasional bagi pasien. PTO dilakukan pada pasien seminoma mediastinum di RSUP Persahabatan untuk memastikan kemoterapi berjalan dengan efektif dan maksimal.Analisis PTO dilakukan dengan menganalisis terapi obat yang didapatkan pasien berdasarkan data yang terdapat pada rekam medis pasien. Data didapatkan dari Catatan Perkembangan Pasien Terintegrasi SOAP dokter, perawat, dan apoteker data terapi dan obat yang diterima pasien selama dirawat dan hasil pemeriksaan laboratorium pasien.Masalah terkait obat terkait reaksi obat tidak dikehendaki ditemukan efek samping kemoterapi cisplatin dan dexamethasone menyebabkan pasien cegukan dan kemoterapi etoposide menyebabkan konstipasi. Indikasi tanpa obat ditemukan bahwa pasien belum buang air besar sejak hari pertama masuk rumah sakit.Pemberian obat dengan baik dan benar kepada pasien perlu diperhatikan supaya tidak terjadi kesalahan dalam pengobatan karena pasien termasuk kategori polifarmasi dengan jumlah obat yang diterima lebih dari 5 obat. Efek samping cegukan yang muncul dan konstipasi perlu dievaluasi setelah diberi pengobatan.

PTO analysis is carried out by analyzing the drug therapy received by the patient based on the data contained in the patient's medical record. Data were obtained from Integrated Patient Development Notes, doctors, nurses and pharmacists, data on therapy and drugs received by patients during treatment and results of laboratory examinations of patients. Drug-related problems related to adverse drug reactions found side effects of cisplatin and dexamethasone chemotherapy causing hiccups in patients and etoposide chemotherapy cause constipation. An indication without medication was found that the patient had not had defecate since the first day of admission to the hospital. It is necessary to pay attention to administering the drug properly and correctly so that there are no errors in treatment because the patient is included in the polypharmacy category with the number of drugs received more than 5 drugs. Side effects that appear hiccups and constipation need to be evaluated after being given treatment."
